• more concern for professionalism and accountability
on the part of the healthcare community;
• the development of a more highly educated, keenly
aware and demanding patients;
• rapid economic changes, new market conditions, and
the ever-changing needs and demands of individual
patients and the public;
• the increasing impact of quality of life and healthcare.
Acquiring the necessary competencies and maintaining
these competencies are clearly vital for providing
contemporary, evidence-based, up-to-date, safe and
ethical care that meets the professional standards and
complies with the needs and demands of individual
patients and the public. On the other hand, as stated
by Christie et al (3) ‘Competence relies on knowledge
and experience, but also on intellectual maturity, self-
confidence and professional values’. Thus, an important
outcome of the basic dental education needs to be
‘self-evaluation’ of knowledge, skills and competencies.
Throughout his/her professional career, each dentist is
expected to determine the particular areas that need
improvement (continuing professional development)
and to know his/her own limits in each dental discipline
(when a case is beyond personal competency, referring
the patient to appropriate colleagues/facilities) (4).
According to Low & Kalwarf (5): ‘As health professions
evolve and standards of care shift over time, the
continuous development of the scientific and practical
knowledge base and of cognitive and clinical skills
becomes a more expected standard’. As a natural
IntroductionDentists are an integral part of the medical community
that is concerned with the monitoring and care of
public health through their provision of oral healthcare.
A dentist, like any other physician, must acquire this
capacity through the achievement of a set of fundamental
competencies to start an independent practice. After
obtaining the required professional degrees, a basic
level of attained knowledge and skills, coupled with
the professional conduct, behavior and attitudes
necessary in order to respond to the entire spectrum of
circumstances encountered during daily practice, must
be acquired. These competencies demand the dentist to
integrate all the basic disciplines for the benefit of his/
her patients (1).
Although the dental profession has always attributed
significant importance to all phases of dental education,
in both basic studies and continuing professional
development, nowadays, more emphasis is being
placed on advanced studies, due to the broadening of
the professional competencies and abilities expected of
dental professionals from various internal and external
driving forces, including (2):
• the changing social and professional environments,
which have expanded the scope of the dentist’s role and
professional responsibilities;
• the development of the dental practice, as a result of
the shorter half-life of professional knowledge, concurrent
with the advances in science and technology, and the
increasingly stringent professional standards;

After analyzing the results of a questionnaire received
from 32 dental associations, the ERO has come to the
conclusion that the profession should adhere to the major
competencies as formulated in this draft, while taking
into consideration the fact that secondary competencies
may vary between countries and faculties.

• Oral Medicine: After upgrading by a comprehensive
continuing education system, a dentist must be competent
in contributing to the general (medical) health of people
of all ages through recognizing the bilateral relationship
between oral health and general health, by diagnosing
the oral findings of many systemic (medical) disorders,
and by diagnosing and managing the most common oral-
mucosal diseases.
• Caries and Endodontics: A dentist must have perfect
knowledge of cariology and pulpal pathology and peri-
radicular disorders.
• Periodontology: The graduate in dentistry must be
competent in diagnosing and managing periodontal
consequence of the expansion of the European Regional
Organization (ERO), fundamental differences between
education systems (e.g., organization, teaching/learning
methods, evaluation, quality management, etc.) have
developed between many of the member countries (6).
There seems to be a clear need for the development of
some national and international standards for professional
competencies and clinical guidelines for evaluating
different aspects of these competencies. Obviously, it
is not possible to improve professional competencies
through a single, concrete pathway. Nonetheless, we
feel that the main tasks of this organization should
include the provision of general guidelines for a long-
term and efficient holistic continuing education program
that complies with the increasing needs and demands,
meets the expectations from the dental profession, as
well as periodically emphasizes the aims, context and
philosophy of continuing education - from a professional
perspective.
Concurrent with the development of this educational
discipline, several approaches have been actually proposed.
Following is a summary of the main guidelines (7).
In this paper, we have identified the areas that
represented the most basic disciplines of the general
practitioner’s (G.P.) professional activity (including, of
course, interdisciplinary aspects).
Spheres of conpetencies • Basic (Primary) Competencies: This is the outcome
of education and represents the dentist’s ability to
perform a specific, but complex, service or task. The
complexity of the task indicates that multiple and more
specialized abilities are indispensable in order to support
the performance of any particular competency.
• Complementary (Secondary) Competencies:
Specific abilities may be considered subdivisions of
educational training. The achievement of a primary
competency requires the acquisition of all secondary
competencies pertaining to that particular discipline.

dento-alveolar growth and the functional abnormalities
of primary, mixed, and permanent dentition.
At this stage, he/she must be capable of performing minor
orthodontic treatments, or, if the case exceeds his/her
competence, should refer the patient to an orthodontic
specialist in order to formulate a proper diagnosis
and treatment planning (e.g., there are considerable
differences between countries; an obligation to refer to a
specialist is feasible only in countries that have orthodontic
specialists) (8).
• Basic Technical and Clinical Sciences: It must be
compulsory for a dentist to acquire a good knowledge and
understanding of the basic bio-medical, technical and
clinical sciences in order to understand the physiological
or pathological conditions that are relevant to oral health
care. He/she also must be competent in applying this
information to daily clinical situations.
• Gathering Clinical Information: At this stage, a
dentist must be capable of obtaining and recording a
complete medical history of a patient’s state of general
and oral health. This must also include psychological
and social information, as well as biological and medical
data. Furthermore, a dentist must be competent in
performing an appropriate and comprehensive physical
and oral examination.
Moreover, he/she must be capable of interpreting the
findings of the diagnostic tools used and of organizing
further investigations, if necessary (9).
• Diagnosis and Treatment Planning: One of the
fundamental principles is to ensure that dentists are
competent in critical thinking, clinical reasoning,
judgment and clinical decision-making, in order
to develop a differential, provisional or definitive
diagnosis, after interpreting and correlating all gathered
information, including examination data (e.g., clinical and
radiographic), and data pertaining to the psychological,
social and cultural background of each patient. A G.P.
must be competent in formulating a diagnosis and a
treatment plan for patients of all ages, in understanding
pathologies, understanding the etiology and pathogenesis,
the treatment techniques, including the most recent
approaches, and the early recognition and prevention of
such pathological conditions.
• Surgery: A dentist must be competent in treating
and managing conditions requiring elementary surgical
procedures of the hard and soft tissues, such as
tooth extractions, removal of roots and minor surgical
interventions, and must be competent in applying an
appropriate pharmaceutical support to the treatment.
(This is likely to be a general competency, not only related
to surgery, but to all aspects of dental treatment as well).
• Restorative Dentistry & Prosthodontics: A dentist
must be competent in restoring partially or totally destroyed
hard tissues of a tooth in an anatomically and functionally
acceptable form. In the instance of missing teeth, a dentist
must be competent in rehabilitating the patient’s teeth with
functional and esthetic prosthetic devices.
• Implantology: After a progressive basic and clinical
calibration, a dentist should be competent in evaluating
patients in need of dental implants, and, after special
training, should also be able to carry out simple implant
procedures.
• Pain and Anxiety: A dentist must be competent in
employing all appropriate techniques in order to manage
oro-facial pain, discomfort and psychological distress.
• Emergency Situations: A practitioner must be
competent in effectively preventing and managing the
majority of medical and dental emergency situations
encountered in a routine general practice.
• a. Pedodontics: A dentist must also be competent in
formulating an accurate diagnosis and in managing the
dental and periodontal treatment, taking into consideration
the special approach towards dental care for children. A
dentist also must be competent in performing all the latest
professionally accepted preventive dental measures for
children.
• b. Orthodontics: Today, a dentist should be competent
in understanding limited developmental or acquired

professional development allied with a permanent
continuing educational system, in order to sustain a high
level of clinical competence. He/she must be aware of
his/her obligations to cooperate with other members of
the dental profession for the achievement of health and
safety standards.
The G.P. must be aware of his/her moral and ethical
responsibilities towards patients, colleagues and the
population as a whole (13).
• Promoting Good Health, Preventive Dental Care:
Special attention must be paid to ensure that dentists are
competent in improving the oral health of their patients,
families, the community and the whole country.
From preventive dental care to community dentistry, he/she
must pay the necessary attention to his/her professional
duties on an ongoing basis. These professional duties
include: approval of broader responsibilities for the
overall health of individual patients and the community,
the provision of health/oral health information to their
individual patients and to the public at large, including
information regarding risk factors, such as those posed
by cigarette smoking, monitoring of the public’s changing
needs and demands, and actively participating in the
planning of oral health policies (14).
• Special Care for the Handicapped / Gerodontics:
G.P.’s should have a good knowledge and adequate
skills in order to ensure competencies in providing
primary care to patients with special needs. The
socio-demographic differences among patients must
be addressed, with treatment planning and clinical
solutions adapted in a manner appropriate for the
frail, the disabled, the disadvantaged and for elderly
patients (15).
RecommendationsSince the target population has significant basic
knowledge gaps, it is necessary to identify these
issues in order to deliver an appropriate educational
program. A problem-based learning system could be
their individual needs, demands and reasonable
preferences. A dentist must be competent in selecting
and evaluating all appropriate diagnostic tools. It is
also essential that a dentist be capable of recognizing
when a treatment is beyond his/her knowledge/skill and
recommend or refer to a specialist or to a dental setting
with appropriate facilities (10).
• Communication and Interpersonal Skills: As many
aspects of dental care involve dentist-patient interaction
and lucid interactions are particularly essential in dentistry,
more so than in many other professions, every dentist must
be trained to achieve competence in basic communication
skills, encompassing interactions with patients, their
families, environment, associations and other health
professionals involved in their care. Such competencies
are clearly important for a constructive dentist-patient
relationship, in order to be capable of encouraging
patients to participate in the decision-making process,
in order to better understand people’s decision-making
process in general, to motivate patients to modify their
behavior/habits, which is necessary in order to achieve a
better oral health status and to improve their adherence
to recommended/prescribed personal care. Interpersonal
skills are particularly needed when communicating with
the disabled and/or disadvantaged, who naturally deserve
particular additional attention (11).
• Professionalism: It is critical that G.P.’s are imparted
with a contemporary knowledge and good understanding
of the actual ethical and professional issues in the
dental practice. Every G.P. must be familiar with the
context and elements of professionalism, have a correct
understanding of dental ethics and of their significant
impact on the respected status of dentistry and on the
public trust in the dental profession (12).
He/she must be competent in a wide range of skills, such
as research, analytical problem solving, communication
and inter-professional relations.
He/she must be actively pursuing his/her continuing

While speaking, speech was impaired; at smiling the
maxillary and mandibular anterior teeth were overly
displayed (Figs.1-3).
The patient was completely edentulous and wore a set
of complete dentures. Vertical dimension at rest position
was at the same level or even lower by 1 to 2 mm than
the vertical dimension at occlusion. There was no inter-
occlusal rest space between the upper and the lower
dentures.
At the intraoral examination, the edentulous arches
and ridges were normal in form, texture and relation.
Slight pressure spots were present at the muccobucal
fold in the posterior region of the edentulous mandible.
The hygiene of the oral tissues and of the dentures was
impeccable. The patient wore the dentures only when
requested by social or functional needs. When alone at
home she removed, at least, the lower prosthesis.
The maxillary denture base was considerable taller
than the mandibular one. The maxillary anterior teeth
were tilted labial from the edentulous ridge crest.
The occluding mandibular anterior teeth were placed
vertically, on top of the anterior mandibular edentulous
ridge. This occlusal scheme produced a horizontal
overlap of 5 mm (Figs. 4-5).
After the clinical examination and observing the old
photographs, provided by the patient (Figs. 6-7), the
esthetic and functional problems expressed by the
patient seemed justified, and indeed caused by the
existing set of dentures. The lack of interocclusal rest
space, the poor distribution of the interarch vertical
Anamnesis and main complainA 68 years old female was referred by a colleague for
diagnosis and treatment. Widowed 6 years ago, the
patient recently emigrated from Argentina to Israel. She
was mentally alert and physically active. She had no
particular problems with cultural, social adaptation, a
new language and different environment.
General healthFive years ago, following a breast removal, the patient
received chemotherapy for cancer therapy. She was
under medical supervision, without specific therapy for
her oncology condition. She received medications for mild
osteoporosis. In addition, she had moderate, controlled
high blood pressure.
For the last two years the patient’s main medical, dental
and emotional complains were her facial appearance,
phonetic difficulties and eating limitations (in her words
,and in that order).
At her first visit to our clinic, the patient was an unhappy
person. She was unable to speak, smile, swallow or chew
properly. She complained of oral discomfort, dentures
instability, and chronic pain at the dentures supporting
tissues in the maxillar and the mandible. ״I have a horse’s
face. I speak, eat and laugh like an equine״.
Clinical examinationThe patient looked her age; the facial appearance
was of a tense person with protruding upper and
lower lips. The facial oral muscles seemed distended.

Fol lowing the tr y in dentures sessions, when the
patient was satisfied with the esthetic and phonetic
arrangements, the dentures were completed and
delivered (Figs. 8-9).
The patient adapted well, was content and felt
comfortable with the new set of dentures. After six
additional sessions, required for minor occlusal
adjustments and pressure spots relieve, the patient
was dismissed and scheduled every six months for
regular check ups.
Specifically, the main changes performed (with the full
collaboration ״and help from the patient) while making
the new set of dentures were:
A. The provision of three millimeters distance for
interocclusal rest ״space, between the vertical
dimension of rest and the vertical dimension while in
occlusion״
B. The full anterior and premolars maxillary teeth and
the incisal ״third of the mandibular anterior teeth were
selected, arranged and ״articulated to be displayed
when patient was speaking, smiling and ״laughing. No
artificial gums were displayed during any normal facial״
or oral activities.
C. The occlusal plane was established after the
selection of the ״anterior teeth. The prosthetic plane was
made parallel to the ala״ tragus (Camper’s) lines
General summaryFive complete edentulous patients were rehabilitated
with conventional dentures. The cases illustrated some
of the difficulties encountered at the different levels in
complete removable dentures treatment.
Clinical and laboratory procedures are available, in
the professional literature and armamentarium, for the
proper resolution of most cases.
By using sound clinical judgment, detecting the
individual problems, applying proper variations in the
treatment course, successful solutions were achieved.
space and the excessive horizontal overlap were,
at least partly, the cause of the multiple, legitimate
complains of the patient.
Treatment planning and care providedA decision was made to make a new set of complete
dentures with emphasis on the individual esthetic
considerations, on a proper division of the inter-ridge
distance between the upper and lower dentures, for
an adequate interocclusal rest space, as a part of the
proper requisites for adequate complete dentures (1-8).
Special attention was given to the collaboration of the
patient during the selection of the form and contour of
the dentures bases; on the shape, color and positions
of the artificial teeth. Importance was accorded
to phonetic considerations. All these factors were
explained and agreed at the inception of the treatment.
We asked for preferred (by the patient) old photographs
for guidance and clues for the ensuing procedures. By
asking her advice, we gained the patients confidence
and collaboration during registrations, try-ins, dentures
deliveries and recall sessions.
The golden rule of universal esthetics is a positive
theoretical base ”from where to begin treatment” the
proper time for constructing a set of complete dentures
(6-9). From that base line and following the particular
characteristics of our patient (prominent nose and a large
chin) we provided the individual esthetic arrangements for
facial harmony, proper fullness of the lips, teeth selection
and positioning, minimal display of the denture base during
rest position, speaking and smiling. We emphasized and
changed several times the prominent chin contour by
adding or by subtracting wax denture base material in the
labial region of the anterior mandibular denture complying
with the reasonable patient esthetic requests (6-14).
Emphasis was given to provide a set of complete
dentures that will help to develop a harmonic face with
the individual facial characteristics of the patient.

4. מתן אנטיביוטיקה סיסטמית וחיסון אנטי טטנוס;שורש טיפול והתחלת שבועיים, למשך השן קיבוע .5
בתוך שבעה עד עשרה ימים )כשהשן עדיין מקובעת(;6. היגיינה מיטבית של הפה.
ד. הטיפול בשן עם אפקס פתוח, כאשר המטופל הגיע לאחר שהשן הושתלה מחדש במקום האירוע: המטרה לאפשר היא פתוח אפקס עם בשן בטיפול העיקרית טיפול מבצעים לא לכן השורש. התפתחות המשך את שורש בשן אלא רק אם מופיעים סימנים של נמק מוך השן או ספיגת שורש. כבשאר החבלות הדנטליות, ישנה חשיבות מיוחדת לביקורת ומעקב אחר מצב השן. כאן
יש לבצע את הפעולות הבאות:1. תפירה של חניכיים פגועות;
2. לוודא מיקום השן באמצעות צילום רנטגן;3. מתן אנטיביוטיקה סיסטמית וחיסון אנטי טטנוס;
4. קיבוע השן למשך שבועיים;5. היגיינה מיטבית של הפה.

- במטופלים מבוגרים עשוי טיפול שורש להיות הטיפול המועדף, למרות שניתן לשקול כיפוי מוך או כריתת מוך
חלקית. - אם עבר זמן רב מהפציעה לבדיקה, ואם מאובחן נמק
של רקמת המוך, יש צורך בטיפול שורש.לשקול יש השן בכותרת נרחב שבר של במקרים -

התאמת ההנחיותלרפואת השיניים בישראל
.2001 בשנת פורסמו IADT של הראשונות ההנחיות באותה שנה פורסם בפני רופאי חדרי המיון בישראל גם
הפרוטוקול לטיפול בשן שיצאה מהמכתשית )23(.הצעה השיניים רופאי בפני הוצגה 2007 בשנת זה פרוטוקול הישראלי. לציבור המותאם לפרוטוקול
נבדל מפרוטוקול 2001 במספר נקודות:עם בשיניים תזוזה של במקרים יבוצע שורש טיפול .1זעזוע של ממקרים )חוץ שנייה בישיבה סגור, אפקס
ותת תזוזה(. אין לחכות לסימני נמק. 2. במקרה של שירוש, אם רק אפשר, יש להשתיל מחדש
את השן במקום האירוע. יש לאחסנה ניתן להשתיל את השן מיידית, 3. אם לא

אידיאליים לשמירה על חיות התאים )Hiltz & Trope .)27 - 25 הראו ש-HBSS שמר על חיות תאים פיברובלסטיים למשך 72 שעות )26(. אולם חומר זה אינו נפוץ בארץ ועל כן סביר להניח שלא יהיה נגיש
במקרה הצורך. זה משמש בהשתלת איברים Viaspen: מדיום אחסון -הוא העיקרי חסרונו ההשתלה. למקום העברתם לצורך חיי מדף קצרים - חודש ימים בלבד. עובדה זו, בשילוב עם
מחירו הגבוה, הופכים את השימוש בו לנדיר.
- רוק: נמצא כמדיום אחסון המסוגל לשמור על ויטליות .)29 ,28( שעתיים עד שעה חצי למשך PDL-ה תאי החשש היא ברוק משימוש להימנעות העיקרית הסיבה נגיפית( צהבת )כגון וירלי או חידקי זיהום של למעבר גם המטופל. אל השן, לאחסון שימש שרוקו מהנשא, עדיין ,)30( בפיו השן את נושא עצמו המטופל אם השורש, שטח ופני המוך של עצמי לזיהום החשש קיים

הביקורת )31(. Lateral( בכל הפרוטוקולים מומלץ על מעקב במקרים שלבמקרים רק שורש טיפול וביצוע ,)Extrusive-ו Luxationרנטגנית עדות וחוסר חיות לבדיקות תגובה חוסר של לתהליך סב-שורשי. גישה זאת מבוססת על האפשרות לרה-
וסקולריזציה נמוכה יחסית לאחר חבלה בשיניים עם אפקס באחוזים קיימת לרה-וסקולריזציה הסיכוי .)38( סגור נמוכים, אולם אם וכאשר מתפתח נמק מוך סיכויי ההצלחה בטיפול שורש נמוכים בהשוואה לטיפול שורש בשן ויטלית. בשן עם אפקס סגור מומלץ לבצע טיפול שורש מוקדם ככל האפשר )עדיף בפגישה השנייה(, כל עוד המוך הנמקי עדיין בסיכויי ההבדל היא זו גישה להעדפת הסיבה הזדהם. לא שבה סגור, אפקס עם בשן שורש בטיפול )96%( ההצלחה ההצלחה סיכויי לעומת שורשי, סב נגע התפתח לא עדיין
הנמוכים יותר )86%( בשן עם נגע סב שורשי )39(.

לרפואת האמריקאית האקדמיה הגדירה 2003 בשנת שיניים לילדים AAPD, את המונח עששת ילדות מוקדמת עששת נגע כנוכחות )Early childhood caries - ECC(או מעששת כתוצאה שן חוסר קויטציה(, בלי או )עם )חמש חודש 71 גיל עד בילד ראשונית שן בכל שחזור שנים ו-11 חודשים )1(. האקדמיה האמריקאית הגדירה חריפה כעששת יותר חמורה המחלה בה קבוצה תת חריפה עששת .)severe ECC( המוקדמת הילדות של לעששת סימן כנוכחות מוגדרת המוקדמת הילדות של עד 3 בגילאי ובילדים ,3 גיל עד בילדים חלק במשטח יותר עם קויטציה, חוסר נגע עששת אחד או כנוכחות 5בשיניים חלק במשטח שחזור וכן מעששת כתוצאה שן
קדמיות עליונות נשירות או dmf≤ גיל הילד + 1.התופעה זכתה בעבר לכמה שמות )4-2(, בהם:
• Nursing bottle mouth (EN Fass, 1962-מיוחס ל(• Nursing caries • Nursing bottle caries • Nursing bottle syndrome • Milk bottle syndrome• Baby bottle caries

המינוח בעבר התייחס לעששת בחותכות קדמיות בילדים זה מינוח מוגדר. דפוס עם כמחלה ספר בית גיל לפני נכון״ ״לא בין שימוש ותוצאה, התייחס לקשר של סיבה במגע להיות ממותק לנוזל או לחלב המאפשר בבקבוק,

מהלך עששת ילדות מוקדמתלבקיעת סמוך מתחילה עששת של זו אלימה צורה מתקדמת חלקים, משטחים על מתפתחת השיניים, מהר וגורמת להרס המשנן. העששת מתפתחת בחותכות העליונות בטוחנות מכן ולאחר העליונות הנשירות מאחר פחות, נפגעות התחתונות החותכות והתחתונות. והן קריוגני מנוזל עליהן מגנה יניקה במצב שהלשון המנדיבולריות. הרוק מבלוטות רוק ידי על נשטפות )צמיג( מוקוזי רוק מגיע העליונות לחותכות לעומתן, שלהן ההדוק המגע לכך, בנוסף קטנות. רוק מבלוטות עם השפה מקשה על שטיפתן. קיימת טענה כי החותכות בקיעה מועד עקב מהטוחנות יותר נפגעות העליונות מוקדם יותר ומכאן גם משך חשיפה ממושך יותר להרגל. יותר דק שהאמאיל משום מהיר הינו העששת תהליך

שכיחות המחלהECC, מאחר שילדים קיים קושי בקביעת השכיחות של נגישים אינם ולכן ספר בית גיל לפני הם לה החשופים לבדיקה כמו ילדים מבוגרים יותר. בדיקת ילדים המגיעים היו שהוריהם מאחר מוטה, שיניים במרפאת לטיפול במרכזים השכיחות בדיקת בטיפול. לצורך מודעים לבריאות המשפחה עלולה להיות מוטה עקב שוני במעמד

וקבוצה תרבות שלכל ומאחר מרכזים בין סוציואקונומי המרכז. באותו שיתבטאו ייחודיים הרגלים יש אתנית בדומה, מחקרים ממדינה אחת לא יחזו תוצאות מחקרים תינוקות בבדיקת קושי קיים בנוסף, אחרת. ממדינה המחקרים בין הבדיקה בקריטריוני הבדלים וקיימים
השונים )4(.סקר ממצאי את מתארים )6( וחב׳ Romez-gomez 3rd National Health and Nutrition( NHANES IIIמ-40% שליותר שמצא ,)Examination surveyיש שנתיים בגיל מהילדים ול-8.4% 5 בגיל מהילדים לפחות שן אחת עם עששת או שחזור. במחקרם, שבדק רובם חודש, 55 עד שלושה בגילאי בארה״ב ילדים 146עם ECC 16% היו מהם של-43% נמצא היספני, ממוצא עששת. ללא היו ו-41% באמאיל מקומית דמינרליזציה שכיחות ה-ECC נעה בין 1% ל-12% במדינות מפותחות
ומגיעה עד 70% במדינות מתפתחות )7(.בישראל נבדקה שכיחות העששת אצל ילדים בני 5. בשנת 1992, בסקר ארצי שכלל 767 ילדים, נמצא ש-41.3% מהם בשנת .)8( 2.7 על עמד deft-ה וממוצע עששת ללא היו 1994 נמצא ש-43% מהילדים באשקלון היו ללא עששת וממוצע ה-)t(dmf עמד על 2.64 ± 2.08 )9(, ובשנת 1978 נמצא ש-84% מהילדים בירושלים היו עם עששת וממוצע
ה-dmf עמד על 3.6 ± 4.7 )10(.
אתיולוגיהפקטורים הכוללת מוליטיפקוריאלית מחלה הינה ECCחידקים של גבוהה רמה עששת. בכל כמו ביולוגיים אצידוגניים בשילוב עם צריכת פחמימות בתדירות גבוהה יוצרים חומצה המורידה את pH הפלאק לתקופות ארוכות
וגורמת לאובדן חומר שן )3, 11(.

השימוש באנטיביוטיקה בזמן ביצוע שתלים דנטליים, למניעת סיבוכים

Suggested guidelines for basic dental education in Europe

With the enlargement of the European Community, a great
effort was done. In order to abolish the inequalities that exists
in the provision of oral health education and Oral healthcare
delivery between Eastern and Western Europe. On the
other hand, each day more emphasis is placed on the basic
competencies and abilities gained during basic dental
education due to internal and external driving forces. The
differences between the systems are often very important
prominent and seem to depend mainly on the national
institutions or the government policy. Therefore there is a
major responsibility for the profession in helping to integrate
the dental practitioners in an updated approach. As a similar
basic dental education will progressively been developed, the
Continuing Education could become more complex. For this
purpose, following a basic dental education complying with
the current needs and demands, a more adapted Continuing
Education program must be elaborated and promoted

The importance of correct diagnosis in complete dentures treatment

A 68 years edentulous female, wearing unsuccessful complete
‘dentures, was treated by making a new set of dentures. The
main initial ‘complains were unaesthetic teeth, excessive
gums display, speech ‘impairment and severe difficulties in
mastication.
‘With the patient’s full collaboration and following sound
established ‘prosthodontics principles, a successful set of full
dentures was provided.
‘The main changes performed with the new set of dentures
were:
A. Provision of three millimeters interocclusal rest space
between the ‘vertical dimension while in occlusion and the
vertical dimension of ‘rest.
B. The full anterior and premolars maxillary teeth and the
incisal’s ‘third of the mandibular anterior teeth were displayed
when patient was ‘speaking, smiling and laughing. No artificial
gums were displayed ‘during any normal facial or oral activities.
C. The occlusal plane was established after the selection of
the anterior ‘teeth .The plane was made parallel to the ala
tragus (Camper’s) ‘line.

Early childhood caries

ECC was defined by the American Academy of
pediatric dentistry at 2003 as the presence of 1 or
more decayed (noncavitated or cavitated lesions),
missing (due to caries), or filled tooth surfaces in any
primary tooth in a child 71 months of age or younger.
This is a virulent type of dental caries that start soon
after the tooth erupts and progress rapidly.
The prevalence is 1-12% in developed countries and
70% in developing countries, and changes in different
cultures, communities, socioeconomic status, etc.
The etiology of the disease is multifactorial like in any
dental caries.
The risk factors include high levels of SM and LB, enamel
defects, oral habits, complication at pregnancy and birth,
social and demographic factors and the child’s age.
The disease implications are: high risk of new caries
defects in both permanent and deciduous dentitions,
insufficient physical development, hospitalization
and emergency room visits, loss of school days and
increased days with restricted activity, increased
treatment costs and time, diminished ability to learn,
and diminished oral health-related quality of life.
Due to the aggressive pattern of the disease, treatment
should be specific for each individual patient, and
should be given by an expert dentist with experience
who could manage the young child and the process
of the disease.
Treatment options are:
1. Conservative approach which includes recalls and
topical fluoride
2. Aggressive restorative approach. In both we should
first stop the carious habit and encourage prevention.
Before choosing the type of treatment, we should
consider the severity of the lesions, child’s age, caries
risk, child’s behavior, and parents’ cooperation.
Prevention at home includes:
1. decreasing the mother’s/primary caregiver’s mutans
streptococci levels
2. avoid sharing the same utensils in the family
3. implementing oral hygiene measures as the first
primary tooth erupts
4. dental home
5. avoid inappropriate feeding practices of infants and
toddlers.
Prevention at clinic includes:
1. topical fluoride application
2. proximal slices in appropriate cases.

The new IADT guidelines for dental trauma management: an introduction for the Israeli dentist

Dental trauma is, unfortunately, not uncommon in
the Israeli population. High risk populations include
children, sport participants, military personnel and
patients with oral (tongue) and/or lip piercing. In
2007, the International Association of Dental Trauma
(IADT) updated the guidelines for the management
of traumatic dental injuries. This paper is aimed
to discuss and present the new guidelines for the
management of traumatic dental injuries and to offer
some highlights to the Israeli dental population. Good
prognosis of injured teeth largely depends on prompt
and appropriate management. Thus, it is important for
healthcare-givers, especially dental practitioners, to
maintain an adequate level of updated knowledge.
Endodontal and periodontal aspects of root perforations

The purpose of treatment of perforations is to
achieve a tight and permanent seal that will prevent
bacteria and their by-products in the root canal from
entering into the surrounding periodontal tissues.
It is important to consider the type of perforation
according to the classification for selection of
the proper material and technique for each case.
Modern techniques for treatment of perforations
by orthograde and surgical approach result in
prolonged survival of the tooth.

even oblique or curved image planes - a process referred
to as multiplanar reformation (MPR). In addition, CBCT
data are amenable to reformation in a volume, rather than
a slice, providing 3-dimensional (3D) information. Data on
the effective dose is not totally conclusive. There is an
agreement that the doses from CBCT are significantly
lower than conventional CT. Some claim that, CBCT
needs doses higher than doses from the traditional views
used in dentistry. Other say that the average dose of a
CBCT image is nearly the same as a full-mouth intraoral
dental series, and higher than a panoramic radiograph.
However, the exact dose for any scan depends on the
specific equipment, the volume of tissue exposed,
and the settings used on the machine. The potential
benefits of CBCT in dentistry are undisputed; however,
it is imperative that their use be fully justified over
conventional techniques before they are carried out. The
next generation of CT imaging in dentistry is imminent.
Some professionals predict that in the near future, a newly
developed CT scanner with enhanced features will be
available to the dental world. The enhanced CT scanner
will be classified as “Ultra Cone Beam CT Scanner” and
it will provide important information about the three-
dimensional structure of blood vessels, nerves, soft
tissue, and bone. These images will help clinicians to plan
treatment strategies with greater chance for success.

Radiology is important in the diagnostic assessment
in dentistry. Computerized tomography as an imaging
diagnostic mean is advancing rapidly. Computed
tomography can be divided into two categories based
on acquisition x-ray beam geometry; namely: fan beam
and the more recent cone beam (CBCT). The imaging
source-detector and the method of data acquisition
distinguish cone beam tomography from traditional
CT imaging. Traditional CT uses a high-output rotating
anode X-ray tube, while cone beam tomography utilizes
a low-power, medical fluoroscopy tube that provides
continuous imaging throughout the scan. Traditional
computerized tomography records data with a fan-
shaped X-ray beam onto image detectors arranged in
an arc around the patient, producing a single slice image
per scan. Each slice must overlap slightly in order to
properly reconstruct the images. The advanced cone
beam technology uses a cone-shaped X-ray beam
that transmits onto a solid-state area sensor for image
capture, producing the complete volume image in a single
rotation. The sensor contains an image intensifier and a
CCD camera, or an amorphous silicon flat panel detector.
The single-turn motion image-capture used in cone beam
tomography is quicker than traditional spiral motion,
and can be accomplished at a lower radiation dose as a
result of no overlap of slices. CBCT allows the creation in
“real time” of images not only in the axial plane but also
2-dimensional (2D) images in the coronal, sagittal and

The journal will also serve as a stage for exchangingviews and information among dentists in Israel.
CirculationThe journal is quarterly, and its target readers are all members of the Israel Dental Association.

For names of diseases and medical terms please use the terms that are customary among dentists in Israel, rather than the Hebrew translation. Names and other professional terms are to be used consistently throughout the article. After the first occurrence of the term in the article, it is advisable to add in brackets itsEnglish translation. Abbreviations may be used.
Cover Page The cover page will include the title of the article (not exceeding 50 letters), names of authors, theirorganizations and their positions.
English Summary An English version of the summary is to be enclosed on a separate page. The English summary page will include the title of the article, names of authors and names of their organization. The summary, 800 words in length, has to emphasize the conclusions of thearticle.
