APPLICATION PROCESS Nursing applications will be available on the Allied Health webpage: http://www.gavilan.edu/academic/ah/index.php starting November 15, 2019.

A complete LVN application should include the following:

1. Complete application with signature 2. Current BLS Provider card from the American Heart Association 3. High School diploma or equivalent. Requirement can be fulfilled by submitting official

final high school transcript, official GED certificate or college/university transcript with posted awarded degree.

4. Official college/university transcripts. Submit all official sealed transcripts for colleges/universities where prerequisites were completed and Gavilan transcripts. Foreign high school, college/university transcripts must be evaluated by: International Education Research Foundation, Inc. Credentials Evaluation Service P.O. 5. myDegreeWorks Report (Degree audit system) is located on MYGAV Student Portal. Applicants must meet with Nursing Counselor, Jessica Weiler, to have transcripts

evaluated for course equivalency of nursing prerequisites and general education courses on MyDegreeWorks. Bring course descriptions and official transcripts from all colleges attended to the appointment unless they are already on file at Gavilan. This report must be printed out and included with your application.

**If you have completed ALL courses at Gavilan College, you do not need to meet with Jessica however, you must include your myDegreeWorks report.

Proposed LVN Program Courses beginning Spring 2021 Completion of AH 71-73 will grant eligibility to take the NCLEX-PN licensing exam.

DEADLINES Students are expected to meet the requirements and deadlines when applying to the Gavilan College LVN Program. November 15, 2019 Applications available online at

January 6, 2020 Deadline to submit the Transcript/Record Request Form for Nursing Students to the Gavilan Admission Office

July 1, 2020 Acceptance letters and letters of regret mailed (approx.) July 30 , 2020 Deadline for accepted students to inform the Allied Health Department

in writing of intent to enroll in the nursing program December 2020 Up until January 29, 2021 Alternates notified of any vacancies SELECTION OF ELIGIBLE APPLICANTS All applications must be complete in order to be accepted into the program. Incomplete applications will not be processed for admission. A complete application includes all supporting documentation.

1. Minimum grade of “C” or 75% average in all courses. “C-” is not accepted. 2. Random selection will be applied if applicant pool is greater than space in the program allows. 3. 24 students are selected for admission. Alternates are identified and will be called as space

becomes available. No students will be admitted after the first week of classes (January 29, 2021) 4. If you are not admitted, you must reapply for the following year. There is no wait list. 5. Note: Although our admission requirement to the LVN Program is a C in each course, please

note that application to the RN Program is a competitive process. If you are intending to apply to the RN Program, your GPA in coursework will be used for admission to the RN Program. Your goal should be to strive for the highest GPA in each course that you take so that you qualify for admission into the RN program!

General Education Requirements for the Associate Degree – Nursing Additional courses are required for transfer. Please see a Nursing Counselor, Jessica Weiler.

Area A – English Language, Communication and Critical Thinking Complete each of the following:
