Because the recordkeeping practices for the stormwater permit are not conducive to utilizing the Departments Annual Report Form, the required components of the Annual Report are enclosed as separate items (Annual Compliance Evaluation, DMR's. Corrective Action to Issues Identified during the Annual Compliance Inspection 1. Comments: The weeps on the three Main Power Transformers (Phases A, B, and C) and the Spare Transformer will be addressed by having the fittings tightened in an effort to reduce or eliminate the weeps. Additionally, the drip pans beneath the fittings on the transformers are being modified to ~nclude oil-sorbing spill pillows and metal cages to keep spill pillows in place during high winds. The stained soil adjacent to the Spare Transformer will be excavated and properly disposed. As previously mentioned in Item No. 2, the other noted items have been, or are scheduled to be remedied in the near future.
Flint Creek Power Plant Corrective Action Plan for Stormwater Outfall SO01
(Drainage from northern portion of plant)
On October 12,20 12, a stormwater sample was collected from stormwater Outfall S001. The sample was analyzed for COD (chemical oxygen demand), TSS (Total Suspended Solids), O&G (Oil & Grease), and pH, and Total Iron. The Total lron parameter exceeded the benchmark value.
Total Iron Analytical result: 1.23 mg/L. Total lron Benchmark parameter: 1.0 mg/L.
The ADEQ Industrial Stormwater Permit (Permit No. ARR000000) requires that a facility experiencing an exceedance of a benchmark parameter comrncnce the following activities within 30 days of the exceedance:
1. Investigate the cause and/or source of the elevated pollutants, 2. Review the SWPPP and make updates if necessary, 3. Determine and document a corrective action plan to address the benchmark exceedance.
Stormwater Outfall SOOI: the source of stormwater flow to Outfall SO01 is a combination of industrial activities and structures, officc buildings, parking areas. laydown areas, rail spur, and vegetated areas, covering an area of approximately 16 acres. Stormwater accumulates in the drainage ditches in these areas until it is manually drained by opening the gate valves on the drainage ditches, allowing flow to the metal culvert which drains into SWEPCO Lake.
Probable cause and/or source of elevated pollutants: beginning in 201 1, Total Iron was monitored at all stormwater outfalls at the plant (not required by previous ADEQ stormwater permits). The three prior sample results for Total lron at Outfall SO01 are as follows:

Each of the previous samples was below the Total lron benchmark of I .O mg/L. Upon receipt of the laboratory report showing that the benchmark parameter for Total lron collected on October 12,201 2, at Outfall SO01 had been exceeded. a visual reconnaissance ofthe area drained by the outfall was made. No structures, activities, rakb materials, or othcr potential sources of elevated Total lron were observed. In fact, no changes in activities, structures, or ground cover have been made to the drainage area of Outfall SO0 I since Total Iron monitoring commenced in April 201 1 .
Corrective Action Plan:
Review of SWPPP: a review of the SWPPP did not identify the need for changes with respect to management of the area drained by Outfall SO0 1.
Corrective Actions: due to the results of the visual rcconnaissance and review of previous analytical results, there are no corrective actions planned. However, an action plan to begin an investigation to determine if this benchmark excursion was an anomaly, or if background concentrations of iron in thc native soils may be contributing to the periodic elevated iron concentrations in stormwater samples has been put in place.
In order to conduct this investigation, the following activities are planned: 1 . Conduct repeat stormwater monitoring for Total lron at Outfall SO01 in order to determine
whether an anomaly occurred on the October 12.20 12 monitoring event. If Total lron
concentrations below the benchmark value of 1.0 mg/L are present in subsequent samples, then evidence of an anomaly will have been found.
2. Conduct background stormwater sampling at one or more sites which consist of stormwater flow not affected or influenced by plant operations or activities. Two such locations have been identified (see attached map). These samples will be conducted concurrently with the repeat stormwater monitoring at SO01 when possible.
3. Results of the above monitoring will guide subsequent actions.
Implementation Schedule: the repeat and background stormwater monitoring will commence immediately, and will be dictated by the availability of qualifying stormwater sampling events.
