This addendum is issued to explain, modify or correct the original Request for Proposals document. This addendum and any subsequent addenda are hereby made a part of the Request for Proposals document and related specifications. In the event of a conflict, the most recent document will govern. Please acknowledge the receipt of all addenda when submitting your Proposal. Addenda should be inserted under Tab 8 of your proposal.
1. HISD RFP in MS Word format.
2. Due date extension until 5/26/2015 3pm CST. Please note that 5/25 is a holiday and HISD will be not be receiving deliveries.
3. Sample contract.
4. Contract Terms – HISD would like to sign a 3 year contract with two 1 year renewals.

- (Vanessa McGinnis) MWBE Requirements – office of business assistance MWBE Section of RFP-explained the MWBE requirement, three ways to participate (1) certified MWBE with COH, Houston Minority Supplier Development Council, Women Business Enterprise Alliance (2) Subcontract with certified MWBE, must be certified with one of the MWBE listed above and meet district goal% (3) Good faith efforts - sub options, just get 100 pts.- one is get 10 points at pre-bid. Other options to subcontract at different percentages. See attachment B MWBE for details.

- Jenn Roberts will discuss scope of work – 4 elements (1) Turnkey wellness provider – general wellness services – partner for a comprehensive program that delivers engagement for all risk levels and return on investment. Some provide coaching and custom care mgmt., that’s broken into discreet sections, option for vendor to check each questionnaire based on what is being quoted. (2) Custom care management provider- to improve chronic care management and improve outcomes, by proactively outreaching and managing some of the highest risk and cost patients. Would like team to have specialist in maternity, cancer care and complex

cases, with integrated teams. (3) Onsite clinic management (HISD Only) one on site at Hattie Mae White, other is at Attucks, want to look at what onsite services are available. Currently clinic format, with schedule listed in RFP, open to different provider schedules and services, goal is to maximize utilization at clinics, want to bring up numbers, improve reporting capabilities. Part of this section of the RFP is to explore the addition of an on-site pharmacy. (4) Direct contracting – partner with near-site clinics for population, offer $50 copay for Select plan members, open to different opportunities.
- Brandi Clancy– Katy and Aldine ISD participating through interlocal arrangement, independent evaluation committee- could choose different vendors and select different program designs. For pricing please price on stand-alone basis as well as combined, and clearly note any caveats to pricing.
Questions about scope:1) Q: Will there be an opportunity to have HISD required forms in Word format – vendors indicated
this would be helpful? A: See above
2) Q: Is clinic or physician office one in the same? (regarding direct contracting provider eligibility)A: Yes
3) Q: Clinic side, now HISD uses 2 clinics via Concentra – any other driving factors to be made aware that’s driving the RFP?A: Looking to explore additional services, and addition of on-site pharmacies, looking at expanding services
4) Q: Is HISD satisfied with current level of service?A: We are always looking for ways to improve current services to make it a better experience for employees that are accessing services
5) Q: You don’t have a pharmacy now, want to expand?A. Looking at doing pre-packs or Class D pharmacy, not full scope pharmacy, looking to
potentially expandQ: Are you happy with current staff, keeping current staff (on site clinic)?A: Open to different types of staffing models, exploration of evaluating space and staff accordingly, not limited to current arrangements, example storefront for health and wellness open to suggestions and modeling, unclear on retention of current employees
6) Q: Are there any utilization reports available that can be provided?A: Listed in RFP, below our goal, would like to see mid 20’s, could see 25-30 visits per day, not meeting goal, visits are 30 minutes long, want a holistic approach open to models
7) Q: What are the expectations of data sharing? Between all vendors?A: Expanded reporting capabilities in scope – important to have cross referrals between different programs, wellness vendor, EAP, onsite clinics, medical carrier, very important to share data; includes sharing with data warehouse as well
8) Q: What are your preferred staffing models for onsite clinics?A: We are open to suggestions, currently acute care model but open to primary care and we are interested in on-site coaching, if available
9) Q: Wellness coaching, DM, are you looking to expand staff at each center or share staff?

A: We are open to coaching including but not limited to telephonic, telemedicine pieces, open to suggestions of best practice in this space with hours
10) Q: Is Katy and Aldine looking to add on-site clinics as well?A: This is not an option now, but may include Katy and AISD in direct contracting
11) Q: The other two districts do they have Aetna Select productA: No they do not have Select, HISD only
12) Q: As far as physical sites are they adequate to meet the need?A: Yes there is adequate room space, lab space to manage 20-30 visits per day, again looking for best practices open to new and innovative in terms of staffing models
13) Q: Is onsite clinic equipment owned by HISD?A: HISD owns property and majority of equipment and furniture, again share best practices and innovative ideas, there are questions in the RFP
14) Q: Direct contracting – Katy and AISD will there be an opt-in provision?A: All will be evaluated on a per district basis
15) Q: Current model with Concentra is it a cost plus model?A: Current clinic is a mixed arrangement – currently going direct on lab services, looking to make changes for efficiency
16) Q: Current provider for case management Is that Aetna?A: Yes
17) Q: Case management is team open to new model as carve out on ASO?A: Yes, open to various vendor options, select option that you want to quote open to mixed model.
18) Q: Any thought to expansion of additional sites?A: Not at capacity now, may look at once capacity is exceeded
19) Q: Who has access to the on-site clinics currently?A: Employees and spouses, no retirees or contractors
20) Q: Per contract term length 3 years, 2-1year renewals?A: Five year guaranteed arrangement preferred, but will accept 3 year, with (2) 1-year options to renew
21) Q: Biometric screenings listed as optional, clarify if this would be included?A: This is a point of consideration, have done in the past, looking at different models to collect that data, could be on site, lab vouchers, physicians visit faxing in forms
22) Q: Is there a benefit design incentive to use health centers to increase utilization?A: Yes, there is an incentive for direct contract for lower price point, both for onsite and direct contract, as well as wellness incentives, interested in using incentives for active engagement
23) Q: Can we get a copy of Districts standard agreement for services, and will we be able to redline?A: Will be provided as an addendum
24) Q: Any discussion on Cadillac tax?A: Yes, always a point of discussion
25) Q: Concierge advocacy – what type of services are you looking for?A: Included because some wellness vendors bundle with services, want to know if included in base pricing
26) Q: MWBE – as health center are you hiring staff, representing staff of HISD, if staff is minority, is that part of qualifying criteria?A: Run that through Vanessa might be yes
27) Q: Wondering if HISD would consider extension to due date?A: Stephen yes looking to extend, open to pushing out a week
28) Q: On MWB if not subcontracting anything not possible to reach point total?A: Take off line with Vanessa, feel free to email her that question

29) Q: Regarding wellness program, is there information on what they are doing to drive engagement?A: Information for each district is in attachments, as well as online through the district-specific benefits websites noted in the RFP would like to get goal of 70% engaged in wellness program, districts have strong incentives
30) Q: Will there be a metric to evaluate savings opportunities and not strictly cost?A: Yes, this is part of the underlying scoring methodology
31) Q: Exceptions relate to T&C or proposal tech?A: T&C exceptions form is specific to HISD, ProposalTech has boxes to input an explanation as to why you may have exceptions to specific questions, not included in exceptions form on pg. 45
- Stephen – how to win the business from HISD. Scoring criteria listed in the RFP. Paperwork – conflict of interest questionnaire (pg. 38-39) needs to be notarized. Exceptions to T& C need to be listed out on pg. 45.
All other terms, conditions, and requirements of the original RFP remain the same at this time.
