A: The most common is the coronary angioplasty. This is the bread-and-butter procedure interventional cardiologists perform due to the prevalence of coronary artery disease. With advancing, new and innovative technology in PCI (Percutaneous Coronary Intervention), more heart patients are expected to be treated using catheter-based, non-surgical treatment modalities. Q: Most people have heard of angioplasty, angiogram and stents. What are some of the newer or more cutting-edge procedures to be aware of? How do they differ in terms of risks, recovery time, long-term viability and other factors? A: Coronary angioplasty using the transradial approach (targeting the artery at the wrist), as opposed to transfemoral approach (targeting the artery in the leg), includes less bleed- ing complications, shorter recovery time, earlier mobility after angioplasty and improved patient comfort. Transradial PCI has become my default practice for several years. My patients prefer this method as it is more comfortable and safer. The latest in coronary stent technol- ogy is the “disappearing” stent. These Bioresorbable Scaffolds (BRS) work the same way as current coronary stents in the market. But unlike the current stents, the BRS dissolve in the blood- stream after a few years. Almost noth- ing is left behind in the newly opened blocked artery. The heart condition Chronic Total Occlusions (CTOs) used to be predom- inantly treated with open heart bypass surgery. But with the advent of new wires and techniques, CTOs are increasingly replaced by PCI. However, even for experienced doctors, CTO intervention can take many hours to complete. Dr Julian Tan, an interventional cardi- ologist practising at The Heart Special- ist Clinic at Mount Elizabeth Medical Centre, answers commonly asked questions on medical procedures available to address heart disease. Q: What do interventional cardiologists do? A: Interventional cardiology is a subspeciality of cardiology. Interven- tional cardiologists are like “plumbers” who address heart diseases or unclog the heart’s blocked blood vessel(s) with non-surgical or minimally invasive techniques. An example is the coronary angio- plasty. To unblock the heart’s clogged blood vessel(s), stents and “balloons” are inserted through tubes called catheters via small arteries in the leg or wrist to reach the heart. Q: What are some other fields of cardiology? A: Other fields include cardiac imaging by doctors dubbed as “photographers” and electrophysiology and rhythm by those dubbed as “electricians”. Q: What is the most common interventional cardiology procedure performed in Singapore? A: The most common is the coronary angioplasty. This is the bread-and-butter procedure interventional cardiologists perform due to the prevalence of coronary artery disease.

With advancing, new and innovative technology in PCI (Percutaneous Coronary Intervention), more heart patients are expected to be treated using catheter-based, non-surgical treatment modalities.

Q: Most people have heard of angioplasty, angiogram and stents. What are some of the newer or more cutting-edge procedures to be aware of? A: Coronary angioplasty using the transradial approach (targeting the artery at the wrist), as opposed to transfemoral approach (targeting the artery in the leg), includes less bleed-ing complications, shorter recovery

time, earlier mobility after angioplasty and improved patient comfort.

Transradial PCI has become my default practice for several years. My patients prefer this method as it is more comfortable and safer.

The latest in coronary stent technol-ogy is the “disappearing” stent. These Bioresorbable Scaffolds (BRS) work the same way as current coronary stents in the market. But unlike the current stents, the BRS dissolve in the blood-

stream after a few years. Almost noth-ing is left behind in the newly opened blocked artery.

The heart condition Chronic Total Occlusions (CTOs) used to be predom-inantly treated with open heart bypass surgery. But with the advent of new wires and techniques, CTOs are increasingly replaced by PCI. However, even for experienced doctors, CTO intervention can take many hours to complete.

Dr Julian Tan, an interventional cardi-ologist practising at The Heart Special-ist Clinic at Mount Elizabeth Medical Centre, answers commonly asked questions on medical procedures available to address heart disease.
