1 The Care Act: Continuity of Care Julie Sorensen Social Care Group June 2014 Slide 2 2 Objectives & Conditions Objective: People with care and support needs who wish to move from one local authority area to another, will be able to do so with confidence that there will be no gap in their care and support when they move Continuity of Care is for people who already receive care and support and who are moving to a new local authority area to become ordinarily resident there. Continuity of Care is also available for carers who already receive support and are moving together with the person they care for, to a new local authority area to become ordinarily resident there. University of Hull: Messages to Care Managers: Ensure that relocation referrals are allocated quickly Work closely with the other local authority throughout the relocation process Maintain close contact and communication with the person moving preparing, planning, reassessing, monitoring and responding to changes and challenges and keeping them up to date Share information, care and support plans, assessments with the other authority as soon as possible Consider interim plans to meet needs Make sure people moving have all the information they need, including information about the implications of changed financial circumstances Slide 4 4 Principal Underpinnings of Continuity of Care The process should be person-centred: The person moving should be kept involved in the preparations for the move. Local authorities should work together to support the process: Information should be shared in a timely manner. Interim care should be personalised: local authorities should take account of the persons needs, outcomes, preferences and relevant differences between their old and new home. Slide 5 5 What does Continuity of Care look like? 2nd local authority provides the person with information 1st local authority enquires on behalf of the person Person contacts 1 st or 2nd local authority: Intention to move Preparing for the move: 1 st authority submits relevant information Ideally, an assessment is carried out before the move. Care and support is in place before the move 2 nd local authority confirms the intention to move 2 nd authority takes over care provision from the moment the person is in their new home. 2 nd authority involves the person in discussions about their interim care plan: Outcomes, preferences & views If an assessment cant be carried out before the move: Slide 6 6 What should local authorities should consider when making an interim care plan: Suitability of the new accommodation Equipment on loan and adaptations made to the old/new home Proximity and access to services and facilities Proximity and access to other types of support (friends, family and wider community) Difference in universal services provision Change in support from carers Slide 7 7 Consultation Question What further circumstances should be considered when people with care and support needs and their carers want to move?
