WHITEHEADAct iv ity o f phytoes trogens in w om enIn pre-menopausal women, controlled studies have shown that phyto-estrogen-rich diets (45 mg/day) modify the hormonal status and exertsignificant physiological effects on the regulation of the menstrual cycle(Cassidy et al, 1994, 1995). In post-menopausal women, gonadotrophinlevels are reduced (Wilcox et al, 1990), and oestrogen-like changes invaginal cytology occur (Wilcox et al, 1990) following administration ofphytoestrogens.Activi ty o f phytoestrogens in animal modelsThis work has largely been carried out by the group at Bowman Gray Schoolof Medicine, North Carolina, USA. The animal model is the cynomolgusmacaque monkey fed a high cholesterol diet. Various studies have beenundertaken (Anthony et al, 1996; Clarkson et al, 1996; Honore et al, 1995,1996) which have involved feeding young macaques casein (as a control),or feeding them soy with the phytoestrogen component intact (soy+), oralternatively feeding them soy with the phytoestrogen component extractedby ethanol (soy-) .Soy+ was associated with an approximate 28% reduction in total plasmacholesterol, a 30% increase in HDL and a 30% reduction in LDL and verylow density lipoprotein-cholesterol. Soy+ exerted a significantly greatereffect than conjugated equine oestrogens in the elevation of HDL-C.Additionally, soy+ decreased iliac artery atherosclerosis by about 80%compared with soy- and casein. Finally, unlike conjugated equineoestrogens, soy+ enhanced the dilator responses of atherosclerotic coronaryarteries to acetylcholine.No increase in uterine weight was observed during treatment with soy+.

S U MM A R YProbable developments in HRT and non-HRT treatments for menopausaland post-menopausal problems have been reviewed.More information is required on potential benefits and side-effects ofHRT. The major potential benefit is prevention against stroke ameliorationof Alzheimer 's disease: the major potential side-effect discussed in thischapter is ovarian cancer.At present, techniques for delivering oestrogens are more varied andadvanced than those for progestogens. Non-oral delivery systems forprogestogens which minimize side-effects will be introduced during thenext decade. It is not clear whether benefits expected with newprogestational agents will be realized.Preliminary data suggest that SERMs and phytoestrogens are worthy offurther evaluation. Their development will provoke intense interest over thenext 10 years.
