1 Warnings, Cautions, and Notes
Warnings
A Warning indicates the possibility of injury to you or theoperator of the device.
• This manual serves as a reference. The instructions inthis manual are not intended to supersede theinstructions of your care giver.
• You should read and understand this entire manualbefore using the Synchrony.
• The Synchrony is not intended to provide your totalventilatory requirement
• The prescription must only be adjusted by a trainedhome care provider.
• Use only the breathing circuit provided by your homecare provider.
• When using a breathing circuit that contains a mask withan integrated exhalation port or a circuit with a separateexhalation device, do not tape, seal, or otherwise blockthe vent openings. Doing so could result in suffocation.
• Oxygen supports combustion. Oxygen should not beused while smoking or in the presence of an open flame.
• Do not use the Synchrony in the presence of flammableliquids or gases. Do not clean the Synchrony withflammable fluids.
• If oxygen is used with the Synchrony, the oxygen flowmust be turned off when the Synchrony is not in use.
• Do not use the device in ambient temperatures above95 °F. If the device is used at ambient temperaturesabove 95 °F, the temperature of the breathing air mayexceed 106 °F, which could cause thermal irritation orinjury to your airway.
• Do not operate the Synchrony in direct sunlight or near aheating appliance because these conditions can increasethe temperature of the air coming out of the Synchrony.
• When the Synchrony is used with a humidifier, positionthe humidifier such that the water level in the humidifieris lower than you, and the humidifier is on the samelevel or lower than the Synchrony.
• Repairs and adjustments must be performed byRespironics-authorized service personnel ONLY.Unauthorized service could cause injury, invalidate thewarranty, or result in costly damage.
• Periodically inspect electrical cords for damage or signsof wear.
• To avoid electrical shock, unplug the Synchrony beforecleaning it.

Caution: Federal law restricts this device to sale by or onthe order of a physician.
• The Synchrony may only be operated at temperaturesbetween 41 °F and 95 °F.
• A properly installed, undamaged reusable foam inletfilter is required for proper operation.
• Do not immerse the Synchrony or allow any liquid toenter the case or the inlet filter.
• Condensation may damage the Synchrony. Always allowthe Synchrony to reach room temperature before use.
Warnings continued• Immediately report any unusual chest discomfort,
shortness of breath, or severe headache toyour physician.
• If you detect any unexplained changes in the perfor-mance of the Synchrony, if the Synchrony is dropped ormishandled, if water is spilled into the enclosure, or ifthe enclosure is broken, seek the assistance of yourhome care provider.

Intended Use
The Synchrony is intended to provide non-invasiveventilation in adult patients (>30 kg) for the treatment ofrespiratory insufficiency (a condition in which the patientcan maintain ventilation without mechanical support forsome period of time) or obstructive sleep apnea. Thisdevice may be used in the hospital or home.
The Synchrony is intended for use with nasal masks andfull-face masks as recommended by Respironics.
Contraindications
The Synchrony should not be used if you have severerespiratory failure without a spontaneous respiratory drive.
If any of the following conditions apply to you, consultyour physician before using the Synchrony ventilator:
• inability to maintain a patent airway or adequately clearsecretions.
• at risk for aspiration of gastric contents.
• diagnosed with acute sinusitis or otitis media.
• allergic or hypersensitive to the mask materials wherethe risk from allergic reaction outweighs the benefit ofventilatory assistance.
• epistaxis, causing pulmonary aspiration of blood.
• hypotension.
Precautions
Immediately report any unusual chest discomfort, short-ness of breath, or severe headace to your physician.
If skin irritation or breakdown develops from the mask,refer to the mask instructions for appropriate action.
The following are potential side effects of noninvasivepositive pressure therapy:
• ear discomfort
• conjunctivitis
• skin abrasions due to noninvasive interfaces
• gastric distention (aerophagia)

TYPES OF SERVICE
The Synchrony optional internal modem is designed to beused on standard device telephone lines. It connects to thetelephone line by means of a standard jack called theUSOC RJ-11C (or USOC FJ45S). Connection to tele-phone-company-provided coin service (central officeimplemented systems) is prohibited. Connection to partylines service is subject to state tariffs.
TELEPHONE COMPANY PROCEDURES
The goal of the telephone company is to provide you withthe best service it can. In order to do this, it may occasion-ally be necessary for them to make changes in theirequipment, operations or procedures. If these changesmight affect your service or the operation of your equip-ment, the telephone company will give you notice, inwriting, to allow you to make any changes necessary tomaintain uninterrupted service. In certain circumstances, itmay be necessary for the telephone company to requestinformation from you concerning the equipment that youhave connected to your telephone line. Upon request of thetelephone company, provide the FCC registration numberand the ringer equivalence number (REN); both of these
items are listed on the equipment label. The sum of all theRENs on your telephone lines should be less than five inorder to assure proper service from the telephone com-pany. In some cases, a sum of five may not be useable on agiven line.
I F PROBLEMS ARISE
If any of your telephone equipment is not operatingproperly, you should immediately remove it from yourtelephone line, as it may cause harm to the telephonenetwork. If the telephone company notes a problem, theymay temporarily discontinue service. When practical, theywill notify you in advance of the disconnection. If advancenotice is not feasible, you will be notified as soon aspossible. When you are notified, you will be given theopportunity to correct the problem and informed of yourright to file a complaint with the FCC. Contact yourtelephone company if you have questions about yourphone line. In the event repairs are ever needed on theSynchrony, they should be performed by Respironics, Inc.or an authorized representative of Respironics, Inc. Forinformation contact Respironics at the location shown onpage 13.

Industry Canada Notice
NOTICE: The Industry Canada Label identifies certifiedequipment. This certification means that the equipmentmeets telecommunications network protective, operational,and safety requirements as prescribed in the appropirateTerminal Equipment Technical Requirements documents.The Department does not guarantee the equipment willoperate to the user’s satisfaction.
Before installing this equipment, users should ensure thatit is permissible to be connected to the facilities of thelocal telecommunications company. The equipment mustalso be installed using an acceptable method of connec-tion. The customer should be aware that compliance withthe above conditions may not prevent degradation ofservice in some situations.
Repairs to certified equipment should be coordinated by arepresentative designated by the supplier. Any repairs oralterations made by the user to this equipment, or equip-ment malfunctions, may give the telecommunicationscompany cause to request the user to disconnect theequipment.
Users should ensure for their own protection that theelectrical ground connections of the power utility, tele-phone lines and internal metallic water pipe system, ifpresent, are connected together. The precaution may beparticularly important in rural areas.
CAUTION: Users should not attempt to make suchconnections themselves, but should contact the appropriateelectric inspection authority, or electrician, as appropriate.
Ringer Equivalence Number (REN): The REN assignedto each terminal device provides an indication of themaximum number of terminals allowed to be connected toa telephone interface. The termination on an interface mayconsist of any combination of devices subject only to therequirement that the sum of the Ringer EquivalenceNumbers of all the devices does not exceed 5.

Definitions
Apnea—A condition marked by the cessation ofspontaneous breathing.
CPAP—Continuous Positive Airway Pressure. TheSynchrony produces a single pressure level.
EPAP—Expiratory Positive Airway Pressure
High Priority Alarm —Alarm signal requiring animmediate response.
IPAP—Inspiratory Positive Airway Pressure
Low Priority Alarm —Alarm signal requiring operatorawareness.
Medium Priority Alarm— Alarm signal requiring aprompt response.
Low Minute Ventilation —A condition in which you arenot receiving a specified volume of air on a per minutebasis.
OSA—Obstructive Sleep Apnea.
Pressure Control (PC)—This is an optional bi-levelmode which responds to your inhalation by increasing thepressure. The length of the inhalation is controlled by theSynchrony. If you do not start inhaling within a set time,the Synchrony will automatically start inhalation.
Ramp—Ventilator Ramp, a feature that may increaseyour comfort when therapy is started. The Synchronyoutput starts at a low pressure (EPAP) and is increased ateach breath until IPAP is reached.
Rise Time—Rise time is the time it takes for theSynchrony to change from EPAP to IPAP. You can adjustthis time for your comfort. See Figure 2-1.
RR—Respiratory Rate
Spontaneous (S)—This is a bi-level mode whichresponds to both your inahalation and exhalation byincreasing pressure when you start to inhale anddescreasing pressure when you start to exhale. There is noautomatic delivery of a breath should you not inhale.

UPGRADESpontaneous/Timed (S/T)—This is an optional bi-levelmode which responds to both your inahalation andexhalation by increasing pressure when you start to inhaleand descreasing pressure when you start to exhale. If youdo not start inhaling within a set time, the Synchrony willautomatically start inhalation. When the Synchrony startsinhalation, it controls the time of inhalation andautomatically decreases the pressure for exhalation withina set time.
Timed (T)—This is an optional bi-level mode in whichthe Synchrony controls both inhalation and exhalationindependent of spontaneous breathing.

What is Bi-level Ventilation?
Bi-level ventilation with the Synchrony VentilatorySupport System helps you to breathe by supplying twolevels of air pressure. The Synchrony provides a higherpressure—known as IPAP (Inspiratory Positive AirwayPressure)—when you inhale, and a lower pressure—known as EPAP (Expiratory Positive Airway Pressure)—when you exhale. The higher pressure makes it easier toinhale and the lower pressure makes it easier for you toexhale.
The Synchrony can also provide a single pressure level,known as CPAP (Continuous Positive Airway Pressure).
If you require a higher level of ventilatory support, theSynchrony can be upgraded to provide modes ofventilation that reinforce your breathing with machine-triggered breaths.

What is the Synchrony?
The Synchrony, shown in Figure 2-2, supplies air pressurethrough a breathing circuit. The circuit, shown in Figure2-3, consists of:
• circuit tubing to deliver the air from the Synchrony toyour interface (e.g., mask)
• a mask or other interface to deliver the prescribedpressure to your nose or nose and mouth, depending onwhich interface has been prescribed for you
• an exhalation port to vent exhaled air from the circuit
NOTE: The exhalation port may be part of the mask ormay be part of a separate exhalation device.
The system senses your breathing effort and changespressure levels when you inhale and exhale depending onthe mode of operation.
The Synchrony can operate on AC or DC power. The DCpower option is not intended as a battery backup. WhenDC power is obtained from a vehicle battery, the Syn-chrony should not be used while the engine of the vehicleis running.

The control panel contains the controls and indicators youwill need when you are using the Synchrony:
• AC power indicator• DC power indicator• Standby key• Control keys (Down, Up, Enter)• Ramp/Alarm Silence key• Yellow LED Alarm indicator• Red LED Alarm indicator• Display screen
The AC and DC power indicators light when theSynchrony is connected to AC or DC power.
The Standby key starts and stops the Synchrony.
The Control keys allow you to display information and setcontrols.
The Ramp/Alarm Silence key starts the ventilation ramp(see page 24) and silences an audible alarm for oneminute.

Figure 3-3 shows where the circuit tubing connects tothe Synchrony.
Rear Panel
Figure 3-4 shows the rear panel.
The DC inlet is where the DC plug is connected if you arerunning the Synchrony from a battery source.
The communications port is used by the home careprovider.
The DC cord retainer holds the DC cord. Used to keepthe plug from coming out of the Synchrony.
The telephone jack is where the telephone line is pluggedin (if you have the optional modem) when you are asked tosend the home care provider data from the Synchrony.
The AC inlet is where the AC receptacle is connected ifyou are running the Synchrony from house voltage.
The air filter cover retains the air filters and helps toensure quiet operation.

CAUTION: A properly installed, undamaged foamfilter is required for proper operation.
The Synchrony uses a gray foam filter that iswashable and reusable, and an optional white ultra-finefilter that is disposable. One filter of each kind is suppliedwith the Synchrony.
If your home care provider did not install the inlet airfilters, you must install at least the gray foam filter beforeusing the Synchrony.
STEP 1 Place the foam filter on top of theultra-fine filter (if using the ultra-fine filter).
STEP 2 Push the filters down into the air inlet recess atthe rear of the Synchrony as shown in Figure 4-1.
STEP 3 Place the bottom of the air filter cover into thebottom of the air inlet opening, making sure thatthe catches engage the lip of the opening.
STEP 4 As shown in Figure 4-2, swing the top of thecover into place and press down on it to engagethe cover catches in the air inlet opening. See Chapter 7 to clean or replace the filters.

STEP 1 Connect one end of the circuit tubing to theoutlet of the bacteria filter (if using one) andconnect the inlet of the bacteria filter to thelarge connector on the Synchrony as shown inFigure 4-3.
If you are not using a bacteria filter, connect theend of the circuit tubing directly to the outletconnector on the Synchrony.

Where to Place the Synchrony
Place the Synchrony on its base somewhere within easyreach of where you will be using it. Make sure that the airinlet on the rear of the unit is not blocked. Place the uniton a hard, flat surface. If you block the air flow around theunit, the unit may not work properly.
If you are using a humidifier, be sure to place thehumidifier and unit on a placemat or other waterproofmaterial to protect your furniture from moisture. Positionthe humidifier such that the water level in the humidifier islower than you, and the humidifier is on the same level orlower than the ventilator. See the humidifier instructionsfor the complete setup.

STEP 2 Connect the open end of the circuit tubing to theexhalation port as shown in Figure 4-4. If youare using a mask that has the exhalation portbuilt into the mask, connect the tubing directlyto the mask and skip to Step 4.
STEP 3 Connect the exhalation port to the maskconnector as shown in Figure 4-5.
STEP 4 Attach the headgear to the mask—see theinstructions that came with your headgear.

STEP 1 Plug the receptacle end of the AC cord into theAC inlet of the Synchrony as shown in Figure4-7.

Starting the Synchrony
STEP 1 Press the STANDBY ( ) key. The Synchronywill sound two beeps, briefly light the alarmindicators, and display the startup screenfollowed by the self-test screen. This is theinternal test performed by the Synchrony.
STEP 2 Put on your mask assembly when the air startsto flow.
NOTE: If the Synchrony does not sound the two beeps orilluminate the alarm indicators, do not try to usethe Synchrony. Contact your home care provider.
STEP 3 Make sure that no air is leaking from your maskinto your eyes. If it is, adjust the mask andSoftcap or headgear until air stops leaking intoyour eyes. See the instructions that came withyour mask.
STEP 4 If you are using the Synchrony while sleeping,try placing the tubing from the Synchrony overyour headboard. This may reduce tension on themask.

NOTE: A small amount of mask leak is normal andacceptable. Large mask leaks or eye irritationfrom an air leak should be corrected as soon aspossible.

Using the Optional Ramp FeatureThe Synchrony is equipped with an optional ramp featurethat your home care provider can turn on or off. The Rampfeature starts the pressure at the EPAP level. The pressureis increased at each breath until IPAP is reached.
To start the Ramp feature, follow Steps 1 to 5 above, thenpress . The Ramp indicator will be shown at the topof the display. The diagram below illustrates how the rampchanges the pressure levels:

The screen that you see normally is called the Monitoringscreen. It shows you the current pressure measurementsand breathing rate.
The CPAP mode Monitoring screen is shown inFigure 4-9. If your Synchrony is not using the optionalmodem, the screen will show “1/1” in the upper right.

5 Alarms and What to Do
This chapter describes the Synchrony alarms and what youshould do if you get an alarm.
The Synchrony provides three alarm levels: high, medium,and low priority.
High priority alarms require immediate response. Thealarm signal consists of a flashing red alarm indicator anda sound that is either continuous, beeps, or repeats apattern of three beeps followed by two beeps. The displayhas the message “ALARM” at the top of the screen.
Medium priority alarms require prompt response. Thealarm signal consists of a flashing yellow alarm indicatorand a sound that repeats a pattern of three beeps. Thedisplay has the message “WARNING” at the top ofthe screen.
Low priority alarm require your awareness. The alarmsignal consists of a continuous yellow alarm indicator anda sound that repeats a pattern of two beeps. The displayhas the message “INFO” at the top of the screen.
The alarms indicators are LEDs located on the right side ofthe front panel, as shown in Figure 5-1.

Cleaning the Synchrony
Before cleaning or performing any routine maintenance,always be sure the Synchrony is not operating anddisconnect the Synchrony from the power source.
NOTE: The following cleaning instructions are for theSynchrony only. To clean the accessories, refer toeach accessory’s instruction sheet.
CAUTION: Do not immerse the Synchrony or allowany liquid to enter the enclosure, inletfilter, or any openings.
Clean the front panel and exterior of the enclosure asneeded using a cloth dampened with water and a milddetergent. Allow the Synchrony to dry completely beforeplugging in the power cord.
Gently wash the reusable circuit tubing in a solution ofwarm water and a mild detergent. Rinse thoroughly andallow to air dry.

Inlet Filters
Cleaning or Replacing the Inlet Filters
The Synchrony has two removable filters at the air inlet.The gray foam filter is washable and reusable. Theoptional white ultra-fine filter is disposable.
NOTE: Dirty inlet filters may cause high operatingtemperatures and may affect Synchrony perfor-mance. Regularly examine the inlet filters asneeded for integrity and cleanliness.
Step 1 Verify that the Synchrony is not operating, anddisconnect the power cord from the wall outlet.
Step 2 As shown in Figure 7-1, remove the filter panelby pressing down on the top of the panel torelease the tabs, then swinging the panel outfrom the Synchrony’s body.
Step 3 Remove the filters from the enclosure as shownin Figure 7-2. The top filter is the reusable grayfoam filter. The bottom filter is the optionaldisposable white ultra-fine filter.
Step 4 Check the filters to see if they are dirty or torn.

Step 5 If needed, wash the foam filter in warm waterand a mild detergent. Rinse the filter thoroughlyto remove all detergent residue. Allow the filterto completely dry before reinstalling it. If thefoam filter is torn, replace it.
Step 6 If the ultra-fine filter is dirty or torn, replace it.
Step 7 Reinstall the filters, with the ultra-fine filter onthe bottom. Slide the filters into the air inlet atthe rear of the Synchrony and push them downinto the recess.
Step 8 Replace the filter panel.

8 Using Oxygen with theSynchrony
If your doctor has prescribed oxygen to be used with yourSynchrony, follow these instructions to connect oxygen tothe breathing circuit.
WARNING: If you are using oxygen, yourSynchrony must be equipped with theoptional oxygen valve, shown in Figure8-1. Failure to use the oxygen valve couldresult in a fire hazard. Contact your homecare provider if you require an oxygenvalve.
WARNING: Oxygen supports combustion. Oxygenshould not be used while smoking or inthe presence of an open flame.
WARNING: Turn off the oxygen supply when theSynchrony is not in use.

To add oxygen to the circuit, the oxygen source mustcomply with the local regulations for medical oxygen. Theoxygen flow into the oxygen valve cannot exceed 15 lpmand the pressure cannot exceed 50 psi.

If the optional modem has been installed in yourSynchrony, you may be asked to dial your home careprovider and have the Synchrony send information usingyour telephone line. You can send the data when themachine is starting up or during therapy.

Limited Warranty
Respironics, Inc. warrants that the BiPAP Synchrony System shall be free from defects of workmanship and materialsand will perform in accordance with the product specifications for a period of one (1) year from the date of sale byRespironics, Inc. to the dealer. If the product fails to perform in accordance with the product specifications, Respironics,Inc. will repair or replace – at its option – the defective material or part. Respironics, Inc. will pay customary freightcharges from Respironics, Inc. to the dealer location only. This warranty does not cover damage caused by accident,misuse, abuse, alteration, and other defects not related to material or workmanship.
Respironics, Inc. disclaims all liability for economic loss, loss of profits, overhead or consequential damages which maybe claimed to arise from any sale or use of this product. Some states do not allow the exclusion or limitation ofincidental or consequential damages, so the above limitation or exclusion may not apply to you.
This warranty is given in lieu of all other express warranties. In addition, any implied warranties—including anywarranty of merchantability or fitness for the particular purpose—are limited to one year. Some states do not allowlimitations on how long an implied warranty lasts, so the above limitation may not apply to you. This warranty givesyou specific legal rights, and you may also have other rights which vary from state to state.
