1. Introduction

PETRONAS Carigali (Hong Kong) Myanmar Limited (here in after referred to as PCML) is the

operator of the Yetagun gas/condensate field. The Yetagun field is located in 105 meters of

water, approximately 500 km south of Yangon. The Yetagun Gas Production field consists of an

offshore drilling (Platform A) and the production platform (Platform B),Booster Platform

(Platform C),Satellite Platform (Platform D) and FSO moored about 2 Km from the fixed

platforms. The platform A is linked to platform B via a bridge and platform C is linked to the

platform B via a bridge and Platform C is linked to the Platform D via a 1

gas (460 MMscf/day) is exported via DN 600 (24 in) offshore and onshore pipeline to Thailand.

The condensate is transferred from the Yetagun Platform to the Floating Storage and Offloading

Vessel (FSO) via the DN 150 (6 in) condensate line.

2. Scope of Supply

Provision of skill personnel for UT and NDT inspection service to perform the following at YETAGUN Platform & POC

1. Ultrasonic Thickness measurements on Piping, Pressure Vessel and Heat Exchanger.

2. To take UT readings which points are identified by General Inspector/ API Inspector/ PCML.

3. Ultrasonic Thickness measurements at high points and low points with rope access.

4. Ultrasonic inspection on welds.

5. B-scan measurement.

6. C-Scan measurement.

7. To carry out Magnetic Particle Inspection on ferromagnetic material (MPI) and/or Dye Penetrant Inspection (DPI) on miscellaneous welds or suspected areas on Vessels and Piping if require.

There shall be three options to perform the scope:-

1. NDT inspector with no rope access for UT A-Scan, B-Scan and C-Scan measurement.

2. NDT inspector with rope access, IRATA Level I,

3. NDT inspector with rope access, IRATA Level I, + IRATA Level III Supervisor (Team).

Contractor shall provide one of the above three options upon PCML request depending on work activities, as and when require.

3. Equipment

Contractor shall provide Test Equipment, Consumables and Tools required for Ultrasonic Thickness Measurement A-Scan, B-Scan & C-Scan including the following (but not limited to):

a) Test Equipment for A & B Scan thickness gauging and consumables required for the Ultrasonic Testing.

b) Test Equipment for C Scan thickness gauging and consumables required for the Ultrasonic Testing.

c) Test Probe - 0º Probes (probes diameter 5mm, 10mm and 15mm) and Angle Probe (45º, 60º & 70º).

d) High temperature (up to 180º C) 0º Probes (probes diameter 5mm & 10mm)

e) Calibration Wedges CS Step Wedge and SS Step Wedge for 0º Probe and V1 Block & V2 Block for Angle Probe and the Welding Institute, U.K. (I.O.W.) Beam profile with BS 4331, Part 3.

f) Magnetic Particle Yoke (DC).

g) Lamination Scanning.

h) Root Scanning.

i) Contractor shall provide Rope Access Gear & Associated Materials upon request.

j) Contractor shall provide consumables for DPT and MPT.

Couplant

A suitable non corrosive couplant of good (high) acoustic impedance shall be used such as water, soluble oil or glycerine.

4. Qualification Requirements for NDT Technician

The proposed personnel shall possess the following qualifications:

a) Degree, Diploma, or Certificate in Engineering, or equivalent.

b) Possess a valid certification of ASNT Level II or PCN in UT, DP and MPI and be able to operate UT B-Scan and C-Scan.

c) Possess a valid Abseiling Certificate IRATA Level I for NDT operator & IRATA Level I for Rope Access Supervisor.

5. CONTRACTOR Responsibility

1. CONTRACTOR shall attend the kick-off meeting at PCML office, YANGON.

2. CONTRACTOR shall attend Permit to Work (PTW) 3 days training at PCML office.

3. CONTRACTOR shall attend the safety briefing presentation at PCML office during office hour upon his arrival.

4. Inspectors will be called upon as and when it is necessary to fulfill the service required. In addition, the CONTRACTOR must also have the ability to supply the inspectors during

inspection will be provided prior to mobilization by PCML.

5. The CONTRACTOR shall make arrangement to mobilize and demobilize the experienced NDT certified personnel ASNT level II (Up to 5 personnel) to and from PCML mobilization/ demobilization points.

6. HSE Requirement

Compliance to Laws/Regulations/PCML Requirements

a) The CONTRACTOR personnel must have valid Offshore Safety Passport and valid Medical Certificate from PETRONAS approved AME, BOSIET (Basic Offshore Safety Induction & Emergency Training) certificate and equip with Personal Protective Equipment (PPE) to compliance with PETRONAS CARIGALI Safety Rules and Regulations.

b) The CONTRACTOR shall comply with PCML HSE Policy, Stop Work Policy, Drug and Alcohol Policy.

c) The CONTRACTOR shall comply with PCML ZeTo Rules to avoid any incident/accident at worksite.

d) The CONTRACTOR must provide Job Safety Analysis (JSA) or Job Hazard Analysis (JHA) in PCML standard form together with site specific procedure for job upon award the order.

e) Valid confined space entry certificate to perform the job inside pressure vessels.

7. Codes and Standards

1. ASME BPVC Section V - Non-destructive Examination.

2. ASME BPVC Section IX - Welding and Brazing Qualifications.

3. ANSI/ASNT CP-189: ASNT Standard for Qualification and Certification of Non-destructive Testing Personnel, (2006).

5. PTS 15.31.02 - Inspection Manual for Offshore platform & Associated Equipment.

8. Reports

UT technician shall provide the following together with technical proposal.

a) Laptop, computers, digital cameras and reporting tools for onsite reporting and data collection.

b) Daily progress report.

c) Draft in-service report together with sketched isometric drawing and photo shall be provided upon demobilization.

d) All the final reports shall be provided within two weeks after demobilization from worksite.

e) To submit report after Ultrasonic Testing, MPI and DPI.

9. Procedure and Equipment

Contractor shall provide the following together with technical proposal.

a) To provide proposed UT Equipment and probe detailed.

b) To provide NDT Test Procedures & equipment calibration procedure.

c) To provide proposed rope access gear (List of Materials) and certificates.

d) PCML will review Test Procedures and equipment information before Service Order award.

10. Provision by PCML

1. Personal Mobilization and Demobilization

PCML will provide transportation from Yangon to Yetagun Platform and Onshore Facility and vice versa. Mobilization and Demobilization point for required personnel is PCML check-in counter at Yangon International Airport, Union of Myanmar.

2. Accommodation, Laundry and Catering Services

PCML will supply accommodation, laundry and catering services for personnel at Yetagun Platform and Onshore Facility.

3. Materials, Equipment, Tools Mobilization / Demobilization

i) Technicians/ Engineers could bring handy tools equipment limited under 15kg.

ii) Equipment that is over hand carry weight limit, CONTRACTOR can deliver & collect test equipment, tools to PCML forwarder (JOS logistics, Offshore Supply Base, Loyang, Singapore and MM Logistics, Offshore Supply Base, Ranong, Thailand).

iii) Mobilization / demobilization of equipment are subjected to PCML boat schedule. Normal boat schedule is once in a month.

1 GENERAL

1.1 GENERAL PREAMBLE

1.1.1 The Prices and Rates contained in this schedule shall be deemed to be fully inclusive and cover inter-alia all of the CONTRACTOR's costs for the satisfactory performance of the CONTRACT as per scope of WORK.

1.1.2 The Prices and Rates contained hereinafter shall include all costs associated with the preparation and execution of all WORK.

1.1.3 COMPANY shall pay the CONTRACTOR for satisfactory performance of the CONTRACT in strict accordance with the Prices and Rates set forth herein. Other costs for items or services not specifically set forth herein shall not in anyway be claimable and shall not be entertained by COMPANY.

1.1.4 Unless otherwise stated in the SERVICE ORDER, COMPANY shall, subject to any disputed over the invoice, make payment within thirty (30) days of receiving the invoice. Invoices shall be submitted in duplicate, one (1) original and one (1) copy each complete with the necessary supporting documentation verified by the COMPANY Representative. Invoices shall be addressed to:

1.1.5 COMPANY shall have the right to withhold taxes on income, excess profit, royalty, and other taxes from payment due to CONTRACTOR under this CONTRACT; to the extent that such withholdings may be required by the Government of Myanmar or any relevant authorities thereof, or by the government of any other country; and payment by COMPANY to the respective governmental office of the amount of money so withheld shall relieve COMPANY from any further obligation to CONTRACTOR with respect to the amount so withheld.

1.1.6 All GOODS and/or SERVICES provided within and/or outside the territorial

boundaries of Myanmar are currently subjected to a withholding tax as per Myanmar Government taxation requirement. The rates quoted by CONTRACTOR should be inclusive of the prevailing amount of withholding tax. Accordingly COMPANY will make this Withholding Tax deduction on all invoices received for GOODS and/or SERVICES provided under this CONTRACT.

1.1.7 COMPANY will provide CONTRACTOR with a statement of any amounts withheld on behalf of the CONTRACTOR including any original receipts received for payment on behalf of the CONTRACTOR.

1.1.8 Should COMPANY provide the CONTRACTOR with any supply or service, which should have been provided by the CONTRACTOR, whether or not through the fault, omission or inability of the CONTRACTOR, and then the CONTRACTOR shall reimburse COMPANY for any and all costs and expenses of the supply or service provided by COMPANY.

1.1.9 COMPANY is not obliged to procure all elements necessary to perform the WORK from the CONTRACTOR and may obtain same from other sources. In such cases, the applicable prices and rates, or parts thereof, shall not be payable by COMPANY to the CONTRACTOR.

1.1.10 Except where it is expressly provided that an obligation shall be performed at the cost of COMPANY all WORK to be performed by CONTRACTOR under the CONTRACT shall be at the cost of the CONTRACTOR and deemed to be included in this EXHIBIT II - SCHEDULES OF COMPENSATION.

1.1.11 The estimated quantities stated in this Exhibit II are indicative only .

1.1.12 All unit rates or prices stated in EXHIBIT II - SCHEDULE OF COMPENSATION shall remain firmed and fixed throughout the duration of this CONTRACT unless where specified.

1.1.13 COMPANY cannot and will not guarantee the quantity or volume of work.

The rates are remain fixed for the duration of agreement and shall not be subject to revision, escalation, or any other form of adjustment whatsoever.

Mob/Demob days shall not be considered as either working day or standby day from YGN to Platform and vice versa.

Reporting Days shall not be

During mobilization/ demobilization, PCML shall not consider as standby day in case of CONTRACTOR cannot mobilize to worksite due to unforeseen circumstances that is caused by CONTRACTOR.
