We are pleased to announce that the University ofAlabama School of Dentistry Continuing DentalEducation Program, in joint venture with the AlabamaAcademy of General Dentistry, is offering mastershipcredit programs approved by the AGD Council onContinuing Dental Education.

The programs are in a “round-table” setting and areoffered in the disciplines of Fixed and RemovableProsthodontics, TMD/Occlusion, Operative Dentistry,Oral and Maxillofacial Surgery, Periodontics, PediatricDentistry, Orthodontics, Endodontics, Basic Sciences,Oral Medicine/Oral Diagnosis, Implants, Special CarePatients and Esthetics. These programs vary in theircontent and logistics, but all consist of adidactic/practical portion and an in-office portion.

Course Description: The BLS Healthcare ProviderCourse teaches CPR skills for helping victims of allages (including doing ventilation with a barrier device,a bag-mask device, and oxygen); use of an automatedexternal defibrillator (AED); and relief of foreign-bodyairway obstruction (FBAO).

PRIMARY PULP THERAPY IN THE 21ST CENTURY -THE CHANGES THAT COULD CHANGE EVERYTHING...

Course Description: Formocresol vital pulpotomies have served

children's dentistry well over the last 100 years. New materials

and procedures have oft attempted to "de-throne" formocresol as

the medicament of choice when performing this procedure, but

few have demonstrated its consistent success. However, exciting

new data emerging from the University of Iowa and UAB are

independently demonstrating that sodium hypochlorite may be an

excellent alternative. When the pulp is non-vital, pulpectomies

are indicated, but can be challenging to perform. Success or

failure of either procedure often hinges on proper diagnosis. This

course offers insight into diagnosis of primary pulp health,

triaging which pulp therapy is indicated, and the latest in pulp

treatment techniques that can be used in the primary dentition.

From indirect pulp caps to NaOCl pulpotomies to Vitapex

pulpectomies, this course will present the "how" and the "why"

behind the changes in primary pulp therapy available to the 21st

century children's dentist.

Course Description: This seven hour course is intended for both

dentists licensed in OCS and ancillary staff who participate in

oral conscious sedation (minimal sedation) in direct patient care.

It will satisfy the CE requirement for AL OCS recertification.

Covered in the course will be:

1. New ADA reclassification of oral conscious sedation.

2. New training guidelines for the proposed reclassification.

3. Review of medications routinely used in oral minimal sedation.

4. Review of new dosing recommendations specific to length of

patient appointments.

5. Review of requirements for monitoring and recordkeeping.

6. Brief overview of "red flag" medical conditions contraindicated

for minimal/OCS.

7. Review of management of medical emergencies specifically

seen in minimal/OCS.

8. Hands on airway management for entire dental team.

Dr. John Coke graduated in 1973 with a DDS from

Loyola University School of Dentistry in Chicago.

Immediately following, he completed a one-year

General Practice Residency at Portsmouth Naval

Hospital, Portsmouth, VA. After two years as a

Naval Dental Officer in Guantanamo Bay, Cuba, he

completed a two-year certificate program in Oral Medicine and Oral

Diagnosis from the University of Illinois - Chicago. Currently, Dr.

Coke is program director for both the AEGD and GPR post-graduate

programs at the University of Alabama School of Dentistry. He is

board certified by The American Board of Oral Medicine, a

Diplomate of the American Board of Special Care Dentistry and a

Fellow of the American Association of Hospital Dentists. Dr. Coke

has been involved in teaching sedation to general

dentists for over 25 years.

Dr. Mike Edwards graduated with a DMD degree

from the UAB School of Dentistry. He completed an

externship with the United States Public Health/

Indian Health Service in St. Ignatius, Montana.

Dr. Edwards began private practice in 1979 and has

received both his Fellowship and Mastership in The Academy of

General Dentistry. Dr. Edwards has held has numerous lectures

nationally on parenteral and enteral sedation as well as other dental

continuing lectures. He has practiced sedation dentistry for over 28

years and has been involved with developing both state and national

guidelines for the safe use of sedation. Dr. Edwards co authored the

article on sedation published in the Journal of the American Dental

Association in 2006.

Course Description: This dynamic duo, with their unique

expertise in Oral Medicine and Oral Pathology, will take you on an

entertaining journey encompassing the many facets of infectious

diseases--from medical history through final treatment. This

clinically-oriented course is designed for dentists, dental hygienists

and dental assistants to present current information regarding

infection control requirements in the dental office. Specific

pathogens and infectious diseases, modes of disease transmission,

susceptibility and risk factors unique to the busy dental office will

be presented. New emerging and re-emerging diseases and their

implications on the dental team will also be discussed.

Immunizations and other ways to reduce infection and transmission

will be emphasized. Numerous clinical examples will be used to

highlight the oral manifestations of infectious diseases.

Educational Objectives:

1. To review the scientific rationale and initiate measures for

disinfection and sterilization.

2. Use of the health questionnaire as an effective screening tool.

3. Be able to recognize the dentally relevant infectious diseases.

4. Recognize clinical manifestations of these infectious diseases.

5. Describe methods to reduce disease transmission.

Dr. John Coke graduated in 1973 with a DDS from

the Department of Diagnostic Sciences at the University of

Alabama School of Dentistry, and in the Department of

Pathology within the School of Medicine. After receiving

his D.M.D. from Southern Illinois University School of

Dental Medicine, Dr. Tilashalski completed both of his residencies along

with an American Cancer Society fellowship at the University of Alabama.

He subsequently became board certified in pathology. Dr. T maintains an

active private practice, treating both endodontic and oral pathology patients.

In addition, he is an avid researcher with his most recent work in the areas

of smokeless tobacco, smoking cessation, and endodontic treatment

outcomes.

and the functional impression techniques into a single appointment. The participants will be introduced

to the Massad/Thornton Tissue Character Mobility Test (M/T) and the clinician will explain how this

information is used to determine the correct viscosity of impression material to use to achieve the

greatest accuracy in edentulous impressioning.

v A method of denture adhesive removal from the edentulous arch tissues and how to condition the

edentulous tissues for impressioning.

v A method for evaluating soft-tissue displacement and character of edentulous jaws.

Joseph Massad, DDS is an internationally renowned clinician in the field of

prosthodontics. He has given more than 500 lectures in nearly 50 states and a dozen

countries around the globe. Named one of the top clinicians in Continuing Education

(CE) for 2001, 2002, 2003, 2004, 2005 and 2006 by Dentistry Today, Massad regularly

lectures at meetings of dental organizations such as the American Prosthodontic

Society, the American Dental Association, the Canadian Dental Association, and the

Federation Dentaire International. Massad is the creator, producer, director, and moderator of two of

the most popular teaching videos on the subject of removable prosthodontics-1997's Predictable

Complete Dentures and 2001's Helpful Hints - Predictable Complete Dentures Part 2. A graduate of the

Loyola University School of Dentistry, Massad has published scientific articles in publications such as

the International Journal of Periodontics and Restorative Dentistry, Compendium of Continuing Dental

Education, Dentistry Today, Dental Economics, and others.

Course Description: Provisional restorations for fixed prostheses are usually made in a hurry and often takemore time than the tooth preparation. Quality can easily be improved for your patients with cost and timesavingtechniques. This one day course provides current information in fabricating the provisional restoration for dentalauxiliaries. Single anterior and posterior provisional restorations utilizing prefabricated crowns and matrixtechniques will be reviewed. More advanced techniques such as multiple unit and veneer provisionalrestorations will be covered in lecture and in the technical, hands-on portion. Casts and several provisionalmaterials will be provided for hands-on practice. Cementation and removal of provisional restorations will alsobe taught.

Educational Objectives:This one-day course is designed so that, upon completion, the participant will have:1. The theoretical knowledge of provisional restorations.2. Practical experience necessary to fabricate a variety of provisional restorations.3. The ability to properly contour the provisional restorations.

Course Description: The program will cover in detail the

techniques of scaling and root planing and their overall role in

the treatment of periodontal disease. A significant portion of the

Dr. Philip Vassilopoulos is a graduate of The

University of Athens (Greece) School of

Dentistry. He received his Periodontics

Certificate from UAB in June 2003 and is

currently working on his thesis for his M.S.

degree. Dr. Vassilopoulos is presently the

program director for the undergraduate dental students, and

lectures in periodontology and implantology. He is Coursemaster

for Clinical Periodontics I and II, and Periodontology IV. Dr.

Vassilopoulos is an Assistant Professor and the Predoctoral and

Clinic Director for the Department of Periodontics and is a

Diplomate of the American Board of Periodontology.

Course Description: This course outlines the most recent

OSHA and Centers for Disease Control and Prevention

requirements for the dental office pertaining to employee

protection, record keeping and practical applications. The

American Dental Association and Alabama Department for

Environmental Management recommendations will also be

discussed. Dr. Filler will provide brief reviews of hepatitis,

tuberculosis and HIV/AIDS. The various types of hepatitis and

their significance will be covered, as well as the concern about

drug-resistant TB. This material will emphasize the diseases

behind the need for infection control rather than the

technicalities of infection control procedures.

Educational Objectives:

1. Define OSHA regulations which affect the dental office.

2. Define ADA and CDCP guidelines for infection control.

3. Define guidelines for hazardous waste disposal as required

by ADEM.

4. Know primary features of diseases of Hepatitis, HIV/AIDS

and Tuberculosis.

5. Be aware of recent disease demographic data/trends.

Ms. Nancy Tomlin is the Quality Assurance Officer at

the University of Alabama School of Dentistry, where

she also serves as the Infection Control Coordinator.

She earned her BS in Dental Hygiene from UAB in

1984. Ms. Tomlin has worked in private practice and

served as the Adult and Professional Coordinator for

the Dental Bureau at the Jefferson County Department

of Health. She has also provided numerous OSHA and Infection

Control presentations to the dental community, locally and statewide.

Dr. Steven Filler is a Fellow and former president of

the American Association of Hospital Dentists. A

graduate of Loma Linda University School of

Dentistry, he completed his postgraduate training in

hospital dentistry and holds masters degrees in oral

biology and education at the University of Alabama at

Birmingham. For many years, Dr. Steven Filler served

as the Director of Hospital Dentistry. During this time

he and his hospital dental residents were on the frontline of medically

complex patient care. Dr. Filler currently serves as Associate Dean for

Student, Alumni, and External Affairs.

CONE BEAM CT AND

USES FOR TODAY’S DENTISTS

(3 LECTURE HOURS)

Course Description: Cone Beam Volumetric

Computerized Tomography Imaging (a.k.a., Cone Beam

CT) Radiography is a relatively new imaging technique for

dentistry but is rapidly gaining a place in dental diagnosis

and treatment planning. The essence of this extraoral

technique relies on multiple skull or maxillofacial images

taken from 360 degrees of rotation about the subject

combined by relational software into numerous possible

viewing aspects. This results in images that can simulate a

full medical CT scan of the head and neck region while

greatly reducing radiation exposure concerns for the

patient. The ability to reconstruct these slice images into a

wide variety of planar and three-dimensional projections

has greatly aided the pre-surgical assessment of implant

placement and restoration. Additionally, the investigative

potential for bony and dental pathology for endodontics

and surgery has been greatly enhanced with this new

radiology technique.

This course will provide an overview of the significant

differences between Cone Beam CT and current

conventional radiologic techniques in dentistry. The

diagnostic capabilities of a practitioner who can utilize and

interpret these images should be improved. While not

focusing on the unique physics of Cone Beam radiography,

the contrasts between tomography and plane film imaging

will be highlighted. Examples of reconstruction images

including three dimensional constructs will be provided to

help outline the great detail now available in properly

produced images. Examples of discovered anomalies and

Dr. Don Karaki, Assistant Director for the

UAB Hospital Dentistry graduate residency

program and Assistant Professor in the

department of Diagnostic Sciences, joined the

UAB School of Dentistry in July 2003. He

received his D.D.S. degree in 1983 from the

University of Colorado School of Dentistry and

practiced in Fort Collins, CO for 9 years. Dr.

Karaki completed the Dental Diagnostic Sciences program for

Oral Radiology and Oral Medicine in 1995 at the University of

Texas Health Science Center-San Antonio. As a full-time

educator, Dr. Karaki also taught at the University of Oklahoma

College of Dentistry from 1995 through 1998 and then directed

the Oral Radiology division at the University of Tennessee Health

Science Center College of Dentistry from 1998 until 2003. He is a

Fellow of the American Academy of Hospital Dentists (Special

Care Dentistry) and an active member of the American Academy

for Oral and Maxillofacial Radiology.

2008 Restorative Dentistry Update

(9 Lecture Hours)

Course Overview: This clinically oriented course compares

traditional and newly developed dental restorative materials and

reports their clinical success. The course is designed to improve

your practice of dentistry, demonstrate which material works

where and will show successfully techniques to handle many of

your everyday clinical problems. It will not make solve your

staffing problems, provide instant financial success nor promote

weight loss but it will provide thoughtful insight on controversies

surrounding clinical dentistry and supply answers to many of your

questions. Understand why dental materials work rather than hear,

"It works for me". You will see the results on many clinical trials

and in vitro studies which will be presented and discussed.

Ceramics. Ceramic materials have evolved from weaker

feldspathetic porcelain to pressed to high strength zirconia cored

materials. CEREC and other ceramic materials, mechanical and

physical properties will be discussed. Indications, where to use

these materials and contraindications will be discussed.

Cements. This lecture covers all classes of luting agents from

zinc phosphate to glass ionomer and resin modified glass

ionomers to adhesive resin cements. The proper use of these

luting agents is demonstrated and their application in specific

clinical cases (veneers, all metal, post cementation, resin inlay,

high strength ceramic materials and conventional metal ceramic

restorations) is demonstrated. The proper manipulation and

application of the newest materials such as Principle, Advance,

Fuji Plus, Fuji Cem, RelyX, RelyX ARC, Panavia 21, BisCem, G

Cem and UniCem will be emphasized to ensure success with non

retentive preparations. Provisional cementation will also be

discussed.

Restoring endodontically treated teeth. When are posts

necessary and which post system should be used for anterior and

posterior teeth? Advantages and disadvantages of active and

passive posts are discussed. Using esthetic ceramic, carbon fiber

and fiber containing composite resin posts to their fullest

potential, the esthetic treatment of discolored roots will be shown.

Advantages and specific limitations of post materials are

emphasized. Core and foundation materials are compared and

advantages of each are provided. Finally the final restoration is

discussed.

Dr. John O. Burgess is a graduate of Emory

University School of Dentistry. He received his M.S.

in Biomedical Sciences from the University of Texas

Health Science Center in Houston, Texas, completed a

one year General Practice Residency and a two year

General Dentistry Residency. Currently Dr. Burgess is

the assistant Dean for Clinical research at the

University of Alabama at Birmingham. He was

Professor and Chairman of Operative Dentistry and

Biomaterials as well as the Assistant Dean for Clinical

research at Louisiana State University Health Sciences Center-School of

Dentistry. He was a military consultant in general dentistry to the Air

Force Surgeon General. He received certification from the American

Board of Dentistry and is a diplomat of the Federal Services Board of

General Dentistry. He is a member of the American Academy of Esthetic

Dentistry, The Academy of Restorative Dentistry, American Dental

Association, the American Association for Dental Research and the

Dr. John Coke graduated in 1973 with a DDS from

Loyola University School of Dentistry in Chicago.

Immediately following, he completed a one-year General

Practice Residency at Portsmouth Naval Hospital,

Portsmouth, VA. After two years as a Naval Dental

Officer in Guantanamo Bay, Cuba, he completed a two-

year certificate program in Oral Medicine and Oral

Diagnosis from the University of Illinois - Chicago. Currently, Dr. Coke is

program director for both the AEGD and GPR post-graduate programs at

the University of Alabama School of Dentistry. He is board certified by The

Dr. Ken Tilashalski (“Dr. T”) is quite unique in that he

has earned specialty certificates in both oral and

maxillofacial pathology and endodontics. He is an

Dentistry, and in the Department of Pathology within the

School of Medicine. After receiving his D.M.D. from

Southern Illinois University School of Dental Medicine,

Dr. Paul Eleazer was born and raised in Georgia. He

earned a DDS degree from the University of North

Carolina in 1970. He served in the US Public Health

Service in Alaska and Arizona. He then earned a

certificate in Endodontics and a Master in Oral Biology

at Temple University in Philadelphia. Dr. Eleazer

practiced endodontics in Albany Georgia for 21 years.

In 1996 he moved to Louisville to lead the residency

program in endodontics and eventually chair the

Department of Periodontics, Endodontics and Dental Hygiene. He moved

to UAB in the Spring of 2000.

Dr. Toni Tien Neumeier is an Associate Professor in the

Department of Comprehensive Dentistry at The University

Prosthodontics. She received her fellowship award from

the Academy of General Dentistry in 1998. Dr. Neumeier

structure caused by acid on teeth. This is a newly developed course for

practitioners and dental staff which will address current patient

conditions including enamel erosion from methamphetamine use, acid

reflux and acidic foods in the diet.

The first part of the presentation will discuss the trend of

methamphetamine consumption, signs and symptoms of recognizing

the user and the meth laboratory and damage to teeth from

methamphetamine ingestion. The second half of the lecture will focus

on restoration of the dentition destroyed by erosion and comprehensive

treatment planning for patients requiring full mouth rehabilitation.

Dr. Waldo is an Assistant Professor in the Department of

Comprehensive Dentistry and received her DMD degree

practiced in Cincinnati, Ohio for several years and

Advanced General Dentistry at the School of Dentistry

He received his Prosthodontic Certificate, MS and DMD

Degrees from the UAB School of Dentistry and is

Comprehensive Dentistry. Dr. Liu has lectured nationally

LOCATION

The School of Dentistry is located at 1919 Seventh Avenue South. Parking

is available in Deck #3 on University Boulevard. The main doors to the

Dental School are locked during the weekends and evenings after working

hours. CDE course participants should go to the back of the Dental School

where someone will be posted at the door at least 30 minutes before and

after the start of the weekend programs. A map will be included with your

Confirmation Letter sent approximately two weeks before the start of the

program.

HANDOUTS, BREAKS AND MEALS

The price of your course includes handout materials and coffee breaks,

unless otherwise noted. All-day courses include lunch, unless otherwise

noted. If you have special dietary needs, please call 205.934.3753 at least

five business days before the start of the course to inform us of your dietary

requirements.

LODGING

Along with your confirmation letter, you will receive a list of area hotels. It

is up to you to contact the hotel and make reservations. Inform the hotel you

will be participating in a Continuing Education Program at the School of

Dentistry for special rates, when available.

TOPICS OF INTEREST

If there are topics of interest which have not been previously addressed by

Continuing Dental Education, please contact Ms. Anita Wilson,

BAD WEATHER POLICY

Occasionally the Birmingham area experiences bad weather with snow and

ice on the roads making it difficult for employees and participants to get to

the School. Please check with WBHM (90.3 FM), or call 205.934.2165,

and select option 2, for a pre-recorded message about the University’s

status. If UAB is closed, all CDE courses scheduled for that weekend

will be canceled and rescheduled. The CDE office will make every effort

to update the voice mail message on 205.934.3753 and the website

(www.dental.uab.edu) with cancellation and rescheduling information.

When canceled courses are rescheduled, pre-registered participants will be

notified by mail. If a participant cannot attend on the new course date,

he/she may register for another course as seats permit or receive a tuition

refund.

PREREGISTRATION: Preregistration for all courses is necessary.

Applications for registration will be processed in the order received,

and the Office of Continuing Dental Education reserves the right to end

registration when the enrollment limit has been reached. The

registration fee must accompany the application.

REGISTRATION FEES: All registration fees include tuition, course

materials, meals and refreshments (when applicable). See individual

course descriptions for the tuition fees. The registration fee must

accompany the registration form. One check or company credit card

may cover several registrations. Please clearly state all participants’

names and the courses for which they are registering.

REGISTRATION BY MAIL, FAX OR INTERNET: You may choose any of

the following methods to register for courses:

ON-SITE CHECK-IN: Please arrive 30 minutes prior to the start of the

course to check in and pick up packets. Registration confirmations and

signs in the Dental School Lobby will list room numbers for the day’s

courses. Same day walk-in enrollments will be accommodated on a

space-available basis.

CONFIRMATION: Confirmations will be sent to participants

approximately two weeks before the program. For registrations

received less than two weeks prior to the program, confirmations will

be faxed to those participants who include a fax number on their

registration forms.

GENERAL REFUND POLICY: Tuition is completely refundable if a

course is canceled by the Director of Continuing Dental Education, or

if the registrant cancels more than 30 days prior to the course. For

cancellations made less than one month prior to the course, the

registrant may apply the entire fee to another course or receive a 50%

refund. No refund will be given for cancellations made less than 7 days

prior to a course; tuition may be applied toward a future course. “No

shows” will forfeit the full tuition. The University requires proof that

checks cleared the bank before refunding tuition paid by check.

Tuition paid by credit card will be refunded to the card charged for

the tuition.
