Message from the Head Janet P. Engle

The Department of Pharmacy Practice, one of the largest in the country, had an extremely productive year in 2010. At the end of 2010, the department embarked on an extensive strategic planning process. The process examined all aspects of the department including research, service, entrepreneurial and educational activities. Planning will continue throughout 2011 until all units within the department have completed planning exercises. Then, a clear blueprint for future directions of the department will be produced. The department developed a new mentoring program. The program, a requirement for anyone joining the department, matches new faculty members with established practitioners in their area of interest. A new faculty orientation program was developed and delivered for the first time in 2010. Six new faculty members participated in the program. The program will be offered on a regular basis to all new faculty hires in the department. In conjunction with the orientation, a faculty handbook was developed and distributed to all new faculty as well as others as requested. The handbook is also available electronically on the departmental website. The department studied its current procedures for promotion on the clinical track, forming an ad-hoc committee to study the issue. After several months of meetings, the ad-hoc committee presented their report. It was reviewed and edited by the Department Advisory Committee and the new policies became effective in November 2010. The residency and fellowship program had a very strong year. Applicant numbers and quality of candidates continue to increase. In 2010, all

available PGY1 and PGY2 specialty residency positions were filled. In addition, a third community pharmacy residency was added and filled. A leadership component to the program was added that allows residents to meet with the department head on a monthly basis to discuss various topics. Residents continue to do well, advancing to positions in academia, clinical practice and industry. Several networking events were held to foster increased interaction among faculty. These included an event held at US Cellular Field to see the Chicago White Sox play on Health Professionals Night and a Poster Networking Event where all departmental faculty displayed their scientific posters that had presented at national meetings throughout the year.

The strategic planning process includes assessing the physical facilities available to the department and making renovations as needed. In 2010, the entire residency suite was remodeled to include new work space for the residents; a state of the art

conference room with videoconferencing capabilities; and small private office spaces for interviews, conferences with students, and other

activities. Department space on the first floor was also remodeled to provide eight new offices for research personnel. Several faculty offices also were updated and new laboratory equipment was purchased. With our new campus in Rockford, efforts have been made to become acquainted with Rockford students. Each

semester the department head and assistant department head for education travel to Rockford to meet with the students and faculty to discuss topics of mutual interest. Faculty recruiting in Rockford has gone well with the addition of two new faculty members in 2010 to supplement the faculty clinicians who have been practicing in Rockford for several years. Efforts to market the seven ambulatory pharmacies also continued throughout 2010. The pharmacies have a presence on Facebook and Twitter, gaining many new followers. A blog covering health-related topics continues, supported by the Drug Information Group. New marketing pieces for campus buses, lightboxes, campus student centers and patient exam rooms were developed. Other targeted projects such as our flu vaccine campaign were also executed. It has been a busy and productive year for the department. These endeavors could not have been accomplished without the hard work of faculty, staff, residents and fellows. While not all accomplishments can be featured in this report, these highlights are just a few of the many contributions the department has made to the college, university and profession of pharmacy.

Department of Pharmacy Practice Mission Statement

The Department of Pharmacy Practice is an academic department within the University of Illinois at Chicago College of Pharmacy, Chicago and Rockford Campuses. The department is also a service unit that provides pharmacy services within the University of Illinois Medical Center and affiliated sites. Among the most comprehensive units of its type in the United States, the department is composed of faculty and staff who are teachers, scholars and/or practitioners. Members of the department continue a tradition of exemplary service through the following activities:

Educating and training Doctor of Pharmacy students, undergraduate and graduate students, post-PharmD residents and fellows, and practicing pharmacists, to become nationally and/or internationally recognized for their leadership, knowledge and competence in practice, research, and/or teaching roles.

Department Section Updates

Center for Pharmacoeconomic Research Glen Schumock, Director

In 2010, the Center for Pharmacoeconomic Research added a new faculty member, Denys Lau, PhD. Dr. Lau came from Northwestern University, with his primary faculty appointment in the Department of Pharmacy Administration. Dr. Lau is an expert in quantitative and qualitative research methodologies used to study the safety of pharmaceuticals and health services in vulnerable older adult populations. Dr. Lau’s research contributes to the impressive listing of new grants and contracts received in 2010 by the center. In total, the funding attributed to CPR faculty in 2010 was $1.5 million. There is heightened national interest in comparative effectiveness research. CPR faculty and students have been at the forefront of this movement. Efforts in this area stem from the CPR serving as the coordinating site for the Agency for Healthcare Research and Quality funded Chicago-Area DEcIDE Center. DEcIDE stands for Developing Evidence to Inform Decisions about Effectiveness. In the fall of 2010 that contract was re-competed, and CPR was one of only eight of the original DEcIDE centers to be renewed. There are now a total of 11 DEcIDE centers.

The faculty and students of the CPR continue to be actively involved in disseminating the findings of research via presentations and publications. In 2010 faculty members had a total of 39 publications in professional journals, made 56 presentations of papers/research at conferences, and were invited to deliver 29 seminar presentations at national and international meetings. Drug Information Group Mike Gabay, Director In 2010, the Drug Information Group had its most financially successful year to date. During the past year, DIG entered into new relationships with Amgen, Walgreens, Scripps Health, and Takeda Pharmaceuticals. The drug information services provided by DIG faculty continue to grow in scope as clients present unique needs. In addition, the DIG faculty completed their second year of teaching literature evaluation at Chicago State University College of Pharmacy, expanding the educational reach of UIC College of Pharmacy beyond the school itself. The DIG expanded in 2010 by hiring a new faculty member, Heather Ipema, PharmD, who completed both a PGY1 general residency and PGY2 specialty residency in drug information at UIC. Sherilyn Van Osdol, PharmD, was brought on as a new PGY2 specialty resident for the year. The Prior Authorization Group is currently in the fifth year of its contract with the Illinois Department of Healthcare and Family Services. By providing timely, clinically appropriate drug prior authorization

determinations for the state's Medicaid recipients, the Prior Authorization Group has helped the State of Illinois achieve tens of millions of dollars in savings in the last year alone. Some of the programs recently initiated to ensure appropriate drug utilization include psychotropic medications for young children, antipsychotic medication use in long-term care recipients, and stimulant medications for adult patients. Experiential Education Kristen Goliak, Assistant Head and Director, APPE Twenty-ten was a very successful year for the experiential education group. Negotiations began with Rush University Medical Center for their Department of Pharmacy to accept UIC College of Pharmacy students on both IPPE and APPE rotations. This exciting new partnership was successfully finalized in early 2011. Allison Schriever, PharmD, was welcomed as the director of experiential education for the Rockford campus. Dr. Schriever will work closely with the experiential team in Chicago and will be responsible for acquiring sites in and around the Rockford area. Dr. Schriever also coordinates the rural pharmacy program, RPHARM. The first UIC Walgreens PGY1 resident was accepted in 2010. Sheila Allen, PharmD, accepted the opportunity to be the faculty liaison to the program. In the fall of 2010, the IPPE program expanded to Rockford with the P1 Shadow Week. Thirty nine Rockford P1 students and 156 Chicago P1 students were placed with pharmacists, pharmacy residents and P4 students throughout the State of Illinois.

In November 2010, a preceptor conference was held. The conference was well attended by more than 100 external and internal faculty preceptors. There were also approximately 20 preceptors from Rockford who attended via ITV. This was the first interactive conference with Rockford and it went well. After the plenary sessions, each campus broke into small groups at their own site to address issues pertinent to their students and residents. Small break out group sessions in Chicago were held related to precepting residents that met ASHP requirements for CE for preceptors. It was a good opportunity for preceptors to experience the college’s new distance education modality.

In November Dr. Allen was appointed director of IPPE, working closely with Dr. Schriever in her roll with Rockford experiential program. Under the direction and guidance of Dr. Allen, several faculty, staff, residents and numerous P4 students administered 1,900 flu shots at the annual State of Illinois flu shot clinics and assisted with the administration of nearly 2,800 doses at University of Illinois Medical Center events.

Hospital Pharmacy Services Andy Donnelly, Director

The Departments of Hospital Pharmacy and Ambulatory Care Pharmacy play an important role in all aspects of the medication system related to the ordering, dispensing, administration and monitoring of medications at the University of Illinois Medical Center. In the past year, the department enhanced processes related to UIMC’s goals to improve patient satisfaction, patient access, and performance related to national quality measures. This was done by expanding the discharge prescription service and

decreasing length of wait times for prescriptions as well as expanding programs for patients with heart failure through hospital discharge medication instruction, adding inpatient vaccination screening program and outpatient vaccination programs for pneumonia, and addressing the need for anticoagulation by creating an inpatient oversight program and an outpatient pharmacy-run anticoagulation service. New hospital initiatives taken on include pharmacist real-time verification of all emergency department medication orders; development of processes related to ordering, verifying, dispensing and administering hazardous medications; and conversion of medication orderables to support true e-prescribing and improve accuracy of home medication lists. The ambulatory care pharmacy continued initiatives to maintain overall patient satisfaction at or above 98 percent, grow the specialty pharmacy business by 10 percent and expand the number of patients helped through the medication assistance program. Ambulatory Medicine Team Michelle Martin, Team Leader

The Ambulatory Medicine Team collaborates to host the Advanced Ambulatory Care Advanced Pharmacy Practice Experience Immunization Clerkship for fourth year students. During this rotation, students screen patients for immunizations, administer immunizations, and update patients’ medical records to reflect their current immunization status. They also maintain a database to record these interventions and work on other projects. In 2010, data that had been previously collected was analyzed and used to create four posters to showcase research. One poster was presented at American College of Clinical Pharmacy Annual Meeting in Austin, Texas in October. Three posters were presented at the

American Society of Health-System Pharmacists Midyear Clinical Meeting in Anaheim, California in December. The team plans to continue to offer this rotation to P4 students and pursue publication upon completion of a manuscript. Individual team members have been involved in multiple other publications and grant projects in 2010.

Antithrombosis Clinic Edith Nutescu, Clinical Manager Nancy Shapiro, Operations Manager The pharmacist-managed Antithrombosis Clinic is a clinical care unit part of the University of Illinois Medical Center and Heart Center. Currently, the clinic provides care to approximately 450 active patients and averages 8,000 patient visits a year. Since its inception, the clinic has received regional and national recognition by being selected as a Joint Commission Model Site for Disease Management and it its selection as a national training site for the American Society of Health Systems Pharmacists Research and Education Foundation Antithrombotic Traineeship. At the present time, eight clinical pharmacist faculty provide direct patient care and clinical services in the clinic. In addition to provision of direct patient care in the ATC, faculty is extensively involved in research, scholarship, teaching, and professional service activities. Critical Care Jeff Mucksavage, Team Leader The section of critical care clinical pharmacy uses the critical care environment to meet the mission goals of service, teaching, and scholarly activity. The group is comprised of eight members whose practice areas include medical surgical intensive care, cardiac intensive care, neuroscience intensive care, emergency medicine, toxicology, and anesthesia services. Members are at the forefront of clinical pharmacy practice in their respective areas and are integral members of the multidisciplinary critical care teams. Recently, there have been new additions to the team. In August 2010, Michael Johnson, PharmD, joined the group covering the medical surgical intensive care unit. The group continues to be deeply committed to the education of pharmacy students, pharmacy residents and other practitioners. Postgraduate pharmacy residency training is a notable strength of the group. In June 2010, the group graduated an American Society of Health-System Pharmacists accredited post-graduate year-2 resident in the areas of critical care. Scholarly activity is another key component to the critical care clinical pharmacy team. Critical care pharmacists are involved in investigator initiated trials as well as co-investigators on numerous multidisciplinary trials. Most notably, Keri Kim, PharmD, completed her master’s degree in clinical and translational science in May of 2010. Her additional skill set provides a welcomed resource for the group to assist in research design and implementation.

Family Medicine & Women’s Health Clinical Pharmacist Team Louise Parent Stevens, Team Leader The Family Medicine and Women’s Health Team provides clinical services to the Family Medicine Centers at University Village and Outpatient Care Center as well as the Women’s Health Clinic at OCC, including practitioner consults and direct patient care addressing disease state, medication management and patient education. In 2010, the team completed a project on the impact of clinical pharmacy services on the management of asthma. The team continues to participate in quality improvement projects related to patient care. With the team leader for the outpatient medicine group, the Family Medicine and Women’s Health team leader is involved in a project to evaluate the impact of the morning conference portion of the ambulatory care clerkship.

Medication Therapy Management Clinic Jessica Tilton, Manager

Medication Therapy Management Clinic is a pharmacist run clinic whose vision is to lead in medication therapy management through innovation, education and research to decrease hospitalizations, improve healthcare utilization, reduce costs and improve patients' quality of life. MTMC assists with the difficulties of accessing medications, along with simplifying the refill process to one time a month. The clinic strives to improve and sustain an acceptable medication adherence rate and prevent time lapses between refills. It ensures continuity of care with providers to improve healthcare utilization and minimize healthcare cost. MTMC sets out to prevent duplication of therapy and avoid drug interactions. It evaluates therapy to avert or minimize adverse drug events, along with assist providers in monitoring a patient’s response to therapy. Preventative measures such as immunizations and routine labs are implemented. The aim is to incorporate patient participation in self-management of medications through education. Ultimately, the clinic aspires to improve patient satisfaction with their healthcare as it relates to their drug therapy. Today MTMC is housed in the Outpatient Care Center Pharmacy where there are two private patient rooms and an additional prep area for writing notes and phone follow-up with physicians and patients. The clinic is located across

the hall from Heart Center, GI Clinic and general medicine, allowing for easy access to the patients’ physicians and nurses. There are 2.5 FTEs which includes five full-time clinical pharmacists and one manager. All pharmacists service at least one additional clinic. Patients are identified by referral from any medical center staff member. Patients are seen during half-hour face to face appointments that are scheduled using UIMCC’s scheduling system, Pathways Healthcare System. Each month the clinic averages approximately 250 patient visits, or 11.27 patient visits per day. This number continues to grow. More than 155 patients, ranging from 11 to 92 years of age, have their medications filled by the OCC pharmacy. This helps ensure the patient

receives the therapeutic regimen intended by their physicians along with providing a way to assess adherence.

Patients take anywhere from one to 41 medications and roughly 90 percent of patients are dual-eligible. After each patient visit, a note is placed in their electronic medical record for viewing by all their healthcare providers. In 2010, MTMC patients’ average monthly medication adherence was 95 percent. Overall, patients were above the 80 percent adherence benchmark set by multiple outcome studies. Additionally, MTMC patients blood pressure control is at 63 percent—above the national average of 34 percent stated by the seventh report of the Joint National Committee on High Blood Pressure and the 40.9 percent announced by the Agency for Healthcare Research and Quality. MTMC’s medication reconciliation rate is up to four times that of medical center physicians. Additionally, in 2010 the MTMC pharmacist team created and implemented a state wide training program called The IL Project in collaboration with Medco and the State of Illinois. The project’s goal is improve patient adherence and address omissions in drug therapy. The program has received such positive outcomes that it has been adopted in additional states. Inpatient Medicine Adam Bursua, Team Leader In addition to several awards received and publications authored by members of our inpatient medicine team, inpatient medicine faculty accomplished a great deal in 2010 to expand the influence and impact of the college. Internationally, Charles McPherson, PharmD, and Adam Bursua, PharmD, delivered educational workshops in Taiwan and Hong Kong. All faculty have enriched international relationships in the past year by mentoring trainees from overseas as arranged by the department and Alan Lau, PharmD. The college’s influence at the national level expanded as well, as Mat Thambi, PharmD, was asked to serve on an influential FDA advisory committee. Locally, outreach to the region’s seniors has continued to flourish under the leadership of Mike Koronkowski, PharmD. Demand for pharmacy faculty-led seminars has led to the addition of several senior center sites. Medicine team members also continue to serve important roles on hospital, department, college and university committees. Over the past year, members of the inpatient medicine team have lectured and provided various forms of interdisciplinary education with the Colleges of Medicine, Dentistry and Nursing. Through all of these activities, faculty remained focused on their most important roles—educating pharmacy students and improving the care of hospitalized patients. Oncology Sandra Cuellar Puri, Team Leader

The oncology pharmacy team consists of two ambulatory clinicians and two inpatient clinicians. The team collaborates on the delivery of patient care as well as academic endeavors. The academic achievements of the oncology pharmacy team vary broadly. This year, two faculty members were invited to give hematology/oncology lectures internationally. Karen Sweiss, PharmD, traveled to Japan to give lectures to Meiji University pharmacy students. Latha Radhakrishnan, PharmD, conducted an oncology workshop and presented a plenary session, titled Integrating Oncology Pharmacy Services

Into Comprehensive Healthcare, at the 2010 Pharmacy Practice Scientific Conference in Kuala Lumpur, Malaysia. Sandra Cuellar Puri, PharmD, was appointed as Secretary of American College of Clinical Pharmacy hematology/oncology PRN and appointed as editor-at-large for a new peer reviewed journal, Journal of Hematology Oncology Pharmacy. Christina Haaf, PharmD, presented an abstract poster at the 2011 Hematology/Oncology Pharmacy Association meeting. Dr. Haaf also serves on numerous committees within the College of Pharmacy and medical center. Recently, Dr. Radhakrishnan was elected as a member of the college’s admissions committee. The oncology pharmacy team teaches and precepts pharmacy students, PGY-1 residents, PGY-2 oncology residents, hematology/oncology medical fellows and international pharmacists throughout the academic year. Drs. Sweiss and Radhakrishnan were both nominated for the Preceptor of the Year award by the college’s Class of 2011.

Pediatric Team Update Leslie Briars, Team Leader

The pediatric team consists of six clinical pharmacists covering both inpatient and outpatient services. In the past few years, the team has collaborated on a variety of projects including an evaluation of high cost drugs like albumin and cost-savings endeavors such as providing vaccinations in the hospital through the Vaccines for Children’s Program. In the fall of 2010, three of the pediatric clinicians travelled to Hong Kong and provided a comprehensive pediatric therapeutics course to approximately 20 pharmacists. Two of the Hong Kong pharmacists then traveled to UIC in the spring of 2011 to rotate through the various clinical sites for three months of onsite training.

Solid Organ Transplant Team Jim Thielke, Team Leader

The transplant team is a newly formed team. Previously, its members were part of both ambulatory care and critical care. The expansion of the team has allowed increased coverage of all aspects for the transplant program, especially on the outpatient side. Clinicians now see transplant patients in the pre-transplant clinic, helping clear them for transplant from a medication standpoint. In addition, team members attend the pre-transplant listing meeting and evaluate all donors and recipients prior to listing for transplant. The group has already begun collaborating with multiple teams and groups from within the college. In addition, the team is working with the inpatient medicine team to pilot a new student evaluation form for clerkship rotations. Faculty hope to use this new tool in the upcoming year. Plans for the next year also include evaluation of resources to allow for the expansion of

the post-graduate transplant training program. We would like to be able to expand from our one PGY2 resident to also include a transplant fellowship/second year clinical residency program in solid organ transplant. The second year of the program would allow the trainee to focus on transplant research under the direction of Patricia West-Thielke, PharmD.

Rockford Pharmacy Practice David Bartels, Vice Dean

At the beginning of 2010, the Department of Pharmacy Practice had four long-standing faculty members based in Rockford. David Bartels, PharmD, vice dean and clinical professor, Rockford campus, provided administrative support for the developing Rockford campus. Dr. Bartels continued to see patients at the University Primary Care Clinic in Belvidere one day per week while lecturing and conducting workshops on a variety of topics for College of Medicine at Rockford second year pharmacology course. He continues in these capacities beyond the enrollment of the inaugural pharmacy class on the Rockford campus in August 2010. Linda Chang, PharmD, and Isaac Cha, PharmD, have joint faculty appointments with the College of Pharmacy and the College of Medicine at Rockford Department of Family and Community Medicine. Dr. Chang maintains a clinical practice at the University Family Health Center at Rockford where she sees patients and instructs family practice residents and pharmacy students during their ambulatory care rotations. She also provides a number of lectures and workshops in the PDAT series for the College of Pharmacy and the M2 pharmacology course for the College of Medicine. Dr. Chang is the course coordinator for the essentials of patient care course for second year medical students at the College of Medicine and lectures extensively in the course. Dr. Cha maintains a clinical practice at the University Primary Care Clinic at Mt. Morris where he sees patients and teaches both pharmacy and medical students during their clinical rotations. Dr. Cha also lectures in the PDAT series for the College of Pharmacy and is the course coordinator for the College of Medicine second year pharmacology course. He lectures extensively in the M2 pharmacology course and conducts many active learning workshops for the students. Jennifer George, PharmD, is a clinical assistant professor who is jointly funded by Crusader Clinic in Rockford where she maintains her clinical practice. Dr. George teaches pharmacy students during the ambulatory care rotations and has become more active in the College of Pharmacy with the expansion of the Rockford campus.

The development of the Rockford campus required additional pharmacy practice faculty members to help with both didactic and clinical teaching. In March 2010, Allison Schriever, PharmD, was hired as the director of experiential education for the Rockford campus. Dr. A. Schriever develops clinical sites for pharmacy student IPPE and APPE rotations. She has been

instrumental in the development and implementation of the Rural Pharmacy Education Program at the Rockford campus. Christopher Schriever, PharmD, joined the department in September 2010. Dr C. Schriever is jointly funded by Crusader Clinic. He is an integral part of the HIV patient management team and he serves on a number of critical infection control and quality assurance committees at Crusader Clinic. Dr C. Schriever is also a lecturer in the infectious disease portion of the College of Pharmacy PDAT series.

Research Activities Jim Fischer, Assistant Head for Research

The department continued to advance its research mission with several new initiatives during the last year. In November, the first Department Research Poster Forum was held with more than 20 individuals participating. The forum provided the opportunity for faculty to educate others in the department about their research activities, receive feedback from colleagues, and establish collaborations. A program of monthly research seminars organized around the topics of clinical pharmacology and individualized drug therapy also started in November. The seminar series was supplemented by a pharmacogenomics lecture series of invited external speakers from January through April. An important need identified in the department’s research strategic plan was the need to provide structured programs to enhance the research knowledge and skills of the faculty. The department, in conjunction with the Office of the Vice Chancellor for Research, is planning to offer a day-long program titled, Writing Winning Grants and Coping with Changes at NIH, to begin to address the identified concerns. Both the quality and size of the research program grew during the last year. The department added new tenure track faculty. A 1,500-square-foot expansion of laboratory space and much needed renovation of the current laboratories are planned. Information on publications as well as externally funded grants and contracts is listed in a separate section of this report.
