NEGLECT AND DIAGNOSTIC DELAY OF SOFT TISSUE SARCOMAS
Brad Culotta, MD, Herrick J. Siegel, MD from UAB Medical Center
_________________________________________________________
Delays in the diagnosis of soft tissue sarcomas remain a considerable problem,
which may adversely affect management and prognosis. In that soft tissue
sarcomas (STS) are rare, often indolent and have variable growth rates has led to
a universal acceptance of lengthy delays in diagnosis and treatment. The impact
of this delay is not clear from the present literature in terms of health care cost,
treatment options and prognosis. The study group consisted of 112 patients
identified from a cancer center sarcoma database consisting of 194 patients.
Patients were sub-grouped based on the length of referral delay from the initial
discovery of a soft tissue mass until presentation to a sarcoma specialist (SS).
The subgroups included: Timely (<3 months), Delayed (3-12 months) and
Neglected (>1 year) were evaluated to determine the factors contributing to
a delayed referral, the impact of delay on prognosis, and characteristics of a
soft tissue mass on presentation that would place a patient at risk for a delay
in treatment. Common causes for delay upon initial presentation include
insufficient imaging studies by general practitioner; reassurance of a benign
process based on physical exam, poor documentation of mass in follow up
visits. Additionally, >70% of the delays were in part contributed to radiology
and pathology reports with either wide differential diagnoses or misleading
descriptive findings suggesting an atypical benign mass led to either unplanned
excisions or delays in treatment. Patients with neglected sarcomas most
commonly (>80%) had large, slow growing, low grade soft tissue sarcomas,
therefore resulting in a better prognosis than those presenting earlier with rapidly
growing high grade sarcomas. Despite of the new NIH guidelines focused on
early referral of soft tissue malignancies, our study found the average referral
time >9 months from the initial discovery of a mass. Continued efforts are
important to encourage early referral of soft tissue masses to SS.
