Introduction

The Uniform Application for Community Scholarships was developed to help high school seniors be more efficient and effective with the use of their time. In addition, and more importantly, utilizing this application will allow seniors to spend meaningful time writing essays for scholarships and providing other more specific information, in order to spotlight themselves as applicable for the individual scholarships. Seniors, and for any parents reviewing this application, it is an outstanding initiative on the part of the student to take the necessary steps to apply for any of these scholarships.

Thanks to a generous donation from the Fishers Rotary Club, FHS and HSEHS in Partnership with the Hamilton Southeastern Schools Foundation have launched an online application platform called SmarterSelect. Before beginning the Uniform Application for Community Scholarships Application online process, please read through the specific Scholarship Eligibility Requirements for each scholarship found in this booklet. Students also have the ability to preview scholarship applications through the SmarterSelect system.

Application Items1. Uniform Application for Community Scholarships Form (complete Scholarship Application

3. In addition to basic contact information and future plans, students should be prepared to enter information regarding employment history, extra-curricular activities, and volunteerism. Some scholarships require letters of reference. Students may upload letters already in their possession or contact potential references via the SmarterSelect system.

Section A – Application submittal Instructions

1.Each application should be submitted using the SmarterSelect program. 2.If a Scholarship requires additional information, upload the required documentation to the

electronic application.3.If scholarship requires sealed recommendation letter(s) complete the Email portion of the

reference section to request the recommender send directly to scholarship sponsor. You will need contact information for your recommender.

4.Required items not included with your application may disqualify you for the scholarship.5.Please thoroughly read the essay questions before composing your essay.6.Double check your contents! It is the student’s responsibility to submit application and

any additional documentation by the due date. The due dates vary and are indicated both on the Available Scholarships List on the next page and in the Eligibility Requirements for each scholarship.

Belfry Memorial Scholarship Due 4-15-16

This scholarship is for a Hamilton County high school senior who is planning to attend college in the fall and major in theater, drama, and/or performing arts.

In the past, this award has been from $1,000 to $1,250.

Two recommendation letters are required; include the evaluation form with each one: One from your drama teacher One from a community member, clergymen, civic leader, or employer, etc.

Write an essay about your dramatic experience and include the reasons why you are requesting this scholarship.

Brooks School Elementary/Luke Butler Senior ScholarshipDue 3-30-16

Two $500 scholarships will be awarded in memory of Luke Butler to graduating seniors from either Hamilton Southeastern High School and/or Fishers High School who attended Brooks School Elementary for a minimum of one full school year.

Eligibility Requirements: Completed application Written explanation of post-high school plans Written explanation of how the scholarship will be used (200 word maximum) Written essay on a life lesson learned while attending BSE that impacted

applicant’s life (1-2 page maximum-minimum font size of 10 point) Letter of recommendation from a current counselor, teacher or employer who can

attest to student’s character and academic potential.

Selection Process: A selection committee which consists of BSE staff members willevaluate student applications and select scholarship recipients. The winners will behonored at the High School Senior Awards Night. The scholarship winners will be notified.

Section C – Scholarship Eligibility Requirements

Carrie Colglazier Memorial Scholarship Due 4-15-16

Hamilton Southeastern High School, in cooperation with the Colglazier family, has established a memorial scholarship in honor of Carrie Colglazier who was tragically killed by a drunk driver on June 6th, 2003. She was an HSEHS two sport, All State student- athlete in soccer and track. She was an active member of the FFA program and a member of National Honor Society. She was extremely passionate about her family, friends, teammates, teachers and coaches, and took great pride in being a Royal!

The $12,000, four year scholarship ($3,000 awarded annually) is funded by an endowment through the Hamilton Southeastern Schools Foundation and by the Carrie Colglazier Community Service Workday. The workday, which will be held in the spring ( the Saturday before the first Saturday of spring break, March 26, 2016), includes workers from the Girls’ Soccer and Track teams, along with members of FFA and National Honor Society. Workers are asked to work up to an 8 hour day (8:00am-4:00pm) in the HSE community for a minimum $25.00 donation per worker. All proceeds go toward the scholarship.

Criteria for the scholarship: Must be a senior planning on attending a 2 year or 4 year college, university,

vocational or trade school  Must have participated in HSEHS Girls’ Soccer, HSEHS Girls’ Track, HSEHS National

Honor Society, and/or FFA  Must have consistently participated in the Community Service Workday  Must have demonstrated academic achievement, leadership roles, and community

service  Must submit two letters of reference: one from teacher or administrator and one

from community member who knows you well  Must make a statement of your education and career goals  Must provide a photo  Must submit an essay using the following prompt: Describe a non-classroom

experience (positive or negative) during your four years at HSEHS/FHS that has impacted your personal growth. Describe the situation and the lessons you learned through this experience.

Citizen’s Police Academy Alumni Association Scholarship Due 3-25-16

One $1,500 scholarship will be awarded to either a Fishers High School or Hamilton Southeastern High School senior. This scholarship is available to a high school senior who expresses a strong interest in the criminal justice field. Applicant should possess leadership skills, community commitment and desire to contribute to society.

Eligibility Requirements:1. Submit a 500 word (or more) essay indicating why you have chosen a law

enforcement related career and how you specifically plan to meet that goal. 2. Provide TW letters Two letters of recommendation (include the evaluation form

with each one) are required:a. One from a school teacher, andb. One from either a school counselor, coach or community leader

***The letters must be sealed and the envelope signed by the author

3. Must have a GPA of 2.5 or higher

After the Alumni Board reviews all applications and essays, you will be notified of your status and may be contacted to set up an interview time if you are selected. Do not make inquiries regarding your status. You will be informed if you have been selected to continue in the process.

Sponsored by Geist Elementary PTO, the Claire Tatom Memorial Scholarship is a $500 scholarship awarded to one male and one female student graduating from either Hamilton Southeastern H.S or Fishers H.S.Eligibility Requirements:1. Must have attended Geist Elementary for one complete school year K-42. Must have a minimum 3.0 GPA3. Must be planning on attending a four-year college or university4. Be active in a variety of activities5. Make a statement of your plans as they relate to your educational and career

Coach Don Nagy Memorial ScholarshipDue 3-30-16

Hamilton Southeastern High School Football Program in cooperation with the Nagy Family, has established a memorial scholarship in honor of Don Nagy who was a longtime volunteer football coach for the Royals. Coach Nagy was extremely passionate about his family, the Michigan State Spartans and the Hamilton Southeastern Royals. Coach Nagy believed in the life lessons that are learned on the football field. He was a tremendous asset to the Football Program and was a role model to the football players he coached.

A scholarship of $2000 will be given to a Hamilton Southeastern High School senior football player who best exemplifies the qualities and characteristics of Coach Nagy.

Criteria for the scholarship: Must be a current Hamilton Southeastern High School graduating senior

and a 4 year member of the football team School activities Academic achievement Community service Leadership roles Must be planning to attend a 4 year college

ActivitiesPlease list all school organizations, clubs and athletic teams in which you have participated. Include any honors, awards or leadership positions. Also, include plans after high school including college choice, major and future career goals. Cumberland Road Elementary School Senior Scholarship For Fishers High School Seniors Only

Due 4-15-16

Cumberland Road Elementary school will present two scholarships to graduating seniors from Fishers High School who attended Cumberland Road Elementary School for a minimum of one full school year.

A selection committee which consists of CRES staff members will evaluate student applications and select our scholarship recipient. The winners will be honored at the Fishers High School Senior Awards night.

Provide a letter (include the evaluation form) from a current counselor, teacher or employer who can attest to your character and academic potential.

Eligibility Requirements: 

1. Written explanation of post-high school plans (In a sentence or two)2. Written explanation of how the scholarship will be used (200 word maximum on a

A $1000 scholarship will be awarded to a senior at both Fishers and Hamilton Southeastern high Schools. One mission of the Sertoma Club is to serve those at risk or impacted by hearing loss. As a result, preference for this scholarship will be given to students that have experienced hearing loss, hearing impairment or speech impairment. Preference will also be given to students who serve in this field either in a volunteer or future career capacity—include but is not limited to ASL experience, majoring in audiology or speech pathology in college. Students without this experience remain eligible to apply provided they meet the eligibility requirements.

Fishers Junior High School National Junior Honor Society Alumni Scholarship Due 4-15-16

Two $500 scholarships will be awarded to former members of the Fishers Junior High National Junior Honor Society.

In 2003, Fishers Junior High School chartered the district’s first National Junior Honor Society based on the principles of Scholarship, Leadership, Service, Citizenship, and Character. As a chapter, we emphasize building leaders through service. The faculty council has made it a goal for the chapter to reward members that went on to put this area of emphasis into action at the high school level. Through the monies raised with our chapter’s recycling program and through a portion of our chapter dues, we will be able to make this reward a reality.

Eligibility Requirements:1. Must be a senior in high school2. Applicant must have been an inducted member in the Fishers Junior High School

chapter of the National Junior Honor Society3. Applicant must have been an active member of a service organization during his or

her high school years. Examples of school service organizations are: National Honor Society, Student Council, Class Officer, Best Buddies, and Key Club. Service involvement is not limited to school organizations.

4. Applicant must have had a cumulative GPA of 3.0 or a B averageLetter(s) of recommendation (include the evaluation form with each one) from the service organization sponsor(s). Maximum of three. Multiple letters are not as important as the commitment and service reflected in a single letter. The letters are meant to show the depth of your service, not the breadth.

Hamilton County Extension Homemakers ScholarshipDue 3-1-16

The Hamilton County Extension Homemakers will award four $500 scholarships to Hamilton County students. Applicants must be a high school senior entering college or a currently enrolled undergraduate, majoring in Health and Human Science or related field. Included would be majors in departments of Foods and Nutrition; Family and Consumer Science Education; Interior Design; Textile and Clothing Design; Hospitality & Tourism; Child Development and Family Studies; Consumer Science & Retailing; Nursing; Fitness and Health; Speech, Language and Hearing; and Health Sciences. NOTE: for the 2016-17 school year, the Hamilton County Extension Homemakers will be awarding two additional $500 scholarships to two students majoring in Elementary Education.

The scholarship will be awarded at the end of the first semester provided that at least a C average or decimal equivalent is maintained in the first semester. Recipients must submit grade reports to Purdue Extension Hamilton County office, Extension Homemakers Scholarship Committee, 2003 Pleasant Street, Noblesville, IN 46060 to receive funds for the second semester.

Any previous applicant may reapply, including current college students.

A scholarship committee of the Extension Homemakers Board will review the applications and may interview applicants. In judging, the following points will be considered:

This is a scholarship for FHS/HSEHS seniors who intend to become teachers. School involvement, community service, character, work ethic, and deservedness considered. One essay and one letter of recommendation must be submitted with the application.

Choose one of the following essay topics and submit a well-developed essay that is typed, double-spaced, 12 pt. font. The essay should include an introduction with a thesis, body, and conclusion. A panel of teachers will evaluate the essay using the 6 + 1 traits rubric. Please DO NOT put your name anywhere on or in the essay.

1. Explain the three qualities/skills every teacher must possess to be effective.

Jessica Adkins Memorial ScholarshipHSE High School Seniors Only

Due 3-30-16

A $500 renewable Scholarship will be awarded each year to a HSEHS senior. Jessica Adkins was a 2006 graduate of Hamilton Southeastern High School and a 2011 graduate of Indiana University. She died in a tragic accident on July 24, 2012. Her passion in life was to help others and be accepting of all people, regardless of their social or economic status. Prior to her accident, Jessica had plans to continue her education in Social Work. To keep her legacy alive, Jessica’s family has established a scholarship to aid an HSEHS senior with characteristics and goals similar to Jessica’s.

Seniors wishing to apply should have the following: compassion, empathy, supportiveness, strong leadership, organization, attention to detail, good communication, and a genuine interest in working with a variety of people from all backgrounds.

The scholarship will be for $500 each year. This scholarship can be reapplied for in subsequent years as long as the student remains committed to their area of focus.

Eligibility Requirements: 1. Two Letters of Recommendation (include the evaluation form with each letter)

One from a teacher or administrator who knows you well One from a person in the community who knows you well

2. Must intend to major in social sciences or related field

Include responses to the following:

Activities: Describe activities that you have been involved in, both in school and in the community. Include all clubs, organizations, and teams of which you have been a member. Highlight any of the characteristics and skills noted above involved in these activities. In addition, please describe any leadership roles you have undertaken.

The Pay It Forward Scholarship by Hunter Law Office, P.C. will be awarded to the applicant who, in the judgment of the Selection Committee, appears most deserving based on the scholarship criteria. One scholarship of $500 will be given to a student from Hamilton County and one scholarship of $500 will be given to a student from Parke County.

Eligibility Requirements: 1. Must be at least a senior in high school (current college students may apply)2. Must be or will be a graduate of a high school in Hamilton County or Parke County3. Must be enrolling in a college or attending college in 2015-2016 school year4. There is not a GPA requirement

Application Requirements:1. Applicant must complete the Uniform Application for Community Scholarship

Form2. Applicant must submit with their application packet (1) Personal Recommendation

from an instructor, counselor, club advisor, coach, church leader, or employer3. Applicant must submit with their application packet (1) response to the following

Peterson Family ScholarshipDue Date 3-30-16

A $1000 scholarship will be awarded to a FHS senior and a HSEHS senior. The Peterson Family Scholarship was established in memory of Roma Peterson, a 1942 graduate of Fishers High School. She was an English teacher at FHS and HSEHS. Four generations of the Peterson family have either graduated from FHS or HSEHS or have worked/volunteered for the HSE District.

Eligibility Requirements:

1. Must be majoring in a field related to business or education2. Must attend a two year of four year college or university3. Must have a minimum GPA of 3.0 4. Must be involved in a variety of activities5. Make a statement of your plans as they relate to your educational and career

Royals Varsity Club ScholarshipDue 3-30-16

The Royals Varsity Club has established two $1,500 scholarships to be awarded to one male and one HSEHS female senior student-athlete.

Criteria for this scholarship include: 2015 Hamilton Southeastern High School Graduate Participation in athletics as a senior at HSEHS Academic achievement Community service Leadership roles

One letter of recommendation is required: One from a teacher, administrator, or person from the community who can attest to your

character, academic and leadership skills.

Activities: Please list all school organizations, clubs and athletic teams in which you have participated. Include any honors, awards, or leadership positions. Expand on your involvement and responsibilities and include the criteria of any awards you have received. The purpose of this scholarship is to provide a Thorpe Creek Elementary student who is a graduate of HSEHS or FHS a grant in order to help further his/her future endeavors, whether it be college, technical, or trade school. A selection committee which consists of TCE staff members will evaluate student applications and select scholarship recipients. The winner(s) will be honored at the High School Senior Awards.
