FIVE POINTS OF LIFE FOUNDATION

ANNUAL REPORT

The Five Points of Life Foundation is a support affiliate of LifeSouth Community Blood Centers, a private non-profit 501(c)(3) charitable organization. The Five Points of Life Foundation educates and raises awareness of the five ways to share life with others through the donation of blood, apheresis, marrow, cord blood, organ and tissue. The Foundation works with blood centers, organ procurement organizations, marrow registries and community organizations to educate citizens about lifesaving donation.

Programs of the Five Points of Life Foundation

• FivePointsintheClassroom:TocreatetheDonationGeneration,theFivePointsofLifeeducationalcurriculum for elementary, middle and high school students teaches why lifesaving donation is important. The program focuses on the social responsibility and the science behind lifesaving donation.

• FivePointsofLifeRaceWeekend:Foundedin2006,theRaceWeekendtakesplaceinGainesville,Fla.Itincludes a Kids Marathon, 5 Points 5K, Marathon Relay, Half and Full Marathon. Participants, family, friends and the community learn about the Five Points of Life through media coverage, course signage, the Fitness Expo, speakers, flyers, race packets and informational and educational booths at the event. The goal of this event is awareness.

• FivePointsofLifeKidsMarathons:Kidsrun25milesduringtheschoolyear,loggingeachmileincrementally, and running the last 1.2 miles at a Kids Marathon. Kids Marathons currently take place inGainesville,Ocala,andCitrusCounty,Fla.aswellasMontgomeryandHuntsville,Ala.Thegoalistohost Kids Marathons in all of LifeSouth’s seven districts.

Mission: To educate and raise awareness on the five ways to share life with others through lifesaving donations.

Vision:Every eligible donor gives blood, joins the marrow registry, talks to their family about organ donation and learns more about cord blood donation. In addition, every school age child is educated on the importance oflifesavingdonationsowhentheyareeligible,theywilldonateandbecometheDonationGeneration.

CreatingtheDonationGeneration:Imagineagenerationsocommittedtodonationthereisnowaitfororgansormarrow and no blood shortage. That’s our goal. The program plans to reach 12,000 children annually.

In 2011 Five Points of Life Foundation established its identity as a foundation and support affiliate of LifeSouth Community Blood Centers. The goal of the foundation is to educate and raise awareness of the five ways to share life through donation. Started in 1997 with a cross-country bike ride, the scope of the organization has expanded to include educational curriculum,KidsMarathonsandanannualRaceWeekend.

A major development in 2011 was the debut of our Five Points of Life in the Classroom curriculum. The material features lessons about the science behind donation and highlights the impact donation can make in communities. The objective is to give students the information they’ll need to make an informed decision about donation by the time they reach high school.

The foundation also conducted Kids Marathons in five cities in 2011, bringingtheeventforthefirsttimetoMontgomeryandHuntsville,Ala.WeplantohostadditionalracesinAtlanta,Ga.,Mobile,Ala.andHernandoCounty,Fla.inthecomingyear. These events spread the message of donation throughout entire communities, while promotingphysicalfitness.TheFivePointsofLifeRaceWeekend,whichoffersaKidsMarathon, 5K, half marathon, and full marathon, draws runners from across the country toGainesville,Fla.Ineverycity,theseeventshavehelpedforgestrongnetworkingconnections between the LifeSouth foundation and dozens of community organizations.

TheKidsMarathonsandclassroomcurriculumarealreadymakingadifference.Weknowof16year-oldswhonowmarktheirbirthdaysbydonatingbloodasaresultofthiscurriculum. Together, these programs reached more than 12,000 young people.

Grants,sponsorshipsandfinancialgiftssustaintheworkofthefoundation.AnEveningWithMotown,agalaheldinGainesville,Fla.,succeededinraisingfundsandsharingthefoundation’scoremessage.

Overthepastyear,theprogramhasgrownintoafoundationthatcontinuestoteach,motivateandinspire.Wearegratefulforthemanyindividuals,businesses,andorganizationsthatsupportoureffortstoraiseawarenessandcreatetheDonationGeneration.

The Five Points of Life Foundation holds an annual fundraising gala, titled‘AnEveningWith.’Theeventfeatures live entertainment, dancing, delicious cuisine, a signature martini bar, and a silent auction.

The2011eventisAnEveningWithABBA, featuring Almost ABBA performing the group’s greatest hits. The event takes place on Friday, September 23 at the Hilton University of Florida.

Proceeds benefit the Five Points of Life Foundation.

Martinis and Motown:

A signature martini bar is featured at the event and guests receive a Five Points of Life martini glass to remember the evening.

‘AnEveningWith’Fundraising Event

Kids Marathons Build on SuccessWiththeadditionoftwonewKidsMarathons in Montgomery and Huntsville, and record-setting participation in Citrus County and Ocala, Fla., it was a banner year for the Five Points of Life Foundation.

TheDonationGenerationIn 2011, Five Points of Life in the Classroom rolled out its new activity books and expanded to more schools. The activity books are geared toward grades K-2, 3-5 and middle school.

Afirst-handlook: After learning about blood donation in the classroom, these students got to look at a real unit of platelets.

FivePointsofLifeRaceWeekendThe sixth annual Five Points ofLifeRaceWeekendbrought in nearly 2,000 runnersin2011.The26.2mile course winds through GainesvilleandtheUniversityof Florida campus. LifeSouth holds a blood and marrow drive at the race giving runners the opportunity to donate at the event.
