Why Attend

Confluence. It means coming together. And home care leaders

are coming together by:

n Developing new models for assuming risk and responsibility for the care we are providing.

n Putting in place the new relationships required to do so.

Coming together. It’s a good thing for patients, and it’s not a moment too soon for a teetering economy in need of a serious injection of value. Home care and hospice needs to be that source of value not just for the healthcare system but for the communities in which we live and work.

What part will you and your organization play in the upcoming era? How will our sector be central, not peripheral? Who is positioned to navigate the confluence of risk, responsibility and relationships? This is what we will tackle with the 2011 program.

Come together with your peers at Home Care 100 and meet this confluence head on!

Sincerely,

no one ever changes the oil in a rental car. Why invest in something that you don’t own? What about your staff or those you lead? do they own their work, or are they just renting space on the organizational chart? the truth is, you can’t hold people accountable for the things that really matter – caring, pride, loyalty, and passion.

in this eye-opening session, Joe tye will use his recently released book, All Hands on Deck, to provide a roadmap of action steps that you can take to build a culture of ownership and develop a spirit of partnership among the people you lead.

You’ll walk away from this insightful program with:

n Practical and proven strategies for earning employee buy-in and commitment to the values, vision, and purpose of your organization.

n the real-world wisdom of some of the most successful business leaders of the twentieth century, including Walt disney, ray Kroc, Mary Kay ash, Bill Hewlett, and dave Packard.

n Copies of two of Joe’s books: All Hands on Deck and The Healing Tree

Joe tye is a best-selling author and Ceo of values Coach, a training and consulting firm focused on value-based leadership and cultural transformation. He has worked with organizations across north america to help them cultivate a more positive and productive workplace environment.

Before founding values Coach, Joe tye was a senior healthcare executive. as Coo of a 750-bed hospital, he helped guide multiple initiatives that substantially increased volume, revenue, morale, and laid the foundation for record-breaking operating and financial performance in subsequent years.

today, Joe’s purpose is to help people re-spark the spirit of mission in their work and the spirit of adventure in their lives, and to help organizations tap into the invisible spirits of their people.

opening Keynotedo the right thing: How dedicated employees create loyal customers & large profitsas Ceo of Southwest airlines during September 11, Jim Parker continued the airline’s strong customer-focused culture and record profitability during an extremely difficult time – especially for the airline industry.

Jim Parker strongly believes that the culture of an organization can be a great benefit in good economic times but is absolutely essential in bad economic times. He stresses the “round earth” principle, which means developing a strong respect for your employees so that they will do the same for your customers on their own initiative. When times are tough, that respect and appreciation transcend a purchasing consumer’s decision to save money.

Jim will share his experiences and how an employee-first culture, mutual respect and a common mission contributed to Southwest’s remarkable success and how you can apply those lessons to home care.

About Jim Parker: during Jim’s tenure as Ceo, Southwest was named one of the three most admired companies in america by Fortune magazine; and airline of the Year by Air Transport World magazine. Jim was also

named Co-Ceo of the Year in 2001 by Morningstar.com, and was named to institutional investor’s list of Best Ceo’s in america in 2004. Jim’s proudest accomplishment, however, comes from the fact that Southwest airlines was able to protect the jobs of all of its employees, with no furloughs or pay cuts in the aftermath of September 11, while also remaining profitable every quarter and year during his tenure.

Jim presently serves on the Board of directors of the successful texas roadhouse restaurant company, and serves on the advisory Council for the Mit Sloan Business School Leadership Center. He is

also the author of Do the Right Thing – How Dedicated Employees Produce Loyal Customers and Large Profits.

capturing the care management Slice of the Healthcare reform pieChronic care management represents the most costly segment of the healthcare system, and the segment is also considerably fragmented. as reform plays out, there will be an increasing focus on addressing chronic care as well as hospital readmissions to reduce the nation’s healthcare costs. Home care brings a number of competencies to chronic care management, but how can agencies put themselves in the driver’s seat? How well prepared is your organization to provide chronic care management? this panel will address the strategic importance and positioning of this opportunity for both the sector and individual agencies.

the pursuit of therapy excellencethe single most financially impactful and politically charged issue in all of home care is the utilization, documentation and reimbursement of therapy services. the growth in both the complexity of cases and the documentable need for therapy visits requires responsible agencies to provide similarly complex therapy offerings. From a leadership perspective, critical questions need to be answered:n What are the key PPS changes and what is the real

impact to agency operations? n Given the impact, what strategies are agencies

implementing to ensure success for their therapy service programs?
