*For all TEEN competitions, including bikini & physique, athletes MUST be at least 16 years of age NO
exceptions!
*CROSSOVERS-only Teen BB and Masters BB are permitted to crossover. Figure, Bikini and Physique
are permitted to cross over into Over 35 and Masters.
Injury Release: In consideration of accepting this entry, I, undersigned herby for myself, my heirs, and
administrators, waive and release any and all claims for personal injury, or property damage of whatever for and
nature, I have against Brandy Leaver and Brandon Curry, The National Physique Committee and Tennessee
Performing Arts Center, or any vendors, individuals, sponsors with affiliation with the contest or competition, that
may arise from participation in the competition.

The undersigned consents and agrees that the undersigned will comply with all rules and regulations of the Host
Hotel including any rules and regulations prohibiting cooking or food preparation in the hotel room. The
undersigned further agrees that the undersigned will indemnify an hold harmless the N.P.C. and the promoter
from and against any damages caused by the undersigned to the Host Hotel or damage to any guest room

occupied by or registered to the undersigned may result in d isciplinary action against the undersigned including,
but not limited to, suspension as a member of the N.P.C. and/or competing in contests sanctioned by the N.P.C.
Entry Fee: $70 (additional $60 for crossover)
Deadline: Oct. 13th, 2012-after this date, a $40 late fee is added
NO Checks accepted after Oct. 13th-Money order by mail ONLY or CASH at check-in.

2012 Music City Muscle t-shirts will also be available to competitors for $7 and we are accepting pre-
orders to ensure a proper amount of sizes and prints. After that t-shirts and tanks will be for sale for
$15 at the show. If you would like to make a pre-order, please add amount to your entry fee and
circle your size/style below. You will be given a ticket to pick up your t-shirt at check-in and you will
be able to pick up your t-shirt then.
