These activities will be promoted to medical attendees before and during the meeting. • AAOS Electronic Skills Pavilion – Presentations focusing on technology will take

place in this pavilion located in the exhibit hall.

• AAOS Redemption Centers – Distribution of complimentary tote bags and drop off of tickets for drawings of other items will take place in designated AAOS Redemption Centers only located on the exhibit floor.

• Ask an Expert – Interactive learning sessions that take place on the exhibit hall floor where attendees are invited to bring case challenges for presentation to an expert in orthopaedics for diagnosis and recommendation.

• Beverage Breaks in Exhibit Halls – Complimentary beverage breaks will be provided during the 30-minute break between scientific sessions at 3:30 PM on Wednesday and Thursday, and at 10:00 AM on Friday.

• Unopposed Exhibit Time - One hour of unopposed exhibit time will be provided each exhibit day from 12:30 PM to 1:30 PM.

Eligibility to ExhibitOnly those companies that have successfully completed the AAOS Company Review Process are eligible to exhibit, or as otherwise specifically approved by the AAOS Exhibits Committee.

Exhibitors must provide details on any consumer or government litigation, orders, injunctions, judgments or settlements over the last three years regarding the business practices of the company or the products and services to be exhibited. Companies with multiple complaints filed against them with state or federal consumer affairs regulatory agencies, the Better Business Bureau, or AAOS members may be required to provide an explanation of the resolution of those complaints.

Products and services must be related to the practice of orthopaedics and companies must be in good financial standing with AAOS. AAOS reserves the right to refuse space to any company who has failed to fulfill its financial obligations to AAOS, and/or whose products or services, in the judgment of AAOS, do not meet the educational, scientific or practice needs of our Fellows and members.

Company Review ProcessCompanies that have not exhibited at an AAOS Annual Meeting must successfully complete the AAOS Company Review Process conducted by the Exhibits Committee in order to be eligible to participate as an exhibitor. The review takes approximately four to six weeks. The form required for the Company Review Process is available online at www.aaos.org/exhibits.

NEW Company Name Companies must list their official company name, not a product or brand name when submitting an application for exhibit space. AAOS will use company’s official name in all annual meeting publications. If a company name change is required, written notification along with documentation to substantiate the name change must be sent to [email protected]. Changes must be received at AAOS by November 21, 2014, to ensure updated name can be reflected in all meeting publications.

While rating factors are not currently taken into account for priority points, rating factors are used to ensure that the AAOS Annual Meeting continues to provide attendees with a broad spectrum of products and services on the exhibit floor. Space AssignmentAAOS intends to send initial space assignment on August 20, 2014. Only applications received with the required 30% deposit by August 4, 2014, will be considered for initial space assignment. Applications submitted on or after November 20, 2014, must be submitted with full payment.

All exhibitors, their agents and employees, are subject to all terms and conditions of the agreement between the American Academy of Orthopaedic Surgeons (AAOS) and the Venetian/Sands EXPO regarding the use and occupancy of the exhibition halls and to all reasonable rules and regulations prescribed by AAOS or the Venetian/Sands EXPO. In addition, the exhibitor shall comply with all applicable federal, state and local statutes, ordinances, rules and regulations.

Please review the entire prospectus. It includes information and guidelines important to your company’s participation as an exhibitor at the AAOS 2015 Annual Meeting. It is the responsibility of the exhibitor contact to ensure that all company personnel or third party agents that are involved in the arrangements for an exhibit are aware of and adhere to these rules. The rules and regulations listed in this Exhibit Prospectus are part of the application for space, and consequently constitute part of the agreement between the exhibitor and AAOS. AAOS reserves the right to interpret and make final decisions regarding all rules and regulations.

Applications received by August 4: These applications will be assigned space under the priority point system. The date that the application and 30% deposit is received by AAOS will determine space assignment priority for those companies with the same number of priority points.

Applications received after August 4: Should space be available, applications will be assigned space from the remaining open booths after initial space assignments. If all exhibit space is sold out, applications will be placed on a wait-list for space.

Wait-List: Companies that submit applications after all exhibit space is sold will be notified that they have been placed on the wait-list. As exhibit space becomes available, companies will be notified of space availability. If exhibit space is not assigned, AAOS will issue a full refund of any exhibit space fees paid.

AAOS reserves the right, in its discretion, to allocate space on any other basis it deems appropriate.

Acknowledgments: Within five business days of receipt of application and deposit at AAOS, the exhibitor contact will be sent an acknowledgement email. This acknowledgement is not a guarantee of assigned exhibit space, but will provide written confirmation that the application was received by AAOS.

Space LimitationsAAOS reserves the right to place reasonable limitations on the number of booths to be rented to any exhibitor. The limitation will be determined by the relation of total applications to the number of booths available on the floor plan. The Exhibits Committee strives to provide a broad spectrum of products and services.

Booth RelocationAAOS does not normally relocate booths. On those rare occasions when this does occur, exhibitors are notified with an explanation of the reason for the relocation. If the reassigned space is not acceptable, exhibitors may notify AAOS within 10 days from the date of relocation notification to cancel booth space and receive a full refund of any exhibit space fees paid.

Selecting Exhibit SpaceAvoid concentrating all booth choices in one area:

• Review the floor plan and select six locations in different areas on the floor plan

• To assist you in determining realistic booth locations, take into account your ranking based on priority points

• Exhibitors with low priority points should request booth locations in various areas of the exhibit halls to increase the opportunity of a favorable space assignment

A company’s priority points have been included in the email announcing the application for space is available. To assist companies in determining their space assignment ranking, following is an overview of the priority points of all companies eligible to exhibit.

Priority Points: 18 companies have over 100 points

64 companies have 50 points to 98 points

134 companies have 26 to 48 points

133 companies have 16 to 24 points

139 companies have 10 to 14 points

168 companies have 6 to 8 points

Over 500 companies have 2 to 4 points

The exhibit floor plan has been designed to maximize the amount of exhibit space and provide unobstructed aisles whenever possible. Should none of the requested booth choices be available, space will be assigned based on indicated preference of type of booth versus location of booth. In this situation the exhibitor agrees to accept the assigned booth(s). Additional island booths not included on the floor plan will be created at the discretion of AAOS to accommodate exhibitors that require more than 800 NSF of exhibit space. It is not possible to contact each company for new selections of booth space during the space assignment process.

Specialty AreasThe following exhibitors will be assigned space in designated specialty areas as indicated on the exhibit hall floor plan:

Diagnostic Equipment First-Time Exhibitors Practice Productivity Exhibitors Publishers and Educators

Multiple DivisionsCompanies with multiple divisions may choose to average their points in order to be grouped together in the space assignment process. The process will allow each division to contract separately while maintaining their own identities for Final Program listings, badges and hotel rooms. Only written requests received prior to August 4, 2014, from all involved parties accompanied with the Applications for Exhibit Space will be honored.

Joint-Venture/Co-MarketingIf two or more companies wish exhibit space adjacent to one another, each company must state its request in a letter sent with their Application for Exhibit Space. Each request must be submitted on company letterhead and indicate that a copy has been sent to the other company. Space assignment order will then be made by averaging the companies’ points. Only requests received prior to August 4, 2014, will be honored.

Sharing Space Sharing of exhibit space is permitted only for divisions of the same company. Exhibitors may not sublet or assign booth spaces.

Mergers and AcquisitionsCompanies that have merged with, been purchased by, or have purchased another company may use the exhibit history from whichever company is most favorable, but not by combining history of multiple companies. AAOS should be notified in writing that the acquisition or merger between companies has been completed, along with the effective date of this change.

Companies requesting a transfer of points must be directly related to the exhibiting company in order to inherit those points. A directly related company shall be defined as a company that is wholly owned by a subsidiary of or one that has merged with another company. Points will be awarded only if similar product lines are being displayed. The sale of a product, product line, certain technology, or certain assets to another company will be considered to transfer priority points only if and to the extent determined by AAOS in its sole discretion.

All requests for priority point transactions must be submitted in writing to the AAOS Exhibits Manager and confirmed in writing by the parent or subsidiary company.

Market Research BoothsMarket research companies will be assigned space only if research is being conducted for an approved AAOS exhibitor and if space is available. If space is assigned, all surveys must be conducted within the confines of the booth space. A list of exhibitors for which market research is being conducted for must be submitted in writing to [email protected] by December 22, 2014.

Online Application/Contract for Exhibit SpaceThe 2015 application is available online at www.aaos.org/exhibits. All eligible companies will be emailed their login information to access the electronic application. Electronic submissions can include online credit card deposit of 30% of the exhibit space fee. Online applications may be submitted if sending a check or wire transfer separately, but the application will not be considered as received until the 30% deposit is received by AAOS.

NEW The paper version of the application will not be mailed. If you require a paper version of the application please email your request to [email protected].

Terms of PaymentA 30% deposit ($1,140 per 100 NSF) must accompany the Application for Exhibit Space. Applications received after November 20, 2014, must be paid in full.

Applications received without a deposit will not be processed until the appropriate deposit is received.

A company’s cancellation of exhibit space will result in the release of all assigned hotel sleeping rooms and hotel public space previously assigned to the company, and company representatives will not be allowed to attend the annual meeting. For companies reducing exhibit space, sleeping rooms and badges will be decreased to coordinate with the reduction in booth space.

If a balance is still outstanding on November 21, 2014, the space assigned may be resold without notification or refund of any payments. Failure to notify AAOS in writing of intent to cancel will result in the company being denied participation at future AAOS Annual Meetings.

Allied Orthopaedic Organizations Exhibit SpaceA limited amount of space is set aside for allied orthopaedic organizations. These spaces are reserved for educational displays for non-profit and/or charitable organizations that are not selling a product or service. EXHIBITOR INFORMATION

Access to Exhibit HallsDuring show days, exhibitors properly badged may enter the exhibit halls as follows:

NEW Wednesday, March 25 6:30 AM – 6:00 PM Thursday, March 26 7:00 AM – 6:00 PM Friday, March 27 7:00 AM – Midnight

Meetings in the exhibit halls with medical attendees and other non-exhibit individuals can only take place during official exhibit hours. Medical attendees must be properly badged to gain access to the exhibit halls during official exhibit hours.

Access to Other Exhibit BoothsExhibiting companies are responsible for monitoring their own exhibit space. Exhibitors may not enter another exhibitor’s booth space without obtaining permission. This is in respect of the rights of all exhibitors to conduct business during exhibit hours without interference or improper intervention.

IMPORTANT NOTE: Lingering in the aisles surrounding other exhibitors’ booths is prohibited. Exhibitors are not permitted to enter an unstaffed booth of another exhibitor at any time, and doing so may result in immediate dismissal from the meeting.

AAOS, its representatives and employees, shall have free access to any exhibit at all times.

Admittance to Scientific ProgrammingBased upon seating availability, exhibitors are allowed admittance to scientific sessions that do not require a ticket.

Exhibitors may attend instructional courses by purchasing a ticket at Attendee Registration Ticket Sales on the day the course is offered, based upon seating availability. Tickets will not be sold to exhibitors in advance. Ticket prices range from $70 to $80 depending on the length of the instructional course.

BadgesAll exhibitor representatives must register and wear the official exhibitor’s badge for admission to and while in the exhibit halls. For complete details regarding badges please refer to Page 10.

Booth ActivitiesExhibitors are permitted to demonstrate equipment and to make informational presentations regarding products or services in a manner which assures all exhibitor personnel and attendees are within the exhibitor’s booth space and not encroaching on the aisle or neighboring exhibits.

AAOS is the sole provider of Continuing Medical Education (CME) credits at the annual meeting between the hours of 7:30 AM to 6:00 PM. Continuing Medical Education credit cannot be provided for presentations in the exhibit halls or time spent viewing the technical exhibits.

Exhibitors are not permitted to use live or human tissue in any booth to demonstrate techniques or instrumentation; companies that process and market human or xenograf tissues may exhibit them in securely sealed packaging only, and cannot be removed from this packaging.

The use of magicians, costumed characters, dancers, games of chance, drawings or other entertainment of this nature is prohibited.

Only such activities that, in the sole discretion of the AAOS, are in keeping with the professional nature of the annual meeting will be permitted. Any questionable activities must have the written permission of the AAOS Exhibits Manager or they may be subject to termination on-site.

Booth PresentationsAAOS key society leaders are prohibited from giving formal presentations or talks in exhibit booths in the exhibit halls or from promoting exhibitor products in print or other advertisements at the AAOS Annual Meeting. AAOS key leadership includes:

President First Vice-President Second Vice-President Immediate Past-President Chief Executive Officer Editors-in-Chief of AAOS publications

Exhibitors may utilize the services of celebrities or other individuals who are employed as spokespersons for the exhibiting company; however, using such individuals will require written approval from AAOS. Please submit by the individual’s name along with the dates and times requesting their presence in the booth for approval to [email protected] by December 22, 2014. Allow 15 business days for AAOS to review and respond.

Diagnostic Imaging Equipment and LasersExhibitors will be able to display and demonstrate imaging equipment in their booth space. Exhibitors shall not demonstrate diagnostic imaging equipment using either ionizing or non-ionizing radiation. In addition, demonstrations of activated therapeutic lasers are prohibited.

Dismantle of ExhibitsDismantle or removal of exhibit or materials is prohibited before the official closing of the exhibit halls at 4:00 PM on Friday, March 27, 2015. Companies in violation of this rule will forfeit 25% of their accrued priority points for the following year’s space assignment. Repeat violations may result in the exhibitor not being permitted to exhibit at future annual meetings.

Drawings, Lotteries, Raffles or Contests Exhibitors are prohibited from holding contests, drawings, lotteries, or raffles held at any time or place during the annual meeting.

Equipment PassesPackages may not be removed from the exhibit halls at any time without completing an equipment pass. This includes hand-carried items such as cartons, luggage, and equipment. Passes may be obtained on-site from security guards or at the Exhibits Office.

Exhibitor LoungeAn Exhibitor Lounge will be provided for exhibitors to use during show days and hours. The Exhibitor Lounge is for use by registered exhibit staff only and cannot be used to meet with professional attendees.

FDA ComplianceAll products that are not FDA approved for a particular use in humans or which are not commercially available in the United States will be permitted to be exhibited only when accompanied by the appropriate signs that indicate the product’s FDA clearance status. Requests for information and guidance can be obtained at www.fda.gov.Following are sample signs that should be displayed:

• This product is not cleared by the FDA for distribution in the United States

• This product is intended to be used in the United States as described in the product’s labeling

Signs must be easily visible and placed near the product, and on any graphics depicting the product. The size of text must be a minimum of 1/8 inch in height or larger so that the sign is visible from a distance of four feet from the display/product. The AAOS Exhibits Committee will conduct a walk-through of the exhibit halls to monitor exhibitors’ compliance to FDA and AAOS requirements before the halls open on Wednesday, March 25, 2015.

Final ProgramThe Final Program will include an alphabetical listing of exhibitors as well as a listing by products and services. Each exhibitor will receive between one to fifteen Final Programs, based on the net square feet of exhibit space. The programs will be available for pickup at Exhibitor Registration. Final Programs are not available prior to the meeting, however, the Final Program information will be available in March on the AAOS web site: www.aaos.org/amprograms.

CateringExhibitors will be permitted to distribute food and beverage items from their booth provided items are ordered through the Venetian/Sands EXPO’s exclusive food and beverage contractor, SES (Specialized Event Services). Examples of items allowed include cookies, granola bars, ice cream, water, soda, and beverage stations. Popcorn and alcoholic beverages are not allowed to be distributed to attendees. Additional information and catering order form will be included in the Exhibitor Service Manual which will be available in October, 2014. Hand washing stations will be required if any unpackaged food is being hand served. The station consists of a 5 gallon container of hot water, paper towels, soap and drain bucket. These items will need to be placed within an exhibitor’s assigned booth space and any counters or tables will need to be provided by the exhibitor.

Gift and GiveawaysExhibitors are permitted to distribute product samples from their exhibit booth during the meeting and these items do not require approval by AAOS. In addition, the following non-product items do not require approval from AAOS: writing pens, pencils, pads of paper, rulers, tape measures, luggage tags and food and beverage items ordered from the Venetian/Sands EXPO’s exclusive food and beverage contractor, SES (Specialized Event Services).

A form will be included in the Exhibitor Service Manual for exhibitors to request permission to distribute non-product items not detailed above. These premiums should be items that can be used during the meeting or in the professional activities of the attendee and have a cost per item of $25 or less. Golf balls, pocket knives, T-shirts, hats, lanyards and tote bags are items that will not be approved.

Exhibitors are prohibited from holding contests, drawings, lotteries, or raffles held at any time or place during the annual meeting.

Exhibitors may not distribute adhesive backed decals or stickers, unofficial badges, pins, buttons, or lanyards.

Large shopping bags or tote bags are strictly forbidden. Small paper or plastic bags for distributing company literature or products are acceptable if no larger than 14” x 11” including the handle. A sample or drawing with the dimensions must be submitted to [email protected] by December 22, 2014 for review and approval.

AAOS reserves the right to grant or deny permission to distribute souvenirs or other free or special convention offers of any type.

Press Credentials and GuidelinesBefore receiving a meeting press pass, media representatives must register with the AAOS Public Relations Department and provide credentials, such as a driver’s license with a business card, letter of assignment, and/or samples of published works. Details on media registration will be available in the Exhibitor Service Manual which is available October, 2014.

Although reporters are welcome at the AAOS Annual Meeting, audio and video recordings of educational content are not permitted. Camera crews and photographers must be escorted by AAOS Media Relations Staff at all times. Request for general, large picture shots that depict the scope of the meeting should be made in writing to AAOS media relations staff at [email protected].

Photography, Video and Audiotape ActivitiesAAOS has designated an official photographer to handle all photography within the exhibit halls and an order form will be included in the Exhibitor Service Manual. Except for services ordered through the official photographer, exhibitors must have prior written approval for any photographing, videotaping or audio taping activity in the exhibit halls. Requests must be submitted in writing by December 22, 2014 to [email protected]. Provided shots are taken by a full-time company employee, exhibitors may photograph and videotape their booth from the confines of their booth space during non-exhibit hours when attendees are not in the exhibit halls.

Companies in violation of these rules will forfeit 25% of their priority points for the following year’s space assignment. During the meeting, exhibitors should notify the AAOS Exhibits Manager if they believe their booth is being photographed or videotaped by another exhibitor or other party. During the meeting AAOS reserves the right to photograph and/or videotape exhibits and/or attendees through the services of an AAOS official vendor. Some of these photos and/or videos may be available on the AAOS website as b-roll for general use by others or displayed in publications or material related to the meeting. If you do not wish your image to be displayed by AAOS, please submit your request in writing by December 22, 2014 to [email protected].

Porter ServiceBooths that distribute products or giveaways that generate trash will need to order porter service to remove empty cartons and/or containers. Any exhibitor disposing of cartons, containers or other trash in the aisles will be charged for removal and cleaning. A form to order service will be included in the Exhibitor Service Manual.

Products DisplayedExhibitors are required to submit a list of any new products and services planned for display that have not been approved or exhibited at any prior AAOS Annual Meetings by December 22, 2014 to [email protected]. New products refer to completely new product categories, not revisions or improvements to previously approved product categories. The Exhibits Committee may require additional reports, literature or other materials relating to the exhibitor’s claims for the efficacy and safety of such products and services. Failure to comply with this provision shall, at AAOS’s option, void this application.

Publicity and Advertising The use of the name, insignia, logo, logotype or other identifying marks of the American Academy of Orthopaedic Surgeons/American Association of Orthopaedic Surgeons is never to be used in signs, advertising or promotions in any media or on descriptive product literature.• Companies are prohibited from promoting any specific

AAOS Annual Meeting education session, including paper presentations, posters or scientific exhibits.

• Companies are prohibited from using AAOS’s name in any advertising for meetings that are sponsored by another organization or at other meetings sponsored by AAOS.

• The fact that an exhibitor or its goods or services is or was exhibited at the AAOS Annual Meeting cannot be used in advertisements or promotional activities by the exhibitor.

• AAOS does not endorse, either expressly or by implication, the exhibitor or its goods or services.

Selling and Order TakingThe purpose of the exhibits is to further the education of meeting attendees through product and service displays and demonstrations. Sales and order taking are permitted if transactions are conducted in a manner consistent with the professional nature of the meeting. The exhibitor is responsible for permits and paying sales tax, if applicable. AAOS reserves the right to restrict sales activities that it deems inappropriate or unprofessional.

Exhibitors that sell items for delivery on the show floor may be required to obtain city occupational licenses. Additionally, they may be required to collect and remit city and state sales taxes.

Sound and Odor RestrictionsSound systems for formal demonstration areas must be directed into the exhibit space, not facing out toward the aisles, and sound and noise may not exceed 85 decibels. Exhibitors that use or display noisy devices, sound-producing equipment or other devices that prove objectionable to AAOS because of noise, odor or other disagreeable features, must agree to keep the noise and/or odor of such devices at levels determined acceptable by the AAOS Exhibits Manager or be discontinued. Only recorded music can be played in booths and exhibitors should be aware that they may be subject to laws governing the use of copyrighted compositions. License agreements for music covered by the American Society of Composers, Authors, and Publishers (ASCAP), Broadcast Music International (BMI), and other organizations are the sole responsibility of the exhibitor.

Staffing of ExhibitsExhibit booths must be staffed during all exhibit hours. An exhibit company that has failed to set up or properly staff its booth during the exhibition will forfeit all exhibit rights for this annual meeting.

Subletting of Booth SpaceExhibitors may not let, sublet, or transfer the exhibit privilege or space in whole or in part.

Temporary Staff/ModelsTemporary staff/models are considered company personnel and must be registered and wear a company exhibitor badge for entrance to the exhibit halls. These badges must be registered within the exhibitor’s allotment of badges.

Models under the age of 16 are prohibited unless advance arrangements have been approved in writing by the AAOS Exhibits Manager.

Attire of models shall be consistent with the professional atmosphere of the AAOS Annual Meeting. Models are allowed to wear sports attire including sweat suits, shorts and T-shirts. Tight fitting or other inappropriate garments, which include T-backs and short-shorts, will not be permitted on the exhibit floor.

It is the responsibility of the exhibitor to ensure that all company personnel or third party agents that are involved in meeting or travel arrangements are aware of and adhere to AAOS rules and regulations contained in the AAOS Exhibit Prospectus. Those relevant to meeting space or exhibitor events taking place outside of the exhibitor booth space are provided below for your reference. Any exhibitor planning on holding an event or function that will take place outside of their assigned exhibit booth space during the AAOS Annual Meeting must secure approval from AAOS.

Approval of Events and MeetingsAll requests must be submitted to AAOS for approval on the Meeting Space Application which is available online at www.aaos.org/exhibits.

Assignment of Meeting Space• Meeting space is assigned on a first-come, first-served basis.

Hotels may assess room rental charges to events that do not meet required food and beverage minimums.

TIP: To avoid room rental charges, order minimal amounts of F&B (e.g. soft drinks, cookies, fruit, etc.) for your functions.

• Once space is assigned, the organizer is responsible for finalizing all arrangements directly with the assigned hotel. Any penalty incurred between the exhibitor and the hotel is the responsibility of the exhibitor. It is understood that AAOS accepts no responsibility for the event(s).

• Meeting space is not available at the Venetian/Sands EXPO or the Venetian/Palazzo Hotel.

• AAOS does not guarantee that an exhibitor’s sleeping rooms will be assigned in the same hotel as the exhibitor function.

• AAOS does not endorse exhibitor’s function and does not consider them a part of the official program.

Cancellation of Meeting SpaceMeeting space canceled after December 1, 2014, for functions other than Educational Events/Satellite Symposia will be subject to a $250 cancellation fee if AAOS is unable to reassign that space. The fee will be invoiced by and made payable to AAOS. If exhibit booth space is canceled, AAOS will automatically cancel meeting space.

Company/Staff MeetingsFor events that only include staff of the exhibiting company, there are no date or time restrictions; however, these events require approval from AAOS. These meetings cannot include AAOS attendees unless the individual is an employee of the company holding the event.

Educational EventsAAOS charges a non-refundable fee to exhibitors holding educational functions that include AAOS attendees. Only companies exhibiting at the AAOS meeting are eligible to present educational events/satellite symposia to AAOS attendees. Third party organizers must be designated by the exhibitor on the exhibitor’s stationery.

Administrative Fee$5,000 – For up to 3 events with a total attendance of 150 attendees or less

$7,500 – For up to 3 events with a total attendance of 151 attendees or more

The fee includes: Meeting space for up to three (3) events, and listings in the AAOS Exhibitor Directory, the daily edition of AAOS Now, and on the AAOS Annual Meeting website.

Badges & Bar CodesExhibitors are not allowed to use AAOS official badges for admission to exhibitor events, nor scan and capture AAOS badge bar code data at any non-AAOS meeting or event.

CME AccreditationAAOS does not provide Continuing Medical Education accreditation for exhibitor educational events. Exhibitors wishing to provide credit for their educational event will need to obtain credit from other accredited CME providers.

Hospitality SuitesHospitality suites are only available to companies that exhibit at the AAOS 2015 Annual Meeting. Requests for suites are made through the AAOS housing bureau at the same time that sleeping room requests are made. Hospitality functions cannot conflict with the official AAOS scientific programming, and may only be scheduled as indicated.

Publicity and Advertising The use of the name, insignia, logo, logotype or other identifying marks of the American Academy of Orthopaedic Surgeons/American Association of Orthopaedic Surgeons is never to be used in signs, advertising or promotions in any media or on descriptive product literature.• AAOS does not publish exhibitor social events in the Final

Program. • Companies are prohibited from promoting any specific

AAOS Annual Meeting education session, including paper presentations, posters or scientific exhibits.

• Companies are prohibited from using AAOS’s name in any advertising for meetings that are sponsored by another organization or at other meetings sponsored by AAOS.

• The fact that an exhibitor or its goods or services is or was exhibited at the AAOS Annual Meeting cannot be used in advertisements or promotional activities by the exhibitor.

Surgical Skills CoursesA hotel, or similar facility, is an inappropriate setting for surgical skills courses using cadaveric specimens due to the difficulty involved in complying with safety recommendations and requirements. Surgical skills courses utilizing cadaveric specimens are restricted to a clinical setting.

Third Party OrganizersThird party organizers must be designated by the exhibitor on the exhibitor’s stationery and forwarded to [email protected], before any investigation of space can begin. Sleeping room requirements for third party individuals must be reserved through the exhibitor’s housing allotment.

Transportation for Exhibitor FunctionsAAOS shuttle buses cannot be used to transport attendees to or from exhibitor functions. Exhibitor-sponsored shuttle buses may not utilize AAOS shuttle bus drop-off or pickup areas at the Venetian/Sands EXPO. To prearrange a shuttle bus pick up and drop off location at the Venetian/Sands EXPO please send your request to [email protected].

HOUSING INFORMATIONExhibitors must obtain sleeping rooms through AAOS’s housing reservation service. Exhibitors or their agents may not negotiate blocks of rooms directly with the hotels in the Las Vegas area or with hotels for future AAOS Annual Meetings. Exhibitor rooms are for the purpose of housing exhibitor staff, and are not to be used to reserve sleeping rooms for medical attendees.

Misuse of AAOS room blocks by an exhibitor that results in a walk situation of a meeting attendee at that hotel will result in a priority point penalty. If such a violation should occur, the company will lose 25% of their priority points. This action would impact exhibit booth assignments at the AAOS 2016 Annual Meeting.

Housing AssignmentsComplete housing information and instructions to reserve sleeping rooms will be sent to exhibitors along with their booth assignments in August.

Rates and DepositsThe per night room rate ranges from $139 to $359. Specific information will be provided to all exhibitors in August.

Housing for International Medical AttendeesAAOS has designated an official international travel agent for the AAOS 2015 Annual Meeting that will make hotel arrangements for international medical attendees. AAOS will allow exhibitors to reserve a small number of exhibitor sleeping rooms along with the physician sleeping rooms. Further information will be included in the Preliminary Program and in the housing information that all exhibitors receive in August, 2015.

AAOS Housing BureauAAOS’s housing bureau, Wyndham Jade, will staff a housing desk at the Venetian/Sands EXPO during the annual meeting.

PIRATE HOUSING COMPANY WARNINGPlease beware of false and fraudulent housing claims. Exhibitors have reported receiving calls from companies claiming to be the official housing bureau for AAOS Annual Meeting. They are not associated with AAOS or Wyndham Jade in any way.

All hotel reservations are to be made directly with Wyndham Jade, the official AAOS 2015 Housing Bureau. Do not provide your credit card information to anyone other than Wyndham Jade/AAOS 2015 housing bureau. Wyndham Jade is the only official AAOS housing representative.

Note that these other companies:

• Often do not have blocks of rooms; they simply contact another source and resell a room from them at a higher rate.

• May require hefty deposit and full pre-payments along with steep change and cancellation fees.

• Provide no on-site support so if you do incur a problem while checking in and out you will have no help from these companies.

Shuttle Bus ServiceShuttle bus service will be provided between AAOS official hotels not within walking distance to the Venetian/Sands EXPO beginning Monday, March 23, 2015, and operate throughout the meeting dates. Specific details regarding AAOS shuttle bus schedule, which will be available in February, 2015, will be sent to exhibitors. The schedule will be available online at www.aaos.org/exhibits and on-site at the Venetian/Sands EXPO and at AAOS official hotels.

Hotel TV ChannelsThe use of in house TV channels at the hotels may be allowed provided the AAOS grants written approval. Exhibitor request should be sent in writing to [email protected].

BADGE REGISTRATION

Academy members requested that the number of exhibitor representatives on the exhibit floor be limited. Attendees were finding it difficult to navigate the aisles of the exhibit floor due to the large number of exhibitor representatives present. Therefore, exhibitors may register a maximum of five representatives for each 100 square feet of exhibit booth space. There will be no additional badges provided over this allotment.

Exhibitor Badge Registration• Complimentary online badge registration and badge name

changes will remain open through March 21, 2015. • If requested online, for US addresses only, badges registered by

February 4, 2015, can be mailed to the designated company contact person.

• It is the responsibility of the company contact person to distribute all their badges to their representatives, whether mailed or picked up onsite.

• Detailed information and instructions for exhibitor registration will be sent to exhibitors in October, 2014.

On-site Registration• On-site any remaining allotment of badges may be purchased

for a fee of $50 each and badge name changes may be made for a fee of $50 each.

• Representatives registering on-site and/or exchanging a badge for a new name badge will be required to show proof of affiliation (business card) with the exhibiting company and a photo ID.

Exhibitor Badge PoliciesCompany badges will not be accepted instead of the official badge. Supplementing AAOS badges with business cards or altering, adding to or defacing the official badge is not permitted.

All booth personnel must wear an official AAOS exhibitor badge to gain access to the exhibit halls.

• Exhibitor badges are to be issued only to company employees, and/or any other individuals working in the exhibit booth.

• False certification of an individual as an exhibitor’s representative, or any other method used to assist unauthorized personnel to gain admittance to the exhibit floor and/or instructional courses will be cause for expelling the violator from the exhibition, or removing a violator’s exhibit from the exhibit floor without obligation on the part of AAOS.

The exhibitor’s badge allows admittance to the exhibit halls, and on a space available basis to the scientific exhibit, poster presentations, general sessions, symposia, and paper presentations. If available, instructional course tickets may be purchased on-site at Attendee Registration Ticket Sales on the day of the instructional course.

No one under 16 years of age will be allowed in the exhibit halls.

Commercial Representatives The AAOS Annual Meeting is a private meeting for medical professionals. However, after review and approval, AAOS does allow certain qualifying individuals to attend the meeting under the registration category of Commercial Representative with admittance to the exhibit halls only. Qualifying individuals are:

Cancellation of ExhibitionIf AAOS fails or is unable to fulfill its obligations in providing the opportunity to hold exhibits at the annual meeting, AAOS agrees to promptly return to the exhibitor all monies paid. If this occurs, any agreements made shall be canceled by mutual consent and AAOS shall be relieved of all responsibility.

It is mutually agreed that in the event the AAOS 2015 Annual Meeting is canceled due to disasters, strikes, governmental regulations, or other causes that would prevent its scheduled opening or continuance, then this agreement will be terminated and the AAOS shall determine an equitable basis for the refund of such portion of the exhibit fees as possible, after due consideration of expenditures and commitments already made.

This Agreement shall be construed in accordance with and governed by the laws of the State of Illinois. The Parties agree to the jurisdiction and venue of the Circuit Courts of Cook County, Illinois, or the United States District Court of the Northern District of Illinois, for resolving disputes arising under this Agreement.

Cancellation of Exhibit SpaceExhibit space assigned to an exhibitor by AAOS will not be released unless AAOS has been notified on a timely basis in accordance with AAOS rules. This notice shall be in writing on company letterhead and shall state that the exhibitor wishes to cancel or reduce its assigned space. Cancellation fees will be charged as outlined on Page 6.

Compliance with Local OrdinancesLicenses and permits required by local statute, ordinance or regulation (if any) are to be obtained and paid for by the exhibitor. Each exhibitor will be responsible for compliance with local health, fire, and safety ordinances and regulations. All products or services exhibited must comply with all state and local regulations, and with all current FDA regulations for such products and services as highlighted on page 8. AAOS has no further responsibility to notify the exhibitors that this compliance is required.

Damage to Venetian/Sands EXPO Exhibitors will be held responsible for any damage done to the Venetian/Sands EXPO by them, their employees or agents. No nails, tacks, or screws may be driven into the floor, wall, or woodwork of the building.

DisputesAAOS will not settle disputes between exhibitors regarding trademarks, copyrights, patents or other product issues.

Fire Ordinances Exhibitors must strictly observe all city, state, and federal fire laws. Complete fire safety requirements will be included in the Exhibitor Service Manual. Demonstration areas shall not be placed on the aisle line of an exhibit. Sufficient space within an exhibit area must be left to absorb the crowd. Should spectators interfere with the normal traffic flow in the aisle or interfere with other exhibits, AAOS may, in its sole discretion, require that the demonstration be limited or canceled.

IndemnityThe exhibitor agrees to indemnify, defend, and hold the American Academy of Orthopaedic Surgeons/American Association of Orthopaedic Surgeons, its official contractors, and the Venetian/Sands EXPO, and their respective employees, representatives, agents, successors and assigns, harmless against any and all

damages, claims, judgments, losses, costs and expenses (including attorneys’ fees) that may at any time be incurred, suffered, sustained by or imposed upon AAOS and/or the Venetian/Sands EXPO or their respective employees, representatives, agents, successors or assigns by reason of any action which may result because of or after acceptance of this application or the providing of exhibit space. This indemnification and hold harmless agreement includes, but is not limited to, any and all claims, damages, losses or expenses attributable to personal or bodily injury, sickness, disease or death, or to damage to or destruction of tangible property, including the loss of such property.

InsuranceInsurance protection will not be afforded to the exhibitor either by AAOS or the Venetian/Sands EXPO. Exhibitors shall carry their own insurance to cover exhibit material against damage and loss, and public liability insurance of at least $1 million per occurrence and $1 million aggregate, against injury to the person and property of others.

The exhibitor shall, at its sole cost and expense, procure and maintain through the terms of the contract for exhibit space, workers’ compensation insurance in full compliance with all federal and state laws, including any state required limits, governing all of the exhibitor’s employees or agents engaged in the performance of any work for the exhibitor.

Policies shall name AAOS as a named additional insured. If requested by AAOS, exhibitor may be required to obtain and furnish AAOS with a certificate of insurance evidencing the required insurance.

Interpretation of Rules AAOS in determining whether to accept an application for exhibition space in any subsequent year may consider breaches or infractions of these terms, rules and regulations by an exhibitor in any year. In addition, infractions of the spirit of the rules by exhibitors or potential exhibitors at any time may be considered in determining whether to accept an application from such person or company seeking to exhibit at a future AAOS Annual Meeting.

Termination or Modification of Display Privileges at Any TimeAAOS reserves the right to terminate or modify the exhibitor’s display privileges at any time for a breach of any of these terms, rules and regulations. AAOS will be the sole interpreter of whether a breach has occurred. Such termination or modification may occur during the annual meeting, at which time the exhibitor must agree to close the exhibit upon receipt of a notice of termination and to remove or modify the exhibit from the exhibition hall as soon as possible without disruption of the meeting. Such removal shall be done under the direction of the AAOS Exhibits Manager. Expulsion or modification of an exhibit under this rule shall not give rise to any claim, and there shall be no refund of the fees paid by such exhibitor.

Use of Certain Property The exhibitor will assume all costs arising from the use of patented, trademarked, franchised, or copyrighted music, materials, devices, processes or dramatic rights used on or incorporated in the exhibitor’s booth. License agreements for music covered by ASCAP, BMI and other organizations are the sole responsibility of the exhibitor.

Reservation of Right to Make Changes Any matters not specifically covered herein are subject to decision by AAOS. AAOS reserves the right to make such changes, amendments and additions to these rules as it considers advisable for the proper conduct of the annual meeting, with the provision that all exhibitors will be advised of any such changes.

*Any required Freeman services for Sunday, March 22, 2015, must be ordered by Friday, March 20, 2015, at 12 Noon. Direct to dock shipments will not be accepted on Sunday, March 22, 2015.

Violation of Rules and RegulationsAs a condition for exhibiting, each exhibitor shall agree to observe all rules and regulations. The action taken against an exhibitor for violation of rules and regulations will be determined on the basis of the particular circumstances of each case. Some infractions will be communicated in a warning notice that may not result in a violation if in the sole discretion of the AAOS Exhibits Manager it is deemed that corrective measures were completed within a timely

fashion and no other parties were negatively impacted. Prior years’ warnings and penalties may be taken into account in assessing penalties for the 2016 Annual Meeting. AAOS reserves the right to levy a more severe penalty, including refusal of or termination of the exhibit, at its sole discretion without progressing through each of the following successive steps. In the event of such restriction or eviction, AAOS will not be liable for any refunds on rentals or other exhibit expenses. For most violations, following are the penalties that will be enforced:

EXHIBITS SCHEDULE

Installation of Exhibits A target move-in schedule has been adopted for the installation of exhibits, based upon booth size and location. Exhibitors must adhere to their assigned target date for delivery of direct shipments to the Venetian/Sands EXPO. This schedule will be detailed in the Exhibitor Service Manual.

Booths must be set by 5:00 PM on Tuesday, March 24, 2015. If an exhibit is not in the process of being set up by 3:00 PM on Tuesday, March 24, 2015, AAOS may, but shall not be obligated to, order the exhibit erected and the exhibitor billed for all charges incurred. Neither AAOS nor the official service contractor shall be responsible for any cost or damages incurred to said property. Throughout installation, all crates must be labeled for storage as soon as they are empty. For booth spaces 600 sq. ft. and larger, all empty crates, cases and containers must be tagged for removal by 7:00 PM on Monday, March 23, 2015.

Exhibitors that want to request a change in their target move-in time will need to complete and submit to Freeman, by February 19, 2015, the Freight Information & Target Change Request Form that will be included in the Exhibitor Service Manual.

On-site at the meeting exhibitors that require extended installation hours should direct these requests to the AAOS Exhibits Manager no later than 2:00 PM on the day that additional installation time is requested. Exhibitors may be required to hire a security guard for any approved additional hours they will be working in their booth outside of established installation hours.

Dismantle of ExhibitsDismantling of exhibits will start on Friday, March 27, 2015, at 4:00 PM and end on Sunday, March 29, 2015, at 2:00 PM. All exhibits must remain intact until Friday, March 27, 2015, at 4:00 PM, the official closing time, or be subject to penalties enforced by AAOS. Exhibitors should remain with their display materials until crates are returned and all materials are packed. Exhibitors are reminded that the return of empty crates, containers and cartons will take a minimum of eight to ten hours after the close of the show. Please plan your departure schedule accordingly.

All exhibits must be down and packed with an outbound bill of lading turned in to the Exhibitor Service Center by Sunday, March 29, 2015, by 2:00 PM. Any materials left on the floor for which an outbound bill of lading has not been submitted by 2:00 PM on Sunday, March 29, 2015, may be re-routed onto another carrier at the exhibitor’s expense. Neither AAOS nor the official service contractor shall be responsible for any costs, damages or losses incurred.

Exhibit Hall AccessExhibitors have access to the exhibit halls during installation and dismantle hours, and for show days at 6:30 AM on Wednesday, March 25, 7:00 AM on Thursday and Friday March 26-27.

Requests for exhibit hall access before or after the established times must be approved by the AAOS Exhibits Manager. Security will not allow access to the exhibit halls outside of the scheduled hours without prior written approval from the AAOS Exhibits Manager.

Americans with Disabilities ActExhibitors shall be responsible for compliance with the Americans with Disabilities Act with regard to their booth space, including, but not limited to wheelchair access. Further information regarding ADA compliance is available via phone at 800-514-0301 or at www.usdoj.gov/crt/ada/infoline.htm.

Booth RequirementsAll exhibits must be confined to the space limits of their respective booth(s) as indicated on the floor plan. All equipment, products, or materials to be displayed or demonstrated must be placed within the exhibit space in order to ensure that the attendee viewing the exhibit will stand within said booth space and not in the aisles.

All exposed parts of displays must be finished to present an attractive appearance when viewed from the aisles or from adjoining exhibits.

Linear Exhibit Booths Linear booths are 100 feet square (10’ x 10’) or multiples thereof, and consist of 8 feet high back drape and 36 inch high side dividers mounted on aluminum tubular frames. Signs, decorations and equipment may not rise above 8 feet 3 inches or be placed outside the booth space. Solid construction over 4 feet high must be a minimum of 5 feet back from the aisle. Exposed unfinished sides of the exhibit must be draped to present an attractive appearance. Hanging signs are not permitted. All booths are identified with a booth number label that is affixed on the aisle carpet.

NEW Due to facility requirements, booths with solid, hard back walls must have a minimum 18 inches of clearance along the back wall for utility access.

Island Exhibit BoothsAn island exhibit is an open area of exhibit space with aisles on all four sides. Island exhibits may extend to all outer edges of the booth, but must provide for attendee access into the booth from all four aisles.

• Aisles may not be obstructed due to any activities within the booth:

h Counters must be placed a minimum of one foot in from all aisles.

h Formal presentation areas are required to be placed a minimum of five feet in from the aisles. Exhibitors are responsible for providing adequate space for presentations and monitoring attendance to prevent aisle congestion. Presentations will be limited or eliminated in situations where overflow continues to be a problem.

• Raised floors must include ramps for access to the booth space and raised areas that are not ramped must be clearly marked or lighted around the perimeter.

• Sound systems for formal demonstration areas must be directed into the exhibit space, not facing out toward the aisles, and sound may not exceed 85 decibels.

• Neither identification sign nor pipe and drape are provided for island exhibits. All exhibits are identified with booth number labels that are affixed on top of the aisle carpet.

• Floor plans and elevation drawings for island exhibits must be submitted to AAOS for approval by December 22, 2014. A form will be provided in the Exhibitor Service Manual. Island booth floor plans are required to ensure that AAOS rules and regulations are followed and provide AAOS with the ability to resolve potential issues before show site. Floor plans should be drawn to scale and include all exhibit components and their dimensions. If not submitted and the construction is determined to be in violation of booth requirements, AAOS has the right to prohibit assembling of the booth. Island exhibits are subject to show site review and modification where necessary, at the exhibitor’s expense.

Multi-level Exhibit BoothsA multi-level exhibit booth is an island exhibit booth with a second level. All requirements for island exhibit booths (see above) apply to multi-level exhibit booths.

Height LimitsLinear Booths: 8 feet, 3 inches Island Booths: 25 feet

Mobile Units or TrucksExhibitors that wish to display mobile units or trucks on the exhibit floor must request approval from the AAOS Exhibits Manager. Requests should be sent to [email protected] by August 4, 2014, and must include specific space requirements. Mobile units or trucks for display in the exhibit halls will only be located in designated areas and cannot be located in regular exhibit booth space. Mobile units must be in compliance with fire prevention codes according to Venetian/Sands EXPO rules and regulations and the Americans with Disabilities Act.

Peninsula or End-Cap BoothsAAOS does not assign peninsula nor end-cap booths. A peninsula booth is bounded by three aisles; an end-cap booth is a linear 10’ x 20’ booth spanning two different aisles.

LightingExhibitor lighting must be limited to the confines of the booth space. Lighting that projects onto other exhibitor’s space or AAOS aisles or otherwise detracts from the professional atmosphere of the event will not be allowed. Lighting that spins, rotates, pulsates, or other specialized effects must be submitted for approval to the AAOS Exhibits Manager by December 22, 2014. Approval will be granted on a case-by-case basis upon inspection on-site at the Venetian/Sands EXPO.

Island booth exhibitors that wish to have facility lights turned off that are within the confines of their booth space must submit requests to SES (Specialized Event Services) by December 22, 2014. No requests will be granted to turn off lights that project over aisle space or other exhibitors’ booths.

BOOTH CONSTRUCTION

Island exhibits that include lights hung from an overhead truss system or facility overhead catwalks and/or beams must be placed within the perimeter of the exhibitor’s booth space.

The exhibitor is responsible for any costs related to lighting installation and dismantle, including any lights which may be denied approval on-site due to infringing on AAOS aisles, or other areas outside the exhibitor’s booth space.

SignsOnly professionally printed signs are allowed in the exhibit halls. Forms for ordering signs will be included in the Exhibitor Service Manual.

Booth ID Signs: A 7” by 44” identification sign with company name and booth number will be provided for all linear booths at no charge.

Hanging Signs or Banners: Hanging signs/banners are only available for island booths, and may not exceed the perimeter of assigned booth space. The suspended height limit for signs is 25 feet when measured from the floor up to the top of the sign. Freeman will assemble install/dismantle any non-electrical sign or decorative materials from the ceiling under 200 pounds. SES will be responsible for all other work components that pertain to overhead rigging.

Exhibitors, display companies and/or Exhibitor-Appointed Contractors may supervise, but will not be allowed to assemble or install a hanging sign. Hanging signs must be assembled and installed by Freeman. An order form will be included in the Exhibitor Service Manual. Hanging signs must be received at the warehouse by March 12, 2015, using the “Hanging Sign” shipping label that will be included in the Exhibitor Service Manual.

Floor Covering and CarpetBooth floor covering is mandatory and is the responsibility of the exhibiting company. Floor covering or carpet may either be supplied by the exhibitor or ordered from Freeman. All exhibit booths without floor covering or objectionable to AAOS shall, at AAOS’s discretion, be carpeted and/or corrected at the exhibitor’s expense.

NEW Materials used for floor covering must be slip resistant.

All aisle carpet will be provided by AAOS, and the carpet color will be black and white tuxedo.

Facility Fire/Safety GuidelinesThe Sands EXPO follows the Clark County Fire Department (CCFD) requirements regarding acceptable booth configurations from a safety standpoint.

• All exhibit construction and decoration materials must be flame retardant.

• Storage of any kind behind back drapes, display walls or inside the display areas is strictly prohibited.

• Fire extinguishers and fire hose cabinets which may be located on a column must remain visible and accessible at all times.

• All fully-enclosed booths (consisting of four walls and no ceiling) must include a smoke detector and fire extinguisher for each enclosed area.

• Booths that have multiple levels or rooms with enclosures are required to post visible notification on the stairway(s) or outside the room. This notice must state the maximum occupancy permissible or total weight load allowable on the second story at one time. If the second level occupancy level allows for more than 10 people at one time or if the total area is larger than 300 square feet, two (2) stairwells are required.

• Hard wall, inline booths must have a minimum 18” of clearance along the back wall for utility access.

The CCFD requires that covered and/or multi-story exhibit booths submit floor plans for review and approval. Plans for all multi-level booths must stamped by a licensed structural engineer. Union Jurisdictions at the Venetian/Sands EXPOFull time employees of the exhibiting company may set up and take down their display. All work performed must be within the confines of the exhibitor’s booth space. If full-time company employees are utilized to set an exhibit, they must carry company identification such as a medical identification card or payroll stub.

Any labor services that may be required beyond what a company’s regular full time employees can provide must be rendered by the local authorized union personnel. The use of workers hired from a non-union agency or company is prohibited.

Teamsters Union Local #631 has jurisdiction via a labor agreement with Freeman for the erection, touch-up, dismantling and repair of all exhibits when this work is done by persons other than your full-time company personnel.

Local #631’s jurisdiction does not cover the placement of your products on display, the opening of cartons containing your product, or the performance testing, maintenance or repairs of your products.

Labor will be available based upon advance orders. Following are the applicable rates:

Cleaning ServicesAAOS will provide cleaning service for all aisles. Cleaning service for individual booths will be provided upon request from the individual exhibitor at the exhibitor’s expense. AAOS has the right to order cleaning at the exhibitor’s expense if booth appearance is unsightly.

SES is the exclusive cleaning contractor at the Venetian/Sands EXPO. No other contractor or cleaning service will be allowed on the exhibit floor.

Electrical Services SES (Specialized Event Services) is the exclusive provider of electrical services at the Venetian/Sands EXPO. Installation and connection to all electrical services must be made by SES.

Electricity is located overhead in the exhibit halls. For linear booths, power will be placed at the back of the booth behind the drape line. If additional power locations are needed within a booth, installation will be done on a time and material basis.

Sample advance electrical charges, excluding labor charges and subject to change, are as follows:

NOTE: SES has equipment which ensures that an exhibitor’s electrical usage does not exceed the service ordered. Should the electrical usage exceed the service ordered, the circuit will need to be reset. within the limit of the original circuit purchase.

Electrical labor charges will be in a minimum of a one-hour labor installation charge for all booths requiring labor. Labor to disconnect will be at one-half of the installation time. Advance labor charges, subject to change, are as follows:

Prewire and Save! Considerable on-site cost savings can be realized by prewiring and preconnecting electrical equipment. Wherever possible, install standard “twist-lock” receptacles or similar “quick-disconnect” devices at your plant.

Make sure all connections are sealed and that there are no live exposed metal parts that may cause shock or electrical hazard.

Porter ServiceSES is the exclusive cleaning contractor. If an exhibitor has giveaways or other booth material items that generate trash, they will need to order porter service to remove empty cartons and or containers. Any exhibitor disposing of cartons, containers or other trash in the aisles will be charged for removal and cleaning. Following are sample advance rates, subject to change.

SecurityAAOS will provide 24-hour security to control access to the exhibit halls from the start of move-in to the end of move-out. However, AAOS and the Venetian/Sands EXPO will not be held responsible for the loss or damage to exhibitor property. Exhibitors are urged to take precautions to protect their own property to discourage theft, and to arrange for appropriate insurance against loss.

Telecommunications ServicesSES is the exclusive supplier of telecommunication services. Sample advance rates, subject to change, are as follows:

Official Material Handling ContractorFreeman has been contracted to be the exclusive provider for material handling services. Freeman has the responsibility to manage all freight docks/doors and to schedule all vehicles into and out of all designated material handling areas of the AAOS Annual Meeting.

Freeman has the sole responsibility for loading and unloading all trucks, trailers, common and contract carriers at its facility or designated material handling areas.

All shipments must be prepaid. Collect shipments will not be accepted. Complete details including schedules and shipping instructions will be included in the Exhibitor Service Manual.

Accessible StorageAn area for accessible storage of sample materials and other items will be available at the Venetian/Sands EXPO. Labor charges are the responsibility of the exhibitor. Rates for accessible storage are as follows and are for the duration of the show:

$51.40 per skid plus labor, ½ hour labor (two person minimum crew)

Advance ShipmentsMaterial handling rates for advance shipments are:

Loose/uncrated shipments will not be accepted at the advance warehouse. Rates are round trip and inclusive of overtime move-in and move-out and any applicable special handling surcharges.

Advance shipping rates include receiving of crated shipments at the warehouse, up to 30 days storage, delivery to the booth during designated move-in days and hours, handling and storage of empty crates, and reloading for outbound shipment during the designated move-out days and hours.

Mixed ShipmentsA mixed shipment is defined as shipments of mixed crated and uncrated goods, where the percentage of uncrated is minimal and does not warrant the full uncrated rate for the shipment, but does require special handling. Freeman defines special handling for mixed loads as having less than 50% of the volume as uncrated. Mixed shipments will be charged at 50% the rate for crated materials and 50% the rate for loose/uncrated materials unless certified weight tickets accompany the shipment that specify the separate weights of the crated and loose/uncrated portions of the shipments.

Return ShipmentsArrangements for all outbound shipments must be made with Freeman prior to the close of the show. A return material handling order form will be required for each outbound shipment at the close of the show. Outbound material handling order forms should not be left in exhibit booths or on materials to be shipped. All material handling forms should be delivered to the Exhibitor Service Center as soon as the materials are ready for shipment. If shipments are to be sent by a specific carrier, that carrier must be indicated on the material handling form. If no carrier is indicated on the material handling order form, the official material handling contractor will select the most convenient carrier available. Official carrier representatives will be on-site to discuss shipping needs.

DeliveriesDeliveries must be sent through the Venetian/Sands EXPO loading dock, as delivery personnel will not be badged and cannot gain access to the exhibit floor.

Marshaling YardA vehicle marshaling yard will be utilized for the AAOS 2015 Annual Meeting to ease congestion at the Venetian/Sands EXPO and maximize the utilization of available dock space. All delivering carriers and trucks, including privately owned vehicles (POV’s) must check in first at the marshaling yard to be assigned a number and position to be unloaded. This same process will be in effect during the move-out of the show.

MATERIAL HANDLING AND SHIPPING

Guidelines for Use of Exhibitor-Designated Contractor (EDC)Exhibitors may use the services of an outside independent contractor only for the installation and dismantling of exhibits. All other services must be secured through the official contractors.

By December 22, 2014, exhibitors planning to use the services of an outside independent contractor for set up and/or dismantle must notify the AAOS by completing the “Request for Use of Exhibitor-Designated Contractor” form included in the Exhibitor Service Manual. Exhibitors not providing this request by the deadline date will be required to use labor provided by the official contractor for installation and dismantling.

The exhibiting company is responsible for the actions of their independent contractors. The exhibitor agrees to indemnify and hold harmless AAOS and the Venetian/Sands EXPO (and all related organizations) for any and all expenses (including attorney fees, judgments, demands, claims or other amounts) resulting from damage or death caused in any part by the exhibitor’s appointed contractor. AAOS reserves the right to remove exhibitor-designated contractors whose actions jeopardize the on-time opening of the exposition or whose employees fail to observe the regulations and procedures in the Exhibit Prospectus and the Exhibitor Service Manual.

Upon receipt of the Request for Use of Exhibitor-Designated Contractor form, AAOS will send directly to the contractor specified information detailing AAOS requirements and policies. Exhibitors should note that the Exhibitor Service Manual will only be emailed to the participating exhibitor, and it is the exhibitor’s responsibility to provide third parties with the necessary information and forms from their manual.

Exhibitor-Designated Contractors must:• Provide an original Certificate of Insurance to the AAOS

Exhibits Assistant by December 22, 2014, that: h Indicates the policy will be in effect over all installation

and dismantling dates, March 19 through March 29, 2015.

h Indicates comprehensive general liability insurance against claims for bodily injury or death and property damage of not less than $1,000,000 for each occurrence, and an active workers’ compensation insurance policy covering all permanent employees and temporary labor hired to perform work during this event is in force over this same period.

h Names AAOS as certificate holder and as an additional insured.

• Upon arrival at the Venetian/Sands EXPO, show proof of affiliation with the exhibiting company or approved exhibitor-designated contractor to receive a work badge that will provide access to the exhibit halls.

• Not engage in solicitation of business on the exhibit floor for present or future conventions.

• Follow all of AAOS move-in and move-out rules in a timely and professional manner, and must perform duties within established AAOS deadlines.

• Adhere to deadlines. Deadlines are not negotiable. If Exhibitor-Designated Contractor information is not received by the deadlines listed the exhibitor will be required to use AAOS official contractors.

Exhibitor-Designated Contractor (EDC) BadgesBadges for approved exhibitor-designated contractors will be available at the EDC security desk located at the Venetian/Sands EXPO. Unregistered exhibitor personnel and exhibitor-designated contractors that need access to the exhibit floor during installation and dismantling hours will be required to show proof of affiliation with the exhibitor or EDC to receive a badge. Note that no EDC personnel will be allowed on the exhibit hall floor for dismantling of booths prior to 4:00 PM on Friday, March 27, 2015.

Different badges will be provided for installation, dismantle, and show days only valid during non-show hours. Exhibitor badges will be required for admission to the exhibit floor on show days during show hours. Specific details on the process for obtaining these badges will be communicated to exhibitors and EDCs upon receipt and approval of the completed Request to Use Exhibitor-Designated Contractor (EDC).

These activities will be promoted to medical attendees before and during the meeting. • AAOS Electronic Skills Pavilion – Presentations focusing on technology will take

place in this pavilion located in the exhibit hall.

• AAOS Redemption Centers – Distribution of complimentary tote bags and drop off of tickets for drawings of other items will take place in designated AAOS Redemption Centers only located on the exhibit floor.

• Ask an Expert – Interactive learning sessions that take place on the exhibit hall floor where attendees are invited to bring case challenges for presentation to an expert in orthopaedics for diagnosis and recommendation.

• Beverage Breaks in Exhibit Halls – Complimentary beverage breaks will be provided during the 30-minute break between scientific sessions at 3:30 PM on Wednesday and Thursday, and at 10:00 AM on Friday.

• Unopposed Exhibit Time - One hour of unopposed exhibit time will be provided each exhibit day from 12:30 PM to 1:30 PM.
