Income Protection and Career Ending Injury Plan Options Income Protection Benefits and Premiums Below is the monthly Income Protection benefit You will receive depending upon the option that You have chosen. The below benefit will be paid to You if you suffer Temporary Total Disablement due to an Accident which occurs during the Operative Time whilst Your Policy is in force. Table 1:- Monthly Benefit Maximum Benefit Annual Premium £1,000. £12,000 £588.48 £1,500 £18,000 £882.96 £2,000. £24,000 £1,176.96 £2,500 £30,000 £1,471.44 £3,000 £36,000 £1,765.92 £3,500 £42,000 £2,059.68 £4,000 £48,000 £2,353.92 £4,500 £54,000 £2,648.64 £5,000 £60,000 £2,942.88 £6,000 £72,000 £3,602.09 £7,000 £84,000 £4,370.53 £8,000 £96,000 £5,294.58 All premiums stated above are inclusive of Insurance Premium Tax (IPT) at 6%. Please note the following key points:- 1. The benefits in Table 1 are only payable after a Deferment Period of six (6) months. 2. The benefits in Table 1 are payable for a maximum period of twelve (12) months after the six (6) month Deferment Period has expired. 3. If you are between 31 and 33 years of age then you will receive the following reduced maximum benefits:- Age Maximum Benefit 31>33 10 x Monthly Benefit stated above 33>34 8 x Monthly Benefit stated above 4. The premium stated above is for both Income Protection and Career Ending Injury benefits. 5. The monthly benefit You select cannot exceed 60% of Your gross monthly salary prior to the inception date of Your policy. 6. There is no cover available if You are 34 years of age or over. Career Ending Injury Benefits The below benefits will be paid to You if You suffer a Career Ending Injury due to an Accident which occurs during the Operative Time whilst Your Policy is in force. Table 2:- Age Maximum Benefit 18 > 26 30 times your monthly benefit £240,000 26 > 29 24 times your monthly benefit £192,000 29 > 31 18 times your monthly benefit £144,000 31 > 33 10 times your monthly benefit £80,000 33 > 34 8 times your monthly benefit £64,000

31 > 33 10 times your monthly benefit £80,000

Please note the following key points:- 1. The Career Ending Injury benefit payable to You is based on Your age at the inception date of Your Policy and is stated in

Table 2. 2. Any Income Protection benefits that You have been paid by Us will be deducted from Your Career Ending Injury benefit.
