This guide provides an overview of NBTY’s benefit choices and enrollment information so you can live your passion for wellness in 2016. Choosing Benefits and Making Changes The benefits you choose will be effective from your effective date through December 31st. You won’t be able to make changes to your coverage during the year unless you have a life status change. Some examples include:• Marriage, legal separation, or divorce• Birth, legal adoption of a child, or placement of a child with you for legal adoption• Death of your spouse or dependent child • Change in residence (only if your current coverage isn’t available in the new location or if you’re offered an

option that you weren’t previously offered)• If your employment status changes

After a life status change occurs, you have 31 days to make any updates through YBR. You can find more information at www.ybr.com/nbty.You can enroll in or make changes to your benefits each year during the Annual Enrollment period, which is normally held in November, for benefits effective January 1st - December 31st of the following year.*

New Hire BenefitsAs a newly hired associate, you’ll receive an Enrollment Worksheet within 21 days of your hire date. Your benefits elections will be effective the first of the month following 30 days of employment. If you don’t make your elections by your enrollment deadline, you’ll have to wait until the next Annual Enrollment period unless you experience a life status change.Example: If you were hired on February 12th, your benefits will become effective April 1st.You must make your elections on YBR if you want to enroll in:• Medical (including prescription drug), dental, or vision coverage• Health Care and/or Dependent Care Flexible Spending Accounts (FSAs)• Optional associate, spouse, or child life insurance• Optional disability coverage• Group legal services

EligibilityIf you work at least 30 hours per week, you’re eligible to elect coverage for the benefits listed in this guide. As a reminder, life and disability rates are based on age and salary level, therefore any salary and age (if crossing over an age band) change will result in a premium change.

How Healthcare Reform Affects CompaniesAt NBTY and businesses across the country, healthcare costs are rising due to a number of factors. One cause is healthcare reform, often referred to as the Affordable Care Act (ACA).Healthcare reform has impacted businesses in many ways, and has caused an estimated 1-5% increase in benefit costs for employers. Some factors leading to increased benefit costs include:• A requirement for large employers like NBTY to provide a minimum level of insurance coverage to all full-

time associates• Mandatory coverage of emergency and preventive care• Coverage requirements for children up to 26 years old

How Healthcare Reform Affects YouUnder ACA, everyone is required to have health insurance. Therefore, it’s very important that you review all coverage options available to you from NBTY and elsewhere to choose the best benefits for you. Regardless of where you get your coverage, you must have insurance or will have to pay a fine to the IRS while filing your taxes. The good news is that under ACA, insurance companies can’t deny anyone coverage based on factors like age, gender or pre-existing conditions. Also, ACA requires that plans cover preventive care such as children’s recomended vaccines, annual exams and certain lab work and screenings. So, while costs might be rising, you’re guaranteed comprehensive coverage and care.

• People are living longer, and there are costs associated with increased life expectancy.

• People are continuing to make unhealthy lifestyle choices like poor diets, stress, smoking, and drinking. These choices increase the likelihood of major health conditions like cancer, diabetes, and heart diseases, which in turn plays a factor in increasing healthcare costs.

• There is an increasing trend for the misuse of healthcare. For example, when an individual goes to the emergency room for routine care like a checkup or for the flu, the visit costs significantly more than going to an in-network provider or urgent care clinic.

• Cost increases can also be attributed to the advancement of medical technology, including things like vaccines and medical devices and specialty drug costs.

TOP 5 HEALTH TIPS1. Get enough exercise. Experts recommend adults

get at least 150 minutes of moderate aerobic activity per week.

2. Don’t use tobacco. Among other negative effects, smoking is estimated to increase the risk of coronary heart disease by 2 to 4 times.

3. Maintain a healthy diet. Many diseases have been linked to poor diet, including type 2 diabetes and some cancers.

4. Take advantage of annual exams. Yearly medical and dental check-ups can detect and help you prevent any potential health problems.

5. Get enough sleep. A sufficient amount of sleep (7-9 hours per night) helps your brain recharge and allows you to focus throughout the day.

FYI: How Some Companies Are Dealing With CostsContinuously rising healthcare costs are affecting everyone. For future consideration, to address this issue, NBTY is exploring what other companies are doing to contain rising healthcare costs to make sure our associates continue to have access to competitive benefits. Many companies are offering Consumer-Directed Health Plans (CDHPs), which are high-deductible plans with lower associate contribution rates. NBTY is exploring this option as well as others that give associates more control over how their healthcare dollars are spent.

WAYS TO SAVE ON HEALTHCARE IN 2016

You can decrease your monthly contributions by participating in a biometric screening. Participating associates will receive a $20 reduction in monthly medical plan contributions. Participating associates whose covered spouse also gets a biometric screening will receive an additional $20 per month reduction. A total savings of $40 per month ($20 for you and $20 for your spouse).

Our Maximum Motivator Program is available to you if you’re enrolled in the Delta Dental plan. Simply have a preventive dental exam by December 31st of the calendar year and increase your dental calendar year maximum from to $1,000 to $2,000 for the following plan year.

As part of our tobacco cessation program, you’ll pay lower life insurance premiums if you are a “non-tobacco” user.

health related questions or concerns. 3. Get generic prescriptions when available. When prescribed a prescription drug, ask your doctor if there is

a generic version available so you can save on your refills. 4. Understand what you’ll pay. Be sure you know what your deductibles, copayments and coinsurance will be

before receiving care. 5. Prepare for emergencies. Evaluate the impact of out-of-pocket expenses to prepare for emergency care if

needed.

MEDICAL

You have access to two medical plans through UnitedHealthcare, both of which include prescription drug benefits through CVS Caremark. As you evaluate your 2016 options for you and your dependents, here are some points to consider for each plan: Choice EPO plan:

• You pay copays for office visits• Provides in-network coverage only

Choice Plus PPO plan: • Provides out-of-network coverage, but the cost of in-network care is typically less money • The annual deductible must be met before any expenses (including prescriptions) are paid by the plan

Note: Medical (including prescription drugs), dental, and vision options will differ for associates in Hawaii, Guam, and Puerto Rico. Please see the Appendix for more information or view the Health Plan Comparison Chart (HPCC) on YBR for coverage details and pricing.

Wellness starts with preventive care, which NBTY wants you to have total access to. All medical plans provide preventive care such as annual exams, vaccinations and well-baby care and are available at no cost to you.

Live Well

MEDICAL |

Coinsurance: A percentage of costs you pay out of pocket for covered expenses after you meet the deductible.

Copay (Copayment): A fee you have to pay out of pocket for certain services, such as a doctor’s office visit or prescription drug.

Network: Doctors, pharmacists, and other health care providers who make up the plan’s preferred providers. When you use in-network providers, you pay less because they have agreed to negotiated pricing.

Deductible: The amount you pay out of pocket before the health plan will start to pay its share of covered expenses.

Out-of-Pocket Maximum: The most you pay each year out of pocket for covered expenses. Once you’ve reached the out-of-pocket maximum, the health plan pays 100% for covered expenses for the remainder of the year.

Preventive Care: Services you receive when you aren’t sick or injured. Preventive care services include annual physicals, wellness screenings, and well-baby care.

Know Your Vision RisksAccording to the Centers for Disease Control and Prevention, approximately half of American adults at risk for vision loss haven’t visited an eye doctor in the past year. Get an eye exam to find out your vision risks. If you enroll in vision coverage, your eye exam is covered. If you don’t enroll in vision coverage, consider enrolling in the Health Care FSA and using it to pay for the exam.

flexible spending accounts (fsas)

NBTY offers two types of FSAs that can help you save, no matter which plan you choose. • You can use the Health Care FSA for eligible health care expenses such as deductibles, copays, coinsurance,

prescription drugs, dental, and vision expenses.• You can use the Dependent Care FSA for eligible child and elder care expenses so you (and your spouse, if

married) can work or go to school.Each year, you can contribute up to $2,500 to the Health Care FSA and up to $5,000* to the Dependent Care FSA. Your entire Health Care FSA contribution amount is available to you right away, even if you haven’t made a deposit yet. However, Dependent Care FSA claims can only be reimbursed when you have accumulated enough money in your account to cover the claim. Your contributions go from your paycheck into your FSA before taxes are taken out. This lowers your taxable income, so you’ll pay less in taxes. Keep in mind, FSA dollars cannot be rolled over from year to year, so you must use all of your FSA balance during the calendar year or you will lose it at the start of the new year. Visit YBR for a complete list of eligible expenses. In accordance with local law, FSAs are not available for associates in Puerto Rico.

If you choose to participate in a Flexible Spending Account and you’re enrolled in NBTY’s medical, dental, prescription, and/or vision plans, your claims will be automatically reimbursed through Your Spending Account on the YBR website. If you prefer submitting manual claims, you can opt out of the automatic claim reimbursement program by calling the YBR Customer Service Center.

*Highly Compensated Employees (HCE) may have contribution limitations due to Non-Discrimination testing, as required by the IRS.

Life insurance

Basic Life InsuranceNBTY provides basic life insurance through Liberty Mutual in the amount of one times your annual base pay, rounded to the next $1,000, at no cost to you (maximum of $250,000).

Optional Life InsuranceYou can also purchase supplemental life insurance for yourself, your spouse, and your child(ren). How much your family might need depends on your situation (debts, expenses, etc.). Use the Estimate Your Life and Disability Needs tool on YBR before you enroll and be sure to choose beneficiaries so they can receive your life insurance benefit if you pass away. You’ll need to have the beneficiaries’ names, birth dates, and Social Security numbers handy when you enroll. Depending on your election, you may be required to submit Evidence of Insurability (EOI) to prove good health.• Associate Supplemental Life Insurance: If you’re eligible for basic life insurance, you can also purchase

associate supplemental life insurance from one to five times your base annual earnings (BAE) up to $750,000 with EOI. Rates are based on your age, salary, and the amount of insurance purchased. As a newly eligible associate or if you experience a life status change, you may purchase the lesser of three times BAE or $300,000 without EOI.

• Spouse Life Insurance: You can purchase one-half to three times your BAE of life insurance for your spouse.* Rates depend on your spouse’s age, your salary, and the amount of insurance purchased. Your coverage is limited to the lesser of associate basic plus supplemental insurance amount, or $400,000 (with EOI). As a newly eligible associate or if you experience a life status change, you may purchase up to $25,000 in coverage without EOI.

• Child Life Insurance: You can purchase $10,000 of coverage for your child(ren) up to age 26 (children less than 14 days old are covered at $2,000).* The amount you pay for child coverage is based on one flat rate, regardless of age and number of children covered. EOI is never required for child life insurance.

Basic Accidental Death and Dismemberment (AD&D) InsuranceNBTY provides basic AD&D insurance in the amount of one times your annual base pay, rounded to the next $1,000, at no cost to you through Liberty Mutual (maximum of $250,000). This coverage pays a benefit if you die or are seriously injured in an accident.

Rates for Tobacco Users

You’ll pay less for supplemental and spouse life insurance if you and/or your spouse are “non-tobacco users.” You’re considered a tobacco user if you have used tobacco products in the past 12 months.

What Is a Beneficiary?A beneficiary is the person who receives the death benefit when an insured member dies while covered under our insurance plan. Even if you’re not electing additional life insurance, you must complete beneficiary details on YBR since you automatically receive basic life and AD&D insurance. In the event a named beneficiary is not on file, your policy death benefit may go to your estate rather than to your loved ones. Be sure to update your beneficiary information on YBR at any time.

Disability InsuranceNBTY provides basic short term and long term disability insurance at no cost to you through Liberty Mutual. You also have the option to buy additional coverage in case basic disability insurance alone doesn’t meet your needs. pay (up to $2,750), for up to 25 weeks after your first week of approved disability. Short term disability benefits begin after a 7 day waiting period of being continuously disabled. NBTY pays the full cost of this coverage.

• Supplemental Short Term Disability: You can also purchase supplemental short term disability coverage in addition to the company-paid plan for a total income replacement of 66 2/3 subject to a maximum benefit of $2,750 a week.

• Long Term Disability: This company-paid benefit pays 50% of your income up to $10,000 if you continue to be disabled after your short term disability benefits end.

• Supplemental Long Term Disability You can purchase supplemental long term disability coverage, which will replace an additional 10% of your annual base salary (for a total benefit pay replacement of 60%) up to $10,000 per month.

This option provides you and your eligible dependents with access to affordable, convenient legal services from in-network attorneys. The NBTY Retirement Plan offers a convenient, tax-advantaged way to help you save for the future. The Plan has two components:

401(k) ComponentWhen you contribute to the 401(k) plan, NBTY will match your contribution dollar for dollar of the first 3% you save, plus 50% of the next 2% you save for a maximum match of 4%. You automatically own all your contributions, NBTY’s match contributions, and investment earnings. You become 100% vested as soon as the money is deposited into your account.Fidelity will mail you an Enrollment Kit shortly before your 401(k) eligibility date. Once you’re eligible, you may begin, change, or stop your contributions to the plan at any time.You’ll become eligible once you are at least 21 years of age, are scheduled to work at least 1,000 hours per calendar year, and have completed six months of continuous employment. Eligibility then occurs on the first of the month following completion of eligibility.Please note: For Vitamin World Associates eligibility begins at store manager level and above.

Profit Sharing ComponentNBTY may contribute an additional amount to increase your retirement savings through a profit sharing contribution. If NBTY meets its annual performance goals and you’re an eligible participant, a contribution will be allocated to your plan account based on your eligible compensation.You’ll automatically become a participant in profit sharing on your one-year anniversary of employment with NBTY or your 21st birthday, whichever occurs later. Once you’re a participant, you must work 1,000 hours in the calendar year and be actively employed on December 31st of such year to receive any profit sharing contribution for that year.
