When a long-time chief executive officer retires, it gives an organization the chance to

take stock of where it’s been and where it’s going. Such was the case for Chapters Health System in 2017, as we prepared to say farewell to Kathy Fernandez, our president and CEO for 25 years.

Under Kathy’s leadership, Chapters Health accomplished much for patients and families in our service area, including: • The first Joint Commission accreditation

survey and subsequent re-accreditations;• Original expansion of LifePath Hospice

into Polk, Highlands and Hardee counties;• Subsequent affiliations with Good

Shepherd Hospice and then HPH Hospice, which expanded our reach into Citrus, Hernando and Pasco counties, plus Pinellas County for home health;

• The construction of four freestanding hospice houses and the addition of our hospital-based palliative care consultation service, which brought more resources and care options to more people; and

• Development of our in-house durable medical equipment and pharmacy programs, which provide patients with the medical equipment and medications they need, quickly and conveniently.

All of these accomplishments helped transform Chapters Health into a nationally respected post-acute health system, caring for nearly 15,000 patients in 2017.

The scope of Kathy’s achievements meant that Chapters Health had to look carefully for the right person to build upon them. After a thorough search, we were pleased to find that candidate in Andrew Molosky, a creative, equally engaged executive who came on board in mid-December. Andrew will provide a steady hand in Chapters Health’s continuing pursuit of excellence.

So as we move forward, a look back: This annual report brings you the important events that became part of the Chapters Health story in 2017.

We attained “deemed” status from The Joint Commission for all affiliates, which recognized our standards and processes at the highest levels. We began laying the foundation for an inpatient hospice unit at Tampa General Hospital. Our children’s bereavement camps marked milestone anniversaries of serving families coping with loss. And yes, we also survived the impact of Hurricane Irma. Throughout, our Board continued its pursuit of innovative, cost-effective solutions for delivering quality patient care and worked to ensure that our employees have the resources and tools they need to deliver that care.

We can’t know what challenges are in store for us, but we can be confident we’ll meet them from a position of strength. Kathy helped make that possible. All of us owe her a debt of thanks for the work she did and the great organization she built.

CEO’s Report

What does a chief executive officer (CEO) say in an annual report when he’s been on the

job for just a few months? The answer’s easy: I’m privileged to lead a post-acute healthcare provider that’s in a strong position to deliver exceptional end-of-life care to the patients and families of West Central Florida.

This is exciting and gratifying, and I owe many thanks to my predecessor, Kathy Fernandez, who led Chapters Health with selflessness and integrity for 25 years. Kathy helped ensure that the organization moved forward so effectively that I could hit the ground running when I arrived. Any CEO would be thrilled for such an opportunity, and I’m looking forward to even further success in the coming years.

And there is no doubt that we will face challenges. The healthcare landscape is changing from fragmented to value-based — a world in which the quality of care, not just its quantity, counts. But the challenges these changes bring aren’t insurmountable. Chapters Health will overcome and exceed them as we become more integrated — tearing down silos, reducing fragmentation — and as we continue to provide compassionate care in a way that other hospice organizations do not.

Four strategies will help us accomplish these goals:

Attracting the best employees – My Listening Tour in early 2018 with visits to every Chapters Health location showed me just how outstanding Chapters Health employees are. We want more just like them.

Retaining those employees by becoming an employer of choice – Engagement is paramount in ensuring the delivery of high-quality care. We need to support our employees’ commitment to patients and families by providing a working environment that values diversity, transparency, flexibility and opportunity.

Offering chances to learn and grow –Hospice puts unique demands on every employee. It’s a calling, not just a job. All employees, whether they touch patients and families or support a co-worker who does, deserve encouragement and guidance in their current positions and continuous opportunities to advance in their careers.

Providing technological tools that support the best care – Putting quality over quantity means we must have technology that frees us to focus on patients’ and families’ needs.

This 2017 Annual Report will show you the progress Chapters Health has already made toward these goals. I’m proud to say that we enter 2018 standing on a foundation of great leadership and sound financial performance. And looking to the future, I’m confident that Chapters Health will continue to make a difference in the lives of those we’re honored to serve.

Chapters Health turned in a strong financial performance for fiscal year 2017, generating a $3.5 million (2.3

percent) operating margin, primarily due to a 5.8-percent increase in the average number of hospice patients served.

In addition, our investment portfolio generated a $10.9 million return on investment and the Development Department raised $4.5 million, a 13.2-percent increase over 2016. As a result, Chapters Health’s total revenue over expenses was $17.5 million, or 11.3 percent.

Our solid balance sheet and cash position supported the successful restructuring of our long-term debt, which lowered our effective interest rate and provided additional funding for new and expanded programs.

Unrestricted cash and investments equal 274 days of operating cash requirements. Total assets are $229 million, and we have net assets (equity) of $163 million. Chapters Health manages more than $90 million in long-term investments.

Our consistently strong financial position has allowed Chapters Health to weather

changes in reimbursement, regulations and competition. That was especially important when we had to deal with the impact of Hurricane Irma in September. Thanks to Chapters Health’s investment of more than $28 million in freestanding hospice houses, we were able to provide shelter, care and comfort to our most medically compromised patients. Our financial strength also allowed us to bring in additional staff to serve patients and families at our hospice houses, in shelters and in patients’ homes.

As always, Chapters Health serves patients and their families regardless of their ability to pay. In FY2017, that meant providing more than $2.6 million in charity care. We also offer a broad range of essential, but unfunded, services to assist patients, families and others in need in fulfillment of our not-for-profit mission. That cost exceeded $1.2 million last year.

We look forward to a continued strong financial position in FY2018, as we continue to expand our post-acute care services throughout our service area.

Clarence Prevatt Jr. became a funeral director not just to follow in his father’s footsteps but because he cared deeply

about others.

“Clarence was probably one of the nicest, kindest people I ever met in my life,” said Elaine Prevatt, Mr. Prevatt’s wife of 33 years. “He was concerned with everybody.”

When Mr. Prevatt, an HPH Hospice board member and chairman of the HPH Hospice Foundation, died in October 2017, the families served by Prevatt Funeral Home, HPH Hospice and Chapters Health all lost a good friend.

“He was a great listener,” Mrs. Prevatt said. “He would sit forever with a family to discuss funeral arrangements. He had all the time in the world to spend with them.”

Mr. Prevatt believed so strongly in this credo that in 1992 he co-founded the Independent Funeral Directors of Florida.

“Clarence wanted to look out for the little guy,” Mrs. Prevatt said. “He knew that to be a successful funeral director you have to immerse yourself in the community, know your neighbors and make sure that things were done right for them.”

“At HPH Hospice, Mr. Prevatt was a deeply involved and committed board member, treasurer and foundation board member,” said Tom Barb, HPH Hospice’s president and CEO at the time of its affiliation with Chapters Health.

“He was a very good steward and believed that joining Chapters Health was the right thing to do for the future of HPH Hospice,” Mr. Barb said. “He was also very interested in our employees. He always tried to attend the employee recognition and service award ceremonies.

“Clarence had a natural empathy for the service hospice provides,” he added. “Spending time with families who were planning funerals is much the same as a good hospice nurse, social worker or chaplain does when working with a patient.”

How does a multi-site healthcare provider prepare for and ride out a dangerous hurricane? Chapters Health

did so with planning, precision and teamwork — but above all, with a selfless commitment to putting patients and families first.

When Hurricane Irma struck southwest Florida in September 2017, Chapters Health employees came together to prepare patients, help them evacuate to shelters and hospice houses, care for them during the storm and reach out to them as soon as the skies and roads were clear.

To accomplish these goals, employees had to set aside personal concerns for their own comfort, homes and families. The hurricane actually brought them closer together than ever. Here are some of their stories.

The Week BeforeNobody knew for sure where in Florida Hurricane Irma would strike, but Chapters Health had to prepare as if its entire service area would take a direct hit. Its hurricane preparedness plan went into effect shortly after the long Labor Day weekend with Chief Clinical and Compliance Officer Pam Saucier in the lead.

“By the morning of September 6, what had started as preparations for a tropical storm became much more serious,” Ms. Saucier said. “Now it was a hurricane, and we were dealing with evacuations.”

Chapters Health had two chief concerns. The first was ensuring all patients had at least two weeks’ worth of medications ahead of the storm. The other was determining where patients planned to ride it out so their care could be as seamless as possible.

Gathering this information was a massive logistical undertaking, with Chapters Health staff and volunteers spending hours on the phone.

“We called every single one of our patients to see what their plans were, if they had everything they needed and if they were going to a shelter or needed to come to a hospice house,” said Allyson Moskowitz, Director of Volunteer, Bereavement and Support Services.

Once callers determined the status of patients’ medications, Chapters Health Pharmacy swung into action to fill and deliver prescriptions. At the same time, the pharmacy began to set up medication carts for patients who were planning to evacuate to the special-needs shelter at the University of South Florida’s (USF) Sun Dome.

Being able to rely on the resources of a parent organization like Chapters Health helped the affiliates get over any logistical bumps in the road.

“Having our own medical equipment service was fabulous,” said Jackie Kendrick, Executive Director, HPH Hospice. “They were able to bring in extra beds. And the home office was always there for us.”

With the hurricane forecast to hit late on Sunday, September 10, Chapters Health employees worked in shifts to have everything in place by Friday night so that by Saturday morning at the latest, those who were planning to work during the storm could take their places, batten down and be on duty around the clock.

“It was very stressful because we didn’t know what would be left standing,” Ms. Saucier said. “Everyone was feeling it on a personal and professional level.”

Even in the midst of the stress, staff and volunteers were more than willing to make sacrifices.

“We never had to ask people to help during the preparations,” said Paul Marois, Executive Director, LifePath Hospice. “They all volunteered. That kind of spirit is embroidered in our culture.”

Irma Bears DownOn Saturday, with staff members and volunteers in place, Chapters Health executives fanned out across the service area, visiting hospice houses and other locations to check on preparations.

Team Sun Dome arrived at the USF arena and made a dismaying discovery: no blankets. The Red Cross’ shipment hadn’t arrived, which would have left hospice patients on bare cots. Chapters Health hospice houses, taking in additional patients and family members, were caught short, too.

“I said, go to Walmart, K-Mart and J.C. Penney and buy all the blankets you can,” Ms. Saucier

said. “Whatever they needed, we supplied. There were no questions about whether we could afford it — we just did it.”

“The linens were crucial to patients’ privacy and dignity,” said Diana Yates, Business Unit Director, LifePath Hospice, who worked at the Sun Dome. “Without blankets, you’re just lying on the green cot. Chapters Health did a phenomenal job of getting them. There were some patients at the Sun Dome who weren’t even ours, and we gave them blankets, too.”

“Other than the blanket issue, preparations at the Sun Dome were impressive,” said Chad Farmer, MD, Medical Director, LifePath Hospice. For the first time, a separate section was reserved for hospice patients only. Chapters Health’s IT department ensured that every patient’s medical record would be available with just a click of a laptop mouse. Crucially, the medication carts were stocked, locked and in place.

“Having our own Chapters Health pharmacy was incredible,” Dr. Farmer said. “I can’t describe what an advantage it was to have them in-house and able to work with us directly.”

“How can you get to your patients if you have to wait in line for gas for six hours? But Chapters Health bought every gas can they could, filled them and drove them down to us from northern Polk County. We triaged it so the nurses and other essential staff could continue to provide a continuum of care.”

In the days that followed, employees and volunteers whose power at home had been knocked out were invited to the hospice houses for showers and hot meals (and some highly coveted time in the air conditioning). Chapters Health also footed the bill for food from the few area restaurants that were up and operating — in one case, buying 50 hamburgers to feed hungry workers.

“And because a lot of our people had lost power, we told them to bring what they had in their freezers to Lakeland Hospice House and make potluck for everybody,” said Mary Grindel, Clinical Administrator, Good Shepherd Hospice. “As crowded and crazy as it was, everybody pitched in and had some fun.”

Payday fell on the Friday after the storm, but without functional ATMs, some employees weren’t able to access their money. “We took cash to them,” Ms. Saucier said.

Chapters Health was able to get through the hurricane in large part because of its size and strong financial position, according to David O’Neil, Chief Financial Officer.

“Overall, we were able to absorb about $500,000 in extra expenses,” he said. “Our ability to prepare so thoroughly and recover so quickly reflects the capital investments that we’ve made through the years — like our hospice houses, which are hurricane-proof, and our in-house pharmacy and durable medical equipment services, which meant that we didn’t have to rely on third-party vendors. If we were a smaller provider, we wouldn’t have been able to do that at all.”

“Team Sun Dome was fully clinically staffed,” Dr. Farmer added. “We had the medical staff, administrator, volunteers, a chaplain and a social worker, the full team,” he said. “Every day we were there, we’d do a huddle at 8 a.m. and 8 p.m., reviewing what had happened during the previous 12 hours and getting prepared for the next 12.”

Irma Hits HardAllyson Moskowitz and her husband, Dan, who was a patient at Good Shepherd Hospice, spent the hurricane at the Lakeland Hospice House.

“I have to say the care was wonderful,” Ms. Moskowitz said. “My sons and I stayed with Dan in his room the whole time. Although the hurricane was all around us, the staff’s attention was completely on us, not on the storm.

“If the staff was stressed, even I as a colleague didn’t know it,” she added. “They were always smiling. I’d ask them if they wouldn’t rather be home, and they’d say, ‘No, this is where I need to be.’”

At the Sun Dome, circumstances varied from dramatic to upbeat. Early in the admitting process, one patient began actively dying.

“We were able to move her to a more private area,” Dr. Farmer recalled. “She was very close to her dog, which was in the kennel there. So we got permission to bring the dog in through a back hallway and let it lie with her as she took her last breath. It was very peaceful. I don’t think anyone else was aware of what was going on.”

By Sunday evening, with the hurricane at its height, the Sun Dome was buoyed when LifePath Hospice Chaplain Jerome Lowe, who had his keyboard and saxophone in his car, played during dinner for the entire shelter.

“The whole mood of that shelter changed,” Ms. Yates said. “That’s what we do — making the moment, wherever we are.”

Beginning to RecoverAfter the hurricane, the biggest challenge was distributing the resources Chapters Health needed to continue serving patients and families: portable A/C units, generators, cash to buy food and supplies, and, most of all, gas.

“Our power infrastructure in Highlands County was almost completely wiped out, so the gas shortage was unbelievable,” said Jennifer Forde, Regional Director, Good Shepherd Hospice.

As the recovery continued, Chapters Health asked all home office employees who had escaped the worst of the storm to donate their unused hurricane supplies for Good Shepherd Hospice employees and others who were hard-hit.

“We turned our boardroom into a collection center,” Ms. Saucier said, “and we sent truckloads of batteries, bottled water, charcoal and bleach.” Later, Chapters Health sent leftover supplies to Puerto Rico to help in the aftermath of Hurricane Maria.

Irma’s LegacyEvery member of the Chapters Health family agreed: Hurricane Irma was stressful, but it brought out the best in everyone.

“Staff and volunteers wanted to help in any way possible,” said Audra Farish, Chief Human Resources Officer. “They provided care during the hurricane and got back out to patients afterward any way they could, even in boats and canoes. They helped patients, families and staff who didn’t have power or who couldn’t get home right away. Everyone across the system stepped up.”

At the debriefing sessions following the storm, the spirit of teamwork stayed steady and strong. “Everyone was giving kudos to each other and hugging,” Ms. Saucier said. “I’m the compliance officer; nobody hugs me! But I got tons of hugs.”

For Ms. Moskowitz, whose husband died the Friday after the storm, the experience was life-changing.

“I have such a renewed passion to make things right for patients,” she said. “Going through the hurricane with my husband as a hospice patient made me so proud of the organization I work for.”

Some hospice volunteers seek opportunities to get involved and meet new people. Others see a niche that they know they’re perfect to

fill. Still others had loved ones who were cared for by hospice, which inspired them to give back.

But one thing is true for everyone: Chapters Health’s 1,700-plus volunteers help the organization fulfill its mission.

“Without our volunteers, we wouldn’t be able to hold our grief and bereavement camps, conduct regular outreach to patients and families, run our thrift stores or officiate pinning ceremonies for veterans,” Ms. Moskowitz said.

Spanning GenerationsChapters Health has opportunities for volunteers of any age, but if there’s one age group that seems to be stepping up, it’s teenagers.

“We have a group of teens at the LifePath Hospice Temple Terrace Resource Center that’s very strong,” Ms. Moskowitz said. “They provide companionship, sitting with patients, reading to them and helping with their meals if they need it.”

Every week, Trey Hurt, 16, and Megan Ghillyer (pictured below, left), 19, devote afternoons to the Resource Center. One of their frequent duties is making regular calls to patients and families to ensure they have enough medications for the weekend ahead.

“The patients and families seem to enjoy having someone to talk to,” said Trey, who is earning community service hours for high school. “It’s good for me as well because it gives me a new perspective on how much I’m helping people.”

USF student Megan had a family experience that inspired her. “My grandfather was in hospice in England, so when I got old enough to provide some support in return, I jumped at the chance,” she said.

She urged her fellow teens to get involved. “Even if they’re busy with work and school, everyone can make a contribution,” she said.

On the other end of the age spectrum is Good Shepherd Hospice volunteer Betty Sutton (pictured below, right). At 95, she volunteers weekly at the Lakeland Hospice House, visiting with patients and families, talking with them and holding their hands.

Hospice Volunteers Show Love of Giving

“It helps me feel useful,” Ms. Sutton said. “I’m giving comfort to others, and I’m also getting comfort from them.”

Pet Therapy Brings Respite Wrigley the “doodle” is a true celebrity at Good Shepherd Hospice. He and owner Bob Funk visit patients in the Bud and Donna Somers Hospice House and at nursing homes across the Sebring area every week.

“Wrigley is almost the perfect therapy dog,” Mr. Funk said. “He has a pretty good understanding of what’s going on with people. He can do tricks for them, or just let people pet him.”

Mr. Funk said pet therapy is effective because it gives patients and families a welcome distraction.

“Wrigley takes their minds off things,” Mr. Funk said. “He brings a little change of pace to everybody involved.’”

Mr. Funk began volunteering six years ago because he felt blessed. “I do this because of the grace that God has shown me in my life, and I want to give some of that grace back to other people,” he said.

Can’t Stop These VolunteersFor 10 years, a half-dozen dedicated Good Shepherd Hospice volunteers have come to the Auburndale office for four hours every week. They stuff and postmark envelopes for 40,000 mailings sent throughout the year to Polk, Highlands and Hardee county hospice families “in need of support, direction and love,” said volunteer Faye Martin.

The mailings include hospice-related information, such as support group schedules, and grief and bereavement advice. Collectively, the volunteers

devote 750 to 800 hours every year to the project.“I hope our service provides comfort to people who have lost their loved ones,” said Ms. Martin, who began volunteering after her husband died in 2008. “We get great satisfaction out of being able to give back and help.”

The inspiration to help with the often-moving pinning ceremonies springs partly from Mr. Whitford’s service in Vietnam.

“I’m not career military, but I got involved because what I go through isn’t anywhere close to what those guys went through,” he said. “We do it for the patient but also for the family, to give them something they can keep forever.”

Employees who are highly engaged stay longer and perform better than employees who aren’t. Focusing on

employee engagement, Chapters Health made it a priority to provide programs and initiatives that reinforce the deep commitment that its employees have to patients and families — and keep them inspired on the job.

“We spend as much time at work as we do with loved ones, so we want employees to enjoy what they do,” Ms. Farish said. “If team members take part in real, two-way communications with their leaders, it provides a foundation for increased engagement. In turn, this leads to enhanced productivity, job tenure and improvements in patient care quality.”

Efforts in improved employee engagement link to Chapters Health’s ongoing initiatives in leadership development and planned changes to the performance management process. To measure engagement, Chapters Health conducts confidential employee surveys, after which teams across the system develop action plans that reflect their priority needs and opportunities for improvement. In essence, it was making change happen from an “employee-centric” focus as opposed to top-down efforts.

The process relies extensively on two-way communication at all levels. “We want survey data to turn into meaningful conversations between employees and supervisors across the organization,” said Ms. Farish.

At the end of 2017, Chapters Health conducted a quick-check survey to measure progress. The “pulse” survey showed statistically significant 4-percent increases in employee engagement levels across the system. Scores rose most dramatically at HPH Hospice,

thanks in part to efforts spearheaded by Clinical Administrator Fay Van Fleet and Ms. Kendrick.

One of those initiatives addressed implementation of the new electronic medical records system (EMR), which employees found difficult to use. Ms. Van Fleet convened a task force that developed a tool to simplify functions of the EMR. Their goal was improving patient care plans by making the EMR system more user-friendly.

“Care plans are communication vehicles between caregivers,” Ms. Van Fleet explained. “You can open a care plan and instantly get a clear picture of what’s going on. Our task force helped create a better experience for the nurses, which translated into even better care.

“Ideas about how to make things better can only come from our team members,” Ms. Van Fleet said. “And our employees know that when they speak up about something, we’re going to try to do something about it.”

When Change Rises from the Grass Roots

One of the benefits of hospice is it provides care that reflects the wishes and values of

patients and families. Now, a Chapters Health initiative based on POLST (Physician Orders for Life-Sustaining Treatment) is another step forward in ensuring patient preferences are followed at the end of life.

POLST is a type of advanced directive that not only outlines a patient’s preferences but does so in specific detail aimed at the patient’s particular condition and with the force of a physician’s order behind it. It’s designed to give patients more control in their end-of-life care, but it does so much more.

“POLST is meant to stimulate an ongoing conversation between the patient and provider,” said Stewart Stein, MD, Associate Vice President of Medical Services, Chapters Health. “It’s not a sign-this-and-put-it-away kind of document. Instead, it fosters a meaningful conversation to help patients understand more about their particular diseases.”

POLST is especially helpful for patients with progressive diseases such as cancer, dementia, amyotrophic lateral sclerosis (ALS), congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) or end-stage renal disease. That’s because while traditional healthcare directives are necessarily long-term and vague, the more-specific POLST can be easily modified to reflect changes in treatment as a disease advances.

“In hospice, things can change quickly, and our goals don’t always remain the same,” Dr. Stein explained. “Once a POLST is in place, we can modify it or void it any time there’s a change in the level of care, the location of care or the progression of the disease.”

While a POLST carries the authority of a physician order, the conversation can take place with

nurses, nurse practitioners, social workers and chaplains to discuss treatment options, alternatives for care, goals of care, what to expect as the disease progresses and how the plan of care can best reflect the values of the patient. Signed by a physician, a completed POLST serves as a template for care going forward.

POLST documents are legally recognized in some states, but they have not been legislatively enacted in Florida. Nevertheless, Chapters Health completed its POLST pilot program in October 2015, and it generated nearly uniformly positive results.

“There’s no obligation for other healthcare providers in Florida to honor a POLST,” Dr. Stein said. “But we found that in the overwhelming majority of cases, it was. POLST works because it represents a patient’s well-thought-out wishes.”

Chapters Health now uses POLST in all three of its hospice affiliates: Good Shepherd Hospice, HPH Hospice and LifePath Hospice.

“POLST has provided patients and families with a better end-of-life experience,” Dr. Stein said. “We took it upon ourselves to adopt POLST because doing what patients and families want is an important part of our job. The more people who understand what those wishes are, the better it is for everybody concerned.”

The Chapters Health mission is to provide compassionate care to people at the end of life. And as a community-based, not-for-profit organization, it also helps make its service areas a better place to live, work and raise families.

The organization’s community benefit is clear: Chapters Health creates jobs, attracts new residents, supports local businesses and provides services that contribute to the overall strength of families. Here are a couple of ways it made a difference in 2017.

Supporting Families in NeedLifePath Hospice and HPH Hospice both run food pantries that provide grocery baskets to needy hospice patients and families in Hillsborough, Pasco, Hernando and Citrus counties. The pantries, which are completely staffed by volunteers, are supported by Chapters Health employees and volunteers who contribute non-perishable groceries, paper products and cleaning supplies. During the holidays, needy families also receive gift cards so they can purchase fresh foods to help brighten their season.

Inspiring Community VolunteerismAfter financial adviser Ana Gessman died under the care of LifePath Hospice at the Melech Hospice House last summer, 35 friends and co-workers from Raymond James Financial Services found a wonderful way to honor her: provide a day of service at the Melech Hospice House. Volunteers teamed up to clean the house’s exterior and the chapel, plant a memory garden, fill bird feeders and distribute flowers to patients.

“The employees of Raymond James filled Melech Hospice House’s hallways with love and laughter, honoring a young woman we were privileged to care for and making an environmental impact at the same time,” said Sandy Cody, Volunteer Services Supervisor, LifePath Hospice. “Their volunteerism exemplifies the unique and sometimes unexpected ways in which hospice brings people together in our communities.”

Community Benefit: A Vital Part of the Mission

Good Shepherd Hospice Reorganizes, Achieves Deemed Status Recognition

Good Shepherd Hospice took stock of its operations in 2017 and decided to reconfigure its hospice care teams

to better serve patients and families. The reorganization, an eight-month project that took effect in November, resulted in more efficient delivery of care, Ms. Grindel said.

“We went to a six-team matrix and made our teams of equal size,” she said, “Staff don’t have to travel such long distances between patients now.”

The fiscal year also brought good news to Good Shepherd Hospice in the form of deemed status accreditation from The Joint Commission. Announced in August, deemed status is official confirmation that Good Shepherd Hospice’s care meets or exceeds federal standards for participation in Medicare and Medicaid.

“Deemed status singles Good Shepherd Hospice out for our commitment to providing high-quality care,” Ms. Grindel said. “As the oldest hospice provider serving Polk, Highlands and Hardee counties, deemed status helps assure our patients and their families that outstanding care is and will remain our top priority.”

To Executive Director Kyle Zenkner, deemed status is official recognition of Good Shepherd Hospice’s approach to end-of-life care.

“Deemed status recognizes something that’s hard-wired into our culture,” he said. “It reflects our commitment to high-quality care today, continuous quality improvement tomorrow, and compassion for patients and families coping with life-limiting illness, always.”

HPH Hospice Goes Above and Beyond in 2017

A fter a rigorous on-site Joint Commission survey and a nervous wait for final notification, HPH Hospice learned in

2017 that it had joined its fellow Chapters Health hospice affiliates in receiving deemed status accreditation, the highest level for the delivery of safe and effective hospice care.

“Deemed status reassures the community that we not only meet The Joint Commission’s guidelines, we go above and beyond them,” Ms. Kendrick said. “HPH Hospice staff rose to the occasion during The Joint Commission’s survey.

“It was a great team-building opportunity,” she added. “We came together as a team and celebrated as a team. Deemed status gave everybody such a sense of pride.”

The year saw expanded grief and bereavement programs: HPH Hospice grief experts visited 145 schools in Pasco, Hernando and Citrus counties to outline available services and support and hosted 75 grieving children at its renamed annual children’s bereavement camp — Camp REACH OUT. HPH Hospice’s grief and bereavement programs also benefited

from the renovation of the home office in Hudson, which freed up rooms at its Center for Grieving Children for teens, volunteers, support groups and one-on-one counseling.

Next on tap: HPH Hospice is looking toward expanding its reach in Citrus County. “With staff there who are local, we are deepening our ties in Citrus and becoming an active part of the community,” Ms. Kendrick said.

L ifePath Hospice made its clinical mark on 2017 with a successful initiative to reduce and/or eliminate hospice patients’ readmissions to Hillsborough County’s hospitals. Sending skilled nurses to case-manage hospitalized

patients helped shorten their stays. And changing communications protocols and speeding up response times helped prevent readmissions from happening in the first place.

“We want to be a patient’s 911 call,” said Kathleen Miller, RN, Clinical Administrator. “If a patient has an emergency at home, we strive to get there within 60 minutes.”

As a further preventive measure, LifePath Hospice placed emergency medication kits in the homes of COPD and CHF patients to help deal with symptoms such as shortness of breath, dizziness or coughing.

“Our commitment is to get to the home immediately, but until we can, these kits allow us to start intervention over the phone,” Ms. Miller explained.

The year was also one of growth. Extending its outreach to under-served communities and serving nearly 300 additional patients overall, LifePath Hospice added a 13th hospice team. And the children’s grief camp, Camp Circle of Love, marked its 25th anniversary, hosting more than 110 children and teens.

“LifePath Hospice found every opportunity in 2017 to be proactive in delivering compassionate end-of-life and bereavement care to patients and families,” Mr. Marois said.

HPH Home Health Improves Outcomes, Keeps Patients at Home

HPH Home Health spent 2017 working to improve services and availability The affiliate is enhancing access in Pasco and Pinellas counties so patients can receive clinical care to remain independent in

their homes and engaged with their families and friends.

Another goal of HPH Home Health is to help patients with progressive diseases transition safely between home health and another Chapters Health service.

“The objective is always to have patients come home and be among the things and people they most love,” said Jane Witkowski, Director. “To help prevent hospitalizations, we provide education and support new skills that families and caregivers can use in their day-to-day lives.”

The “Call Us First” program, implemented in 2017, helps HPH Home Health nurses work collaboratively to identify and address triggers that might otherwise send a patient to the ER.

“Call Us First is a priority for our nurses to help keep patients out of the hospital,” Ms. Witkowski said.

“Home health is in a unique position to ensure patients are in supportive healthcare environments,” she said. “Our goal going forward is to continue to explore ways to expand service to better meet patients’ needs and keep them in their homes.”

Two years ago, Chapters Health made an important commitment: expand palliative care services to more hospital inpatients.

This year, that commitment saw results, with a 75-percent increase in new patients seen at Florida Hospital Tampa and a new program about to launch at Florida Hospital Heartland Medical Center in Sebring.

“By the end of 2017, Chapters Health Palliative Care was serving more than 10 percent of all admitted patients at Florida Hospital Tampa,” said Ronald Schonwetter, MD, Chief Medical Officer. “The patients and families are benefiting, and so is the hospital — through lowered costs, decreased lengths of stay, decreased mortality and better outcomes.”

Palliative care is for patients who are coping with life-limiting and/or chronic illnesses such as CHF, COPD, dementia or cancer. While many are still receiving treatment, and can even go into remission, they can require palliative care because their symptoms, such as pain, shortness of breath or anxiety, are not being adequately addressed. They also want to understand their conditions and what they should expect as their illnesses advance.

“Palliative care seeks to give patients and their families a clear understanding of how their illness experiences are interrelated,” said Mary Alfano-Torres, MD, Medical Director, Chapters Health Palliative Care. “Patients may see their experiences as isolated events. We have the complex conversations that will give them a more complete understanding of their disease.”

Why is palliative care important in a hospital setting, and why is providing it to inpatients a goal of Chapters Health?

“Understanding where they are in the trajectory of their disease allows people

to make choices and make realistic plans that are most consistent with their personal goals and values as their disease progresses,” Dr. Alfano-Torres explained.

“The staff at the 475-bed Florida Hospital Tampa has received the program enthusiastically,” Dr. Schonwetter said. “Acuity in patients is far greater today than it used to be, requiring doctors to provide more care and education in the areas of complex medical decision making, advance directives and end-of-life care. Those are the kinds of conversations we help with,” he added. “Hospitals, regardless of their size or demographics, are beginning to understand the benefits of palliative care.”

The success of the Chapters Health Palliative Care program at Florida Hospital Tampa caught the attention of the chief medical officer at Florida Hospital Heartland Medical Center in Sebring, leading to an agreement to launch a Chapters Health Palliative Care program there in early 2018.

“It’s a smaller hospital, but our approach will be the same in terms of using our multidisciplinary Chapters Health team,” Dr. Schonwetter said. “We’re very excited to be starting that program.”

Chapters Health Expands Inpatient Palliative Care

Chapters Health’s Development team enjoyed a successful and productive 2017, netting $3.1 million on gross receipts of

$4.7 million — a 26-percent increase over 2016.

“One of the reasons we did so well is that we were able to create exciting new opportunities for donors and volunteers to support the work we do,” said Sharon Jones, Vice President, Development. “We combined those new opportunities with our existing ones; and together, they offered an array of ways in which people could help Chapters Health deliver compassionate hospice services to patients and families.”

Caring for the CaregiversThe Hospice Women of Philanthropy helped LifePath Hospice care for the caregivers by spearheading a campfire chat for parents and guardians who sent 115 kids off to Camp Circle of Love, LifePath Hospice’s long-running grief camp.

“The Caregiver Campfire Chat’s purpose was to provide support to bereaved adults who, after

dropping off their children for camp, were going home for three days to an empty house,” said Liz Anderson, Associate Vice President of Development.

The Hospice Women of Philanthropy conceived and funded the event and coordinated the on-site logistics. Eighty caregivers heard a presentation by Chapters Health Palliative Care pediatric physician Dr. Robert Bash, viewed a video about camp and received “self-care boxes” with small gifts and camp-related materials.

“We thank the dedicated Hospice Women of Philanthropy for funding the video and for assembling all the gift boxes,” Ms. Anderson said.

Breakfast Hits a Home RunHome Runs for Hospice, which debuted in May at The Pavilion at Tampa’s George M. Steinbrenner Field, had a special mission — hospice education.

“We’d continued to hear from our donors that people don’t understand the depth and breadth of

hospice services, what hospice does and what it really means,” Ms. Jones said. “So we wanted to educate the community on what we do and the power of their gifts in supporting us.”

Emceed by Chapters Health Board Member Gayle Sierens Martin, the event featured a video and powerful testimonies from speakers who had benefited from hospice. The first-time breakfast raised $194,000.

Comforts of HomeThe Hospice Women of Giving put an inspired idea into motion this year: linen kits for patients and families cared for by Good Shepherd Hospice.

“Many patients don’t have the kind of tall twin sheets that fit the hospital beds delivered to their homes,” said Nikki Powers, Director of Annual Giving. “The Hospice Women of Giving funded linen kits that our nurses could keep in their cars and hand out to families who needed them.”

With initial funding of the project in 2016, the Hospice Women of Giving bought and assembled 400 linen kits for distribution in this year. The kits were received so enthusiastically, the group immediately chose to fund the project again in 2017.

Life’s Treasures Thrift Stores Break RecordsChapters Health’s thrift stores recorded nearly $2.5 million in gross revenue for the year — a 1.18-percent increase from 2016. LifePath Hospice’s Life’s Treasures stores in South Tampa, Carrollwood and Brandon brought in $1,641,842, while the HPH Hospice thrift stores in New Port Richey and Zephyrhills totaled revenues of $765,943.

“We must give credit not just to the increased amount of great merchandise we had but to all the volunteers who were and are consistently creative with layout and design in the stores,” Ms. Jones said.

All thrift store revenues “stay local” by funding important services and programs, such as unfunded and charity care, for patients and families in their communities.

Festive FundraisingGood Shepherd Hospice’s annual Holidays for Hospice hosted 120 guests at the historic Hotel Jacaranda in Avon Park at the end of November. With wine tastings, delicious food from local chefs and a steel-band musician offering island-inspired music, the event raised $22,585 to benefit the Bud and Donna Somers Hospice House in Sebring.

“We owe great thanks to Joan Hartt and our wonderful volunteers from Highlands and Hardee counties who, once again, beautifully decorated the Hotel Jacaranda,” said Jolene Bryant, Special Events Coordinator.

More Donors and Supporters JoinChapters Health’s Sustaining Society, donors whose annual gifts support programs across the entire service area, increased to 65 members in 2017. Hospice Women of Giving at Good Shepherd Hospice finished the year with 40 members, and LifePath Hospice’s Hospice Women of Philanthropy welcomed 26 new members for a total of 113.
