To get started on your Health Track, you (or your covered spouse) need to know a few things …

Step 1: Complete your Biometric Screening by Oct. 14, 2016 (see pages 8–9 for schedule).

Step 2: Register for Houston Methodist MyChart using the special code emailed to you following the Biometric Screening.

Step 3: View your lab results in Houston Methodist MyChart, typically three to five business days after you complete your Biometric Screening (see page 11).

Step 4: Discover your 2017 Health Track assignment and focus area. Approximately two to three weeks after you complete your Biometric Screening, you’ll receive an email from Keas to let you know your results and Health Track assignment are available.

Step 5: Set your course for 2017.

• If you are on the Guided Track, get started by contacting a Care Navigator Nurse at 713.441.0001 or [email protected].

• If you are on the DIY Track, you get to customize your path. See pages 16–17 for more details.

• Don’t know where to start? Call a HealthyDirections specialist to get yourself oriented at 1.800.520.0726.

Step 6: Know your track and what is required to reach completion (see pages 14–17 for more details).

Step 7: Get started as soon as you receive your 2017 Health Track. The 2017 Health Tracks start officially on Oct. 1, 2016!

Step 8: Know the deadline — Aug. 31, 2017.

Step 9: Earn the HealthyDirections premium for 2018. Completing your Health Track means hundreds of dollars in savings.

Generic Medication SupportIf you are currently managing chronic health conditions like diabetes, high cholesterol or high blood pressure and are assigned to a Health Track, you’ll pay $0 for generic prescriptions to treat your specific condition(s) in 2017.

The Do It Yourself (DIY) TrackWe’re all on this wellness journey together, and no matter your health status, there is always something you can do for your health. The DIY Track offers you a variety of eligible activities and/or fitness activities to help you stay focused on achieving and maintaining good health. Get involved. It’s your life.

DIY Track Focus AreasYour results will highlight an area of your health that may need attention. We call this a focus area. Knowing your focus area is good information to have and may help you find a Health Track activity that pinpoints and addresses the health issue. Your focus area is determined by the following:

Who is on the DIY Track? Those who are assigned to this Health Track do NOT meet the criteria for the Guided Track (see pages 14–15 for Guided Track criteria).

Houston Methodist is committed to helping you achieve your best health. Our wellness program is designed to give you tools and resources to make healthy changes and be rewarded for it. We recognize that everyone is different and may need alternative activities to qualify for prizes and rewards. If you or your spouse are unable to meet the standard for a reward under HealthyDirections, you may qualify for an opportunity to earn the same reward by different means. Also, if your doctor states that the standard for a reward isn’t medically appropriate for you, we will work with him or her to develop an alternative qualification to earn the same reward or obtain a waiver.
