Many organizations understand the theory of Social Accountability and some basic definitions; however, getting beyond knowledge and progressing towards practice often presents a challenge. It is essential to document and report your impact on your community. This session will focus on integrating social accountability and philanthropic efforts to further an organization’s charitable mission and reporting your success to the community. The speakers will incorporate interactive learning through periods of questions & answers along with tabletop discussion.

3. Taking the Confusion Out of DementiaRob Leffler, Vice President of Clinical Services, PCA Pharmacy

The incidence of dementia and cost of treating dementia has skyrocketed as people are living longer and longer. The world of medicine has impacted so many disease states but the advancements in the treatment of dementia has lagged. This session will discuss the symptoms of dementia and some of the triggers that can cause patients with dementia to act out. The different types of dementia and how they affect patients differently will be covered as well as the ways to treat dementia with medications and through non-pharmaceutical modalities.

4. Understanding Your Medicaid ReimbursementSarah W. McIntosh, Director of Operations, Hargis & Associates LLC

This session focuses on the importance of understanding Kentucky’s Medicaid reimbursement programs. Your team’s overall purpose is to provide excellent care to your residents. In order to support this purpose, the facility has to receive reimbursement. Additionally, it is imperative to capture and document services administered by your staff. While some of today’s workforce is made up of deep-rooted, dependable staff, a large number of positions are now a revolving door. With the revolving door, some job responsibilities get overlooked. This session will help ensure that you are not omitting items that could affect your reimbursement. This session will provide an overview Medicaid Rate and how different components affect your rate. Topics will include Case Mix Index (CMI) reports, downloading CMI reports, residents that should be included in the CMI, case mix management strategies, capital rate component, requesting a capital re-appraisal, and various other Medicaid reimbursement caveats. Learn how defining the roles of your team can fuel their purpose and yours.

5. Hot Topics in Infection ControlAndrea Flinchum MPH, BSN, CIC, HAI Prevention Program Manager, Infectious Disease Branch, Dept of Epidemiology & Health Planning

This session will focus on “hot topics” in infection prevention and control. Topics will include: the CMS Mega-Rule, antibiotic stewardship, and care transitions. This session will also highlight key findings from the enhanced infection control assessments conducted by the Healthcare/Associated Infection Prevention Program in Kentucky.

7. Senior Living Affiliation Trends: Updates & Sector ImplicationsTom Meyers, Senior Managing Director, Ziegler

It is important for providers and industry professionals to understand the dynamics of recent trends and what the key drivers are when affiliations, acquisitions or dispositions occur. Providers will share their organization’s histories of affiliations and how it plays into their overall growth strategy. Case studies of recent affiliations will be shared along with lessons learned and advice for providers who are considering a formal affiliation or partnership.

It will also be important to talk about the for-profit sector and their role in these trends. Specifically, an examination of how for-profit providers view not-for-profit acquisition opportunities and how valuation differences exist between for-profit and not-for-profit providers. This information will also assist not-for-profit providers who may be considering an acquisition of a for-profit facility or who may be looking to capitalize on assets by disposing a community to a private investor group. This particular topic will be general in nature and not an in-depth discussion of finance activities related to an affiliation, acquisition or disposition.

8. Advancements in MDSEleisha Wilkes, RN, RAC-CT, Proactive Medical Review

Session participants will receive a review of MDS changes effective October 2018 with emphasis on Section GG coding and collaborative data collection, an overview of current and changing Quality Measures and the Implementation of Medicare PDPM for 2019 (updated as information becomes available)

9. Montessori Methods: Improving Quality of Life for Older Adults with DementiaKathleen Farfsing, Director, Montessori Dementia Program, Xavier UniversityMarcia Wessels RN, Twin Towers Director of Resident Experience

Get a full description of the implementation of a Montessori based program in a long-term facility in which older adults with dementia reside. Environmental modifications included staff and family training, adapting and widening the scope of activities offered, and modifying the environment to increase engagement. This was a research project conducted in collaboration between Xavier University and Dr. Jennifer Kinney of Miami University Scripps Gerontology. Dementia Care Mapping (Brooker & Surr, 2005), employed before and after implementation, revealed Montessori interventions engaged residents and promoted their sense of personhood.

We are experiencing the most challenging labor market in decades. Notwithstanding these challenges, opportunities exist to mitigate the growing labor costs and maximize ROI by investing in people.

Operators and capital partners in seniors housing and skilled nursing will hear best practices for attracting, growing, and retaining an effective, engaged workforce. Data and cost analysis will be presented on employment rates, contract labor, turnover, employee disengagement, and other factors that have an impact on the seniors housing industry. This session will share proven strategies that lead to improved employee experiences, including compensation, benefits, perks, culture, and leadership.

11. Medication Administration Mysteries RevealedNatalie Daum, RN, LNC

The mysteries of medication administration that circle around Assisted Living within the state of Kentucky will be revealed during this presentation. What does the law say is allowed in Assisted Living Communities? Can you provide assistance with self-administration of medications? Are you allowed to “read labels” for the residents? Can you assist with taking medications out of containers? What is your responsibility to ensure medications are taken properly and how do you do this?

After completing this session these mysteries will be eliminated and you will be able to confidently ensure medication administration is provided within the requirements of the state of Kentucky.

The Improving Medicare Post-Acute Care Transformation Act (IMPACT) of 2014 requires continued specification of quality measures for the SNF Quality Reporting Program (QRP). Measures were implemented in October 2016 with more measures on the way as finalized in the SNF FY 2018 Final Rule. Come join us to double check your understanding of existing measures, discuss new and future measures, including updated coding options/items for Section GG, that are on the horizon and explore options for monitoring of measures to ensure your facility is on the road to success.

14. Know Your NarrativeKevin O’Connor, Vice President, Providigm, (Medline Partner)

The Affordable Care Act (ACA) has set into motion efforts to slow the rate of growth in health care spending and to transform the current fee for service payment system into a value-based payment system. In addition, the IMPACT Act allows hospitals to create preferred provider’s lists and recommend certain providers to patients, so long as those providers are determined based on Quality Metrics.

Even with the current administration, Accountable Care Organizations (ACOs), Bundled Payments, and Primary Care Transformation all will impact the way long term care providers are assessed as appropriate continuum partners, a key factor in business viability.

This program will provide insight into how to use existing Quality Metrics to best market your facility to continuum partners thus increasing referrals and census.

15. Housing Policy Update from Washington, DCLinda Couch, Vice President, Housing Policy, LeadingAge

Get up to date on federal housing policy: what’s happening with RAD for PRAC, the new funding for new homes in the Section 202 program, proposals on rent reforms, the Low Income Housing Tax Credit, and more. Understand the latest policy proposals and bills, learn your best advocacy options in Kentucky, and dive into shaping the future of housing policy for older adults.

16. Medicaid Policy and Program Updates for 2018Jill R. Hunter, MA, Commissioner, Department for Medicaid Services, Cabinet for Health and Family Services and Beth Vail, Myers and Stauffer

Staff from the Department for Medicaid Services will provide high-level updates on programmatic changes that have recently occurred in the Kentucky Department for Medicaid Services. Specific topics will include of the Level of Care Process for Nursing Facilities and ICF/IDD, Medicaid eligibility determination process, changes with MAP 552 and an overview of common issues arising out of Medicaid RUG Audits for Nursing Facilities.

18. An In-depth Look at Reporting and Investigating Abuse and NeglectSandra Houchen, R.N., Director, Division of Health Care, Office of Inspector General

This session will focus on two main components of a comprehensive abuse prevention program as required by the federal regulations, reporting and investigation. The session will cover what, when and to whom to report allegations and incidents of suspected abuse and neglect. It will also cover the components of conducting a thorough investigation and what facilities need to consider to develop and implement necessary actions to address abuse or neglect situations.

19. Kentucky Court Rulings Impacting Non-Profit Housing and Long Term Care ServicesBetsy Weber, Attorney, DBL Law Representative from Department of Revenue

The Kentucky Court of Appeals recently determined that possession of a residential unit in a non-profit Senior Living Retirement Community by a resident constitutes a taxable leasehold interest. The units at issue are part of a non-profit Senior Living Retirement Community. Non-Profit Senior Living and Disability providers have historically been considered a purely a public charity and therefore exempt from property taxes under Section 170 of the Kentucky Constitution. In their final ruling the courts held residents liable for paying any property taxes associated with their living unit.

20. Aging Matters: Human Aging 101Keith R. Knapp, PhD, CNHA, HSE, FACHCA, CNA, Associate Professor Chair, Department of Health Services and Senior Living Leadership, Bellarmine University

Knowing some basics about human aging can really enhance a “non-clinical” service staff member’s tool chest…from dining services to activities/life enrichment, maintenance, laundry, business office, human resources, medical records or the beauty shop. What is normal vs usual aging? What differences can be expected as the Baby Boomers arrive? This session provides an overview of the major aspects of advancing age – physical, mental, emotional, social and spiritual - as well as some examples of high impact practices that have resulted in positive outcomes in the senior living field.

21. Food Nutrition and Dining: Critical Components for Successful Outcomes in Post-Acute CareBrenda Richardson, MA, RDN, LD, CD, FAND

Healthcare teams today are breaking down silos and creating personalized holistic treatment plans for their residents. Yet one area that often goes overlooked is a resident’s nutritional status, despite the growing body of evidence that demonstrates its positive impact on overall care and cost reduction. This session will explore the consequences of malnutrition such as longer hospital stays, more frequent readmissions, increased costs of care, and higher risk of complications and mortality. The impact of malnutrition will no doubt become more prevalent as the population ages and has more chronic diseases. About half of all adults – 117 million people – have one or more chronic health condition, according to the CDC. As this group ages, they are at higher risk for poor nutrition, due to factors like loss of appetite, limited mobility or even inflammation from the disease itself.

Despite its pervasiveness and severe consequences, malnutrition far too often goes undetected or is deprioritized. This session will present ways to easily and cost-effectively solve this growing issue in post-acute care.

22. Home Community Based Waiver TransformationJill R. Hunter, MA, Deputy Commissioner, Department for Medicaid Services, Cabinet for Health and Family Services

This session will offer an update on Kentucky’s 1915(c) redesign. The redesign is an effort to ensure the best use of resources to deliver services that meet people’s needs, enhance dignity and quality of care while ensuring all efforts align with Commonwealth and federal regulations. State representatives will discuss results of Navigant’s assessment, stakeholder feedback, and the Commonwealth’s next steps in the process. The Department of Medicaid Services is committed to ensuring that stakeholders are educated and engaged in the process.

Continuing Education With 33 hours of education to choose from, any attendee participating in an education session during each offering time eligible to earn 9 credit hours over the 1.5-day conference. LeadingAge Kentucky is an approved provider of education for the Kentucky Board of Licensure for Nursing Homes Administrators and the Kentucky Board of Nursing. (Please note: Kentucky Board of Nursing approval of an individual nursing continuing education provider does not constitute endorsement of offering content).
