What’s New?

A new vision plan is being added for 2019.

You will now have a choice between the Vision Discountprogram at no cost, or the comprehensive Vision Choice plan with a monthly premium.

Please see the Open EnrollmentGuide for details.

The Vision Discount plan will remain the default if you do not make an active election.

- Maximum benefits with PPO Delta Dentists

- $50 annual deductible

- $1,700 annual maximum benefit (PPO Dentist)

- $1,500 annual maximum benefit (non- PPO Dentist)

▪DeltaCare USA

- HMO dental plan– Must use DeltaCare USA dentists only (except in emergencies)

- Available in California only

- Co-payments vary based on services performed

- No annual maximum benefit or deductible

Becoming Medicare eligible

▪ You can be eligible for Medicare as follows:

- Normally at age 65

- Before age 65 if disabled and receiving Social Security disability benefits

- Are you eligible for Medicare? Contact Social Security Administration (SSA) to find out

– If you are currently receiving Social Security check, you will be automatically enrolled in Part A & B and will be sent an ID card

– If you are not eligible for Medicare, SSA will provide you with a letter that you will need to send to Empyrean as proof

▪ If Due to Disability- You will be notified by Social Security Administration when you

become eligible for SSDI benefits

– Option 1: Enroll in Kaiser Senior Advantage through Empyrean

– Option 2: Enroll in other individual Medicare supplement plans through Via Benefits

– Option 3: Not eligible for Medicare, stay on Non-Medicare plan

– Option 4: Suspend LLNS coverage and enroll through spouse’s plan

- Non-Medicare dependents will continue in the same plan with direct contributions

Medicare - What’s New?

▪New Medicare Card

‐ New cards have been mailed to those living in California

‐ Once received, destroy your old Medicare card and start using your new card right away

‐ Your new Medicare Number is a unique combination of numbers and letters –it no longer uses your Social Security Number

Watch out for scams! Medicare will never ask you to give us personal or private information to get your new Medicare Number and card

‐ Election made through Empyrean (for California only)

‐ Group plan

‐ Cost is $227.68 per Medicare eligible person

– Annual cost is $2,732.16

– $282.16 annual out-of-pocket expense if receiving 100% LLNS contribution to HRA

‐ New enrollees will need to complete a KPSA group enrollment form

‐ Forms must be completed and submitted to Kaiser by December 31st

for January 1, 2019 effective date

▪Medicare 2020 MACRA Legislation –- Medicare Plans that cover the Part B deductible can no longer be sold as of

1/1/2019 to newly eligible Medicare participants

- Therefore Medigap Plans F and C will be eliminated for new enrollments by Medicare

- Those enrolled in Plan F or Plan C as of 12/31/2019 will be grandfathered

- Closest coverage will be Plan G

- Medicare Part B deductible is $183 for the year

▪ The HRA monies can be used to reimburse you for eligible health care expenses on a tax-free basis

- Expenses eligible for reimbursement:

‐ To be eligible for the “Donut Hole” special payment you must been enrolled in a medical plan through Via Benefits

‐ Each Medicare eligible participant is eligible for this payment

‐ The gap covers drug costs from $3,820 to $5,100

– Maximum lump sum payment is $2,550 (50% of the $5,100), prorated based upon month in which donut hole gap is reached

▪ Payment is taxable; you will receive a 1099-R
