Instructions:

• This application form is for the awards with a deadline of February 1st, 2021 (Winter Term). • Carefully review the award description, requirements and application deadline provided on the

Enrolment Services Scholarship Sorter website.

Steps to complete the award application form: (Please read it carefully)

NOTE: Do not fill out the form directly onto your internet browser or Microsoft Edge viewer. Please

download and save the form as a PDF file first. Failure to do so will cause an inaccurate information when

trying to enter responses in some fields.

1. Download the Adobe Acrobat Reader:

• You will need the Adobe Acrobat Reader DC (Free version) or the Adobe Acrobat PRO DC (Paid

version) to complete the award application form with your information.

• If you do not have one of this, download either the free or paid version from:

https://get.adobe.com/reader/. The software is compatible with Mac and PC.

2. Download the PDF award application form on your computer:

• Open the “Award Application Form” which can be found on the Enrolment Services

Scholarship Sorter website. You will have to search for the award you applying for.

• Click “Download” located on the top right corner on the form. Download this form in a folder

on your computer and select “Adobe Acrobat Document” under “Save as type:” Change the

“File Name” section to “Last Name, First Name 2021 Winter Award Application”. Click Save.

This will automatically save in “Adobe PDF Files (*.pdf)” Format.

• Locate the folder on your computer where you downloaded the form then right click on the

form. Select “Open With” either Adobe Acrobat Reader DC or Adobe Acrobat PRO DC.

Depending on what Adobe Acrobat version you have. For the free version, you’ll have to open

the form with “Adobe Acrobat Reader DC”.

• Complete all sections of this application form by entering the information directly into the

fields. If numeric fields do not apply, indicate “0”. If text fields do not apply, indicate “not

applicable”.

• Insufficient information and/or documentation may impact the consideration of your

application.

• Remember to save the application form again with your information.

3. Email the completed application form (PDF format) and the required documentations:

• Application form and all required documentations must be submitted as “ONE” PDF File (if

applicable) via email as an attachments to Enrolment Services (awards.uoft@utoronto.ca)

• Subject line must be “2021 Winter Award Application”.

Paper or scanned applications will not be accepted.

• Select awards where you meet the award requirements. • Each form allows you to select up to 5 awards. If you wish to be considered for more than 5 awards, you may

submit multiple forms. • It may be helpful to have the Awards website open in another window/tab while completing the application form to

ensure you have provided all of the required details and application components.
