2020-21 DANCE PROGRAM REGISTRATION & CONTRACT *Total family income must meet federal income eligibility requirements. Completion of scholarship form & proof of income required. Class & Payment Information: Fees are based on total class sessions for the year (including recital & rehearsal), regardless of the number of classes in any particular month. Payment for the entire year receives a 5% discount on full price classes only. *Total family income must meet federal income eligibility requirements. Completion of scholarship form & proof of income required.

Class & Payment Information:

Fees are based on total class sessions for the year (including recital & rehearsal), regardless of the number of classes in any particular month. Business Hours: We are open Monday-Friday, 9am-5pm For payment after hours and on weekends, a drop box is located in hallway next to the front desk or a drop slot on the front of the building near the sign. Put payments in an envelope and indicate on the envelope your child’s full name and class title. Checks are strongly encouraged for drop box/slot payments.

Dance Agreement & Liability Waiver:

I understand and will abide by the payment schedule specified on this contract. I realize that if I am delinquent in my payment for classes, costumes or fundraisers, my child will not be permitted to attend class and/or removed from the program without refund. I will be held responsible for the monthly fees & total money owed regardless of attendance or program supsension/termination. I acknowledge that any outstanding balance on my account will be submitted to a collections agency. I understand that poor attendance and/or deliquent payments may result in exclusion from competitions associated with specified classes and/or removal from the program.

LIABILITY WAIVER: All registrants are required to sign the following release. Parents or guardians must sign if participant is under 18. I, the above named candidate for participation in the activities listed above, hereby waive any claim for bodily injury or property damage against the School District of Lancaster, the City of Lancaster, Lancaster Township and the Lancaster Recreation Commission, while a participant in any program at any location. I also permit the Lancaster Recreation Commission to use any photographs or videotape of me or my child(ren) for promotional purposes. In the event of a medical emergency, I authorize the Lancaster Recreation Commission staff to obtain medical treatment for me, my son and/or daughter or minor for whom I am the guardian.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate on behalf of Lancaster Recreation Commission athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,4. Lancaster Recreation Commission PARENT CODE OF CONDUCT

The essential elements of character building and ethics in sports are embodied in the concept of sportsmanship: trustworthiness, respect, responsibly, fairness, caring, and good citizenship. I therefore agree:

• I will not force my child to participate in sports. • I will remember that children participate to have fun and that the program is for the youth, not adults. • I will inform the teacher/coach of any physical disability or ailment that may affect the safety of my

child or the safety of others. • I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing

respect and courtesy, and by demonstrating positive support for participants and teachers. • I (and my guests) will not engage in any kind of unsportsmanlike conduct with any teacher, staff

member, participant or other parent such as using profane language or gestures. • I will not encourage any behaviors that would endanger the health and wellbeing of participants • I will teach my child to follow the rules and to resolve conflicts without resorting to hostility or

violence. • I will demand that my child treat other everyone with respect regardless of race, creed, color, sex or

ability. • I will teach my child that doing one's best is most important so that my child will never feel defeated by

his/her performance. • I will praise my child for trying hard and giving full effort. • I will never ridicule or yell at my child or other participant for making a mistake. • I will emphasize skill development and individual growth. • I will promote the emotional and physical well-being of participants ahead of any personal desire I may

have for my child. • I will respect the teachers and their authority and will never question, discuss, or confront them during

class or performances field and will take time to speak them at an appropriate and agreed upon time and place.

• I will demand an environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all events and prior to attending any events.

I also agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that may include, but is not limited to the following:

• Verbal warning by teacher, staff and/or department administrator • Written warning • Parental suspension with written documentation of incident kept on file by organizations involved • One year suspension

Lancaster Rec sets reasonable limits that we expect everyone to follow. The basis of our discipline policy is respect for people and property. Appropriate language is expected as well as respect for authority. Hurting another person physically is dealt with promptly in our programs. We dismiss individuals from our program if inappropriate actions are a recurring problem. For severe cases of misconduct, it may be necessary to dismiss individuals immediately from our program. July 2020 Dear Parent and/or Guardian, The Lancaster Recreation Commission would like to welcome you and your child/children to our dance program. Important information for you to know prior to signing up your child:

• Classes are one day a week through May/June (depending on recital) • A monthly fee is due the 1st of every month • A recital is held in late May-mid June – determined by SDoL availability • All dancers need to wear specific dance attire to class • A costume for the recital must be purchased for each dance class in which

your child enrolls –cost is $80 • A fundraiser is offered to help with costume fees if registered by a certain date.

o Will be distributed during the first weeks of class • Review the Dance Contract completely for payment deadlines/expectations • A mandatory dance fundraiser will be held in late Winter/early Spring to support

the recital • Scholarships are available - ask registration desk for information • A Plan for Covid-19 protocols is provided in your registration packet.

* Monday classes are 15 minutes longer to build in time missed for the multiple Monday holidays/closures throughout the program. **Little Dancers is a multi-style class and will learn both tap and ballet. The style listed will be danced in the recital. ***Invitation Only– Receiving an invitation does not guarantee a spot in the class. Dancers still must register & class size is limited.

Dance COVID-19 Guidance

***Failure to comply with these mandated rules may result in suspension from the program/league***

DISCLAIMER: Participants, volunteers and spectators engaging in football and activities related to football do so with knowledge of the risk and potential exposure involved and agree to accept any and all inherent risks to their personal health. NOTE: Guidance was developed using information from Dance USA and the PA state sport safety guidelines. They are subject to change. NOTE TO STAFF: Please inform participants and families of the following guidelines.

Prior to Arrival at Class/Rec Center:

1. Take body temperature to ensure that no fever is present prior to attending any Lancaster Rec activity.

2. Conduct a health screening (see chart below for guidance). a. Anyone with symptoms of fever, cough, or worsening respiratory symptoms, or any

known exposure to a person with COVID-19 should not attend any Lancaster Rec activity unless cleared by a medical professional.

3. Encourage all dancers to practice good general health habits, including maintenance of adequate hydration, consumption of a varied, vitamin-rich diet with sufficient vegetables and fruits, and getting adequate sleep.

4. Clean and sanitize personal and shared equipment before and after each class. 5. Dancers should arrive to the Rec Center in dance clothes. Changing in the restrooms is

discouraged. a. Dancers may wear a cover-ups to and from class but must remove them prior to

entering classroom. 6. Wash hands and do not touch eyes, nose or mouth prior to or during a Lancaster Rec event.

a. Tie hair back to help refrain from touching face during class. 7. Do not congregate in the parking lot or in the lobby. Wait in cars until it is time to check in for

class. 8. Upon each dancer arriving, a health screening & temperature check must be completed by

teacher (see chart below). a. Dancers must maintain distancing while waiting to enter class/get temperature checked. b. All dancers must exit the room prior to allowing incoming students to begin temperature

checks and entering the dance room. 9. Use hand sanitizer to disinfect all dancers & teachers hands prior to class and upon check-in.

a. We encourage everyone to bring a personal hand sanitizer as well, if possible. 10. Provide a mask to wear. If you cannot provide a mask, Lancaster Rec has a limited supply. We

strongly suggest bringing your own, if possible. 11. Dancers, teaching staff, dance helpers and other staff must wear face coverings (masks or face

shields) at all times, unless doing so jeopardizes their health. a. Masks should fit snugly over the nose and mouth, not be touched once placed on the

face, and washed or discarded after each use. 12. Limited family members (1 person/dancer) are permitted in the lobby while classes are taking

place. Everyone must maintain social distancing protocols (6ft from those outside of their household) and must wear masks.

a. We encourage parents to wait in cars to avoid overcrowding in the lobby area, if possible.

Equipment:

1. Dancers should limit any items brought into the dance room and place any required individual costumes/equipment in a well-spaced out manner.

2. Any shared equipment must be sprayed or wiped with cleaner and disinfectant before and after each use.

Drinks and Snacks:

1. Dancers must bring their own personal drinks to class. Drinks should be labeled with the person’s name.

o Water fountains are currently out of service for health/safety reasons. 2. Individuals must take drink containers home for cleaning/sanitization or use single-use bottles. 3. There should be no use of shared or group beverages/snacks. 4. Chewing gum, etc., is not be allowed in the lobby or dance room. 5. No food or eating is permitted in the lobby or dance room. Dancers & family members must refrain

from bringing food into the building.

Limited Admittance:

1. The dance room & classes will be limited to teachers, dance helpers and dancers. In-Class Guidance:

1. Maintain social distance (6ft) between all dancers and teachers when possible. a. Floor & barres will be marked to help encourage and maintain distance.

2. Dancers, teaching staff, dance helpers and other staff must wear face coverings (masks or face shields) at all times, unless doing so jeopardizes their health.

a. Masks should fit snugly over the nose and mouth, not be touched once placed on the face, and washed or discarded after each use.

3. Dancers & teachers are encouraged to sanitize hands during any breaks, when possible.

a. It is recommended that each dancer/teacher provide a bottle of sanitizer for personal use.

4. Limit dancing that requires contact between students. a. For any skills/movements requiring contact, keep dancers with same partner as much as

possible & use hand sanitizer after. 5. All equipment, barres & door handles must be sanitized after each class.

a. It is recommended that only teachers open the door at the start and end of classes. 6. All participants are encouraged to shower promptly and wash all clothing after each class. 7. If a class or dance involves a prop, they are not to be shared by dancers.

Public Restrooms:

1. Restrooms will follow 1 in-1 out policy. Please wait until the restroom is empty before entering a. Exception: 1 adult may accompany 1 young child, if needed.

No Handshakes/Personal Contact Celebrations:

1. Dancers should take measures to prevent all but essential contact to the dance. o This includes refraining from handshakes, high fives, fist/elbow bumps, hugs, group

celebrations, etc. 2. Dancers and family member should vacate the facility as soon as is reasonably possible after the

conclusion of their class to minimize unnecessary contact with other dancers, families, etc. If illness occurs, the following steps will be taken:

1. Participant, teacher/staff member will be designated to separate area. 2. Participant’s parent/guardian will be notified and the player MUST be picked up within 60

minutes. 3. Teacher will notify Director of Athletics and Recreation immediately. Executive Director will then

be notified who will promptly inform the necessary public health officials. Health Screening Chart:

General class rules & information • Families must park in the back parking lot. Front doors cannot be used for entrance after 5pm • Be dressed and ready to enter dance room at start time & be ready to pick up your child when class is over • Students should go to bathroom before coming to class • Label all pieces of clothing- especially shoes • Have bag with shoes in for classes with shoe changes, if applicable • Only 1 parent/guardian is permitted to wait in lobby during class (SEE Covid plan) • No cell phones in dance room (students must leave in dance bag on silent) • If a child becomes a class distraction, he or she may be taken out of class for that day. This is not a punishment, it is

necessary so that the other children can learn. • Please avoid looking through the door window, it becomes a distraction to students and teachers. • Appropriate behavior is expected from participants and parent/guardian. • For updates: check Dance bulletin board in lobby for information that is given out in class • Please reference the Covid Plan for Dance included in registration packet for detailed directions/guidelines/protocols

Attendance

• Call Miss Eloise if your child will not be attending their class due to illness, 392-2115 x452 • In order for our dancers to receive a quality dance experience, it is important that attendance is taken seriously. Attendance

is taken in every class. Injured students are expected to attend and watch. There is leniency for appropriate reasons (school function, family wedding/vacation), but teachers should be notified of such events and approve absences in advance.

• After three absences from competition classes, dancers go on probationary status and evaluated on a case by case basis. • After five missed classes for any student, dancers may be removed from the class with no refunds. There will be no refunds,

credits or discounts for missed classes. • Students receiving scholarships are required to attend 80% of all classes to fulfill scholarship guidelines • Poor attendance and/or delinquent payments may result in exclusion from competitions associated with specified classes.

Dance wear & costumes

• Do not wear dance shoes outside • Must wear proper dance attire to class & long hair must be pulled back • Costumes should only be worn for pictures, competitions, rehearsals, and recital

Schedule & Cancellations (weather or instructor illness)

• Follow Lancaster Rec Dance Class calendar not School District of Lancaster. A Dance calendar distributed first week of class

• If School District of Lancaster schools are closed/or dismiss early due to weather issues, dance is cancelled • If instructor is sick- an email will be sent. If possible, a phone call will be made but is not guaranteed. Make up classes will

be scheduled (may be on a different night than regular class)

Payments • Payment is due by the 1st of the month or the Monday following if the 1st is a Saturday or Sunday. Secure drop slots are

available on outside of building and next to office-put in an envelope with students name and dance class listed (no cash). • For accounts in good standing, a 5 day grace period will be given. Late payments will result in your child being suspended

from class until your account is paid in full. Monthly fees will continue to accrue regardless of attendance or program suspension. Fee adjustments will only be made with a signed doctor’s note. If your account is delinquent, your child may be removed from the program with no refund including costume fees.

• Costume balances are due by 10/30/20. Costumes will not be ordered unless balances are paid. Costumes are $80/class. • Recital Fundraiser (winter/spring): All dancers will be required to sell candy or pay the opt-out amount of $30. • All account balances must be paid up before receiving your costume(s) and/or participating in the recital.
