Enrollment PeriodsYou can only make changes to your benefit elections during Annual Enrollment or within the allowed timeframe of a qualified status change during the year. These status changes include birth or adoption, getting married or divorced, loss of coverage, etc.

Time for a 401(k) Check-up Although you are not required to make any 401(k) changes, Annual Enrollment is a good time to review all of your benefit options. While you can make changes to your 401(k) contributions throughout the year, now may be a good time to make sure your investments are still aligned with your retirement goals. Key Benefits Changes for 2020Medical PlansBeing a self-insured medical plan means employers and employees take on the responsibility for every medical and prescription drug claim that is processed. With the continued increase in medical plan costs, all claims processed have a direct impact on overall costs each year.

New Services

Be prepared and know where to go for quality, cost-efficient care. Use the new Health Advocate team or ConsumerMedical to help you navigate the health care system depending on your medical needs. And check out Benefit Scout to help you with your life insurance questions.

• Health Advocate - a new service from Blue Cross and Blue Shield of Illinois is made up of non-clinical representatives that can address questions about benefits, consult with clinical nurses and pharmacists as well as other experts on specific health care needs. They can help find in-network providers, schedule doctor’s appointments, or assist with pre-authorization steps including cost for health care services. It’s available to you and your covered dependents at no additional cost and could even help you save money through the Member Rewards program.

• ConsumerMedical - a benefit that can help you and your family make more informed medical decisions and get better care at no cost to you. ConsumerMedical is a free benefit that provides personalized, one-on-one support to help you and your family.

• Benefit Scout - an educational tool created by Securian Financial/Minnesota Life that assists employees with life insurance decision making. Employees are guided through a short series of questions to help them compare life insurance options and cost estimates. The tool will then provide coverage suggestions based on these responses and the employees may enroll in additional coverage on www.lifebenefits.com.

Medical Plan OptionsBoth Consumer Choice 1 and Consumer Choice 2 are High Deductible Health Plans, meaning your premiums (payroll contributions) are lower, but you pay the full cost of all non-preventive care services until you meet the full deductible. That means the cost of the insurance is lower, but your costs are higher when you use the plan, up to the amount of your deductible.

While Consumer Choice 2 has a higher deductible, it also has zero coinsurance. So, once you reach your full deductible, the plan pays 100 percent of any other costs you incur for the rest of the plan year. While everyone’s situation is different, this might be a good option for people with higher than average medical care needs.

Save on Preventive Care PrescriptionsPreventive care is important for keeping you healthy today and tomorrow, and prescription medication that helps treat chronic conditions is a big key for your health.

Certain drugs classified as “preventive care prescriptions” are covered at no cost, regardless of whether the deductible has been met. These include drugs that treat chronic conditions such as high blood pressure, high cholesterol, etc. To see which drugs are on the list, visit www.FarmCreditFoundations.com. This list is subject to change throughout the year.

Prescription Drugs

Farm Credit Foundations prescription drug coverage is administered by CVS/Caremark. The cost of your prescription medication will vary, depending on which medical plan you enroll in. There are no prescription drug copayments. Prescription costs paid by you accumulate towards your annual medical deductible.

Coverage by Medical Plan• Consumer Choice 1

You pay 100% of the cost, up to the plan deductible, and then 20% coinsurance up to the annual out-of-pocket maximum.

• Consumer Choice 2 You pay 100% of the cost, up to the plan deductible. Once you have reached the deductible, the plan pays 100% of the cost. The annual deductible and out-of-pocket maximum are the same.

Things to Consider for 2020

• Use the CVS/Caremark Rx Cost Estimator to estimate the cost of your prescription medications. Go to www.FarmCreditFoundations.com.

• Many preventive generic drugs are covered at 100%.• CVS/Caremark is the prescription drug manager, but you are not limited to

only CVS pharmacies.• Revisit your medications with your physician at your next checkup.

Whether you’re at home or traveling, you and your covered family members can speak with a board-certified physician 24/7 without setting foot in a doctor’s office. How? Through MDLIVE* with U.S. Board-certified doctors and pediatricians with an average of 15 years of experience.

With this benefit, you can:

• Save time and money ($44 per virtual medicine visit; counts toward deductible)

• Receive medical treatment for many non-emergency conditions, including colds, flu, asthma, allergies and nausea

Health AdvocateYou have a Health Advocate with Blue Cross and Blue Shield of Illinois

The new Health Advocate team includes registered nurses, social workers and health and behavioral specialists who help you (and your family) navigate and coordinate health care services over the long term. Use a Health Advocate to understand how your benefits will cover a medical procedure or when you have a benefits claim question, find a doctor and schedule appointments.

Health Advocates help with pre- or post-surgery care, or managing a chronic health condition (like coordinating care for diabetes treatment with your doctor, pharmacist and other care providers).

This benefit is available to you and your covered dependents at no additional cost and could even help you save money through the Member Rewards program.

Look for a new Medical ID Card in December 2019!

ConsumerMedicalConsumerMedical is a benefit that can help you and your family make more informed medical decisions and get better care at no cost to you. Receive help with any medical condition, diagnosis or health topic, including prevention and wellness information. Cancer, back pain, asthma, osteoarthritis, high blood pressure, ADHD, diabetes and anxiety are just a few examples. ConsumerMedical is a free benefit that provides personalized, one-on-one support to help you and your family:

• Understand any medical condition and all available treatment options• Find the most qualified doctors and top-rated hospitals in your area• Get a second opinion from elite specialists across the country, either in person or virtually for complex cases• Ask your doctor the right questions• Cope with the stress of having a medical condition

Tax-Advantage Accounts

Health Savings Account (HSA)If you enroll in the Consumer Choice 1 or Consumer Choice 2 plan, you can elect an HSA, which allows you to set aside pre-tax dollars (up to annual IRS limits) to pay for eligible health care expenses either now or any time in the future.

With an HSA, you can make contributions through payroll deductions up to IRS annual limits, which, for 2020 are:

• $3,550 for individual coverage.

• $7,100 for family coverage.

• $1,000 catch-up contributions for participants age 55 or older.

Limited Purpose Flexible Spending AccountIf you enroll in the Consumer Choice 1 or Consumer Choice 2 plan, you can open and contribute to a Limited Purpose FSA. Funds from your Limited Purpose FSA can be used to pay for non-medical eligible charges, such as dental and vision. The maximum you can contribute is $2,750.

Health Care Flexible Spending AccountThe health care FSA reimburses you for eligible health care expenses. The maximum amount you can contribute to your health care FSA is $2,750 per year. You are not eligible for a health care FSA if you are enrolled in a high-deductible health plan.

Dependent Care Flexible Spending AccountThe dependent care FSA reimburses you for expenses associated with the care of a dependent while you or your spouse work or attend school. The maximum amount you can contribute to a dependent care FSA is $5,000 per year. The IRS family maximum is $5,000. If you are married and filing income taxes separately, the maximum amount each spouse can contribute is $2,500.

Things to Consider for 2020

• Tax Advantage Accounts are set to $0 on January 1 of each year. You must enter an amount in the Dayforce enrollment form if you choose to contribute in 2020.

• You can adjust your HSA contribution throughout the calendar year - you are not locked into the amount specified during Annual Enrollment.

• If you are in either Consumer Choice plan, you can use the Limited Purpose FSA for eligible dental and vision services so you don’t have to use your HSA funds.

Dental Plan OptionsYour dental plan is a Preferred Provider Organization plan administered by Delta Dental of Kansas.

You can choose between two plans:

• Basic Plan - Provides benefits for diagnostic, preventive and basic care

• Comprehensive Plan - Provides all of the above, plus offers coverage for a more extensive range of dental care including orthodontia.

Things to Consider for 2020

• You can use a Limited Purpose FSA for eligible dental services not covered beyond your maximum benefit coverage.

• Does someone in your family need braces next year? It might be a good time to switch to the Comprehensive Plan for orthodontia coverage.

• In the 2019 Benefits Survey, 84% of Farm Credit employees said the dental plans meet their needs.

Vision PlanYour vision plan through VSP offers coverage for you and your eligible dependents for eye exams, lenses, frames and contact lenses. VSP will also provide a discount for laser vision correction surgery when you use a participating provider.

VSP pays for the majority of expenses for a number of services when you use a participating provider. Providers can be found on VSP’s website at www.vsp.com. Use the Signature Network when searching for a network provider.

The vision plan covers:

• Exam—Once every 12 months

• Frames—Once every 24 months (child(ren) once every 12 months); $180 allowance

• Eyeglass lenses—Once every 12 months

• Contact lenses—Once every 12 months; contact lens benefit is not available in the same year that frames and lenses are purchased; $180 allowance

• Laser vision surgery—Discounts available through network providers. Go to www.vsp.com for more information.

Things to Consider for 2020

• Routine eye exams are covered under the medical plan. If you need glasses, contact lenses or more comprehensive coverage, you may want to consider electing vision coverage.

• You can use a Limited Purpose FSA for eligible vision services not covered be-yond your maximum benefit coverage.

• In the 2019 Benefits Survey, 77% of Farm Credit employees said the vision plan meets their needs.

Making Changes

Visit Minnesota Life’s secure website at www.LifeBenefits.com to review and make changes.

Employer-Provided Benefits Your employer pays the entire cost for you.

• Basic Term Life and Accidental Death & Dismemberment (AD&D) Insurance Plans Coverage equal to one times your total compensation.

• Business Travel Accident Insurance Coverage equal to three times your total compensation.

• Long-Term Disability (LTD) Insurance Coverage that provides 66 2/3 of your monthly total compensation in the event you become disabled and unable to work.

Employee Optional Benefits Your employer will pay a substantial portion of the cost of your coverage. You pay your portion of the premiums pre-tax. Domestic partner benefits are taxable.

• Optional Basic Employee Term Life and Accidental Death & Dismemberment (AD&D) Insurance Elect coverage for an additional one times your total compensation. You pay premiums pre-tax.

• Group Universal Life (GUL) Insurance Elect up to 10 times your total compensation for yourself, up to $250,000 for your spouse. Cash accumulation fund option also available. You pay premiums after-tax.

• Dependent Child(ren) Life Purchase up to $25,000 in life insurance for each eligible dependent child. You pay premiums after-tax.

• Voluntary Accidental Death & Dismemberment (AD&D) Insurance Elect coverage for you and/or your family up to 10 times your total compensation to a maximum of $750,000 in $25,000 increments. You pay premiums after-tax.
