To support your enrolment to Norfolk Village State School we require the following documentation:
Students Birth Certificate (original or certified copy, birth extracts will not be accepted)
Proof of residency (only required if no siblings attend NVSS or if sibling will be in Year 7 in 2021) o Rental Agreement or Rates Notice with current dates o Electricity Account with current dates o Drivers Licence showing current residential address
Passports (required only for students born overseas)
To assist parents to learn about school policies and procedures please read through the Preparation for Prep 2021 guide to give you an idea of what will be on offer at our school. Any dates for activities that are detailed in this guide are yet to be confirmed. On completion of paperwork please return all relevant documentation to the school office via email – emails are only to be sent to [email protected]. Enrolment forms will only be accepted once all additional paperwork as listed above has been received. To ensure enrolment forms are accepted, they must only be emailed to the above address. Welcome to our School
Norfolk Village State School staff, students and families work together as a team, building relationships as we work towards achieving quality learning outcomes. Respect for self, each other, our school and the wider community are the guiding principles in developing our relationships.
All members of Norfolk Village State School have rights and responsibilities they must undertake to ensure it is a productive environment in which to learn. This Enrolment Agreement sets out the responsibilities of the students, parents and staff associated with NVSS for the duration of the child’s enrolment.
It is the STUDENT’S responsibility to:
Attend school regularly, on time, ready to learn and take part in school activities.
Act at all times with respect and show tolerance towards other students and staff.
Work diligently and comply with requests or directions from teachers and staff members.
Follow school rules, meet all course requirements and wear the school uniform.
Demonstrate respect for themselves, other members of the school community and the school
environment.
After school hours, students in uniform are expected to comply with the school rules and
regulations as outlined in the Responsible Behaviour Plan for Students.
Take responsibility for their own behaviour and learning.
It is the PARENT’S/GUARDIAN’S responsibility to:
Parents are expected to show an active interest in their child’s schooling and progress.
Inform the school of any reason for absence from school.
Work with school staff in an environment of trust and respect.
Contribute positively to behaviour support plans that concern their child.
Follow the school’s policy regarding access to school grounds before, during and after school
hours.
Attend parent functions where possible.
Any concerns regarding students, staff, other parents and issues pertaining to school operations
must be dealt with through the school or Education Queensland (not through the media).
Ensure that their children comply with the dress code for students.
Cooperate with the school to achieve the best outcomes for their child.
Initiate and maintain constructive communication and relationships with school staff regarding their child’s learning, wellbeing and behaviour.
It is the SCHOOL’S responsibility to:
Clearly articulate the school’s expectations regarding the Responsible Behaviour Plan for students
and the school’s Dress Code Policy.
Inform students, parents and carers about what the teachers aim to teach the students each term.
Teach effectively and to set the highest standards in work and behaviour.
Take reasonable steps to ensure the safety, happiness and self‐confidence of all students.
Inform parents and guardians regularly about how their children are progressing.
Contact parents and guardians as soon as is possible if there are concerns about the child’s school
work, behaviour, attendance or punctuality.
Consult with parents/guardians on any major issues affecting students.
Be open and welcoming and offer opportunities for parents and carers to be involved in the school
community.
Ensure parents and guardians are aware of the school’s record keeping policy including the use of
a transfer note should the child be enrolling at another school.
Deal with complaints in an open, fair and transparent manner.
Treat students and parents with respect.

I understand that the school provides my child with access to the school's information and communication technology (ICT) facilities and devices (including the internet) for valuable learning experiences. In regards to internet access, I understand that this will give my child access to information on computers from around the world; that the school cannot control what is on those computers; and that a small part of that information can be illegal, dangerous or offensive.
I accept that, while teachers will always exercise their duty of care, protection against exposure to harmful information should depend upon responsible use by students/my child. Additionally, I will ensure that my child understands and adheres to the school's appropriate behaviour requirements and will not engage in inappropriate use of the school's ICT facilities and devices. Furthermore I will advise the school if any inappropriate material is received by my student/child that may have come from the school or from other students.

AnimalAssistedTherapy&AnimalInteractionStudentConsentForm
WhatisAnimalAssistedTherapy?
AnimalAssistedTherapy (AAT) incorporateshuman‐animal interactionsand is anaide to supportalreadyexisting interventions.Norfolk Village State School are proud to have an animal assisted therapy dog to support our students. Training for this dog iscompletedthroughTherapyDogsAustraliaandtheyhaveatrainedhandlerwhowillcloselymonitorthesafetyofthestudents,staffanddog.
Benefits of AAT include; increase in emotion recognition and regulation, reduction of anxiety and depression, ability to addressgrief/loss issues, reduction of blood pressure, self‐esteem enhancement, task completion motivation, increased communication,improvedsocialisationandimprovedabilitytotrust(Velde,2015).
PotentialRisksofAnimalAssistedTherapy:
1. Dogs have their own natural defences. While the dog trainer and dog have both had training, and continue to do so to minimise risk involved with working with animals in schools. The dog will be removed immediately, should any risk be foreseen for either him or those around him.
2. While the dog has been screened by a veterinarian before commencing at school, animals do sometimes carry disease(s). Our therapy dog is up to date on vaccinations, worming, flea and tick and will receive regular and routine veterinarian care as well as regular hygienic grooming.
3. Risk for an allergic reaction is present when working with dogs. Below, please indicate any concerns for your child relating to dogs. 4. Dogs use their body to communicate and may brush against and lean into a person. Other body language such as tail wagging or
body wiggling may also occur. Such behaviours create a risk for loss of balance, falling, or possible light bruising. Our therapy dog will be accompanied by his trainer at all times, during his learner phase and will only be ‘on duty’ for selected periods of the school day.
5. While our therapy dog will be a valued member of our school community, please be mindful that he is in his learner phase and will be slowly integrated into school life. Our therapy dog has a fabric crate as well as an enclosure where he will have the opportunity to rest and re‐hydrate with no human interactions.
6. Animal welfare is taken very seriously. Should a student ever treat our therapy dog un‐kindly, processes learnt through Therapy Dogs Australia will apply and care will be taken to re‐build the human‐animal bond.
