Many times I have

asked myself this

question; from child-

hood, all the way to the present

and each time I come up withthe same answer – Sell Some-

thing! As a child, if I had noth-

ing to sell, I would pick-up

things along my morning news-

paper delivery route, treasures

that others discarded as

trash.With a little re-tuning, I

was able to pick-up, fix-up andsell hundreds of dollars’ worth of 

lawn mowers, televisions, bicy-

cles, wagons, baby carriages, fur-

niture items and more, all from

my front yard every Sunday

when parishioners went to

church on my street corner. I

made ten times more in salesthan I did delivering the morn-

ing paper and as a result earned

a fortune selling treasure that

others just threw out, making

Cold Hard Cash!

Other times I manned my

lemonade and iced tea stand.

Anything, to make a few extradollars for all of the “toys” I de-

sired to buy. Whenever inven-

tory was low, I just went looking

at the garbage people threw out

and to a few garage sales. Today

there’s EBay and the internet to

buy and sell items we want to

market; all in the name of mak-ing a few more dollars – imme-

diate cash!

But, now with the respon-

sibility of a regular job and fam-

Pawn Shops and Auctions. The

most popular infomercials on

TV are based on the latest

money making systems of a few

prolific marketeer’s such as:

“Get Money from the US Gov-

ernment”, “Build Wealth

Through Real Estate Using

  None of Your Own Money”,

“Make a Fortune in Purchasing

Tax Liens”, “Trading Your Wayto Wealth on the Forex (cur-

rency trading),” “Buy Gold and

Silver to protect your assets and

make a Bundle,” “You Can Give

Up Your Job and Make More

Money Buying and Selling

Stock options” and many more.

Do all of these programs

offer a legitimate way to earn

immediate cash? Are the “ex-

perts” portrayed really experts or

are they something much less?

these shows real people or just

are, but, there is also an oppor-

tunity here that is often over-

looked. The motivation to try

something new, whether it be anew idea or a new direction, a

new career or a new opportunity

and another chance to really

make it this time if I put my

mind and my money to work for

me. I call this putting some per-

spiration behind the inspiration

and having a goal! This motiva-

tion is a key ingredient to

wealth creation and without it

success is virtually unattainable.

If your plan to create cash cen-ters around a second job, a part-

time or full-time business

opportunity or selling items you

picked out of the trash, I must

ask you two questions. First, for

what reason do you wish to gen-

erate this cash and second, how

much of this new found cash

flow do you intend to save or in-

vest for your future? Looking a

little down the road, will your

current savings be enough foryou to retire one day and to stay

retired for the rest of your life.

What we once thought was an

important acquisition and a

must have now item, pales in

comparison to a comfortable re-

tirement. It is vital for your fi-

nancial security, longevity and

quality of life that building up

your retirement plan is amongst

your most important goals and

endeavors. Outside of goodhealth, having adequate retire-

ment and emergency funds,

should be everyone’s long-term

financial plan with short-term

sacrifices to bolster long-term

needs. In this economic climate

we find ourselves in, it is imper-

ative that we live below our

means funneling extra dollars

into IRA’s, 401k’s and other tax-

deferred and tax-advantaged in-

vestment vehicles. If we do notput away for ourselves every-

thing that we will need in the

future,who will be there to

make up the short-fall? The

Government, our children,

charities, I would think not. It is

our responsibility to be self-re-

liant, self-sufficient and self-

funded as there are few, if any,

acceptable alternatives if we fall

short.

Most people are ignorantwhen it comes to retirement

ternatives. Is your portfolio in-

vested too aggressively or too

conservatively to be able to pro-

vide a substitute income the day

after you retire and for the next

35 years of days that will follow?Does your portfolio have the

live, even if the account value

exhausts itself from negative

stock market performance cou-

pled with monthly withdrawals

necessary to maintain your stan-

dard of living? Is a portion of 

your portfolio designed to rise

when the stock market is up, yet

insulated from loss when thestock market is falling? If not,

works show, be sure the guests

arrive early enough so everyone

•Have something for every-

one. Independence Day festivi-

ties are for people of all ages, so

hosts should have something for

everyone. Adults may appreci-ate a cold beer to accompany

their hamburgers and fireworks,

but be sure to have plenty of 

other cold beverages on hand as

well. Stock up on soda, ice

water and other popular non-al-

coholic beverages like lemon-

ade and iced tea. Temperatures

on the 4th of July are usually

pretty warm, so hosts should buy

more than they think they

might need, as guests will belooking to stay cool and hy-

drated throughout the party.

When inviting guests, you

should ask if anyone has any

particular food allergies or di-

etary restrictions. Plenty of de-

licious vegetable dishes can be

cooked right on the grill for veg-

etarian guests, and foods likefish might be more suitable for

guests with preexisting health

conditions who should not eat

red meat.

•Decorate.   Just because the

party is liable to take place out-

doors doesn’t mean hosts can’t

decorate. The 4th of July is all

about the red, white and blue,

so be sure to display the Ameri-

can flag and decorate in the col-

ors of the U.S.A. PurchaseIndependence Day cups and

plates, and encourage guests to

wear red, white and blue to the

party. Visit the local party store

and purchase some red, white

and blue balloons.

Even the food can be dec-

orated. Instead of making regu-

lar cookies, make American flag

cookies complete with red,

white and blue coloring, and do

the same with the cupcakes.Bartenders can even use food

coloring to make the drinks red,

white or blue.

•Host a fireworks display. Fire-

works are illegal in many states,

so hosts should consult the au-

thorities as to the local fireworks

laws before hosting their own

party. If fireworks are legal, en-list the help of friends and host

your own small show. Keep the

neighbors in mind when host-

ing a private fireworks show,

and always keep children away

from the fireworks.

If fireworks are illegal in

your state, it’s still possible to

host a fireworks show. Rent a

big screen television for the oc-

casion and take it out to the

backyard. Once the sun setsturn on a televised fireworks

event (the Macy’s-sponsored

 New York City fireworks show

as is the display that takes place

in Boston) and encourage guests

to sit down on the ground and

enjoy the show just as they

would if they were in atten-

dance. The show can be just as

Whether in patients

with arthritis or in

patients who have

had sports injuries, untreated

loose bodies in the knee can be

a major problem. About three

months ago, we first saw Mrs.

C in the office for knee pain.

She is a most pleasant but very

busy 46 year old female who

works in Manhattan, and

rarely has the time to see doc-

tors. She has had treatment

for knee arthritis for several

years. She has been told that

she is too young for a knee re-

placement. Mrs. has had vari-

ous other treatments including

cortisone injections, synvisc

injections and physical therapy

with varying degrees of suc-

cess.

About three days prior

to our first meeting, she had

an episode of excruciating

sharp catching pain deep in

her right knee that lasted

about 20 minutes and went

away. It was so painful that

she wanted to make sure

nothing was torn or fractured.

With a physical exam, she had

some joint line tenderness and

some grinding in her knee but

nothing unusual from a typical

patient with arthritis. Her x-rays showed mild amount of 

arthritis, but certainly nothing

bone-on-bone. However, her

history was quite worrisome of 

loose bodies and we explained

that x-rays can miss loose bod-

ies in about 40 percent of pa-

tients. We proceeded to order

an MRI, and it showed threeloose bodies measuring 1 cm to

1.5 cm in the suprapatellar

pouch, which is the area above

and behind the knee cap.

We explained to Mrs.

that smaller loose bodies meas-

uring 1 or 2 mm may be re-

sorbed by the body, but at the 1

cm to 1.5 cm size range, the

odds are stacked against her.

She was advised to have the

loose bodies arthroscopicallyremoved. We went ahead and

booked her surgery three weeks

later, because she had major

deadlines for the next two

weeks.

…. Then, at 10am two days

later, I received a phone call

from Mrs. in severe pain. Her

right knee was stuck in one po-

sition for the past three hours

and she was unable to fully ex-

tend it. We explained to Mrs.

that the one or more loose

bodies had likely moved to a

new location, between the car-

tilage surfaces, and had lodged

in there. We saw her back in

the office and, that day, Mrs.

was taken for arthroscopic

knee surgery. At the time of 

the operation, we found 5

loose bodies measuring over 1

cm each and subsequently re-

moved them. A large loose

body was found in the weight

bearing joint likely causing her

horrific pain. We also found

an area of cartilage fragmenta-

tion with unstable cartilage

edges, where new loose bodies

may have been sloughing off.

The cartilage edge was

smoothed out with a shaver to

prevent further loose bodies.

Mrs. was lucky in the sense

that we got to her loose bodies

in a relatively quick manner.

About a year ago, I re-

member a 21 year old lacrosse

player who had been avoiding

surgery despite a 1.7 cm loose

body in her knee for 9 months.

She put off surgery despite fre-

quent episodes of catching and

severe pain. She was masking

her pain with regular Percocettablets. At the time of surgery,

we found severe diffuse arthri-

tis in her knee, likely resulting

from the loose body repeti-

tively scraping along her

weight bearing cartilage sur-

faces.

The physical therapist

tells us that Mrs. has been one

of the hardest working patients

that they have seen, and her

rehab has been remarkably

smooth. About a month after

surgery, she was complaining of 

some aching with weather

changes but was pain-free with

almost full range of motion.

She had already returned to

full time work at her job in

Manhattan for 3 weeks.

Hypotonia is our topic

of the month. We

will provide valuable

information about this treat-

able condition.

Things to know about hypo-

tonia:

The degree of stiffness

required to move the body for

the child with hypotonia is

inadequate.

The child cannot gener-

ate enough force at the joints

to support movements like sit-

ting up, rolling or standing.

Low muscle tone can be

associated with a feeling of 

heaviness or limpness when

the infant or child is picked

up. Parents often describe it as

“dead weight.”

There is a large contin-

uum of low muscle tone rang-

ing from slight involvement to

severe.

Basic problems of the infant

or child with hypotonia:

Parents notice excessive

floppiness of the infant and

general inactivity. The infant

feels very flexible and has dif-

ficulty controlling their head

and trunk during feeding,

dressing , bathing and carry-

ing.

The infant is limited in

physical ability to maintain se-

cure and balanced positions

like sitting, standing or crawl-

ing. If the infant cannot move

around to explore the envi-

ronment, cognitive delay may

result.

A school age child with

low muscle tone may be con-

sidered a couch potato, floppy,

clumsy in dance class, uncoor-

dinated or uninterested in ath-

letics.

If hypotonia is left un-

treated the infant or child may

be at risk for shoulder and hip

dislocation, scolisosis, pelvic

torsion and oral motor prob-

lems.

The cause of hypotonia may

be:

Hypotonicity may be a

characteristic of a central

nervous system dysfunction

such as cerebral palsy, Down

Syndrome or developmental

delay.

Connective tissue disor-

ders, metabolic and endocrine

disorders and a variety of syn-

dromes can result in hypo-

tonicity.

How is hypotonia treated?

Postural tone is change-

able and treatment relies on

the infant’s or child’s self ini-

tiated activity to change low

muscle tone.

Automatic responses to

changes in posture are stimu-

lated and more independent

movement results.

As the infant or child re-

peats and practices these

movement patterns, they be-

come more independent and

develop the ability to roll, sit

walk, jump and hop without

help.

function in gross motor, fine

motor, oral motor and cogni-

tive development.

Maintaining a healthy

back requires a bal-

ance of flexibility

and strength between the key

postural muscle groups of the

body. These muscle groups sta-

bilize the pelvis and lower

back, and the head, neck and

shoulders. Because these are

postural muscles, meaning they

play intrinsic roles in main-

taining correct upright posture

and alignment, these muscle

groups are vitally important to

back health.

The key postural muscles

of the pelvis and lower back

include the quadriceps and

hamstrings of the thigh, the

hip flexors, the gluteal mus-

cles (buttocks), the abdomi-

nals and finally the erector

spinae of the low and mid-

back. As a general rule, the

leg muscles (quadriceps and

hamstrings) and hip flexors

are the muscle groups most

prone to developing flexibil-

ity deficiencies. Chronic

weakness is more of a problem

involving the erector spinae,

abdominal and gluteal muscle

groups. Simultaneous flexibil-

ity and strength deficiencies,

however, are often seen in

several groups, such as the

hamstrings and hip flexors.

Balance between flexi-

bility and strength of these

muscle groups is what is most

critical to maintaining a

healthy back. Exercises to

stretch and strengthen these

muscles are available from

many sources. It is necessary,

however, to first learn which

groups need stretching and

which need strengthening be-

fore embarking on a stretch/

strength exercise program,

and what exercise is best for

Obesity is a growing

problem in today’s so-

ciety. Our feline friends

are no exception. The differ-

ence is that they rely on us to

regulate their food and exercise.

Obesity is a condition that is

both preventable and treatable

but requires some effort and

patience. The re-

wards are having

your cat live a

healthy happy and

most importantly,

longer life.

Prevention is

much easier than

taking off the excess

weight once it is

there. Just like peo-

ple, the name of the

game is diet and ex-

ercise. I am an advocate of 

keeping your cat indoors so this

doesn’t mean to start letting

him/her outside but dedicate

some time everyday to play with

your cat. Not only will it pro-

vide exercise but also stimula-

tion. Two favorite types of toys

are string toys or laser lights.

The other component of 

prevention is diet. First, with

the assistance of your veterinar-

ian, an appropriate food should

be chosen. Cats need different

types and amounts of food dur-

ing their life stages. This does

not include table food or exces-

sive amounts of treats. The

biggest mistake is feeding your

cat free choice, i.e. just keeping

the bowl full. Some cats regu-

late their intake but most will

overeat therefore it’s best to pro-

vide food in measured meals.

The most objective way to

tell if your cat is overweight is

to have your veterinarian assign

a body condition score. Basi-

cally two things are assessed.

One, the ribs should be felt eas-

ily with only a slight amount of 

fat over them. Two, look at

your cat from above and from

the side at the area between the

last rib and hindlegs, they

should have a definite waist or

“hourglass” shape.

Overweight cats have

many of the

same health

risks as over-

weight people.

One of the most

common com-

plications of 

obesity in cats is

diabetes. One

of the early signs

of diabetes are

an increase in

thirst or urina-

tion. Other health risks include

arthritis, heart disease, liver dis-

ease, urinary obstruction in

males and skin problems. All

these can contribute to a de-

creased quality and length of 

life.

If your cat is indeed over-

weight, it is time to start a

weight reduction program now.

As we all know, there’s no quick

fix for weight loss. The first step

is to have your cat examined by

a veterinarian prior to initiating

a weight control program.

Since certain medical condi-

tions can cause obesity in cats,

it must be determined if there is

an underlying cause or if there

are other conditions present,

which could complicate weight

loss. Your veterinarian can also

help you determine a realistic

weight goal. It is important to

understand the process may

take a long time because even a

small amount of weight loss is a

large percentage of the cat’s

overall body weight. Plus, rapid

weight loss in overweight cats is

risky because it can result in

liver disease called hepatic lipi-

dosis (fatty liver).

The focus of the weight

loss program is the same as with

prevention: diet and exercise.

There are two main ways to cut

down on calories: feed less of 

the food the cat is currently eat-

ing or switch to a weight reduc-

tion diet. Table scraps also need

to be eliminated and treats min-

imized. Feeding less may just

entail measuring meals instead

of free choice but weight loss

foods are usually more success-

ful. Weight loss diets are typi-

cally either low calorie/high

fiber or low carb/high protein.

They can provide reduced calo-

ries and also keep your cat satis-

fied so they’re not waking you

up to be fed. Exercise programs

for cats seem more difficult than

those for dogs. Trying new toys,

initiating playtime and getting

the whole household involved

are important.

Do you suffer from

chronic lower back

pain? Do you experi-

ence leg pain, numbness or

weakness? These may be symp-

toms of a herniated disc.

In the past, open surgery

was often the only option for

patients with herniated discs.

Today, new treatment options

for back pain and sciatica exist.

A new minimally invasive

treatment for herniated

discs uses water. It’s called

HydroDiscectomy (hi-dro-

Disk-eck-toe-me). HydroDis-

cectomy uses a high-speed

water stream to remove herni-

ated disc tissue, relieving the

nerve pressure that causes back

and leg pain.

HydroDiscectomy is per-

formed using a fluoroscope,

which is a machine that proj-

ects live x-ray pictures onto a

monitor. This allows the physi-

cian to place the SpineJet de-

vice within the spinal disc

without using a large incision.

The physician will insert

the SpineJet device by dilating

a tiny hole in the disc wall.

Once the device is in place, the

water-jet action works to

quickly remove the herniated

disc tissue. The procedure is

usually performed using light

sedation. The entire procedure

takes 30-45 minutes. Hy-

droDiscectomy is an outpatient

procedure and therefore, allows

you to go home the same day.

Specialty trained physi-

cian, Dr. Amr Hosny, performs

HydroDiscectomy as well as

many other minimally invasive

procedures to alleviate neck,

back and leg pain. Dr. Hosny

discusses and designs a treat-

ment plan that best fits the

need of the patient suffering

from acute or chronic

pain. The goal is to help the

patient restore possible maxi-

mum function and manage that

pain.

 Amr Hosny, MD, MBA, cur-

rently serves as the Medical Di-

rector of New Jersey Pain Care

Center and New York Pain

Care. Dr. Hosny’s training in-

cludes the completion of an

 Anesthesiology residency at St.

Luke’s Roosevelt Hospital Cen-

ter, Columbia University, in

have been doing some spring

cleaning around the house.

Instead of bringing old

clothes, sporting equipment,

and household items to the

dump, you should consider

donating them to a local

charity or thrift store. Some-

one will find a use for your

unwanted items and you

could potentially lower your

taxable income if you qualify

for itemized deductions. You

can qualify for itemized de-

ductions if you exceed the al-

lowable standard deduction,

which in 2011 is $5,800 for

singles and $11,600 for mar-

ried couples filing jointly.

In order to maximize the

tax benefits of your dona-

tions, you must first make

sure that you are donating the

items to an accredited organ-

ization. The IRS specifically

defines a qualified organiza-

tion, but the organization

must, in general, operate for

religious, charitable, educa-

tional, scientific, anti-animal

or child abuse, and/or literary

purposes. Note that donations

made to specific individuals

or political organizations and

their candidates never qualify.

However, donations to an en-

tity which performs a substan-

tial government function,

like a donation to your local

police department, can be de-

ducted. If you have questions

about whether or not a spe-

cific organization qualifies,

please see IRS publication

526, or give us a call here in

the office.

Secondly, if you’re do-

nating clothing or household

items to charity, the items

must be in good condition

and you are only allowed to

deduct the fair market value

of the item. In general, the

fair market value is the price

at which property would

change hands between a will-

ing buyer and seller. This is

sometimes referred to as the

“thrift market” cost of an

item. Moreover, if your con-

tribution recently appreciated

in value, this would mean

more money in your pocket.

Remember that you’ll need to

collect a signed receipt from

the organization as proof of 

your donation. Furthermore,

for contributions equal to or

greater than $250 in value,

you must obtain written ac-

knowledgement from the or-

ganization showing the

amount of cash or a descrip-

tion of any property con-

tributed, and whether the

organization provided any

goods or services in exchange

for the contributions. If you

are feeling especially generous

and plan to donate an item or

group of items worth more

than $5,000, you should have

it appraised. In today’s digital

age, you might even consider

taking a pictures or a video of 

your non-cash charitable con-

tribution as further documen-

tation of your donation. If 

you receive a benefit from do-

nating—like tickets to a base-

ball game for instance—you

may only deduct the amount

that exceeds the fair market

value of the benefit received.

If you donate your old

car and the charity turns

around and sells it your de-

duction is limited to the

amount the car actually sells

for, which could be less than

the fair market value or blue

book value. Some charities

will sell your car at auction

for only a few hundred dollars

which could significantly

limit your tax deduction.

So, if you are planning

on doing some spring clean-

ing, be sure to follow these

simple guidelines. There are

many other ways to leverage

the tax benefits of your de-

ductible charitable contribu-

nergy efficient con-

struction is celebrating

one more milestone. It

is now possible to build a fa-cility — even a home — that

is so energy efficient it pro-

duces zero demand on the

public electricity supply.

Proving this in the past

few months is the opening of 

the Richardsville Elementary

School in Lexington, Ken-

tucky, the first ‘net-zero’

school in North America.

This achievement has utilized

features like an innovativewall system — and the entire

building is expected to push

the efficiency benchmarks be-

yond all expectations.

“In addition to rock-

solid walls, the rooftop solar

panels cover 40,000 square

feet providing for all of the

school’s electricity needs,”

says Todd Blyth at Nudura, a

leading name in construction

efficiencies.Blyth also explained that

any construction project

today can aim at net-zero, but

to achieve it cost efficiently

the building envelope must

provide maximum insulation.

“This school sets a stan-

dard that can be applied to

homebuilding too,” he con-

tinued. “If, at the outset, de-

cisions are made carefully

regarding the walls, windows,roofing, water management,

ventilation and indoor cli-

mate control, you are well on

your way to completely offset-

ting energy consumption.”Here are a few of the net-

zero design components of

Richardsville Elementary:

•A roof designed with solar

panels and electricity grid.

•Geothermal heating and

cooling for environmentally

responsible efficiency.

•Walls of concrete. The

  Nudura system is comprised

of stay-in-place, pre-assem-

bled blocks, steel reinforced,and then filled with concrete.

It replaces traditional build-

ing methods. The durability

and energy efficiency of con-

crete has shown to reduce en-

ergy costs up to 70 percent.

Better still, the entire struc-

ture (www.nudura.com) is re-

ported to be up to nine times

stronger, with far more fireprotection and with far more

sound insulation.

•Ventilation includes a CO2

monitoring system to keep

good air quality indoors and

allow no more outdoor air

than necessary.

•Positioning is north-south,

allowing for effective day

lighting (without glare) so

that all artificial lighting can

be off during 70 percent of school hours.

•Computers are wireless. Five

carts, each loaded with 30

laptops save energy, wiring

and construction of a com-

puter lab. Laptops use a frac-

tion of the energy used to run

a typical desktop computer.

•Kitchen staff is trained in

energy-efficient food prepara-tion. Typically, kitchens ac-

count for 25 percent of a

school’s energy drain.

Rub

1/2 cup brown sugar

meat and rub in thoroughly

The Pequannock ValleyRotary Club held their

annual Scholarship

Luncheon at Ruppert’s restau-

rant on Wednesday 6/8/2011.

Twelve local graduating high

school students received almost

$11,000 in scholarships. The

club supports students and nu-

merous projects in Lincoln

Park, Riverdale and Pequan-

nock, NJ. To date the club has

given over $200,000 in scholar-

ships to local students.The scholarship recipients

were: Pequannock: Daniel

Bernstein, Kathryn Kancz-

ewski, Peter Stivala, Samantha

DeVito, and Abraham Can.

Meghan O’Brien was given a

memorial scholarship in the

name of Gunther Seibert, a past

officer of the club. Lincoln Park:

Krysta Scimeca, and Shawn

Rhoads. Riverdale: Roberto

Soperanes. Scholarships were

also given to graduating highschool students intending to

pursue a career in the medical

field from the Run-For-Marge

with the principal to learn more

about the ideals of the school
