Dear Members, Special Care Advocates in Dentistry (SAID) Is a group of committed professionals, dedicated to the care of the patient who is at highest risk of dental disease; the patient with special health care needs. Our membership today encompasses the entire country. Our annual meeting is a place where we share knowledge and add to our expertise. This gathering is an opportunity to share news, ideas, concerns and innovations. While enjoying each other’s company! The theme of this year’s meeting, Special Care for a Lifetime, promises to address the challenges we see on a daily basis. Our expert speakers will cover a variety of topics that include: Pharmacology, Prevention, Team Treatment, the Complex Geriatric Patient, Dementia, Mental Illness and Forensics. This year we bring our meeting to Philadelphia Pennsylvania. “The City of Brotherly Love” offers many historic sites, museums and fine dining. You may even attend a professional basketball or hockey game! We are fortunate that our venue The Wyndham Hotel is in the heart of the historic district just a short walk from the Liberty Bell and Independence Hall. During lunch Friday Drs. Bob and Joyce Leonetti will give us a light hearted look at the history and lore of our host city. No organization runs without the help of many people. Thursday, October 31st 2013 George Martinez 1:15 PM-3:15 PM The Power of Prevention About the Speaker: George J. Martinez is currently a Regional Preventive Specialist for 3M ESPE with over 32 years of Preventive Dentistry experience. His lectures specialize on simplifying CaMBRA; risk assessment, treatment protocols, the latest in technology for fighting oral disease and coaching needs of the clinicians. About the program: Caries Management by Risk Assessment, or CAMBRA, represents a shift from simply treating caries to proactive prevention and treatment. Through learning about CAMBRA, and implementing the recommendations and protocols, dental clinicians have a valuable opportunity to help patients take charge of their oral health. Upon completion of this course, participants will learn: • to evaluate a patient’s disease indicators, risk factors and protective factors to determine

their level of caries risk. • to understand accepted clinical guidelines and treatment options to help patients manage

existing oral health issues based on their level of risk. • to understand how risk assessment can help to identify the appropriate treatment plan depending on the patient’s risk level.

Evan Spivack, DDS 4:00 PM-5:00 PM The Complex Geriatric Patient; Decision Making Dilemmas About the Speaker: Dr. Evan Spivack graduated from the University of Maryland dental school and completed general practice residency training at St. Barnabas Hospital in the Bronx and a fellowship in special care dentistry at Helen Hayes Hospital in West Haverstraw, NY. Currently a professor and acting director of the Special Care Treatment Center at the Rutgers School of Dental Medicine, Dr. Spivack has devoted his career to both direct patient care and to the education of future dentists in the provision of dental care to the full spectrum of the special needs population. About the Program: Patients with special needs often provide dentists with decision-making dilemmas, but perhaps the most difficult situations arise when caring for the complex geriatric patient. This case-based presentation will introduce several tools that can assist in decision making and help participants develop an “outside the box” approach to geriatric patient care. Upon completion of this course, participants will: • recognize problems that commonly occur in assessing complex geriatric patients • be able to develop dental treatment plans specifically addressing the needs of these complex patients • have knowledge of several decision-making tools used in assessing complex patients with special needs

Friday, November 1st 2013 Tom Viola R.Ph. 8:30 AM-10:30 AM Pharmacology De-Classified About the Speaker: In addition to his daily practice of the profession of pharmacy, Thomas Viola also serves the professions of dentistry, dental hygiene and dental assisting as an educator, published writer and professional speaker. Tom is a member of the faculty of eight dental hygiene and dental assisting programs, as well as several national board exam review courses. He is a contributor to several dental professional journals, with columns appearing regularly in AGD Impact and RDH Magazine. In addition, Tom currently serves as the Subject Matter Expert for Mosby’s Dental Drug Reference. About the Program: This program will provide participants with an update and overview of the most frequently prescribed FDA-approved medications for the past year. Throughout the program, special emphasis will be given to those medications whose actions, side effects, or interactions with other medications may impact dental therapy. Upon successful completion of this program, participants will be able to: • Identify the most frequently prescribed FDA-approved medications for the past year. • Review the basic mechanism of action, principle therapeutic indications, and potential adverse reactions and drug interactions of these medications. • Discuss the clinical dental considerations of these medications and their potential impact on dental therapy. Burt Nussbaum DDS 11:00 PM-12:00 PM Team Treatment About the Speaker: Dr. Burton Nussbaum graduated from dental school in 1974. He then went on to Pediatric Dentistry graduating in 1976. While in graduate school, he enjoyed treating unique patients. After graduating from a specialty program, he spent two years in the Navy stationed in Naples, Italy treating children. After the Navy, he bought a practice in New Jersey which had many patients with disabilities. He bought it and transformed it to include people that were very fragile. He joined the faculty and surgical staff of The Children's Hospital of Philadelphia and the Thomas Jefferson University Hospital. This provided a sophisticated avenue for treatment of Special Needs Patients. He is a Clinical Professor at the University of Pennsylvania School of Dentistry a well as the Jefferson Medical School. About the Program: This presentation will deal with the treatment of medically complex dental patients in a coordinated “team” approach versus a singular approach. It will define the types of providers necessary to treat dental patients with medical and behavioral issues and help the provider decide which approach is best for each particular patient. After completion of the course, participants will be able to:

• Define and describe the different singular practitioners involved with the dental care of the medically/ behaviorally complex dental patient

• Define and describe what constitutes team care • Decide which patients can be treated in an office or clinic setting and

which patients need the O.R. • Locate an existing team or create one of their own

Sam Zwetchkenbaum DDS Rita Jablonski PhD CNRP 1:00 PM-3:00 PM The Patient with Dementia About the Speakers: Dr. Zwetchkenbaum is currently a fellow in the Geriatric Dentistry Fellowship, part of Rutgers School of Dental Medicine and the New Jersey Institute for Successful Aging, Rowan University- School of Osteopathic Medicine in South Jersey. Previously, he was faculty at University of Michigan, providing clinical care and directing the General Practice Residency program. He received his DDS from the University of North Carolina and a Certificate in Prosthodontics from UMDNJ- New Jersey Dental School. Dr. Jablonski is an Associate Professor at the University of Alabama at Birmingham School of Nursing. She is also a Scientist in UAB’s Center for Aging. Dr. Jablonski has worked with older adults in multiple clinical settings, including intensive care units, general medical units, emergency departments, skilled nursing facilities, and primary care. In addition to her clinical and teaching responsibilities, Dr. Jablonski is the principal investigator for an NIH-funded study, "Reducing Care-Resistant Behaviors During Oral Hygiene in Persons with Dementia" ($1.4 million, 4 years). She is also the nursing content editor for the Portal of Geriatric Online Education and a member of the Editorial Board for the Journal of Gerontological Nursing. About the Program: Using slide presentation and videos, the presenters will cover some important issues in geriatric dentistry, including working with patients with dementia as well as those exhibiting care resistant behavior, xerostomia, and memory loss. They will discuss the provision of care in different living settings. Following the presentation, the participant will be able to: • Discuss what types of dementia are found in older patients, including incidence, presentation, and current thought on pathophysiology • Review stages of Alzheimer’s disease, along with pertinent issues of function and care • Describe how memory loss may present, how it is evaluated, and what strategies dental providers can employ to assure appropriate communication • Explain care-resistant behavior and why it occurs in patients with dementia • Describe techniques of mouth care when working with patients with care-resistant behavior • Review issues of consent and advanced directive for the patient who lacks competence and discuss how to determine capacity to provide consent for care. • Discuss decision making for care/no care for patients at different stages of functional loss • Review living settings for patients with dementia (including LTC and dementia units at ALs), determinants for placement, and how mouth care is provided in those settings • Discuss strategies for educating the nursing staff in mouth care.

Patricia E. Doyle RDH 3:30 PM-5:00 PM Mental Illness & Oral Health About the Speaker: Patty Doyle has closed one chapter in her life to open another. She retired from private practice June 4, 2013, to pursue education and outreach activities in her special area of interest, the connection between dental health and mental health. Following her graduation from the University of Washington, Patty practiced dental hygiene for 48 years which include 29 years as a volunteer at Harborview Medical Center, Mental Health Services, and Outpatient Programs. She has held an Affiliate Faculty position with the University of Washington School of Dentistry since 1986 and is a Fellow of the Academy of Dentistry for Persons with Disabilities and a member of the University of Washington Oral Health Collaborative. Patty is a longstanding member of American Dental Hygienists’ Association and Washington State Dental Hygienists’ Association. A strong advocate for understanding the needs of people with mental health issues, she has published, presented nationally on the dental-mental connection, and been recognized for her work locally and nationally. About the Program: The presentation is designed to Increase participants’ understanding of mental illness. It will focus on how mental illness and its treatment affect oral health and address specific communication techniques, treatment planning and case management issues. It will explore funding and access to care issues. Upon completion of the course, participants will be able to: • Discuss how mental illness and its treatment impact dental care • Help mentally ill patients through a bio-psychosocial approach • Discuss case management for patients with differing degrees/ severity of mental Illness • Discuss access to care issues regarding patients with mental illness 6:00 PM-8:00 PM Trolley Tour of the City of Philadelphia Tour the City of Brotherly Love from the comfort of a quaint old-fashioned trolley. Paul Kovalski DDS 8:30 AM-9:30 AM Forensic Odontology and Crime Scene Investigation: the Role of the Dental Team About the Speaker: Dr. Paul Kovalski Jr. has been the Forensic Dental Consultant to the Monmouth County, NJ Medical Examiner’s Office for over 20 years. He has participated in over 75 medico-legal investigations and identifications including cases with the NJ Sate Police and FBI. He appeared on America’s Most Wanted and was interviewed by John Walsh in the “Baby Bones” case. Dr. Kovalski is a long-time SAID member and is also a member of the American Society of Forensic Odontology, the American Academy of Forensic Sciences and the NJ Society of Forensic scientists. He currently serves as President of the NJ State dental chefs association and treats patients at Greystone Park Psychiatric Hospital. About the Program: Dr. Kovalski will share his appreciation for the complexities and difficulties involved with forensic dental identifications and bite mark analysis. He will discuss the capabilities, limitations and practical applications of this science as well as some scenarios whereby the dentist treating at-risk patients might become involved. Upon successful completion of this program, participants will be able to:

• Discuss the capabilities and limitations of forensic dental science • Appreciate the practical applications of forensic dental science • Visualize scenarios that might involve them and/or their patients

Doug Veazey DDS 9:30 AM-10:30 AM Special Care Literature Review About the Speaker: Douglas Veazey, DDS retired in 2009 as Dental Director at Broughton Hospital, Morganton, NC after serving 31 years. He has worked at Broughton Hospital and J. Iverson Riddle Developmental Center. He has served over 20 years at Good Samaritan Dental Clinic, a non-profit which serves indigent patients in Burke County. In SAID, he has served as President and been active in many capacities About the Program: An update of all the literature pertaining to Special Needs Patients in the past year. What’s new in literature that could change the way you practice? David Itzkoff DDS 10:30 AM-11:30 AM Pearls, Perils & Parables About the Program: We are pleased to once again include this popular interactive session as part of the program. This year we encourage you to bring your pearls of wisdom on any topic relating to special care patient care including clinical, oral hygiene and administration. In addition we encourage you to share your stories. We are asking you as experienced clinicians to share your unusual cases or cases that caused you to be creative. We want to know the lessons that you learned and the obstacles that you overcame. Dr. David Itzkoff will moderate this lively session. Sign up will be available on-site.

About SAID… Special Care Advocates In Dentistry SAID is a benevolent group of dental professionals whose mission is to improve the oral health of people with disabilities through service, education, and advocacy.

SAID was founded in 1966 by Dr. Gerald Bensberg, Director of the NIMH-supported Attendant Training Project. Dr. Bensberg invited dental professionals who served in facilities for people with cognitive challenges in 15 southern states to participate in a seminar to consider ways of delivering better dental care to people with developmental disabilities. The original seminar was attended by 23 institutional dentists, one dental hygienist, two nurses, and a pediatrician. These attendees represented 26 institutions in 13 southern states.

Since 1966, SAID has grown in scope and membership, and today includes over 200 professionals whose patients reside both in facilities and in the community. The scope of SAID now includes people with intellectual disabilities as well as those with mental illness. While SAID still has its southern heritage, members now hail from all regions of the country and Canada. During the 2010 business meeting the SAID tagline was officially changed to “Special Care Advocates in Dentistry” because the original “Southern Association of Institutional Dentists” no longer represented the diversity of the of the organization’s membership .What has NOT changed is the original vision of improving the oral health of people with disabilities through service, education, and advocacy.
