Pedorthic Management for Patients Suffering from Osteoarthritis Issues Donna Robertson, C.Ped

Description: With almost fifty percent of the people age sixty-five and older having osteoarthritis, this degenerative joint disease is making a huge impact on American society. 27 million people are limited by their symptoms causing losses in annual wages and medical costs that exceed sixty billion dollars, annually. With so many being afflicted there is little doubt that thousands upon thousands are searching for relief from osteoarthritis pain every single day.

As the pedorthic field grows to serve the needs of the osteoarthritic patient, we need to demonstrate to the medical field (Allied Health Care Providers) that we are an important and viable profession that needs to be a part of the team approach in treatment/management of osteoarthritis issues. A pedorthist needs to know the array of materials available for the fabrication of an orthosis and a pedorthist should have knowledge of which appropriate shoe function is going to both enhance the orthoses and serve the patient’s needs.

There are studies available that prove that certain combinations of materials with different types of therapeutic shoes that

work best for a patient that has osteoarthritis issues. Discussion of the pedorthists role in the integrated care of a wide variety of osteoarthritic lower extremity ailments. This lecture enables participants to enhance their skills in the management of lower extremity problems associated with osteoarthritis.

Topics covered include: the pedorthic assessment and treatment of osteoarthritic pain involving the hip, knee, ankle and foot; the assessment of biomechanics static/dynamic; the role of appropriate shoe selection (design/function) and selection of orthotic devices addressing joint dysfunction; foot and ankle injuries that leads to osteoarthritic joint pain.  Presenter will provide patient case studies and discuss a variety of pedorthic treatment options.

Footwear Science Melvyn P. Cheskin, MBS., C.Ped, L.Ped Description: How footwear has evolved from a craft performed by artisans to the present day and beyond including – Engineering, Chemistry, Materials, Medical, Biomechanics and Bioelectromagnetics.

for hallux rigidus, hallux abucto valgus and hallux varus. The biomechanical consequences of this procedure are critical to understand in order to achieve optimal  results post-operatively.

The biomechanical changes due to a first metatarsal phalangeal arthrodesis alter the hindfoot and forefoot resulting due to compensation for loss of first metatarsal phalangeal joint range of motion. This compensation explain the common complications noted in the literature post arthrodesis.

Post-operative biomechanical management of an arthrodesis can prevent many of the complications. The functional outcomes of 1st MPJ arthrodesis have also been well documented to be excellent.

Pedorthics and Podiatry: A Match Made in Heaven?Luke J. Hunter, MD

Description: When to use off-the-shelf braces, what to consider while bracing a surgical patient, and how to get paid for your work are 3 key elements every Pedorthist working with any Podiatrist or Foot and Ankle Physician should know. Off-the-shelf (OTS) braces and splints are the 1st choice in the treatment of foot and ankle problems. But for some patient’s after surgery, OTS bracing may not be the best option. For the surgerized patient, bracing should take into consideration what anatomy was changed/corrected in surgery and patient’s comorbidities. Most surgical procedures are used to correct malformed or malfunctioning anatomy, it is important to know how to protect and support the patient’s surgery and help optimize the patient’s outcome.

Lastly, knowing the insurance coverage and indications for the most common off the shelf braces, when custom braces are warranted,

Deconstructing the Over-Pronation Myth Matt Gooch, MS, C.Ped Description: The presentation will discuss the commonly used term “over-pronation” and will make the case that it should be removed from the lexicon of Pedorthic practice. Despite being one of the most commonly used words to describe foot motion and one of the most common explanations for lower-limb injuries, over-pronation is neither clearly defined nor directly correlated with injury in the literature.

This presentation will review some of the seminal research on the topic and provide a potentially more accurate way forward for quantifying human movement.

Introduction to Kinesiology Taping and Pedorthic Applications Jennifer Howey, PT, CAFCI Description: Spider tape, K-tape, BSN Leukotape-K are all brands of Kinesiology taping. A leader in this tape and research, BSN Leukotape-K has proven benefits for reducing joint pain, improving muscle function and decreasing swelling. Compliment your pedorthic practice by managing patients foot pain and function quickly and effectively.

This presentation is part one of a 2-part Kinesiology taping, foot focused workshop. It will introduce participants on the basic principles and methods of Kinesiology taping and demonstrate how it can be applied to pedorthic conditions such as tibialis posterior tendinopathies. This is part one of a two-part breakout session.

Hands-on workshop on Kinesiology Taping Techniques for Foot Pathologies Jennifer Howey, PT, CAFCI Description: After gaining an understanding of the principles and method of Kinesiology taping during Session One, this workshop will be “hands on” teaching of Kinesiology taping applications for pedorthic conditions. Demonstrations and practice of taping for conditions such as plantar fasciitis and tibialis posterior tendinopathies will be covered.

The time will then be spent to teach how the principles can be incorporated to help with other conditions such as diabetic neuropathies and those pertinent to the audience. This is part two of a two-part breakout session.

Pedorthic Prevention of Wound Development Edith James, C.Ped Description: Prevention of wounds is high priority in lower extremity care. The Certified Pedorthist can play a vital role as a team member in this prevention modality.

Pearls of Pedorthics: Fabrication Facilitator: Jay Zaffater, C.Ped, John Galbraith, C.Ped, Matthew Almeida, C.Ped, Robert Sobel, C.Ped, Jamie Dick, C.Ped Description: This session will feature four (4) 10–minute segments, bundled together to make up one full session with time for Q & A. Our presenters will use personal experience and proven techniques in fabrication that work for them and their patients.

Back to Basics: Diabetic Offloading Teresa Alpert, CO, C.Ped Description: Getting back to the basics understanding different pedorthic modalities. The concept that a shoe and insert are the only options for the diabetic at risk foot is passé. Today the opportunity to offload pressure areas and use technology and material selections are keeping diabetics safe and ambulating. Learn about different lower limb orthotic offloading devices. How and when to apply these modalities to assure the best patient outcomes. I will share real patient case studies and together we will discuss the approach to keeping people healthy on their feet.

Patient education and compliance are the focus of how we can make a difference. The CDC reports that 40% of American’s will have diabetes. This is a huge impact on our

healthcare and economic systems. A little prevention is worth hundreds of thousands of dollars. Pedorthics is positioned to be a major profession in helping set the standards of ambulatory care.

Posterior Tibialis Tendon Dysfunction Pedorthic Management Louis N. Iannuzzi, PT, DPT Description: PTTD, also called the “acquired” adult flatfoot, is the most common type of flatfoot disorder developed during adulthood. (PTTD) is a progressive complex sequence of degenerative changes in the tendon, ligamentous and osseous structures, impairing the ability of the foot to support the arch. This usually results in pain, deformity, and disability.

This lecture will present a brief review of the anatomy and pathomechanics, as well as, provide effective pedorthic management options in the successful treatment of the adult flat foot.

One Foot in Front of the Other: Contributions from Foot Sole Cutaneous Mechanoreceptors Leah Bent, PT, OTRL Description: There are four distinct cutaneous mechanoreceptors that provide tactile input from the plantar surface of the foot. The unique patterns of vibration, slip, pressure and stretch provide valuable information regarding the interface of the foot with the environment.

This session will address the feasibility of foot sole skin to contribute to gait and balance control; with “particular” attention paid to the distribution and density of skin mechanoreceptors across the foot sole. Here we will talk about the potential for receptors in the skin to respond to input from the environment, and evoke changes in patterns of muscle activity; for example, onset and amplitude of lower limb muscles during gait. As such there are implications for rehabilitation advancements such as insoles, orthotics and vibrating devices, whereby skin activation can influence gait outcomes. Importantly, the physical location of mechanoreceptors across the foot sole, and the physical characteristics of the skin (hardness, thickness) have implications on activation of the mechanoreceptors, their perceptual threshold and ultimately the success of clinical testing techniques.

We will address the use of microneurography in parallel with other techniques to weave together the link between standard clinical

Modern Approaches to Traditional Biomechanics Kendrick Whitney. DPM Description: Knowledge of how and why the foot, digits and body compensate for specific foot deformities will lead to more effective treatments and outcomes. The key to success lies in understanding the imbalances produced by foot deformities occurring in all three planes.

A new system that allows for the decompensation of each foot deformity will be discussed. This visual test balance system will help to provide optimal shoe and orthosis re-alignment therapies.

Growing your Podopediatrics Practice Louis J. DeCaro, DPM Description: Tired of the typical elderly or diabetic patient time and time again? An incredible opportunity surrounds your practice that is both beneficial to you and the community at large.

Children are in tremendous need of podiatric care, both preventatively and proactively as a result of complication or injury, but are often unaware of the high level of care you as a practitioner can offer them.

Beyond that, referral sources such as other medical practitioners, athletic directors, school nurses and a multitude of other entities around you are equally unaware.

What this means to us is one thing; Opportunity.

Deconstructing the Link Between Impact Forces and Injury Matt Gooch, MD, C.Ped Description: It has long been assumed that there is a definitive link between the impact forces observed in common activities, like running, and the likelihood of injury. As a result, a great deal of focus has been placed on researching and developing materials and products that are able to reduce peak impact forces.

Despite the assumed causal link, the body of available literature has not fully supported this association. A new understanding of the role of impact forces is necessary to fully appreciate their effect on human movement.

This presentation will review some of the relevant literature on this topic by investigating the way that our bodies respond to impact forces and propose a new lens through which they should be viewed.
