THANKS SAARC Psychiatric Federation (SPF) organized its 5th Conference at Lahore from 25‐27 March 2011. SPF was launched in Lahore in 2004 and it was a great privilege for Pakistani psychiatrists to host this meeting again in the city of Lahore that was attended by more than 250 delegates from all over the country along with representatives from Bangladesh, India, Sri Lanka, Afghanistan, Iran, UK and Australia. 5th SPF enjoyed full support from Pakistan Psychiatric Society, Indian Psychiatric Society, Sri Lanakn College of Psychiatrists, Bangladesh Association of Psychiatrists, Psychiatrists Association of Nepal, Bhutan Psychiatrists Association, Iran Psychiatrists Association and was represented by their office bearers and delegates from their respective countries. Pakistan Psychiatric Society, Fountain House and academic departments of psychiatry from all over the country participated in this meeting and took an active part in the scientific deliberations of the conference. Medical institutes from Lahore including Department of Psychiatry Services Institute of Medical Sciences, Post Graduate Medical Institute, Fatima Jinnah Medical College, Sharif Medical College, Fatima Memorial Medical and Dental College and Lahore Medical University, with collaborations of psychiatrists from other institutions like Punjab Institute of Mental Health need special mention for their support. British Pakistani psychiatrists came in a big group and helped the scientific committee by organizing a number of educational workshops and special sessions. Similarly contribution from Dr Zarrin Siddiqui from Australia was remarkable for organizing a workshop on teaching and training in psychiatry. The theme of the meeting was "Building Bridges to provide Mental Health Care in the SAARC Region” and the conference was dedicated to the memory of late Prof Haroon Rashid Chaudhry who was serving as Secretary General of SPF and President of Pakistan Psychiatric Society (PPS) at the time of his untimely death in Oct 2010. The scientific programme included pre and post conference workshops on: • Rehabilitation Psychiatry (Dr Bilal Ahmad, UK) • Hypnosis (Dr M Farrukh Hussain, UK) • Psycho trauma (Brig Mowadat Rana, Pakistan) • Teaching & training in Psychiatry (Dr Zarrin Siddiqui, Australia) • Course for GPs (Faculty members from local Medical Colleges) • Child Psychiatry (Dr Shahid Munir Ahmad, UK) Special sessions were held on different clinical topics along with a number of Industry sponsored educational symposia on Mood disorders, Depression and Anxiety disorders. Prof Malik Mubashar, Vice Chancellor University of Health Sciences gave one of the plenary talks on Community Psychiatry as well. The quality of the papers was excellent and all the participants rated the meeting as highly scientific and very academic.
A large number of local psychiatrists, PG trainees and psychologists helped the organizers in a big way to make this conference a real success. Prof Nasar Sayeed Khan as chair of the scientific committee and his dedicated team (Dr Imran Ijaz Haider, Dr Umair Rasheed Chaudry, Dr Altaf Qadir Khan, Dr Usman Amin Hotiana, Dr Muhammad Mujtaba, Ms Mirrat Gul, Dr Saira Ejaz, Dr Ayesha Rasheed, Dr Sadaf Rashid, Dr Munir Iqbal and Dr Sumira Qamber Bokhari) did a marvellous job and require special thanks for keeping the scientific programme interesting, informative and more importantly very academic. They were guided and fully supported by Prof Khalid Mufti, Prof Abdul Malik, Dr Afzal Javed, Prof Rizwan Taj and Prof Aziz ur Rehman. Prof S Sultan, Prof Fareed Minhas, Prof Mowadat Rana, Prof Iqbal Afridi, Prof Ghulam Rasool, Dr Naim Siddiqui, Dr Leghari, Dr Wajid Akhundzada, Prof Niaz Maqsood, Prof Imtiaz Dogar and many other key psychiatrists from all over the country were speakers, discussants and facilitators in different sessions along a number of overseas and regional speakers. The meeting did generate a lot of interest in starting and initiating different research projects and it is hoped that the contacts made with the psychiatrists who came from India, Bangladesh, Sri Lanka & Iran will be very beneficial for Pakistani mental health professionals in coming years.
One of the highlight of this conference was that no political figure was present for inauguration or concluding sessions. The Inaugural lecture was delivered by Prof I. A. K. Tareen and the meeting was concluded by a lecture from Prof S Haroon Ahmad. The honour and respect that was given to these two most senior Psychiatrists was greeted by all the delegates. This made particularly the overseas delegates from India, Sri Lanka & Bangladesh very impressed who thought that they will take this message home and will also try to follow this. Social functions included gala dinner at Fountain House where the participants were briefed about the activities of this centre of excellence in rehabilitation Psychiatry with of course a great tribute to late Prof Haroon Rashid Chaudhry.
And last but not the least, the office bearers of SAARC Psychiatric Federation and the delegates from India, Sri Lanka, Bangladesh, Iran, UK, Afghanistan and Australia need a lot of appreciation and thanks for making this meeting a real success.
The organisers of 5th SPF conference would also like express their thanks to the newly elected office bearers of SPF (Prof Rabbani & Dr Shehan Williams) and wish them all the best for the future SPF activities.
INTRODUCTION
SAARC Psychiatric Federation (SPF)
South Asia region represents eight countries‐ Afghanistan, India, Pakistan, Bangladesh, Sri Lanka, Nepal, Bhutan and Maldives‐ which are members of South Asian Association for Regional Co‐operation (SAARC). Most of the people in these countries share a common cultural and political heritage. It was this recognition at the political level which led to the establishment of SAARC when its Charter was adopted on December 8, 1985. SAARC is an organization that provides a platform for the people of South Asia to work together in a spirit of friendship, trust and understanding. It accelerates the process of economic and social development in Member States. It also encourages cooperation in agriculture, rural development, science and technology, culture, health, population control, narcotics control and anti‐terrorism. The SAARC Secretariat is supported by Regional Centres established in Member Countries for promoting regional cooperation. The countries in this region are developing nations with limited resources especially in health sector. Mental Health in particular, is not a priority area in many of these countries. Limited number of professionals, escalating health care costs and cost of drugs in addition to non availability of mental health policies are some of the major obstacle in this region. Such issues in the mental health sectors have always made it necessary for South Asian nations to have a joint dialogue on development & formulating a uniform mental health policy which identifies major issues and objectives, defines the role of public and private sector, providing guidance for prioritizing expenditure, linking problems with resource allocation, highlighting vulnerable groups with special mental health needs like children, the elderly, the women , refugees and displaced persons, caring individuals at risk (with depression, schizophrenia, substance abuse etc.), bringing forth alcohol and drug policies, ensuring respect for human rights, and involving NGOs, consumer groups, mass media and all other stake holders. Mental health professionals working in the region were fully aware of these issues always thought of having co‐operation in mental health in Asia in general and South Asia in particular. This certainly had been a dream of our predecessors for the last many decades. There came an opportunity at the inauguration of GAMHA (Group for the Advancement of Mental Health in Asia) on January 21, 1978 at New Delhi during the Annual Conference of Indian Psychiatric Society & the Regional Conference of World Psychiatric Association & World Federation for Mental Health that proved an important landmark of major significance in this regard. It was the first time in history that leaders of mental health in sixteen countries in Asia declared to initiate an organized effort to promote mental health in their own countries. JS Neki, NN Wig, Col Kirpal Singh, RL Kapur, KC Dube (India), Zarabi and Davidian (Iran), Prof Rashid Chaudhry (Pakistan), Prof Chaudhury (Bangladesh) and many other notable Asian psychiatrists led this initiative. It may be noted that the participation of Prof Rashid Chaudhury from Pakistan at this meeting in India was the first such instance in seventeen years among the two countries. In a way, this
brought nations closer as JS Neki soon travelled to Pakistan in return to attend the annual conference of Pakistan Psychiatric Society and Prof Wig in his WHO Regional Advisor role also made further trips to Pakistan and advocated for more cooperation and strengthening of relationships among the professionals across borders. But despite this good beginning, efforts to form an Asian or South Asian Federation did not bear fruit. While South Asia dithered, the eleven nations of South East Asia formed federation for ASEAN (Association for South East Asian Nations) and decided to strengthen mutual co‐operation in the field of mental health by establishing ASEAN Federation for Psychiatry & Mental health (AFMH). This initiative which was led by Prof R Kusumanto Setyenegoro & MP Deva and others has been functioning well for more than three decades now. The AFMPH success again inspired the initial work for mutual cooperation among South Asian countries. Pakistan Psychiatric Research Centre & Fountain House, Lahore, Pakistan organised an international seminar on schizophrenia in 1998. This witnessed the revival of interest for joint working. At this seminar a committee was formed with Prof Nalaka Mendis (Sri Lanka), Afzal Javed (Pakistan) & Prof Wig (India) as members and they took another lead & restarted the work for setting up a group in the region. Further discussions took place at a WHO sponsored meeting in Colombo in 2001 and a plan of action was prepared by this committee. It was in this perspective that a suggestion was made to have a formal meeting of Asian psychiatrists during the 2003 International Conference of Psychiatry organized by the South Asian Forum on Mental Health to deliberate on this issue. Afzal Javed who was the founder of South Asian forum (SAF) was the main moving spirit behind this landmark initiative. This meeting proved very fruitful and it was decided to continue having these discussions at Lahore on the occasion of WPA Regional and Inter Zonal Meeting to be held in 2004. This is how our long standing dream became a reality and representatives of the Psychiatric Societies of South Asian countries while meeting at Lahore on 19 Sept, 2004 formed the SAARC Psychiatric Federation. This historic meeting was jointly chaired by Prof JK Trivedi, President, Indian Psychiatric Society and Prof Khalid A Mufti, past president of Pakistan Psychiatric Society & chairman of the Organising Committee of Lahore conference. WPA office bearers participated as observer and gave valuable suggestions along with an active input from many senior psychiatrists who attended this special meeting from Pakistan, India, Sri Lanka and Bangladesh. Prof Abdul Malik (Pakistan) was elected as the President and Roy Abraham Kallivayalil as the Secretary General with Afzal Javed and Nalaka Mendis as advisors, besides other office‐ bearers for this newly formed federation. It was decided to draft the constitution and circulate to all member societies for their suggestions and views. It was also decided to affiliate the Federation to WPA and to build up partnership in South Asia in research, community care, training programmes, exchange of Faculty and Trainees, publication of data, health education and other related academic & clinical fields A Special SAARC Psychiatric Federation Session was held at Chandigarh (India) on 30 January 2005, during annual national conference of Indian Psychiatric Society (ANCIPS 2005) in which JK Trivedi presented the Statutes of SPF for approval. Prof Abdul Malik, President SPF was the Chief Guest at the valedictory function of ANCIPS along with Afzal Javed, Prof Haroon Rashid Chaudhry and representatives from Sri Lanka & Bangladesh. Indian Psychiatric Society and in particular its presidents during the last few years including Prof J K Trivedi, Prof Nambi, Prof Ghosh, Prof I Reddy, Prof Shastri, Dr Mohandas & other office bearers especially Prof Roy Abraham Kallivayalil and UC Garg’s efforts are worth mentioning as at the initial preliminary stages of the formation of Saarc Psychiatric Federation, they played an important role in keeping the secretariat & members societies of SPF active, vibrant and lively. It was later on decided to continue holding future meetings preferably along with national association meetings. The First International Conference of SPF was held at Agra in December 2005. Second international conference of SAARC Psychiatric Federation was held at Kathmandu, Nepal on November
17‐19, 2006 followed by 3rd SAARC Psychiatric Federation Conference in Colombo from 2‐4 November 2007 & 4th SPF conference at Dhaka, Bangladesh on 1‐3 November, 2008. Although SAARC Psychiatric Federation is still in its infancy but there has been a number of initiatives that have been started in the region from the SPF platform focusing on developing the mutual interest and joint collaborations in different regional countries. It is hoped that there will be more discussions at our next meetings regarding joint working in different areas of mental health. Some of the projects that are under consideration for the future work include:
• Focusing attention on the current inadequacies in mental health delivery in South Asia region. • Suggesting a suitable action plan for the future, in a cost effective manner, suitable to developing
nations. • Planning for unified mental health policy in the region • Reviewing programmes of training undergraduate medical students in Psychiatry • Training more number of psychiatrists, psychologists, social workers and mental health nurses • Empowering patients and their carers & families in the treatment programmes and ensuring
practices based on human rights and equal opportunities SAARC Psychiatric Federation now plans its 5th conference in Lahore on 25‐27 march 2011. This will be organised by Member Societies of SAARC Psychiatric Federation (SPF) ‐ Afghanistan, India, Pakistan, Bangladesh, Sri Lanka, Nepal, Bhutan and Maldives & Pakistan Psychiatric Research Centre, Fountain House, Lahore. World Psychiatric Association (WPA), World Association for Psychosocial Rehabilitation (WAPR), Asian Federation of Psychiatric Associations (AFPA), South Asian Forum on Mental Health , International (SAF‐I), Indo Pak Punjab Psychiatric Society and British Pakistani Psychiatrists Association (BPPA)have agreed to collaborate with this educational & scientific activity. We are also getting a lot of support from many national, regional & international organisations working in the field of mental health in Pakistan. It is hoped that these issues and many other related matters will be discussed at this conference.

Prof Haroon Rashid Chaudhry was among the founders of SPF and was the Secretary General of SPF at the time of his death in 2010 and had planned this
