5. IF DEWATERING OF THE TRENCH IS REQUIRED, CONTRACTOR TO PUMP WATER TO A FILTER BAG TO DEWATER.
6. CONTRACTOR TO SWEEP STREETS OF ANY DEBRIS OR SEDIMENTS CAUSED BY CONSTRUCTION OPERATIONS AND DISPOSEOF AT AN APPROVED LOCATION.
7. CONTRACTOR TO STABILIZE ALL DISTURBED AREAS WITH SEED & MULCH OR APPROPRIATE STREET REPAIR.
8. CONTRACTOR SHALL CONTACT THE DEPARTMENT OF WATER QUALITY AT 240-313-2600, 72 HOURS IN ADVANCE OF INSTALLINGTHE SEWER CLEANOUT.
9. EXISTING WATER SERVICES LOCATED ON SITE ARE TO BE TERMINATED. THE CITY OF HAGERSTOWN REQUIRES THAT THESERVICES BE TERMINATED AT THEIR RESPECTIVE MAINS IN VIRGINIA AVENUE AND BOWER AVENUE. THE PROPERTY OWNERSHALL BE RESPONSIBLE FOR ALL WORK ASSOCIATED WITH THE TERMINATION. INCLUDING EXCAVATION, SHORING, BACKFILL,AND RESTORATION. THE CITY OF HAGERSTOWN SHALL BE RESPONSIBLE FOR PHYSICAL TERMINATION OF MAINS.
10. CONTRACTOR SHALL FIELD VERIFY CONDITION OF EXISTING CMP SECTIONS TO REMAIN WITHIN MD-SHA RIGHT-OF-WAY ANDNOTIFY ENGINEER IF CONDITIONS ARE INADEQUATE FOR PROPOSED REVISIONS (I.E., PIPE DAMAGED, BLOCKED, ETC.).
11. CONTRACTOR SHALL FIELD VERIFY CONDITION OF EXISTING CMP OUTFALL ON NORTH SIDE OF VIRGINIA AVENUE IS INSATISFACTORY CONDITIONS TO ALLOW FOR PROPER DRAINAGE FROM SITE AND CONTACT ENGINEER FOR DIRECTION IFFOUND INADEQUATE.

5. ROUGH GRADE FOR BUILDING PAD AND UTILITY INSTALLATIONS.
6. CONSTRUCT BUILDING AND UTILITIES. NOTE THAT ALL CONTROLS SHALL BE INSTALLED IN ADOWNSTREAM TO UPSTREAM ORDER. DO NOT CONSTRUCT ANY IMPROVEMENTS PRIOR TOINSTALLATION OF DOWNSTREAM EROSION & SEDIMENT CONTROL PRACTICES. INSTALL INLETCONTROLS ON INLETS AS THEY ARE INSTALLED.
7. CONSTRUCT SITE IMPROVEMENTS WITHIN LIMITS OF SILT FENCE, INCLUDING BUT NOT LIMITEDTO CURBING, PARKING LOT, SIDEWALKS, STORMWATER PRACTICES, ETC.
8. UPON SUBSTANTIAL STABILIZATION OF THE SITE, (AT A MINIMUM: CONSTRUCTION OF BUILDINGFOUNDATIONS AND CONCRETE FLOOR, UNDERGROUND UTILITIES, CURBING, STONE BASE ANDASPHALT BASE) REMOVE PERIMETER SILT FENCE ADJACENT TO ROADWAYS AND CONSTRUCTROAD IMPROVEMENTS. ROAD BASE BOXED OUT SHALL BE STABILIZED BY THE END OF EACHWORK DAY WITH COMPACTED AGGREGATE. DO NOT LEAVE SOIL EXPOSED. CONSTRUCTCURBING, SIDEWALKS AND PERFORM BASE PAVING. ALL OTHER SILT FENCING AND SEDIMENTCONTROLS TO REMAIN.
9. COMPLETE SITE WORK SUCH AS SIGNAGE, PAVEMENT STRIPING, ETC.
10. STABILIZATION SHALL BE APPROVED BY THE ST. MARY'S COUNTY SOIL CONSERVATIONDISTRICT INSPECTOR AND MUST INCLUDE AT A MINIMUM THE BASE PAVEMENT OF PARKINGLOTS AND ESTABLISHMENT OF 75% STAND OF GRASS IN LAWN AREAS AND INSTALLATION OFPLANTINGS AND MULCHED BEDS IN LANDSCAPED AREAS.
11. UPON COMPLETION OF CONSTRUCTION AND APPROVAL BY ST. MARY'S COUNTY SOILCONSERVATION DISTRICT, REMOVE ALL EROSION AND SEDIMENT CONTROLS. 2. ALL SOIL EROSION/SEDIMENT CONTROL MEASURES SHALL COMPLY WITH THE “2011 MARYLANDSTANDARDS AND SPECIFICATIONS FOR SOIL EROSION AND SEDIMENT CONTROL” AND THEPROVISIONS OF THE APPROVED PLAN.
3. ALL GRADING AND STABILIZATION SHALL COMPLY WITH THE “2011 MARYLAND STANDARDS ANDSPECIFICATIONS FOR SOIL EROSION AND SEDIMENT CONTROL”, “SECTION B – GRADING ANDSTABILIZATION” AND THE PROVISIONS OF THE APPROVED PLAN.
4. ALL SOIL EROSION AND SEDIMENT CONTROL PRACTICES (BMP’S) ARE TO BE CONSTRUCTEDAND/OR INSTALLED PRIOR TO OR AT THE INITIATION OF GRADING IN ACCORDANCEWITH “2011 MARYLAND STANDARDS AND SPECIFICATIONS FOR SOIL EROSION AND SEDIMENTCONTROL”, AND THE APPROVED PLAN.
5. A GRADING UNIT IS THE MAXIMUM CONTIGUOUS AREA ALLOWED TO BE GRADED AT A GIVENTIME AND IS LIMITED TO 20 ACRES. WORK MAY PROCEED TO A SUBSEQUENT GRADING UNITWHEN AT LEAST 50 PERCENT OF THE DISTURBED AREA IN THE PRECEDING GRADING UNIT HASBEEN STABILIZED AND APPROVED BY THE ENFORCEMENT AUTHORITY AND/OR THE ST. a) THREE (3) CALENDAR DAYS AS TO THE SURFACE OF ALL PERIMETER DIKES, SWALES, DITCHES, PERIMETER SLOPES, AND ALL SLOPES STEEPER THAN 3 HORIZ TO 1 VERT. (3:1);

7. STOCKPILES MUST BE STABILIZED IN ACCORDANCE WITHIN THE 7 DAY STABILIZATIONREQUIREMENT, AS WELL AS, STANDARD B-4-1 INCREMENTAL STABILIZATION AND STANDARDB-4-4 TEMPORARY STABILIZATION (AS APPLICABLE).
8. ALL CONSTRUCTED CHANNELS AND SWALES SHALL HAVE SPECIFIED TREATMENT INSTALLEDTO THE DESIGN FLOW DEPTH COMPLETED DOWNSTREAM TO UPSTREAM AS CONSTRUCTIONPROGRESSES. AN INSTALLATION DETAIL SHALL BE SHOWN ON THE PLANS.
9. ALL STORM DRAIN AND SANITARY SEWER LINES NOT IN PAVED AREAS ARE TO BE MULCHEDAND SEEDED WITHIN 3 DAYS OF INITIAL BACKFILL UNLESS OTHERWISE SPECIFIED ON PLANS.ELECTRIC POWER, TELEPHONE, AND GAS LINES ARE TO BE COMPACTED, SEEDED, ANDMULCHED WITHIN 3 DAYS AFTER INITIAL BACKFILL UNLESS OTHERWISE SPECIFIED ON PLANS.

12. TEMPORARY SEEDING SHALL BE IN COMPLIANCE WITH SECTION B-4-4, STANDARDS ANDSPECIFICATIONS FOR TEMPORARY STABILIZATION, OF THE 2011 MARYLAND STANDARDS FORSOIL EROSION AND SEDIMENT CONTROL.
13. PERMANENT SEEDING SHALL BE IN COMPLIANCE WITH SECTION B-4-5, STANDARDS ANDSPECIFICATIONS FOR PERMANENT STABILIZATION, OF THE 2011 MARYLAND STANDARDS FORSOIL EROSION AND SEDIMENT CONTROL.
FOR SITES 1.0 ACRE OR MORE, THE FOLLOWING ARE REQUIRED:
A. MARYLAND DEPARTMENT OF THE ENVIRONMENT, GENERAL PERMIT FOR STORMWATERASSOCIATED WITH A CONSTRUCTION ACTIVITY, NPDES PERMIT NUMBER MDRC, STATEDISCHARGE PERMIT NUMBER 14GP, OR AN INDIVIDUAL PERMIT.
B. THE MARYLAND DEPARTMENT OF THE ENVIRONMENT (GENERAL/INDIVIDUAL PERMIT "NOTICE OFINTENT" (NOI)) APPLICATION AND PERMIT SHALL BE POSTED AND/OR AVAILABLE ON-SITE AT ALLTIMES. DURING CONSTRUCTION, ALL SOIL EROSION AND SEDIMENT CONTROLPRACTICES (BMP’S) SHALL BE INSPECTED AND RECORDED ON THE “STANDARD INSPECTIONFORM”, “GENERAL PERMIT FOR STORMWATER ASSOCIATED WITH CONSTRUCTION ACTIVITY” PERTHE MARYLAND DEPARTMENT OF THE ENVIRONMENT (GENERAL/INDIVIDUAL PERMIT "NOTICE OFINTENT" (NOI)).
C. FOLLOWING CONSTRUCTION AND RELEASE OF THE SITE FOR SOIL EROSION AND SEDIMENTCONTROL BY THE ST. MAY'S COUNTY SOIL CONSERVATION DISTRICT, I.E., ALL PORTIONS OF ASITE HAVE BEEN PERMANENTLY STABILIZED, AND ALL STORMWATER DISCHARGES FROMCONSTRUCTION SITES THAT ARE AUTHORIZED BY THE PERMIT ARE ELIMINATED, THEAUTHORIZED PERMITTEE SHALL SUBMIT THE MARYLAND DEPARTMENT OF THE ENVIRONMENT,GENERAL/INDIVIDUAL PERMIT "NOTICE OF TERMINATION" (NOT).
1. PRIOR TO THE START OF WORK, THE CONTRACTOR IS TO OBTAIN MDE AND ST. MARY'S COUNTY SOIL CONSERVATION DISTRICT APPROVAL OF ANY PROPOSED PLAN CHANGES AND SEQUENCE OF CONSTRUCTION,SPECIFICALLY RELATING TO INSTALLATION, INSPECTION, MAINTENANCE AND REMOVAL.
2. SEDIMENT CONTROL MEASURES ARE NOT TO BE REMOVED UNTIL THE AREAS SERVED HAVE ESTABLISHED VEGETATIVE COVER, OR WITH THE PERMISSION OF THE ST. MARY'S COUNTY SEDIMENT CONTROLINSPECTOR.
3. WHEN PUMPING SEDIMENT LADEN WATER, THE DISCHARGE MUST BE DIRECTED TO AN APPROVED SEDIMENT TRAPPING MEASURE PRIOR TO RELEASE FROM THE SITE.4. ALL TEMPORARY STOCKPILES ARE TO BE LOCATED WITHIN AREAS PROTECTED BY SEDIMENT CONTROL MEASURES, AND ARE TO BE TEMPORARILY STABILIZED.5. ALL SEDIMENT CONTROL DIKES, SWALES, BASINS AND FLOW LINES TO BASINS WILL BE TEMPORARILY SEEDED IMMEDIATELY UPON INSTALLATION TO REDUCE THE CONTRIBUTION TO SEDIMENT LOADING.6.
