We all would like to believe that every potentialclient, patient, or acquaintance adheres to the

moral adage…“beauty is only skin-deep.” However,the U.S. advertising giants would prefer us to acceptas true, their supposition that the average Americanconsumer’s decision-making process will be more in-fluenced by an individual’s outward appearance thanby his or her professional credentials. Adding legiti-macy to their arguments is the estimate that 35 mil-lion Americans experience some degree of hair loss,and spend $900 million annually on medication and/or surgical procedures to grow it back, as reported byJournal of Pharmacology and Therapeutics in theirrecent Medical Sciences Bulletin.1

About 95 percent of all cases of hair loss are the re-sult of androgenetic alopecia (also known as male pat-tern baldness in men). Androgenetic alopecia occursmuch more frequently in men than in women. Approxi-mately 25 percent of men begin balding by age 30; twothirds begin balding by age 60.2

complex and are not fully understood. However, what-ever the reasons may be for the thinning hair, it is gen-erally considered to be a hereditary trait, influenced bymultiple genetic factors. A tendency toward baldness inmen on either the mother or father’s side of a man’sfamily indicates a genetic predisposition to baldness.The speed, pattern, time of onset, and degree of baldingare all influenced by heredity. Generally, the earlier theonset of balding, the more extensive the degree of hairloss will eventually be.3 With balding, there is a pro-gressive miniaturization of the follicle resulting inshorter and finer hair. The end result is a miniature fol-licle with no hair. In male-pattern baldness, hair losstypically results in a receding hairline and baldness onthe top of the head. Women can also experience a loss,or thinning, of their hair. In female-pattern baldness,however, the hair becomes thin over the entire scalp.4 Ittypically starts with affected follicles producing pro-gressively finer and shorter hair as one ages. Mostwomen first experience hair thinning and hair loss wherethey part their hair on the top-central portion of the head.5

While the thinning of one’s hair can be a devastatingblow to one’s self-image or esteem, there are medicaltherapies available in the marketplace today—from oralprescription drugs to surgery—that can successfully dealwith the problem. One of the more popular approachesto coping with hair loss, and one that offers a perma-nent solution to the problem, is hair transplantation.According to Alan Bauman, M.D., medical director andfounder of Bauman Medical Group, PA, “With the greatadvances in surgical hair replacement procedures, menand women can now have safe, permanent hair replace-ment and avoid the downside of hair loss. Toupees, hairextensions and wigs can camouflage hair loss, but theonly permanent and maintenance-free solution to male-pattern and female-pattern baldness is a surgical proce-dure called hair transplantation.”

In hair transplantation, productive hair follicles areremoved from a “permanent” donor area on the backand sides of the scalp and transplanted to a balding area.

The follicles are commonly taken in “follicular-units”of one to three hairs (called micrografts) from the do-nor strip, and moved to the front and/or top, with thehairline being slowly reconstructed. Donor sites withan adequate quantity of hair make transplants more suc-cessful.6

“To be a good candidate for a hair transplant, a per-son should have enough hair at the sides and back ofthe head to serve as a donor area,” explained Dr.Bauman, who specializes in hair restoration for menand women at the Bauman Medical Group surgical cen-ter in Boca Raton.

A graduate of New York Medical College in Valhalla,NY, Alan Bauman, M.D., served his residencies in Gen-eral Surgery at Beth Israel Medical Center and MountSinai School of Medicine. “We were doing everythingfrom appendectomies to amputations,” says Dr. Baumanof his residency training, “but I was truly drawn to themore artistic cosmetic end of surgery. I had the oppor-tunity to go to Long Island to watch a hair transplantprocedure. I really didn’t know much about it at thetime…but it instantly appealed to me. On one hand, itwas a procedure that was very creative—using skingrafts to design a natural looking hairline from scratch,and yet it was also very detailed in terms of the ad-vanced microsurgical techniques and specialized in-struments that are used.”

sits on the Ethics and Bylaws Committee for the soci-ety and travels to several conferences each year wherehair transplant surgeons exchange the latest innovativetechniques and technology.

In a recent press release, Dr. Bauman announced theacquisition in his practice of the “Capillicare” hairanalysis machine from France. According to theproduct’s distributor in this country, Harmonix, Inc. inBoca Raton, Dr. Bauman is the first U.S. physician touse the machine for hair loss diagnosis and treatment.He described the unit as a non-invasive, hand-held videomicroscope attached to a computer with powerful im-aging software. The “Capillicare” aids the hair restora-tion surgeon in the diagnosis and treatment of patientssuffering from and/or concerned about hair loss. Dur-ing a consultation, it magnifies the patient’s scalp andhair (30X) to detect and record evidence of early hairloss. The machine is then able to monitor the effective-ness, and subsequent results, of the medical and/or sur-gical protocols implemented in the treatment program.Dr. Bauman expects this machine to revolutionize thetreatment of hair loss, easily allowing physicians andpatients to quantitatively analyze the effectiveness ofall types of hair loss treatments: non-medical, medicalor surgical. He believes that the use of the “Capillicare”might even “eventually stamp-out many of the spuri-ous snake oil ‘cures’ that are aggressively marketed tohair loss sufferers.”

• Trichogram: A microscopic exami-nation of hair-counts in variousstages of life—anagen, catagen,telogen. It determines the percentof hair in the growing or restingstages in the hair cycle.

• Phototricogram: The photody-namic examination of the scalpand hair, which facilitates the ex-amination of the hair cycle.

• Sebum rate: A measurement of se-borrhea of the scalp and hair fol-licles.

“With the ‘Follicular-UnitMicrografting Technique,’ we areable to transplant 1,500-3,000 tinygrafts in a single session, produc-ing a more natural result than theoutdated ‘plug’ methods. Thetransplanted hair will live and con-tinue to grow indefinitely. And itworks equally as well for both menand women experiencing hairloss,” says Dr. Bauman. “Underlocal anesthesia, a long strip-likeellipse of hair-bearing scalp is re-moved. Under magnification, thisdonor-strip is then meticulouslydissected by technicians into thetiny follicular-unit micrografts.The donor area is sutured closed(usually with absorbable stitches)

Us-ing special instruments, I make therecipient sites in the thinning ar-eas of the scalp—determining theangle, orientation and position ofeach transplanted hair. After I’vemade the sites, the technicians as-sist me in placing the micrograftsinto the sites. We can move up to3,000 micrografts in a single‘megasession’ if the donor area al-lows—this is much more than wasever thought possible just a fewyears ago.”

The decision to seek treatmentfor hair loss is a personal one. Andsince it presents no physical risk
