Project Description and Goal

To convene a community health improvement planning process that facilitates cross-agency collaboration and results in an improvement plan with measurable health objectives and strategies for action.

To integrate strategy aligned management techniques and tools such as strategy maps, balanced scorecards and community compact agreements into our Community MAPP and CQI process.

Baseline – A meeting effectiveness survey was conducted among members of the St. Clair County Health Care Commission in 2008. The results showed that while 90% of the HCC members were actively participating in a MAPP Action Team, team strategies were not communicated among the Commission members. The survey also revealed an inability to evaluate program effectiveness. It also showed an overall willingness to participate in more joint projects.
