Introduction

Hello and welcome to Glasgow!

If you are reading this you have signed up for this year’s RCGP pre-conference. This is the first year such an event has taken place and we anticipate that it will form an integral part of future conferences.

The pre-conference is organised jointly by the RCGP Junior International Committee (JIC), the Associates in Training (AiT) committee, and the

First5® committee and will provide an opportunity to find out more about what these groups can do for you. In addition, we aim to offer a unique forum for new and future general practitioners from around the world to come together and reflect on some of the challenges facing us all as GPs. This will in turn help participants to think beyond the confines of their consulting rooms and will provide the perfect start to an RCGP conference with the theme of Global General Practice.

As this is the first event of this kind to take place in the UK we are keen to hear your feedback at the end of the day – both on what you thought went well and perhaps what we might do differently next time. There is a feedback form at the end of this booklet and we will have spare forms to hand out on the day also.

There are several people we would like to thank for their help and support throughout this process. Antoinette Hoyte at the Royal College of General Practitioners has helped with administration. We would also like to thank staff at Profile productions, in particular Sue Elcock and Tara Everist.

Welcome to overseas participants

We would like to extend a special welcome to all those participants who have travelled from far and wide to attend the conference here in Glasgow.

In particular, there are 20 participants from all over Europe who are here as part of the annual Vasco da Gama/Junior International Committee Exchange programme. 15 of these delegates will attend the preconference and 5 will take part in the Society of Academic Primary Care events. They can easily be recognised by looking out for badges corresponding to their country of origin. Please make an effort to talk to these delegates as much can be learned from shared experiences of different systems of general practice.

The Vasco da Gama movement is the WONCA Europe Working group for early career (GPs within 5 years of qualification as a GP) and future General Practitioners (GP trainees). The movement is the continuation of the work that was started during the first preconference meeting for junior doctors during the WONCA Europe Conference in Amsterdam in 2004.

The ideals were set down in a meeting in January 2005 in Lisbon, the home port of Vasco da Gama, from where he set out on a similar voyage of discovery, and for this reason his name has been applied to the movement. It was formally launched at the Kos WONCA Europe Conference in September 2005.

One of the most successful activities of VdGM is the Hippokrates Exchange Programme. This international exchange programme offers an opportunity to visit or host a GP in another participating country for two weeks. The Hippokrates programme has been running for around 10 years in Europe and is now growing to incorporate the emerging junior WONCA networks across the world.

The Leonardo da Vinci Mobility Scheme is part of the European Commission's Lifelong Learning Programme, which funds many different types of activities of varying scales. In 2010 the UK and Slovenia were the first countries to successfully be awarded funding for outgoing Hippokrates participants. In 2011 the Czech Republic and Austria have been awarded Leonardo Funding and are currently selecting applicants to participate. In 2012 Leonardo Funding has been granted to Denmark, the Netherlands, Switzerland and UK. The UK is now accepting applications for exchanges that will take place in 2013. • Meet new UK and international colleagues.

• Explore what “Global General Practice” means and debate the changing role of GPs beyond the consulting room.

• Learn more about and get involved with the RCGP AiT, First5® and JIC committees.

• Warm up for the conference, or get a taster if you cannot make it to the main conference.

Aims

The aims of the pre-conference are:

• To raise awareness of the roles of the RCGP AiT, First5® and JIC committees.

• To consider some of the key challenges of “Global General Practice” and how trainee and First5 GPs – the future leaders of general practice – can contribute to addressing them.

• To meet fellow new and future GPs from the UK and across Europe and to develop professional links and personal friendships that will develop throughout the conference and beyond.

Who’s involved?

To make things a little clearer for all concerned, we asked the current Chairs of the three groups involved in the pre-conference to introduce their groups.

Associates in Training (AiTs)

Associates in Training (AiTs) are the trainee GP members of the Royal College of General Practitioners. With 10, 000 AiTs we represent roughly a quarter of the total RCGP membership. Our committee gives AiTs a voice in the development of policy and future initiatives in relation to GP training and membership. The committee is made up of a representative from each deanery within the GP training system. Both the chair and vice-chair hold a seat on RCGP Council – the national decision-making body within the College.

We aim to help AiTs establish a relationship with the College during their training through membership benefits such as the InnovAiT journal, annual conference and MRCGP exams. AiTs are also supported by their trainer and local training schemes. At the end of GP training many AiTs find they want ongoing support – we aim to help by facilitating the transition from AiT to First5®.

First5®

The first years after training can be a difficult time for new GPs who find themselves in the ever-changing world of modern general practice and the new independent practitioner may feel vulnerable and isolated.

In recognition of this situation, the concept of First5® was developed in 2008/9 as a way for the RCGP to support new GPs through those crucial first five years up to the first point of revalidation.

The pillars of First5® are:

• Connecting - Promoting a sense of belonging and appropriate representation for the First5 cohort within the College

• Facilitating Networks - Encouraging peer support and mentoring through the development of local networks

• Supporting revalidation

• Career mentorship - Career mentorship – making the most of a career in general practice

The regional faculty structure is the local face of the RCGP and offers opportunities for peer support, educational events and local representation. Central RCGP will continue to offer effective leadership and representation within the wider health service which represents the interests of all its members including those in the First5® cohort.

Junior International Committee (JIC)

The JIC's vision is to inspire a global approach to primary health care. We currently carry out a number of activities towards this common goal. These include developing links and projects with international healthcare organisations, providing a global health perspective to GP education and training, supporting and developing involvement in primary care research internationally and facilitating opportunities in international exchange. Through our image team we also aim to provide a hub for information and discussion on international topics relevant to AiTs and First5®s.

It is clear that there is a particular momentum for developing interest and activity in global health issues within the young doctors of the UK. As the JIC we hope to be able to harness and develop this momentum, whilst building on the already significant achievements of our team since starting in 2009.

Invited Speakers

Dr Iona Heath

Iona Heath worked as an inner city general practitioner at the Caversham Group Practice in Kentish Town in London from 1975 until 2010. She has been a nationally elected member of the Council of the Royal College of General Practitioners since 1989 and chaired the College’s Committee on Medical Ethics from 1998 to 2004 and the International Committee from 2006 to 2009. She is currently President of the Royal College of General Practitioners having been elected for a three year term from November 2009.

She has been a member of WONCA (the world organisation of family doctors) world executive since 2007.

Dr Steve Mowle

Steve is a GP Principal at the Hetherington Group Practice, an inner city practice in the heart of the Borough of Lambeth, London. He has held a variety of different educational leadership roles within the London Deanery and Kings College, London. Currently he is a Patch Associate Director with responsibility for Inner South West London and also has Pan-London responsibility for Post CCT GP training. He has a keen interest in Medical Education and Training and was a member of the RCGP First Five Working Group and a founding member of the RCGP Quality Management and Training Standards Committee.

Steve has been an active member of his local faculty board and as its Chair was a founding member of RCGP London. He has also been the lead on the International Committee for the formation and development of the Junior International Committee which has proved to be a major success.

As Vice Chair, Steve will be the lead for the 2012 Conference in Glasgow, lead officer for the Patient Partnership Group and lead for the Centre for Commissioning

What do we mean by “Global Health”?

The theme of this year's RCGP conference is “Global general practice”, but this may mean different things to different people in different contexts.

In an attempt to shed some light on this area, we present a framework for thinking about global health – within which global general practice is set – as outlined in a recent Lancet article.

“There is not yet consensus on a definition of global health, but certain key concepts underpin a global health approach: transnationalism, with national borders increasingly porous to disease, migration, and trade; holism and interdisciplinarity, taking a broad approach that includes subjects beyond the health sciences, such as law and international relations; interdependence and interconnectedness, recognising that global health threats are relevant to richer and poorer countries and can be tackled only through collaboration; and an emphasis on understanding inequalities between and within countries.” [from Johnson et al. Relevant events during the main conference As noted, the theme of the main conference this year is Global General Practice. This will be a celebration of best practice in the healthcare services delivered in the UK, and an opportunity for colleagues to learn about innovative practice in other parts of the world.We have hand-picked a few sessions from the main RCGP conference that may be of interest. But first, here is an introduction to the JIC stand.

JIC standWelcome to the JIC stand! This is the perfect conference to gain insight into worldwide experiences in primary healthcare. And the JIC stand is the ideal place to really find out what this means. Throughout the conference, the JIC stand is hosting a series of mini presentations delivered by doctors from over 8 different countries. These will cover details of different healthcare systems - including the role of the family doctor, models for general practice training and direct experiences of primary care. The structure of the presentations will favour debate and discussion, allowing for unique opportunity to really get to grips with similarities and differences in practice. This is also a focal opportunity to form professional, educational and personal international links with like-minded practitioners. Please see the timetable at the stand for further details. General information about Glasgow
