Abstract

This report examines the need for a paramedic level ambulance service in

the City of Oconto, Wisconsin. Current EMS service is provided by the Oconto Fire

Department at the Advanced EMT Level. The department has been recognizing a

demand for paramedic level service in recent years. The severity in the types of

EMS calls has been increasing along with a demand from the local hospital for a

local paramedic service to transport patients to larger hospitals for definitive care.

Evaluative research on Oconto Fire Department ambulance run data in 2009 is

conducted in an attempt to identify those calls worthy of a paramedic level

assessment or intervention. Evaluative research was also conducted on hospital

patients who required paramedic level ground transport to other hospitals in

2009 in an attempt to confirm the demand from the local hospital.

The report reveals conclusive evidence of the demand for a paramedic

ambulance service in the City of Oconto and recommends proceeding further

towards an EMS upgrade by conducting further research on costs versus benefits

and seeking potential funding sources.

Introduction

The Oconto Fire Department has been recognizing an increase in the

demand for advanced life support care. Pubic demand and expectations for

complete care are on the rise while fire fighters are recognizing their own

shortcomings in their EMT skill sets as they treat patients with more severe and

complex cases. The local hospital in Oconto is in great demand for a readily

available ALS service to support its mission and transport their patients to

definitive care hospitals while the private ALS service based out of the nearby

larger city looks to extend its services further north.

This report examines the need for paramedic level EMS service for the City

of Oconto, Wisconsin. An evaluative research method was conducted on the

nature and severity of ambulance runs provided by Oconto Fire Department in

2009. The research attempted to answer the question, “How many patients

treated by Oconto Fire Department could have benefited from a paramedic level

assessment or skill in 2009?” Additionally, the research attempted to answer the

question, “How many patients transferred from Oconto Hospital and Medical

Center could have been transported by an Oconto Fire Department ALS ground

unit in 2009?”

This report focuses on the need for paramedic level service, but since cost

is always a primary factor when considering a project of this nature, this report

touches on this subject as well. Many unknown variables involved with cost exist

for the Oconto Fire Department and will be listed in detail in the

recommendations section. These intangible factors made it impossible to

conduct a thorough cost/benefit analysis at the time of this report. However,

interviews and information was obtained from comparable fire departments in

Wisconsin that successfully upgraded to the paramedic level in the last five years

as well as ambulance billing data and will be shared in this report as well.

Background and Significance

The City of Oconto, Wisconsin is located thirty miles north of the City of

Green Bay on the shores of Green Bay. Fire protection and EMS service for the

area is provided by the Oconto Fire Rescue Department through a combination

service. Six fulltime line personnel staff the station 24/7 in shifts of two. A

fulltime fire chief oversees sixteen additional paid-per-call fire fighters, eight of

which are also EMTs.

As part of the fire department, the Oconto Ambulance Service serves the

City of Oconto and its three surrounding townships with what is now known

nationally as the Advanced EMT level service. The service provides customary

EMT interventions along with IV therapy and a limited about of drugs.

Recognizing a need in 2004, the service upgraded its skill level from EMT-Basic to

the Advanced level in 2005. In addition to an improvement in patient care, the

city has enjoyed an increase in revenue from the additional billable amount

allowed by Medicare for IV therapy.

The only hospital in the city, Oconto Hospital and Medical Center, is a four

bed hospital with a clinical lab, X-Ray with CT, and a level-IV trauma room. Due to

its size and limited capabilities, often, patients are transferred by ground or air to

more definitive care hospitals in Green Bay after being treated and stabilized

locally. A number of Oconto Hospital’s patients require ALS service when

transferred to other hospitals. Since Oconto Fire Department cannot provide this

service, a private ambulance service responds from the Green Bay area to

transport. On many occasions, critical patients must wait thirty minutes or more

for a ground unit to arrive when the helicopter is unavailable due to weather or

committed to other calls.

While not quantifiable from this report, the public expectation for complete

EMS care is rising, especially the expectations from a fulltime fire department.

EMTs are witnessing the disappointment on the faces of patients or family

members when they find out that the EMT is unable to treat their pain or stabilize

their cardiac condition. Meanwhile, the private ALS service responds to rural

areas in the adjacent jurisdiction fifteen miles south of the city from their station

located in the suburbs of Green Bay. In a manner similar to other fire

departments around the country, Oconto Fire Department must deal with

competing privatization as well. Fire fighters view this as a direct threat to their

job security and are eager to match or exceed the service level of any

competition.

Procedures

Ambulance run data analysis was conducted by using the department’s

electronic web-based run reporting system. The system was set to list all 911

calls in 2009 along with the patient’s chief complaint, the transport mode to the

hospital, and the EMT’s primary impression and secondary impression (figure 1).

Ten major categories were screened for in the EMT’s Primary Impression or

Secondary Impression: Cardiac Emergencies, Pulse-less Non-Breathers, Allergic

Reactions, Respiratory Distress, Trauma, Drug Overdose, Strokes, Abdominal Pain,

Seizures, and Altered Level of Consciousness. For the purpose of this report,

these ten patient conditions were considered to have potentially benefitted from

a paramedic level assessment or intervention. The transport mode to the hospital

was considered whenever the severity of the call was in question. For example,

trauma encompasses a wide range of injuries and could be a simple cut to the

finger or massive head and chest trauma from a motor vehicle accident. Knowing

whether or not the EMT chose to use lights and siren to the hospital reveals the

EMT’s impression of the severity of the call. In an attempt to show the severity of

the calls, a second analysis was also conducted on the same data using only those

calls that were transported with lights and siren.

Patient transfers from Oconto Hospital and Medical Center were also

analyzed. At the department’s request, the hospital conducted an internal

analysis of patients transferred from their facility. A total of all patient transfers

were compiled and analyzed for critical care criteria. Patients falling into the

critical care category were assumed to have been transferred by helicopter or the

helicopter’s ground equivalent with specialized personnel and equipment

operating above the paramedic level. The remaining number of patient transfers

was given to the department for analysis. The department was able to use this

data against the department’s own data to determine the remaining number of

patients that were transferred by paramedic ground units.

In order to gain an inside perspective and a general idea of cost, an

interview was conducted with Antigo Fire Department in Antigo, Wisconsin.

Antigo is comparable in size in the region and successfully upgraded to the

paramedic level five years ago. Plenty of advice was shared. The initiative started

from the bottom up at their department. The union organized a study, performed

all the ground work and presented it to the management along with some

concessions on overtime expenses associated with the training. The city council

agreed, and have been happy with the service ever since. The city has also been

reaping a large reward in the revenues generated from patient transfers from

their local hospital.

Antigo shared a cost/benefit analysis of Merrill Fire Department, another

city comparable in size to Antigo. In 2005, the cost for that department to

upgrade eight fulltime fire fighter/EMTs to paramedic was $125,000, including all

necessary equipment. Merrill Fire Department needed to do fundraising drives in

order to meet this cost and was successful in completing their upgrade. Their

study found that about 33% of their patients could have benefitted from a

paramedic level service.

Finally, the department’s billing company provided information on the

various billing rates. Since the department is already able to bill at the Medicare

ALS1 rate whenever an IV is started, an obvious revenue increase is not readily

apparent. However, the paramedic level allows for an additional billable rate at

the ALS2 level. In order to bill at this level, it must be medically necessary to

administer at least three different IV medications or provide one or more of the

following ALS procedures: manual defibrillation/cardioversion, endotracheal

intubation, central venous line, cardiac pacing, chest decompression, surgical

airway, or intraosseous line. The most frequent billable ALS2 situation that

Oconto Fire Department would encounter is cardiac patients with certain

conditions and Pulse-less Non-Breathers. The number of cardiac and PNB calls in

2009 is presented subsequently.

Results

Out of the 502 scene response calls in 2009, 309 of them, or 62%, fit into

the ten criteria listed that were considered call types that may have benefitted

from a paramedic level assessment or skill. A breakdown can be seen in figure 2.

Secondly, in an attempt to show the severity of the calls, the same data was

filtered to show only those patients that were transported emergent with lights

and siren. 183 of the 502 patients, or 36%, were serious enough to warrant

emergent transport, see figure 3.

Finally, patient transfers that did not require specialized critical care from

Oconto Hospital and Medical Center consisted of 94 patients by ground

ambulance in 2009. Oconto Fire Department was able to transfer 62 of those

patients, or 66%, with their current level of service, leaving 32 patients, or 34%,

transferred by another paramedic agency as seen in figure 4.

Discussion

62% is a very significant amount of patients requiring paramedic level 911-

response. Even if an error was made in the data analysis and only half the

number of patients needed a paramedic, the department would be at the same

results as those found in Merrill, Wisconsin where a successful paramedic

program is in place. Furthermore, the severity report shows that at least 36% of

these patients were critical enough for emergent transport.

From an interfacility perspective, one out of every three patients

transferred from Oconto Hospital and Medical Center required a paramedic level

service. As an average, these patients must wait at least one hour from the time

that the call is made to the transferring agency, until the time they arrive at a

definitive care facility. A local paramedic service would be able to cut this time in

half. Interfacility transfers are an excellent source of additional revenue. It would

be logical to assume that the City of Oconto could expect this dollar amount to

increase by approximately one-third.

ALS2 billable calls would most likely occur with certain cardiac calls, PNBs,

or in situations when IV access absolutely must be established and the

paramedics cannot start one successfully using conventional IV methods in the

patient’s arms or legs. These types of calls most likely account for less than 10%

of all Oconto Fire Department’s EMS calls, but they are life saving interventions

that cannot be measured fiscally.

The billing company submitted a comparison of rates with five other clients

in the region. In nearly every category, Oconto charges less than other paramedic

level services, (see appendix). As an option, these charges could be increased to

current standards in order to expedite the cost recovery for the paramedic

upgrade.

The data shows a compelling need for paramedic level service for the

Oconto area and the Oconto Hospital and Medical Center. Patient care could

drastically improve both in the field and out the doors of the hospital if a

paramedic service were locally available. After the initial start-up costs, a

paramedic system would be self-sustainable since the patients are billed directly

for any medications and equipment used. Over a time to be determined through

further cost analysis, the city will see the upgrade cost become offset from an

increase in revenue generated through interfacility transports and ALS2 billable

rates.

Recommendations

Additional research is required in order for the Oconto Fire Department to

move forward. A detailed cost analysis must be conducted. Current revenues

must be more closely compared to the potential revenues that would be

generated at the paramedic level. Upgrade costs must be compiled. The fire

fighters union and the city must reach an agreement for training pay, benefits,

and any potential salary increases. Oconto Hospital expressed a desire to support

the paramedic upgrade and must be involved and consulted for its resources.

Based on the results and financial support received from the union, city, and

hospital, grants or fundraising activities may or may not be needed. Fortunately,

the State of Wisconsin provides for a 2-year grace period for EMS organizations

attempting to reach 24/7 paramedic coverage. This factor can be a major selling

point to financial investors since the cost to upgrade could potentially be spread

over two years. Once an upgrade plan is finally in place, the Wisconsin EMS

Division must be contacted for permission and approval. Protocols, Operational

Plans, Exposure Control Plans and Standard Operating Procedures would need to

be updated during the upgrade process.

Lastly, within two years after the upgrade, the original problem presented

by this report should be revisited to check for improvements in patient care,

revenue generated and hospital support.
