Mastering extraction and socket preservation

Dental implant placement is a highly demanding surgical procedure. It is all made easier with proper handling of tooth extraction and socket preservation. In this course we will explore current tooth extraction techniques that are minimally invasive and simple to achieve as well as evidence based socket preservation and grafting that will enable you to handle and prepare an edentulous site for implant placement. Some time will also be spent on discussing immediate implant placement as a way to preserve osseous and soft tissue architecture after extraction.

Marketing Dental Implants; Increasing Case Acceptance Strategies

One of the barriers to case acceptance in implant dentistry is still dealing with the associated higher fees unexpected by our patients and minimal coverage by their insurance carriers. Although clinicians have the knowledge base to provide optimal implant dentistry treatment, they are often frustrated with lack of patient acceptance and compliance. This presentation will teach dentists various techniques to improve their case acceptance rates for implant dentistry. In addition, optimal patient education tools will be illustrated to aid in case acceptance rate.

Zygoma Implants and Immediate Load Rehabilitation of the Maxilla

The All-on-4 treatment concept by Nobel Biocare allows for rehabilitation of either the maxilla or mandible with just four dental implants. This treatment concept allows for immediate load fixed rehabilitation of either arch. The maxilla is more challenging due to the presence of the maxillary sinuses. Not all cases may be treated with standard implants in the All-on-4 treatment concept. Even with the tilted implant concept, spread is often limited by the anterior position of the maxillary sinuses. This presentation will discuss the unique Zygoma implant and its use in the All-on-4 immediate-load treatment concept. The presentation will outline the clinical situations when the Zygoma implant is required for maxillary, fixed-immediate-load implant rehabilitation. The various considerations involved with the Zygoma implant treatment planning will be discussed from surgical to prosthetic issues. Complications associated with this type of implant will also be reviewed.

Dr. Lorenzo Vigna graduated with Honors from the University Of Toronto Faculty Of Dentistry. After completing dental school Dr. Vigna completed a one year dental internship at the Toronto General Hospital from 1990-1991. At that time he treated medically compromised patients at the dental clinic and was exposed to the oral surgery program at the University. This created interest in oral surgery and subsequently encouraged him to apply to the oral surgery programs throughout the United States.

In 1995 following oral surgery residency Dr. Vigna took his fellowship exams and obtained his Royal College of Dental Surgery fellowship with the Canadian Board. Dr. Vigna is the new Chief of Oral and Maxillofacial Surgery for the Niagara Health System. He is also the Service Lead for Dental Service with the Department of Surgery at the Niagara Health System. Dr. Vigna is an Assistant Clinical Professor of Surgery with McMaster University and teaches Ear, Nose and Throat residents in both his clinic and hospital.

Dental Implants: The Good, The Bad and The ugly

Dental implants have the potential to rehabilitate the dentition of our patients and provide excellent service for a life-time. It is important to recognize that while implants have had a revolutionary effect on modern dentistry, they do pose a challenge when it comes to long-term care and management. With the growth in the number of dental implants placed annually, by greater numbers of practitioners of varying experience, using more types of implant systems, and ever-increasing pressure to reduce costs, it is our responsibility to prepare ourselves for the tsunami of aging implants and the difficulties we will face managing them.

Complications in Implant Dentistry

Some complications may be relatively minor and easy to correct, while others will be major and result in the loss of the implant or prosthesis. Implant complications can typically be categorized by temporal sequence and defined as either an early or late complication. An early complication can be defined as a problem that arises prior to osseointegration of the dental implant, whereas a late complication occurs after the implant osseointegrates and the final prosthesis is placed. In addition, implant problems can further be subcategorized into biologic or mechanical complications. Biologic complications involve pathosis of the peri-implant hard and soft tissue. Mechanical complications occur when the fatigue strengths of the dental implant or restorative components are reached and breakdown occurs resulting in material failure.

PRF and immediate implant considerations

Recently the use of Platelet Rich Fibrin "PRF" treatment has gained tremendous growth in the dental field. Beginning with the history of platelet concentrates to the use of A-PRF and i-PRF, the everyday clinical use of PRF and its indications will be discussed with a focus on implant therapy.

Immediate placement of dental implants has become common practice, shortening treatment time with success rates comparable to delayed placement. Several factors for success will be discussed along with surgical and prosthetic considerations for esthetic and predictable stability long term.

New Evolution in Implant Overdenture Therapy

Edentulous or soon-to-be edentulous patients require dental implants for stability and retention. For many years locators were the main treatment option for removable prosthesis. Locators are better than no treatment, but patients often complained of instability with locators along with the costly maintenance associated with the changing of the nylon inserts. The fixed-removable overdenture solution has eliminated both problems and provides a rigid fixed prosthesis that is removable.

The fixed removable overdenture solution also allows the surgeon to “follow the bone” and place implants into solid bone without concern for parallelism. The tipping of distal implants in fixed cases creates increased anterior to posterior spread with decreased cantilevers with more functioning teeth within the prosthesis. This same surgical technique can be incorporated into this new overdenture surgical procedure. A one stage procedure allows for extractions, bone reduction, implant placement along with healing collar all at one time. Within 8 -10 weeks, patients are ready for impressions.
