Dear Doctor,As per the World Health Organization statistics, every year the lives of
approximately 1.25 million people are curtailed as a result of a road traffic
crash. Between 20 and 50 million more people suffer non-fatal injuries, with
many incurring a disability as a result of their injury.
About three-quarters (73%) of all road traffic deaths occur among men.
Among young drivers, young males under the age of 25 years are almost 3
times more likely to be killed in a car crash as young females.
Young people are suffering, decapitating, dying, leading to many young
families grieving—we can avoid this misfortune by timely management of
trauma cases.
In the past two years, we have successfully discussed various newer trauma
management methods. The Indian Orthopaedic Association (IOA) is glad to
partner with the American Academy of Orthopaedic Surgeons (AAOS) to bring
the Third Trauma Conclave, a forum to discuss and learn the latest in trauma
care.
The IOA is one of the largest professional organizations, which focuses on
academics, research, and social networking. It is a constantly growing
organization not only in terms of increasing memberships but also in all other
fronts like fellowships, CMEs, orations, and subspecialty meetings.
Once again this year, we look forward to your presence in this knowledge
exchange conclave.

improved standard of living. In this fast-paced life, trauma is becoming one of the
major health hazards, leading to mortality and morbidity.
In such situations, managing trauma in patients is critical. The AAOS with an objective
of tackling orthopedic-related conditions has managed to create a lot of relevant

Dr. Zura, a native of Baltimore Maryland, attended the Gilman School and then the
University of Virginia. He returned to Baltimore to study medicine at Johns Hopkins
and then back to UVA for his orthopedic residency. He studied under Drs. Kellam,
Sims, and Bosses at the Carolinas Medical Center in Charlotte, NC for his orthopedic
trauma fellowship. Before going to Duke, Dr. Zura spent 2 years on faculty at the
Medical College of Georgia and a year in private practice in Fredericksburg, VA. He
spent 12 years at Duke and was the Duke director of orthopedic trauma and
director of the orthopedic trauma fellowship. He has over 80 peer-reviewed
publications. His interests include fracture care, intramedullary nailing, powdered
gloves, nonunions, and osteomyelitis.
A graduate of the University of Virginia and Johns Hopkins School of Medicine,
Dr. Zura completed his surgery internship and residency at the University of Virginia
and an orthopedic trauma fellowship at the Carolinas Medical Center. He currently
coordinates the Orthopaedic Trauma program for all Duke University residents and
fellows.
Dr. Andrew N. Pollak is a professor at the James Lawrence Kernan, Chairman of the
Department of Orthopaedics at the University of Maryland School of Medicine,
and Chief of Orthopaedics for the University of Maryland Medical System. In the
past, he served as Chief of Orthopaedics and Associate Director of Trauma at the
R Adams Cowley Shock Trauma Center at the University of Maryland, which is a
world leader in trauma clinical care, research, and education. As chair of the
extremity war injuries (EWI) project team for the AAOS in the past, he developed
several important programs to assist military orthopedic surgeons in the care of
wounded warriors. In conjunction with Dr. Richard Kyle, he developed a report
following the experience that recognized the outstanding accomplishments of
military medical personnel in developing the trauma care capacity in Iraq and made
recommendations to achieve incremental improvements. He has served in the past
as chair of the board of specialty societies for the AAOS, as treasurer of the AAOS, as
president of the OTA, and as president of the Maryland Orthopaedic Association.
He currently serves as Series Editor for the AAOS Orange Book Series.
Amongst his other responsibilities, Dr. Pollak was recently appointed by the
Governor Larry Hogan as Commissioner of the Maryland Health Care Commission.
He also serves as Medical Director for the Baltimore County Fire Department and as
a Special Deputy US Marshal.

Dr. Cross is a board-certified orthopedic surgeon specializing in Orthopedic Trauma
with special interests in nonunion surgery and sacroiliac joint dysfunction. He is
Vice Chair of the Department of Orthopedic Surgery and Chair of the Division of
Community Orthopedic Surgery.
Dr. Cross completed his medical training at the University of Minnesota Medical
School in Minneapolis, MN in 2009 and completed a 1-year Orthopedic Trauma
Fellowship at Harborview Medical Center in Seattle, Washington. His area of
interests are orthopedic traumatology, complex periarticular injuries, pelvic and
acetabular fracture surgery, malunions and nonunions.
He is an active member of various societies including the American Academy of
Orthopaedic Surgeons and the Orthopaedic Trauma Association. He is actively
involved in several clinical research studies at Mayo Clinic and was recently voted as
Teacher of the Year for the Mayo Clinic Orthopedic Surgery Residency Program. He
has numerous scientific articles to his credit printed in various national and
international journals of repute.

Dr. Wolinsky, a board certified orthopedic surgeon, specialized in the care of
orthopedic trauma patients at a level 1 trauma center in Northern California. He did
his residency training at the New York University Medical Center, followed by a year
of fellowship training in orthopedic trauma at Vanderbilt University Medical
Center. He practiced orthopedic trauma surgery at Vanderbilt for 4 years, was Chief
of Orthopaedic Trauma at Bellevue Hospital Center for 4 years, and has practiced at
UCMDC since 2002. Along with being a committee member of the AAOS and OTA,
he is the chair of the orthopedic surgery trauma group of the Committee on Trauma
(COT), which is a part of the American College of Surgeons. He is actively involved in
developing the process to collect metrics for evaluating the care delivered to
orthopedic trauma surgery patients.

1. Understand the deformities characteristically seen with proximal and distal tibial fractures.
2. Appreciate the advantages and disadvantages of both intramedullary nailing and open reduction and
internal fixation with plate/screw constructs for the treatment of proximal and distal tibia fractures.
3. Be able to apply the techniques employed to obtain and maintain appropriate reductions of proximal
and distal tibial fractures

subtrochanteric and distal femoral fractures.

1. Understand the treatment options for proximal humeral fractures to include non-operative
management, percutaneous pinning, open reduction and internal fixation with plates, intramedullary
nailing, hemiarthroplasty and reverse total arthroplasty.
2. Understand the reasons for fracture fixation failure and be able to apply techniques to minimize
failures in the treatment of proximal humerus fractures.
3. To better understand modern principles of fixation of humeral shaft, distal humeral and distal radius
fractures.
Summary
Case Presentations
1. Fractures of the proximal tibial metadiaphysis and proximal diaphysis are injuries commonly seen in
road traffic collisions.
2. These fractures may have concomitant injuries to the tibia either proximally or distal to this
fracture. In high energy mechanisms, a high suspicion for vascular injuries and/or compartment
syndrome should be maintained.
3. Intramedullary nailing may allow earlier weight-bearing in some cases.
4. Historically, intramedullary nailing has proven to be technically difficult & is associated with the classic
deformities of valgus, apex anterior angulation and posterior translation of the distal segment.
5. Multiple techniques have been developed to help overcome the obstacles related to the treatment of
these fractures.
6. Distal tibial fractures are fraught with many similar challenges as proximal. Multiple techniques have
been developed to help overcome the obstacles related to the treatment of these fractures.
7. Subtrochanteric femoral fractures are challenging due to the classic deformity of varus, shortening,
adduction and varus. Multiple techniques have been developed to help overcome the obstacles related
to the treatment of these fractures.
8. Distal femoral; fractures can be plated or nailed. Nailing these fractures requires care and multiple
techniques have been developed to help overcome the obstacles related to the treatment of these
fractures.
9. Proximal humerus fractures can be difficult to treat whether treating non-operatively or operatively.
10. Non-operative management is a realistic option in a majority of cases.
11. The choice of open reduction and internal fixation versus arthroplasty depends on the patient's
physiologic age, activity level, rehabilitation potential, fracture pattern and bone quality.
12. Humeral shaft, distal humeral, and distal radius fractures are common and challenging fractures.
Multiple techniques have been developed to help overcome the obstacles related to the treatment of
these fractures.
