LATERAL EXPANDERS
POSTERIOR DRIVE
ANTERIOR DRIVEAn A-P Expander is nothing morethan an arch-lengtheningappliance. It can be a major toolin your treatment plan by enablingyou to relieve crowded arches.

Bonded Hyrax with Super Screw
BONDEDRPEWITHSUPER SCREW
The NPE is a thermally-activatedappliance used for slow expansion. Many doctors like to use the NPEbecause there are virtually noadjustments needed. The appliance ischilled prior to seating in order to gainflexibility. Once the metal is warmed inthe patientʼs mouth, it regains itstension.
Some doctors leave this in during thebracketing phase for cross-palatalsupport.
This is similar to the banded RPE butmore commonly used to correct a V-shaped arch. The appliance expandsprimarily in the anterior area (like a fan).
The banded and bonded Hyrax wereboth originally designed for rapid palatalexpansion. The traditional hyrax screw(shown above) expands up to 10millimeters and is still widely usedtoday.
The “Super Screw” (shown below)can be used for additional distances ofup to 18 millimeters.
BONDEDHAAS
Bonded Haas
One advantage the bonded versionoffers over the banded is it preventstipping of the teeth. The Haas appliancealso provides additional palatalanchorage during expansion. Theappliance above is Dr. Steve Galellaʼsversion. Clear acrylic is used so doctorscan check for leakage.

This commonly prescribed appliance isused to develop the pre-maxilla. Thedoctor controls the spring activation, which in turn gives them predictableresults.
The nance button allowsremodeling of the anterior pre-maxilla.OPTIONS: Galella tongue loop, RPEscrews or cross-palatal arch wire.

This easy to bond wire frame Hyrax is aversion taught by Dr. Beistle to USDIDoctors. The absence of acrylic makesfor easy removal and clean up. Photoalso shows RHG hooks often usedduring mixed dentition.

The Magill appliances provide anterioraccess for screw adjustments, which isshown close up in upper right corner.
The bonded Hyrax provides bettertorque control and is a good solution forhigh angle cases. Thick pads helpintrude the posterior teeth by interferingin the freeway space.
Gordon Expander
GORDONEXPANDERThe benefit of the Gordon anteriorexpander is once this step is completed, the doctor can insert a Controlled Archwithout removing the bands.

SPAHL SPLITVERTICALThe Spahl Split Vertical appliance hasbecome an important fixture in modernorthodontic treatment. It allows doctorsto close the posterior open bite tosupport the condyle in its new relocatedposition.
The Spahl Split Vertical functions the same as the Biofinisher, but more doctors are prescribing theSSV for a number of reasons. It doesnot require an additional appliance likethe Biofinisher often does, and it ismade with less acrylic, so it is morecomfortable with fewer speechproblems.

The E-Z bond has gained in popularitybecause it offers less chairtime for thedoctor. Composite pads on eachindividual tooth also help prevent arecurrence of crowding.
TRANSPALATAL BARThis form of a holding arch is one of themost commonly used appliances forholding the molars in a posterior, de-rotated position.
Transpalatal BarHERBST©
The Herbst© appliance can be a goodtreatment alternative for patients whowill not wear removable appliances withregularity. There are two main appliancedesigns available from Johns Dental:the banded and the bonded Herbst.
The appliance advances themandible allowing skeletal and dentalchanges to occur. The patient has arange of vertical and lateral movementmaking it reasonably comfortable after abrief adjustment period.

CONTROLLED ARCH
Controlled Arch
This molar-stabilizing appliance createsproper tip, torque and rotation of molarsprior to the next level of treatment. It isimportant to complete this step first toprevent arch wire deflection andunwanted tooth movement.
This version of the controlled arch, isshown above with Delta-Force brackets.
BANDED RETAINERThe lingual wire retainer is normally used atthe end of orthodontic treatment to maintainincisor position. Both upper and lower archesare available from Johns Dental. Multiplemesh pads can also be requested for eachtooth.
Banded RetainerUpper MARA
MARA®
(Mandibular Advancing RepositioningAppliance)The MARA is used to advance themandible. It started to gain popularity in2003 with lecturers like Dr. Derek Mahony speaking of itssuccesses.
The standard MARA does not haveexpansion screws or arch wires, butthose features can be add-on options.Metal spacers are used to activate theappliance.

FIXED TWIN BLOCK (Dr. Jay Gerberʼs version)This mandible-advancing applianceoffers similar benefits to otherremovable Twin Blocks, but eliminatespatient compliance concerns with his“Banded Block” version. In addition, many of the sharp edges have beenrounded to improve patient comfort.

LINGUAL ARCH(Soldered)This lingual arch is our standardsoldered version and is used as aholding appliance. Some doctors preferthe Wilson versions, which allow the useof interchangeable modules for futuretreatment.
