A groundbreaking cancer-imaging technique pioneered to diagnose human prostate cancer is being repurposed by re-searchersatColoradoStateUniversity(CSU)todiagnosecaninecancerindogswithheadandnecktumors.

MorrisAnimalFoundationisfundingtheresearch,whichwouldallowveterinarianstodevelopoptimumtreatmentplansduring cancer stagingtomaximizeapatient’schanceforapositiveoutcome.

Thetechniqueinvolvesinjectingnanoparticles—inthiscase,microscopicbitsofiron—intoadog’sbloodstream.Thebody’simmunesystemthenengulfstheparticleswithwhitebloodcells(macrophages)thatclearbloodofcellulardebrisbeforemovingintonearbylymphnodes.

IronappearsblackonanMRI,soacompletelyhealthylymphnodefullofnanoparticle-bearingmacrophageswouldlookblack,too.Butcancercellspushmacrophagesoutoflymphnodes,soifcancerhasinvadedevenasmallportionofalymphnode,thatportionwouldappearwhiteonanMRI. In an initial pilot studyofasix-dogsamplegroup,thetechniquedemonstratedan88%successrateindetectingcancer.Researchersplantoenroll50dogswithheadandneckcancersforthenextphaseoftesting. NEWStat:Whataretheadvantagesofthisnewtechniqueovercurrentimagingtechniques?

Lynn Griffin: A lot of the current imaging methods used to diagnose metastatic lymph nodes rely on the size of the lymph node(aspalpatedorasseenwithvariousimagingmodalities)orthecontrastenhancementpatternsthatarerelativelyinsensitive/inaccurate.Thereareanumberofpreviousarticlesthathaveareportedsensitivityrangingfrom12.5%uptoalmost70%.Becauseweareexploitingmoreofaphysiologicconsequenceofmetastaticdisease,thismethodseemstobemoresensitive.

NEWStat:Rightnowyou’refocusingontumorsintheheadandneck.Why?

LG:We’reusingheadandnecktumorsrightnowbecausetoprovethismethodworks,wehavetoextirpatethelymphnodesandhavehistologyperformed.Thisisacommonpracticefordogswithaggressiveheadandnecktumorswithlimitedmorbiditytothepatient(versusifwewereextirpatingsublumbarlymphnodes,forexample).

NEWStat:What’snextforyourcurrentresearch?

LG: We are going to be using these scans to develop some objective quantitative parameters to evaluate lymph nodes, ratherthanjustlookingatthemandsaying“aretheymetastatic,yesorno?”amethodthatwouldbemorepronetoobservererror.Wearealsoworkingoncreatingatruncated,lessinvolved—andthereforelessexpensive—protocolthatwillmakeitmoreclinicallyapplicableforotherpractices,notjustresearchinstitutions.That’sprettyimportanttome,asIdon’tseethepointinspendingallthemoneytocreateanew,betterwaytostageapatient[if]noonecanaffordtodoitintheireverydaypractice.

NEWStat:Andafteryoufinishthisstudy? LG:We’relookingattheimagingofinflammationpre-andpost-SRTwithironnanoparticlesaswellasusingthemtolookatvasculardensityintumors.It’sareallyexciting,newlyemergingareaofresearchthatIthinkisgoingtobeverybenefi-cialtounderstandinghowtheimmunesystemplaysaroleinhowourbodyworkstoeitherhelporhindercancer.

Rabies Prophylaxis

The Centers for Disease Con-trol and Prevention’s Advisory Committee on Immunizat ion Practices published a set of rec-ommendations regarding human rabies prevention. These rec-ommendations state that rabies pre-exposure vaccines should be offered to individualswithhighriskforexposuretorabies,whichincludes veterinarians and their staff,aswellasanimalhandlersand some researchers and labora-torypersonnel.

Most practicing veterinarians in the United States are considered to have a frequent risk for exposure to rabies and should have their titers checked every two years,pertheCDC’srecommendations.

The CDC’s Advisory Commit-tee on Immunization Practices (ACIP) resource indicates thatif a rabies titer has fallenbelowthe minimum acceptable anti-body level, a single pre-exposure booster dose of vaccine is recom-mended for persons at frequent risk, which aremost practicingveterinarians, or continuous risk ofexposuretorabies.

Some question whether education and immunity provisions are keeping pace

Signalingahigh-watermarkineffortstofightanimalabuse,PresidentDonaldTrumptodaysignedalawmakingintentionalcrueltytoanimalsafederalfelony.WhilethePreventing Animal Cruelty and Torture Act does not specify a role or responsibilities for veterinarians in combating cruelty, an increasing number of jurisdictionsdo,reflectinganevolutioninattitudesaboutanimalabusethatischangingexpectationsofveterinarians.

The understanding that animal abuse often occurs in tandem withorasaprecursortocrimesagainstpeople,especiallydo-mesticviolenceandchildabuse,hasledtoanti-crueltylawsineverystate,andtoincreasingfinesandpenaltiesforoffenders.In recognition of the linkbetweencrimesagainstanimalsandcrimes against people, the FBI began in 2016 to track instances ofanimalcrueltyinitsnationalincidentdatabase,alongwitharson,burglary,kidnappingandhomicide.As part of the efforttocurbanimalabuse,18statesrequireveterinarianstoreportsignsofanimalcrueltytolawenforcementorothergov-ernment agencies, according to information compiled by the American Veterinary Medical Association.Mandatoryreport-ing for veterinarians is required in all of Canada’s provinces exceptAlberta.“Oftenveterinariansaretheonlyhumanwitnessestotheeffectsofanimalcruelty,soitiscrucialthattheyareempoweredtoreportandreceivecivilimmunityforreportingingoodfaith,”saidKath-leenWood,astaffattorneywiththeAnimalLegalDefenseFund,whichadvocatesformandatoryveterinaryreportingstatutes.

Amajorityofveterinarianswhorespondedtoasurvey in 2015indicatedsupportforthetrend.LedbyaresearcheratColorado State University College of Veterinary Medicine andBiomedicalSciences,thesurveypolledmembersoftheVeterinaryInformationNetwork,anonlinecommunityfortheprofessionandparentoftheVINNewsService.Of1,209respondents,66%supportedmandatoryreportinglaws.

However,someveterinarianshaveconcerns.Insurveys,onlinemessageboardsandconversationswithVINNews,someprac-titionerssaytheydon’talwaysknowhowtospotabuseorbestpracticesforreporting,andtheyareworriedabouteverythingfrom violating the veterinarian-client-patient relationship to the possibilityofretributionbyangryclients.

Support for concept meets concern about execution

The most recent states to require veterinarians to report signs ofcrueltyareMassachusetts,in2014,andMaryland,in2018.When Massachusetts made reporting mandatory, some vet-erinarians toldVINNewstheyworriedthattherequirementmight prevent people from seeking medical care for their pets, evenincasesofaccidentalinjury.

“Idoubteveniftheydidoffersomeformofprotectionthatitwouldworktoprotecttheveterinariansfromfutureretri-bution,”wroteDr.FrankMarchell,whohasasmallanimalpracticeinMaine.“Wemightassumethatindividualsabusinganimalsmayholdasimilarsetofvaluesforpeople.”

Dr.DonWoodman,whohasacompanionanimalpracticeinFlorida,voicedconcernwhenMassachusettspasseditsreport-ingrequirementandcontinuestobelievethatwhilewell-inten-tioned,suchmandatoryreportinglawsarenotthoughtthrough.

“I one hundred percent believe veterinarians should be empoweredtoreportsuspectedabuse,”WoodmansaidinarecentemailtoVINNews.“BUT...thiswouldinmymindin-cludeshieldlawsprotectingveterinariansfromretaliationbyclients, the development and dissemination of clear diagnostic criteria, and the creation of robust investigatory infrastructures BEFOREveterinariansaretoldthattheyareREQUIREDtoreportsuspectedabuse.”

Woodman became interested in the topic more than a decade agowhenhewastryingtogettheFloridaVeterinaryMedicalAssociationtolobbyforalawthatwouldgiveexplicitper-missionandprotectiontoFloridaveterinarianswhoreportedabuse.Hewasn’tsuccessfulatthetime,buthenotedthataprovisionstipulatingthatveterinarians“may”reportabusewasadoptedinFloridaearlierthisyear.Theseregulationscanactasashieldagainstchargesofviolatingconfidentiality.

Woodmancitestheexampleofphysicians,whoarerequiredtoreportsignsofabuse.Hesaidthathumanmedicinebeganlook-ing at the issue in earnest in the mid-20thcentury.“Diagnosticcriteriaforabuseweredevelopedandthenpractitionersweretrained: ‘When you see this constellation of symptoms, abuse shouldbeonyourdifferentiallist.’Then,asystemofreportingandinvestigationinfrastructurewasputinplace.”

Withoutthesesteps,Woodmansaid,“Ithinkthingswillbeamess.Imagineifstatesstartedrequiringveterinarianstoreportadisease,butveterinarianswerenevertaughtaboutthediseaseandwhatsymptomsshouldmakeonesuspectthedisease.…Somevetswouldignorethemandate,andothervetswouldoverreport.”

Researchers studying the issue of veterinary reporting have found that veterinarians sometimes feel unprepared to iden-tifyabuseand/orknowtherulesandprotocolsfordoingso.

“The diagnosis of abuse, cruelty, nonaccidental injury, neglect, or maltreatment of animals is one of the most chal-lengingsubjectsinclinicalwork,”theauthorsofthe2015surveywrote.“Withouttheappropriateeducationandinformation, veterinarians may be more likely to avoid get-tinginvolvedthaniftheywerebetterprepared.”

Oftheveterinarianswhorespondedtothesurvey,87%said they had seen at least one case of suspected abuse and55.8%reportedit.Ofthosewhodidnotmakea

report,36.8%saiditwasbecausetheywereunsureitwasabuse.Otherleadingreasonswere:

o thinking it better to educate the client than report theabuse(17.8%)

o thinkingitwasaccidentalandnotintentional(9.9%)

o beingunsureoftheprocessforreporting(9%)

Dr.LilaMiller,whoretiredthisyearasvicepresidentofshelter medicine at the American Society for the Prevention of Cruelty to Animals, believes mandatory reporting can help veterinarians.Inapresentationontheveterinarian’sroleinhandlinganimalabuse,sheexplainedthatitcanbedifficultwhenclientstrytonegotiatewithaveterinarian.Withmanda-toryreporting,Millersaid:“Youcanjustsay,‘Look,I’msorry.Ihavetofileareport.Ifyouhaven’tdoneanythingwrong,it’snotaproblem.It’sjustwhatthelawrequiresmetodo.”

She noted that veterinarians are not investigators or prosecu-tors;theydon’thavetomakethefinaldetermination.

Canada moves toward mandatory reporting

InCanada,lawsinnineoftenprovincesrequireveterinar-ianstoreportsuspectedabuse.Albertaistheloneholdoutbutismovinginthatdirection.InFebruary,theAlbertaVet-erinaryMedicalAssociation,whichregulatestheprofessionin that province, passed a resolution adopting mandatory reportingforveterinariansandveterinarytechnologists.ThisresolutionbecamebindingonABVMAmembersonJuly1stbyvirtueofABVMACouncilpolicy.Theassociationwillpropose codifying this amendment in Alberta’s Veterinary ProfessionActGeneralRegulation.

Inadditiontoprovinces,Canadahasthreeterritories.Nonerequireveterinarianstoreportsuspectedabuse.Allprov-incesandterritories,exceptNunavut,explicitlyoffersomesortofimmunitytoanyonewhomakesagood-faithreportofsuspectedabuse.

Despitethewidespreadadoptionofrequiredreportinginthecountry,achieving“thevirtuousgoalofanimalprotection”viamandatedreportingisnotwithoutitschallenges,saidDr.GaryMorgan,registraratthePrinceEdwardIslandVeteri-naryMedicalAssociation,whichregulatestheprofessioninthatprovince.

“People may be reluctant to reach out to a vet for treatment if afraid that they might be violating the Act and be reported bythevet,”hewroteinanemailtoVINNews.“Thereisalsoapotentialconflictofinterestforthevet.Forexample,thepatientwithseveregumdisease,andaclientwhocannotafforddentistry.TheActmayinterpretthatasaviolation,andrequirethevettoreporttheclientwhocannotaffordtheserviceasanimalabuse.”

AninteragencydebateinBritishColumbiaillustratessomeofthechallengesofinterpretingwhattriggersareport.Theprovincial government made reporting suspected abuse mandatory for veterinarians in 2011 in the Prevention of Cru-eltytoAnimals(PCA)Act.Sixyearslater,theCollegeof

VeterinariansofBritishColumbia,aregulatorybody,issuedapositionstatementonthedutytoreport.

“Thepositionstatementwasintendedtoclarifyregistrants’responsibilitieswhenitcomestothePCAAct,asthereseemedtobesomemisunderstandingaboutwhatsortsofthingswouldtriggerthatduty,”CVBCDeputyRegistrarDr.StaceyThomaswroteinanemailtoVINNews.Thomasrecountedthehistory,althoughshewasnotpartofthecollegeatthetime.

Thestatementsaid,inpart,“ReportingaclienttotheBCSPCAshouldbereservedforcircumstanceswherethereisclearand unequivocal evidence of an animal being in distress as a directresultoftheactionsoftheveterinarian’sclient.”

Theprovince’schiefveterinarian,Dr.JanePritchard,tookissuewiththisguidance.WritingintheJuly2017MinistryofAgricul-turenewsletter,PritchardreprovedtheCVBC.“Theemphasisonprotectingclientconfidentialitytodefendnotreportingani-malcrueltyseemstometobelessthanprofessionalwithinthecontext of our oathandtherequirementsofthePCA,”shewrote. Citinghigh-profileactsofextremecrueltyinBritishColumbiaand public outrage, she continued: “The public often ques-tionswhattheroleoftheveterinarianisinthesecircumstances,andifwedonotspeakup,takeaninterest,askquestionsandbecomeengagedinthisarea,Ifearweasveterinarianswillbeseenasirrelevantinprotectinganimalwelfare.”

TheCVBCstatementwasrevisedafewmonthslater.Itnolongermakesreferenceto“unequivocalevidence.”Theup-dated statement says in part, “Without clear evidence of current or imminent animal distress requiring a PCAA report, itisbesttoproceedwithreasonandcaution.”Italsocallsreportspotentially“devastatingtoallinvolved.”

Despitereservationswithintheprofession,momentumisonthesideofmandatoryreportinginCanada.ThenextactionmaycomeinYukon,thecountry›ssmallestterritory.

Yukon does not have a regulatory body for the dozen vet-erinarianswhopracticeintheterritory,andtheprovince’sAnimal Protection Act has no rules relating to a veterinarians’ dutytoreport.However,thegovernmentisintheprocessofupdatingitsanimalprotectionandcontrollaws,accordingtoYukonChiefVeterinarianDr.MaryVanderkop.

To that end, Yukon’s Animal Health Unit conducted a survey in2018.Accordingtoresults, Yukoners like the idea of requir-ingveterinarianstoreportsuspectedabuseofanimals.

“Thiswassupportedby82%ofrespondents,”thesurveysum-marysaid.“Itwasalsoclearthatpeoplewantedveterinarianstobeprotectedfromliabilityorotherbacklashwhenmakingthesereportsingoodfaith.”

Dr. Dustin R. Schmid grew up in Rancho Palos Verdes, a small town in Southern California. He received his Bachelor of Science degree in Animal Sciences from the University of Colorado - Boulder. He earned his Doctor of Veterinary Medicine degree from Kansas State University.

He subsequently completed a rotating internship and residency in emergency and critical care in Southern California. Dr. Schmid is board-certified in small animal emergency and critical care by the American College of Veterinary Emergency and Critical Care. He has published multiple scientific papers in peer-reviewed journals and has co-authored a chapter in a critical care textbook. His professional interests are treating shock, sepsis, and mechanical ventilation.

Outside of the hospital, Dr. Schmid remains active by playing soccer, hockey, golf, and snowboarding/skiing. He is an avid traveler and has visited over 40 countries on 6 continents. Dr. Schmid has 2 cats, Yota (named after he found him in the engine of his Toyota) and Buffalo (after the Colorado Buffaloes).

Not So Sweet - Feline Diabetes Mellitus: Treatment with Concurrent Disease (Rucinsky)Did you make that cat diabetic? We’ll discuss ways to manage inflammatory bowel disease, al-lergies, and other ailments along with diabetes.

Canine Diabetes Mellitus-Dogs are Not Big Cats (Lathan)We’ll review insulin and dietary therapy in dogs during this lecture. We’ll also focus on diabetic monitoring, including clinical signs and continuous glucose monitoring.

Diabetic Ketoacidosis: How to Stop the Ketones (Lathan)In this session, we will study the diagnosis of DKA and also cover the most commonly used treatment protocols, and some newer protocols that also enlist the use of sub-cutaneous insulin.

We’ll talk about early detection, pitfalls in testing, and various treatment options for hyperthyroid cats.

Canine Addison’s Disease-A Case Based Update (Lathan)Cases will be used to illustrate how to treat Addisonian dogs with steroids, and also different presenta-tions of Addison’s disease. Dogs with both typical and atypical hypoadrenocorticism will be covered. Canine Cushing’s Disease-A Case Based Update (Lathan) This lecture will use cases to illustrate practical aspects of the diagnosis and treatment of hyperadrenocorti-cism. We’ll focus on treatment using trilostane, in-cluding the use of the pre-pill cortisol for monitoring.

Deborah Harvazinski, DVM, served on the SDCVMA Board from 2002 - 2019. Dr. Harvazinski supported Independent Contract work in providing correct documentation for relief doctors, as well as practice owners, to prevail in the event of an audit. She was Board President during years 2004 and 2007. She is one of nine Presidents that served a second term as President due to an unexpected vacancy or need. Her second term as President, she was instrumental in microchip reader/scanner standardization. She notified the community that not all shelters or veterinarians had scanners to detect all chip types, and put the company providers on notice to stop selling those chips until readers/scanners became universal.

Keith Hilinski, DVM, served on the SDCVMA Board from 2000 - 2019. He championed the need for colleague mentorship and spearheaded the first meetings to provide insight, guidance and support. His year as President called for a focus on unfair competition in the private sector. His letter to the US Ways and Means Committee remains in the Federal Register, requesting greater guidelines and definitions for charitable organizations’ tax exempt activities. Dr. Hilinski was Board President in 2005. He served most Board positions, as well as serving as CVMA Delegate and CVMA District I Governor.

Both great advocates for organized veterinary medicine. SDCVMA thanks you for your dedication and many years of service, collectively 36 years!
