PSM Training Standard

1. PURPOSE

This standard defines Process Safety Management (PSM) training programmes and their scope, as well as the responsibilities of Training and Development Department and other departments concerned with this training.

2. SCOPE

PSM training is an element in the Company's PSM system and the training conducted shall comply with the PSM training requirement, which includes training in operating procedures, safe working practice, emergency evacuation and response, safety procedures, work authorisation and other topics relevant to process safety and health.

3. DEFINITIONS

3.1. New Employee

A newly recruited employee, who has successfully completed the departmental and Personnel interviews, selection tests (where applicable) and medical test and is employed to fill a position within the Company.

3.2 Fire & Safety Induction

A brief introduction to Bapco's safety and emergency procedures, fire extinguishers and their application. It also include basic practical training on the use of fire extinguishers.

3.3 Safety Orientation Training

Training on all aspects of safety as per the Safety Standard SS/370/SAF/001/SH.

3.4 Process Overview & Process Hazards

An overview of refining processes and associated hazards.

3.5 Hazards Communications (HAZCOM) Training

Training on materials safety data sheets which contain all the information required to handle such materials in a safe manner.

3.6 Safe Work Practice

Training on the Permit to Work System as stipulated by the Permit to Work Standard SS/370/ PSM/408/SH.

3. DEFINITIONS (Contd.)

3.7 Induction Programme for New Employees

A formal introduction to the Company, organised by Personnel Department for all newly recruited employees, to provide them with information concerning the Company's operations, policies, rules and regulations, employee benefits, health, fire and safety training (see QS/520/000/002/TR).

3.8 Operators Familiarisation

Refinery familiarisation training for new employees in Refining Division. It covers refinery terminology, Company processes and industrial safety procedures.

3.9 Initial Training

The first stage of training required for new employees in Maintenance and Refining Divisions. It includes health, fire and safety training as appropriate.

3.10 Job Specific Training

Job related training, which consists of on- the- job and off- the- job training for both job competence training and for training changes by Management of Change (MOC).

3.11 Evaluation

A regular assessment carried out by the trainer in order to determine the standard of trainee performance during the training period.

3.12 Job Competence Assessment

An official record of achievement to evaluate the trainee's success in order to qualify him/her to assume his/her target position.

3.13 Training Needs Analysis

A process conducted on an annual basis by Training & Development Department and concerned departments to review employees' performance and identify training needs.

3.14 Refresher Training

A training activity in which employees reinforce or update their knowledge and/or skill and learn about new developments. Such refresher training shall be provided, normally at three yearly intervals, to meet identified needs and satisfy the PSM standard.

4. RESPONSIBILITIES

4.1 Line Departments

Managers of line departments concerned or their delegates, shall have the overall responsibility for identifying all aspects of training required to satisfy the PSM training requirement.

4.2 Training & Development Department

Manager Training and Development Department (T&DD) or his delegate shall be responsible for ensuring that all PSM training requirements are identified, training provided and that the centralised training record is kept updated.

4.3 Fire & Safety Department.

Manager Fire and Safety Department or his delegate shall be responsible for ensuring that Safe Work Practice training and other fire and safety related training required by PSM for Company and contractor employees are addressed, that records of training completed are passed to T&DD for entering into the electronic database (COMPERE) and that the PSM standard is regularly audited.

4.4 Personnel Department.

Refer to QS 520/000/002/TR concerning induction course for new employees.

6. PROCEDURE

General All departments including contractors affected by this PSM training standard shall

ensure that : i) employees who require PSM training are identified ii) training programmes for such employees are established iii) measurable aims and objectives for such training are decided

iv) all employees attend training courses required under this standard v) T&D and F&S departments are informed of their employees' PSM training

requirements vi) employees are made available to attend refresher PSM courses as appropriate vii) on the job assessments and tests are conducted for their trainees as required viii) T&DD is informed of training completion and employee training files are

maintained.

6.1 Training Programmes

6.1.1 Initial Certification of Competence

Incumbents of line positions will be deemed competent for the purpose of this standard by virtue of their experience in the position, and previous training.

6.1.2 Induction for new employees

All newly recruited Bapco employees shall attend an induction course as specified by Company Standard QS/520/000/002/TR (see Clause 5.ix).

6.13 Familiarisation for new operators

All new employees recruited to work for Oil Processing, Power and Utilities and Oil Storage and Export Departments shall attend familiarisation training. This familiarisation training shall be conducted after the induction course for new employees and shall provide a general overview of the Company's operations and aspects of industrial safety relevant to Company operations. This is a two week course and achievement shall be measured by an end-of-course written test. Upon completion of familiarisation training, trainees shall be allowed to work on process units under supervision.

6.1.4. Initial Training

a) Oil Processing, Power & Utilities and Oil Storage & Export Departments On completion of the familiarisation course for new operators, all new trainees in the

above mentioned departments shall attend initial training within six months of their employment. This shall include:

i) Basic course This programme is of six weeks duration. Achievement shall be measured by an end-of-

course written test and project work. The course shall be designed to give employees the necessary in-depth knowledge required to work in the said departments. (Refer to OB 521 course contents manual with T&DD.)

6. PROCEDURE (Contd.) ii) Job Specific training The said departments shall prepare and implement on the job training programmes for

their new employees . This programme shall be based on the requirements of the job. During the training period, the said departments shall arrange for trainees to attend specific modular training appropriate for their jobs. Such training shall be recorded and submitted to T&DD for entering into the electronic training record database (COMPERE).

b) Plant Maintenance and OS&E Departments On completion of the induction course, all new technician trainees in Plant Maintenance

Department and Oil Storage and Export Department (Marine Engineering Section) shall attend initial training which shall include:

i) Basic hand skills This course shall be from two to six weeks' duration, depending on the experience of the

new employee, and shall be designed to give employees general theoretical and practical knowledge about all the hand tools involved in their work and how to handle these tools in a safe and correct manner.

ii) Specialised Training Specialised training shall include theoretical and practical training in different

disciplines, according to the employee's job in Welding, Metal Trade, Mechanical, Electrical, Instruments and Electronics fields.

6.1.5 Evaluation and Job Competence Assessment

a) OPD, P&U and OS&E departments shall evaluate their trainees' training performance on a monthly basis using Bapco forms specified by appropriate QA documents. Refer to clause 6.1.7.b. Training completion forms shall be completed by the departments concerned and forwarded to T&DD for entering into the electronic record database (COMPERE). Refer to QA document GW 580/000/010/TR for details of training record.

b) Plant Maintenance and Shutdowns & Major Maintenance Departments shall evaluate their trainees' training performance on a monthly basis. On completion of the training programme, the record shall be maintained on employee training record files or Maintenance Division files as appropriate. Training completion forms shall be completed by the departments concerned and forwarded to T&DD for entering into the electronic training record database (COMPERE).

c) Shutdowns and Major Maintenance and Plant Engineering Departments shall evaluate and qualify contractors' employees who will work for Bapco through safety and trade testing. Qualified contractors' employees shall be certified using BP3884M Contractor Trade Certificate.

6.1.6 Refresher training

All personnel involved in operating process units shall attend refresher training at regular intervals, as appropriate. The requirement for such training shall be determined by the line departments concerned.

6.2 Training Need Analysis and Identification

All departments covered by this standard shall, in conjunction with T&DD : conduct an annual Training Needs Analysis (TNA) to identify training needs for the

employees, develop training programmes for those employees with training needs, ensure that employees receive training in accordance with training plans, monitor and record all training activities in employees' training record files.

6.3 Training Documentation and Access

6.3.1 Line departments

Departments concerned shall maintain and update each employee's training record as per appendix (4).

6.3.2 Training and Development Department

a) T&DD shall maintain and update trainees' training files pertaining to OPD, OS&E and P&U. Such files shall include :

Initial training Basic training Specific training On-the-job training records. b) T&DD shall utilise the following forms for updating the electronic training record

database (COMPERE). Departments concerned shall enter a copy of these forms into employees' training record files.

i) BP4001A/B Attendance Report ii) BP3931 Trainee Qualifying Test Evaluation iii) BP4004 I.T. Course Report iv) BP3334 English Language Training Report

6.3.3 Fire and Safety

F&S shall maintain records for training conducted for Safe Work Practice and Permit Authorisation as per F&S standards SS/370/SAF/001/SH.

6.4 Audits

This standard shall be audited by F&S department every three years according to compliance audit standard No.SS/370/PSM/412/SH.
