that she came in contact with poison ivy. Mary has late-onset Alzheimer’s
and therefore her history was provided by her husband. She was wearing
gloves while gardening. The raised maculopapular rash has a linear pattern
on her forearms. The rash is consistent with poison ivy.
VITALS: BP 110/6; Weight: 103; BMI 24.5; PO2 94; Respiration rate: 22.
ASSESSMENT: Poison ivy. PLAN: Solu-Medrol 125 mg 1M X1. Over the
counter Benadryl. Gauze dressing to prevent Mary from scratching. Long
sleeves suggested. MA will dress wound and provide gauze for 1 home
change.
