ABOUT CENTURA HEALTH About Our Sponsors In 1996, Centura Health was formed through an Affiliation Agreement and now functions under the sponsors, Catholic Health Initiatives and Adventist Health System. Since their earliest days, Adventists have been active in establishing hospitals that place an emphasis on whole person care and on illness prevention. Catholics are well known for their tradition of caring for the poor. Both sponsors have preserved an unwavering commitment to their values and an understanding of the vital role that ministering to spiritual needs can play in health care. Centura Health, the region’s health care leader, is focused on connecting Colorado and Kansas to affordable, world-class care. More than 15,800 of the best hearts and minds in medicine, along with more than 6,000 physician partners, have access to the most technologically advanced tools available. As a mission-driven, faith-based nonprofit organization, what distinguishes us more than size is our daily dedication to nurturing the health of the communities we serve. Centura Health is a nonprofit organization, therefore has no shareholders to whom returns are distributed; all profits are reinvested in our mission and communities. Our associates treat and touch the whole person, body, mind and spirit, and reach beyond hospital walls to connect with people at every stage of life. ABOUT CENTURA HEALTH

About Our SponsorsIn 1996, Centura Health was formed through an Affiliation Agreement and now functions under the sponsors, Catholic Health Initiatives and Adventist Health System.

Centura Health 2020 Centura Health 2020 is an integrated system-wide strategic plan that has continually evolved since 2009. It is a dynamic, living plan, reflecting the transformation of the organization to prepare for a health care environment that will be very different from the one we have experienced since the formation of our organization. Centura Health 2020 is rooted in our mission and vision and based upon three simple, yet powerful principles:• Create Systems of Care: Transcend the walls of our hospitals to create true systems of care that

connect our facilities together in a seamless network of excellence that delivers services across the full continuum of need.

• Move Upstream to Manage Health: Expand the scope of our services beyond traditional “sick care” to truly manage health in accordance with our health model.

• Strengthen the Foundation: Our foundation of facilities, organizations and highly committed, mission-centered professionals must continually find ways to improve. A strong foundation will fuel our transformation, allowing us to build systems of care and move upstream.

2014-2016 Strategic PrioritiesThrough the first four years of Centura Health 2020, our focus has been on creating systemness. Over the next three years we will focus on optimizing value, which requires us to both get better at our traditional business model – running hospitals, physician practices, senior living facilities and providing home care – while we get different by developing the tools and services needed to deliver accountability and provide population health. The three tenets of Centura Health 2020 – creating systems of care, moving upstream to manage health and strengthening our foundation – continue to guide our pursuit to deliver value.
