Personal Goals Record Sheet
It is important to have future goals. Your goals could be to narrow your career focus, to find a school that will prepare you for the future, or to register for the appropriate test. Whatever your goals, the following activity may help transform your goals into reality by clarifying and ranking them.
Name:_______________________________English Teacher:______________________
First it is important to know what a goal must be.
A GOAL must be SMART:
Specific rather than general (I want to pass all of my classes with letter grades of A or B.)
Measurable- by the types of classes taken, or grades, or attendance.
Achievable- for your grade level in high school. You must have the interest, the skills and abilities, strengths to make this goal happen.
Rewarding- positive rather than negative.
Time-oriented - able to be finished by the end of the school year or in a reasonable time frame that has been given.
List three goals in each area. Then, rank them in order of importance.
MY SCHOOL GOALS:
________________________________________________________ rank: __________
________________________________________________________ rank: __________
________________________________________________________ rank: __________
MY FUTURE EDUCATIONAL GOALS:
________________________________________________________ rank: __________
________________________________________________________ rank: __________
________________________________________________________ rank: __________
MY CAREER GOALS:
________________________________________________________ rank: __________
________________________________________________________ rank: __________
________________________________________________________ rank: __________
MY PERSONAL GROWTH GOALS:
________________________________________________________ rank: __________
________________________________________________________ rank: __________
________________________________________________________ rank: __________
MY LONG RANGE GOALS:
________________________________________________________ rank: __________
________________________________________________________ rank: __________
________________________________________________________ rank: __________
Now choose one of the #1 ranked goals you have identified. Identify three actions you can take that will help you reach your goal. ACTION 1: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________ ACTION 2: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________ ACTION 3: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________
