• Complete the Chapter Leadership Form• Have the 2013 president sign:
– Warranty Statement – Conformance to Bylaws – 2012 Chapter Affiliation Agreement Review– Brand Protection– Canadian Chapters – Contract for Services
• Complete the online chapter activity report • Collect 2011 financial summary and program info
• Verify submission of 990 or T2 (through chapter or IHQ)

• Send all materials together with a copy of the checklist

• Receive confirmation!
Deadlines• Monday, December 17 – Chapter
Leadership Form due
• January 15 – All other materials due at IHQ
• All materials must be RECEIVED at IHQ by close of business on these days
• Required to receive the 2013 Ten Star Award (awarded in 2014)
