St. Francis of Assisi Catholic School Mission Statement

St. Francis of Assisi Catholic School was founded in 1954 as a mission of St. Francis of Assisi Catholic Church. The school is a beacon of hope in Pike County educating children pre-school through sixth grade. Children are welcome to enroll regardless of race, religion or ethnicity. St. Francis Catholic School is committed to gospel values and academic excellence. We strive to impart to all students an inner self-discipline and sense of social responsibility which extends beyond self to family, church and community.

“Be it known to all who enter that Christ is the reason for this school.”

Goals flowing from the mission statementTo teach and model gospel values. To provide a strong curriculum by implementing the latest educational trends though on-going professional development. To provide for a diverse curriculum that meets the needs of students with various learning styles. To provide a family oriented environment that provides the support needed for each child to succeed.

PhilosophyProclaiming the Gospel is a perennial task and joy for the Church of Jesus Christ. Catholic Education is an expression of this mission entrusted by Jesus to the church he founded. Though education, the church seeks to prepare its members to proclaim the good news and to translate this proclamation into action. (To teach like Jesus did).

St. Francis of Assisi Catholic School, located in the Diocese of Lexington is a co-educational facility which recognizes the diversity of the many world cultures. We seek to create an atmosphere of Christian awareness within a program of academic enrichment. Realizing that parents carry the basic responsibility to educate their children, we endeavor to supplement their intellectual, emotional, social and physical growth. Within the school, the program strives to recognize that this can be accomplished only to the extent that meeting these needs is within the program capabilities of the school.

Being aware that faith in Christian values gives meaning to life, our staff aims to present the Christian message though living out gospel values. Faculty and students endeavor to become a community of faith founded on worship of God and intellectual growth. Recognizing our personal giftedness, in the light of world needs, we wish not only to grow in academic learning, but by awareness and empathy to touch the world through community service.

Perceiving that living a moral life leads to peace within families, the greater community and the world, we endeavor to form in the children a sense of morality. Our moral formation is based on the covenant at Mount Sinai, the Sermon on the Mount, and the teachings of the Catholic Church. Realizing that all things are possible though prayer, we provide times, places, and various forms of prayer for our students.

Admission Policies St. Francis of Assisi Catholic School will admit students of any race, sex, religion, nationality or ethnic origin to all rights, privileges, programs and activities. St. Francis of Assisi Catholic School will not discriminate on the basis of sex, race, color, nationality or ethnic origin in the administration of its educational policies, admission policies, scholarships, loans, athletic programs or any other school administered program. The parent or legal guardian must register all students and provide the necessary forms/documentation.

All new students enrolled at St. Francis of Assisi Catholic School will be on probation in order to determine if the school can meet the student’s educational needs. Reasonable accommodations will be made to assist each student to ensure academic success, but finances may prevent St. Francis of Assisi Catholic School from providing special services that some students may require. Many public school special education programs are required by law and funded only in public schools. If for any reason St. Francis of Assisi Catholic School cannot provide the needed instruction or if the student does not adhere to school policies, the school administrator in consultation with the Diocese of Lexington will notify the parent or guardian that the student must immediately withdraw. Any student enrolling at St. Francis of Assisi Catholic School who is transferring from a non-accredited school or a home school may be required to take tests to determine where the student should be placed or to determine class credit.

In accordance with Kentucky state law, children reaching the age of five on or before October 31st of the current school year shall be eligible to enroll in kindergarten. Any student, who is six on or before October 1st, must be enrolled in school according to KY law. Kentucky School Law requires successful completion of kindergarten before entering first grade. Proof of attendance and a report card/progress report from a certified kindergarten is necessary when enrolling a student in first grade. Students enrolling in first grade must be at least six years old on or before October 1st of the current school year.

It is the policy of St. Francis of Assisi Catholic School to accept students on the following basis:

A parent is a participating member of St. Francis of Assisi Catholic Church A parent is employed at St. Francis of Assisi Catholic School A sibling is presently enrolled in St. Francis of Assisi Catholic School A parent is a graduate of St. Francis of Assisi Catholic School

Other interested families are accepted on the basis of: Space availability Date of registration

Parents must submit the following documents prior to the beginning of the school year when enrolling for the first time and at certain grades where documentation is required:

1) State Law requires a birth certificate and a Social Security card be presented for any student entering a new school. If a student attended St. Francis Preschool this was required at that time.2) All schools shall require a medical examination of each child within six (6) months prior to his or her initial admission to school. (Note:  Under the “initial admission” clause, a student who is

enrolled as a preschool student on the same campus is not required to have a second physical for entry into kindergarten). An official KY form is required. Health records can be obtained if a student is transferring from another KY school. 3) State Law requires the school to have an up-to-date immunization record on file for every student and this must be on the official KY form that has an upcoming expiration date.4) State Law requires the school to have a vision and dental screening record on file for every new kindergarten student entering St. Francis of Assisi Catholic School for the first time as well as entering preschool.5) A Baptismal Certificate is required for every Catholic student; the school can request these from the parish.

Transfer PoliciesA student who wishes to enroll and transfer from another school at a time other than semester break or a new school year shall do the following:

a) parent/guardian and student will meet with the school administrator requesting admissionb) pay the required book/supply fee at the time of registrationc) begin paying tuition, based on the enrollment day, with the first month’s tuition due in 2 weeksd) begin following the dress code on Monday after the first weekend the student is enrollede) sign a records release form which will enable St. Francis of Assisi Catholic School the authority to request recordsf) request records to transfer to St. Francis of Assisi Catholic School

Parents/guardians should be aware that the school administrator will request disciplinary information as well as academic records from the student’s present school. The school administrator may require a student to wait until the end of the current semester or year before enrolling a transfer student. This transfer student is enrolled on a 6 week probationary period. During this probationary period, the parent or guardian will be notified if there are problems or concerns about conduct, grades, or other relevant information that could lead to the student being required to withdraw. The school may inform the parent or guardian that the student must withdraw from St. Francis of Assisi Catholic School at the end of the probationary period and on tuition or book /instructional fee is refundable.

Students transferring from another KY school will have most of these required documents already on file with a school. Parents/guardians must sign a release and all records will be requested from the previous school. Students entering from an out of state school will be required to have a recent physical and an up-to-date immunization form, as though they were entering school for the first time. Copies of all of these permanent school records are sent to other schools upon written request with parental signature. The file containing discipline matters is separate and kept by the School Administrator and is not a part of the student’s permanent cumulative record. Parents wishing to view their student’s cumulative folder must make a written request to see the records and these can be viewed only in the presence of the school administrator or designee. Copies of these records will be made for the parent upon written request and will be available within 10 working days of the request.

School HoursClass begins: 8:00 A.M.Students may arrive at 7:45 A.M.Class dismisses: 3:20 P.M.

Students are to be dropped off at the entrance of the school. Parents are to follow the flow of traffic: enter on the right, circle the parking lot and exit to the left. Parents of Preschool students may park in the spaces facing the school and walk their children into the school. At dismissal parents may park in front of the school. Children are instructed to walk around to their cars and not cross the parking lot. This is for their safety.

At the beginning of the school year, a form must be filed with the office stating who is allowed to pick up your child. If your child’s usual means of transportation is changed, and someone other than listed, is to pick up your child, please write a note to the teacher, giving the name and description and relationship to the child.

Student SupervisionStudents cannot be left on school property without adult supervision. Parents or guardians dropping a student off at school or for a school activity should be sure that the adult who is supervising is present and ready to supervise. Students coming to practice, meeting to leave on a trip, or attending any other school activity should arrive not more than 5-10 minutes before the scheduled time. Adult supervision may not be available when students arrive too early. Parents or guardians must pick up students on time.

Adult Visitors and Sign-InParental involvement and community support is very much appreciated at St. Francis of Assisi Catholic School. Adult visitors are welcome, but there are some necessary restrictions because of safety and the class structure of the school.

Visitors (including parents) must immediately come to the school office when they enter St. Francis of Assisi Catholic School and sign in.

When visitors leave, they are to return to the school office and sign out. Visitors are not permitted to interrupt classes and are to schedule all teacher

conferences in advance. Visitors must enter the building through the main front school doors. Parents observing in classrooms must be approved in advance with the School

Administrator and the classroom teacher. Parents and guests visiting in the cafeteria during lunch must sign in and out at the

school office.

Attendance and AbsencesAt St. Francis of Assisi Catholic School, we work in partnership with parents to enable each child to develop as an independent learner. Parents should allow their child(ren) to enter the school building on their own each morning. The attendance laws of KY are strict and require 177 school days or equivalent minutes for students to be in class. The purpose is to assure each student has a sufficient number of days of instruction to meet the requirements for the class. Therefore, St. Francis of Assisi Catholic School encourages every student to be in school every day.

The academic progress of each student does depend upon regular class attendance. Attendance is both a student and parent or guardian responsibility. If a student misses two (2) consecutive hours, the student will be counted absent one-half day.

The official school day is 8:00 AM to 3:20 PM. A parent/guardian should call the school to report that the student is absent or may send a written note concerning a planned upcoming absence. It is the responsibility of the student and/or parent/guardian to contact the teacher concerning assignments the student needs to make up and a time line for completion.

The following are considered excused absences and tardies and requires a parent to notify the school that the student will be absent and the reason why:

illness of the student critical illness or death of a family member authorized school activities appearance in court medical appointments (doctor’s statement required) pre-approved educational trips (approved by the administrator) other absences will be handled individually between the parent and administrator in

advance to determine whether the absence is excused

Students will be allowed 5 unexcused absences for the school year without a doctor’s excuse. A student who has been to the doctor, dentist, or health department must present a medical statement to the school within 3 days of the absence. This applies to absences, tardies, or early dismissals. Without a medical statement these tardies and early dismissals are unexcused. Please make doctor and other appointments after school hours whenever possible.

10

Page 11: Accreditation Certificate · Web viewProclaiming the Gospel is a perennial task and joy for the Church of Jesus Christ. Catholic Education is an expression of this mission entrusted

Long term illnesses, such as flu, chicken pox, strep throat, mono, staph infection, severe personal accident, will need a doctor’s statement to return to school. The actual number of days missed will be recorded on the report card and permanent record. Students with head lice will require proper treatment that day, students can return to school when they are nit free.

All student illnesses should be reported to the school office by phone or written note by 9:00 a.m. If the office does not receive a call, a parent will be contacted. This policy is for the protection of the St. Francis of Assisi Catholic School students. This is necessary to protect the health of all students, staff, and volunteers.

When a student has been absent from school for three consecutive days due to an illness, a doctor’s statement will be required before returning to school. Students should be free from fever (less than 100 degrees), vomiting and/or diarrhea for a period of 24 hours before returning to school. (Example: If a student leaves school at noon with a fever, they will not be allowed to return to school until noon the following school day.) This policy will assist in keeping all students healthy.

Missed AssignmentsMissed assignments are the student’s responsibility. Please discuss missed assignments/work with the student’s teacher for details and deadlines.

Tardies and Early DismissalsIt is very important that students arrive to school on time. Being on time for school and class is the responsibility of both the student and the parent or guardian. The school day begins promptly at 8:00 am, students arriving later than 8:05 am, will be marked tardy. Parents or guardians should come into the office to sign students in when they are late. Morning tardies count toward attendance. The school day officially ends at 3:20 pm., students leaving before 3:20 pm must be signed out by a parent/guardian.

Uniform PolicySt. Francis of Assisi Catholic School uniform policy is easy to follow. This policy has been established to promote an academic environment that is conducive to teaching and learning. Catholic Schools Office Grievance ProcedureThe following procedures have been established to facilitate reconciliation and to maintain unity within each school and within the Diocese as a whole.

So far as possible, all given grievances should be settled “as close to the problem” as possible. Therefore, the first step of every grievance process requires that the aggrieved person (the “petitioner”) and only the petitioner speak directly and privately with the person with whom he/she has the grievance (“the respondent”). The grievance must be made within ten (10) days of the incident.

The two should make every effort to come to an agreement. In the event that the petitioner decides to continue the grievance the petitioner should next speak to the person to whom the respondent is directly accountable. Again, every effort should be made to resolve the problem.

The petitioner may, if necessary, continue the process by petitioning help up the authority line, one step at a time, until the final authority on the local level is reached. Every effort should be made to resolve the problem immediately. No step in the process may take more than ten (10) working days. Examples of the proper order in parishes are:

Parent Teacher Pastor/Principal

If all attempts to solve the problem at the parish or local level fail the petitioner may appeal to the Catholic Schools Office. In order to make an appeal to the Catholic Schools Office, the petitioner must prepare a written petition. The petition shall include:

Name, address and telephone number of petitioner Name, address and telephone number of respondent Description of complaint Desired remedy Date letter is sent Signature

16

Page 17: Accreditation Certificate · Web viewProclaiming the Gospel is a perennial task and joy for the Church of Jesus Christ. Catholic Education is an expression of this mission entrusted

The Superintendent of Schools shall review the petition, and decide whether the issue falls within jurisdiction of the department. If so, the Superintendent of Schools will talk with both parties, come to a decision, and shall notify the petitioner in writing of the decision within fifteen (15) working days.

A decision of the Superintendent of Schools, if the case does not involve termination or non-renewal of contract may be appealed to the Bishop through the Diocesan Conciliation/Arbitration Process. A decision, which does involve termination or non-renewal of contract, may be appealed to the Secretary of Pastoral Life. The Secretary will notify the petitioner in writing of the decision within fifteen (15) working days.

Medical Treatment PolicyAt the beginning of the school term, each family will fill out an emergency form. This form contains information for use by the school in case of an emergency with your child. Please immediately inform us of any change as they occur.

If you will be out of town for a day/week, please give the school the name and phone number of the person in charge and how long this person will be responsible for your child.

First Aid Parents must inform the school of any medical problems or conditions their child(ren) may have. Physical exams and immunizations as required by state law are required of all children entering preschool, kindergarten and sixth grade.

When possible parents or designated persons should come to school to administer medication to their child(ren), if this is not feasible medications are self-administered by the student under the supervision of school personnel. All medication must be sent to the office with written directions and in the original container. It is against the law to give any medication including aspirin that has not been sent from the home with written instructions. In the event of a medical emergency, medical services will be called and the recommendation of the medical team will be followed. Parents will be called immediately.

MedicationsAny medication that a student is to take during school hours must be brought to school office in the original container by parent or guardian.  Any medication dispensed (prescriptions or over the counter (OTC), must have completed a parent/guardian St. Francis of Assisi Catholic School Medication Form giving the school the responsibility of administering the medication. The school cannot accept a verbal request for administration of any medication.

Medication must be for the named student, not be passed expiration date, and have complete instruction for administration (dosage, time, name of medication, and route).  The dispensing of medication will be documented and kept on file. Please send prescription medication for one to two weeks and the medication bottle will be returned with student or can be picked up for refills as needed.

Any OTC medication (Tylenol, Advil, Ibuprofen, Midol, etc) will be dispensed per label guidelines. A note will accompany the student home with the time and date they received any OTC medication. Due to the danger of Rye's syndrome, school personal will not administer aspirin unless a physician’s written prescription is received. Students should never carry their own medication without approval of the principal and this is done only at the request

of a parent/guardian, and in cases where the medication cannot pose a danger to any other student. Students will not be allowed to take the first dose of any medication at school due to the risk of allergic reaction; first dose should be administered at home. Teachers may dispense cough drops to students in the classroom.

Parents/guardians who have students that need to carry their rescue inhaler with them should see the school office for required forms to be completed by the physician.  The form can be completed by parent/guardian and faxed by school personal to the named physician. The student will only be allowed to carry inhaler on self with physician approval. At that time the student will be responsible for his/her own rescue inhaler.

DisciplineDiscipline is controlled behavior designed to develop within an individual, responsibility for his/her own actions in accordance with Christian values and socially accepted conduct. It’s ultimate objective is the development of a mature individual capable of self-control and direction.

Discipline applies to the rules and regulations of the Christian life. We trust that parents and students realize that all rules and regulations set by the school are made with good reason. Without discipline, the school would not accomplish its purpose nor operate effectively. Therefore, students who wish to attend St. Francis of Assisi Catholic School, are expected to follow the rules and regulations of the school.

If a student is incapable of self-control and persists in disturbing others, he/she must be responsible for the consequences. In serious matters, the parents will be contacted for conferences. These Christian principles include, but are not limited to the following: Parents/guardians are expected to work courteously and cooperatively with the school

to assist students in meeting the academic, moral and behavioral expectations of the school.

Always mindful that we hold to Christian principles and serve as an example to our children, it is necessary that parents/guardians respectfully express their concerns about the school operation and its personnel; however, they may not do so in a manner that is discourteous, scandalous, rumor driven, disruptive, threatening, hostile, or divisive.

Parents/guardians are expected to use the established chain of command and the proper setting for expressing their concerns.

Christian conduct of parents/guardians is expected at, but not limited to, all school-sponsored programs and activities (PTO meetings, fundraising events, field trips, etc.).

The school reserves the right to determine, in its discretion, which actions fall short of meeting the Christian principles of the school. Failure to follow these principles will result in a verbal or written warning to the parent/guardian and will first result in suspension of parent/guardian's privilege to come on the campus grounds and/or participate in parish/school activities for a determined period of time. The school reserves the right to determine, in its discretion, when conduct is of such a severe nature as to warrant immediate action without a warning including expulsion of the family from the school.

Parent Teacher Organization (PTO) All parents/guardians of St. Francis of Assisi Catholic School and teachers are invited and encouraged to be actively involved in the Parent Teacher Organization.

The five major activities of Parent Teacher Organizations are to: Promote communication Provide parents and teachers with information Promote good will and cooperation Direct and coordinate parental support for the school And Organize political action of parents as advocates on local, state, and federal

legislation that affects Catholic schools.

All PTO activities must be discussed and approved by the school administrator.

Field TripsStudents must have a written permission form signed by a parent/guardian and turned in to the teacher before attending a field trip. Any student not having a signed field trip form will not be allowed to participate in the trip and will be required to stay at school. The school will only accept the official field trip permission form. Field trip permission forms will be furnished by the school.

The field trip form can be faxed, emailed or hand delivered to the school but telephone calls cannot take the place of the written form. The Diocese of Lexington and other groups also require a special field trip form for overnight trips. The permission form releases the school from liability. Parents/guardians may refuse to allow their student to attend a field trip and should submit their refusal in writing.

Acceptable Use PolicySt. Francis of Assisi Catholic Schools pleased to offer students access to a computer for use of the Internet. This access will enable students to explore thousands of libraries, databases, museums, and other repositories of information, and to exchange communication with other Internet users around the world. Families should be aware that some materials accessible via the Internet may contain items that are illegal, defamatory, and inaccurate, or potentially offensive.

While the purpose of the school is to use Internet resources for constructive educational goals under close supervision, students may find ways to access unacceptable material. We believe that the benefits to students in the form of information resources and opportunities for collaboration outweigh the disadvantages. Parents and guardians of minors, as well as the school, are responsible for setting and conveying the standards their children should follow when using the media. We support and respect each family’s right to decide whether or not to apply for access. To gain this access, all students must obtain parental permission as verified by the signatures on the attached form. Should a parent prefer a student not to have email and Internet access, use of the computers is still possible for other purposes such as word processing.

Students are responsible for good behavior on school computers just as they are in a classroom or hallway. As communications on the Internet are often public in nature, general school rules for behavior and communications apply. Access to the Internet is given to students to conduct research and communicate with others. It is given to students who agree to act in a considerate and responsible manner. Access is a privilege, not a right. It may be revoked if abused. Students are advised never to access, keep, or send anything that they would not want their parents or teachers to see.
