SUMMARY

The Maritime and Coastguard Agency (MCA) exercises Central Government's statutory

responsibilities for taking action when oil or hazardous and noxious substances

emanating from any at-sea activity threatens the United Kingdom or its surrounding

waters. As part of its commitment to provide cleaner seas around the United Kingdom,

the MCA publishes, via ACOPS, an annual survey of reported discharges attributed to

vessels and offshore oil and gas installations operating in the United Kingdom Pollution

Control Zone (UKPCZ). The annual survey results complement historical data and

records and enable MCA's Counter Pollution and Response (CPR) Team to monitor

trends in the number of incidents, the amount of oil and chemicals spilt, the geographical

distribution of spills, the sources of pollution and the nature of the pollution.

Following analysis of 1,600 incident reports and supporting information, 669 separate

accidental discharges from vessels and offshore oil and gas installations were identified

in the survey area during 2011. An overall increase of 14.9% was evident in the total

number of incidents reported from all sources over the previous year’s total.

A total of 150 vessel-sourced discharges were reported during 2011 which was 1 more

than the previous year’s total but significantly fewer than the mean annual total of 200

reported discharges between 2000 and 2010.

A total of 519 accidental discharges were reported by offshore oil and gas installations

during 2011 representing a 19.9% increase on the previous year’s total and the highest

annual total since the survey commenced.

Further analysis of these statistics showed that the total of 279 oil reports from offshore

oil and gas installations during 2011 was the same number as the mean annual total

reported between 2000 and 2010.

The total of 240 non-mineral oil reports by offshore installations during 2011, primarily

those involving chemical substances, was 73 more than the total recorded during the

previous year.

The reported incidents from all sources during 2011 comprised 62.9% involving mineral

oils, 36.2% involving chemicals (including oil-based mud) and 0.9% involving other

substances (including vegetable and animal oils).

Approximately 84% of all reported accidental discharges were in the open sea, 13% were

in ports and harbours and 3% were across the remaining marine environmental zones.

Survey respondents submitted details of 89 incidents involving accidental discharges

from vessels operating in ports and harbours during 2011 which was 20 fewer compared

to the mean annual total of 109 discharges between 2000 and 2010.

Estimated volumes of accidental discharges were recorded in 618 incident reports, or

92% of the total. The modal class for oil discharges was less than 455 litres but larger

spill volumes of between 456 litres and approximately 25 tonnes were recorded in 5 of

the 11 survey enumeration areas. In total 7% of all oil discharges were greater than 455

litres, where this information was recorded, compared with 6% during the previous year.

The largest reported oil discharge, with an estimated maximum volume of 25 tonnes and

occupying an area of 5km2, was reported by the Byford Dolphin Mobile Offshore Drilling

Unit on 21 November. The surface sheen was attributed to an unidentified passing vessel

and later dispersed naturally without posing a risk to the shoreline.

The modal class for chemical spills was also less than 455 litres but 44 releases, or 18%

of the total, were of 1 tonne or more. The largest chemical incident involving a release

from the Hunter Pipeline of 189,840 kg of methanol and 121.6 kg of KI-5351 was

caused by a pipeline failure during an integrity pressure test on 10 November. The

pipeline was subsequently removed from service and a replacement installed.

At least 31 confirmed discharges were attributed to vessel casualties including 21 fishing

vessels, 8 pleasure craft and workboats, a tug and a barge. The vessel-casualty spills

occurred in all enumeration areas with the exception of the UKCS.

Reporting organisations submitted details of 2 successful prosecutions brought by the

competent authorities following pollution offences during 2011 and another 2 cases were

under consideration or had not been called at the time of reporting. The owners of the

oil/chemical tanker Pretty Time were fined £20,000 with costs totalling £75,000

following a palm oil discharge in the Outer Nab Anchorage. The Baltnautic Shipping

Company was fined £12,391 following a diesel oil discharge from the general cargo

vessel MV Nina 1 in the Port of London on 4 August.

Observations from air surveillance patrols and a satellite surveillance programme were

again employed by the MCA and DECC to help identify discharges at sea.

SECTION 1: INTRODUCTION AND SURVEY DESIGN

1.1 INTRODUCTION

1.1.1 Since 1965 the Advisory Committee on Protection of the Sea (ACOPS) has

compiled statistics and other information on different types of marine

pollution in the waters around the British Isles. The competent authorities

and other stakeholder groups have used this information for a wide range of

purposes including policy and planning decisions, evaluations of particular

programmes, assessments of regulatory compliance levels, development of

risk assessment models and to support educational campaigns. Examples of

organisations that have consulted the survey data include the Department for

Transport, European Commission, International Maritime Organisation,

Local Authority Associations, Marine Management Organisation, the

Scottish Government and the Welsh Assembly Government.

1.1.2 The Maritime and Coastguard Agency (MCA) exercises Central

1.2 SURVEY DESIGN

1.2.1 In accordance with the MCA's survey specification, a two-page questionnaire

was designed to capture information relating to discharges of oil and other

hazardous and noxious substances from vessels and offshore oil and gas

installations. The survey area included the UKPCZ and adjacent areas of

jurisdiction including ports, harbours and tidal stretches of rivers and

estuaries. The boundaries of the UKPCZ are shown in Figure 2. The survey

questionnaire and accompanying guidelines are provided in Appendix 2.

1.2.2 The survey questionnaire was distributed to the appropriate representatives

of coastal local authorities, port and harbour authorities, national

environment agencies and animal welfare groups. In addition, questionnaire

responses were supplemented with Petroleum Operation Notices (PON1s)

provided by the Department of Energy and Climate Change’s (DECC)

Offshore Environment Unit and Pollution Reports (POLREPS) provided by

the MCA. PON1s are submitted to DECC by offshore oil and gas installation

operators for all unpermitted oil or chemical releases to sea regardless of the

volume lost. The resulting information was entered into a database

identifying separate pollution incidents by date, location and other survey

attributes.

1.2.3 Following analysis of data the survey results are presented in a range of

different formats. In Section 2 descriptive statistics are provided for reported

discharges including some numerical trends with 2000 as the base year and

also including categories and types of polluting substances, geographical

locations and distributions of incidents, amount and extent of reported

pollution, sources, details of prosecutions brought by the competent

authorities and reporting effort.

1.2.4 In Section 3 summary reports are provided for all larger discharges of 2

tonnes or more. Further information is provided in Section 4 for each of 11

geographical areas around the United Kingdom, including the causes of

particular incidents and counter-pollution responses where appropriate.

1.2.5 Elements of the survey quality assurance programme are outlined in Section

5 including the number of incident reports compiled from information

submitted by two or more reporting organisations. Moreover, the current

database is provided in Appendix 1 as part of the quality assurance

programme in order to ensure the reliability and accuracy of the survey

results. This is further enhanced by posting the survey report on the MCA’s

website as a publicly available document for scrutiny by all interested

stakeholder groups.

ASSESSMENTS

2.1 NUMERICAL TRENDS IN REPORTED DISCHARGES

2.1.1 Following analysis of 1,600 incident reports and supporting information, 669

separate discharges from vessels and offshore oil and gas installations were

identified within the survey area during 2011. An overall increase of 14.9%

was evident in the total number of incidents reported over the previous year’s

total.

2.1.2 Trends in annual totals for reported discharges between 2000 and 2011 are

displayed in Figure 1, including separate line-graphs for vessels and offshore

oil and gas installations.

2.1.3 A total of 150 vessel-sourced discharges were reported during 2011 which

was 1 more than the previous year’s total but significantly fewer than the

mean annual total of 200 reported discharges between 2000 and 2010.

2.1.4 A total of 519 accidental discharges were reported by offshore oil and gas

installations during 2011 representing a 19.9% increase on the previous

year’s total and the highest annual total recorded since the survey

commenced.

2.1.5 Further analysis of these statistics showed that the total of 279 oil reports

2.1.6 During 2011 offshore operators reported a total of 240 incidents involving

accidental releases of non-mineral oil substances, primarily those involving

chemical substances, and further details are provided in Appendix 1.

2.2 CATEGORIES OF REPORTED DISCHARGES

2.2.1 In Table 1 statistics are provided showing the different categories and

numbers of discharges reported during 2010 and 2011 for each of 11

enumeration areas. Geographical boundaries of each enumeration area and

the complete survey area are shown in Figure 2. It should be noted that the

statistics for discharges in the United Kingdom Continental Shelf (UKCS)

Oil and Gas enumeration area have been presented in two source categories:

offshore oil and gas installations, and vessels.

2.2.2 The discharges from all sources reported during 2011 comprised 62.9%

mineral oils, 36.2% chemicals (including oil-based mud) and 0.9% other

substances (including vegetable and animal oils). During 2010 there were no

reports of any observed discharges of garbage at sea from vessels or offshore

oil and gas installations.

2.3 LOCATIONS OF REPORTED DISCHARGES

2.3.1 The geographical location of each reported discharge has been identified in

Appendix 1 together with other information for each incident. To facilitate

comparisons with previous years’ data, reported locations of all vessel-source

oil discharges in the open sea, nearshore waters and estuaries during 2011 are

plotted in Figure 3. Data for ports have been grouped separately for England,

Wales, Scotland and Northern Ireland.

2.3.2 The overall geographical pattern for vessel-source oil discharges in the

survey area during 2011 showed fewer reports of incidents across 5

enumeration areas, with the highest density of reported incidents in the open

sea occurring in the vicinity of North Sea oil and gas installations.

2.3.3 The geographical locations of all discharges are further analysed by their

distribution across five marine environmental zones in each enumeration

area: open sea; tidal river or estuary; bay or nearshore waters; beach or shore;

and port or harbour. The results are presented in Table 2, which shows that

84% of all reported discharges were in the open sea, 13% in ports or

harbours and 3% across the remaining zones.

2.3.4 Reporting organisations submitted details of 89 incidents involving

discharges of polluting substances from vessels operating in ports or

harbours during 2011, which was 20 fewer than the mean annual total of 109

discharges between 2000 and 2010. Further details are provided in Section 4.

2.4 AMOUNT AND EXTENT OF REPORTED POLLUTION

2.4.1 Information describing observed dimensions and volumes of pollution for

each enumeration area is presented in Table 3. It should be noted that the

extent of pollution for incidents occurring within the UKCS enumeration

area 11 was determined by the observed approximate release area of water

surface pollution in 2 categories: up to 1.6 km2 ; or above 1.6 km2. Estimates

for volumes of discharges were not confirmed in written reports for 51

incidents, or 8 % of the total reported.

2.4.2 Excluding discharges from offshore oil and gas installations and vessels

operating in the UKCS area 11, approximately 4% of oil slicks or sheens

were greater than 1.6 km in length when this information was reported.

Broadly comparable statistics from the 2000, 2005 and 2010 surveys were

12%, 7% and 10% respectively. Within the UKCS area 11 a release area of

water surface pollution greater than 1.6 km2 was reported in 8 incidents

following oil discharges.

2.4.3 The modal class for all oil discharges was less than 455 litres but larger

estimated maximum volumes of between 456 litres and 25 tonnes were

recorded in 5 of the 11 survey enumeration areas. In total, 7% of all oil

discharges (including 3 ongoing releases) were greater than 455 litres, when

this information was recorded, compared to 6% during the previous year

(Refer to Table 3 and Appendix 1 Table 11).

2.4.4 The largest oil discharge with an estimated maximum volume of 25 tonnes

and occupying an area of approximately 5 km2 was reported by the Byford

Dolphin Mobile Offshore Drilling Unit (MODU) on 21 November. The

surface sheen was attributed to an unidentified passing vessel and later

dispersed naturally without posing a risk to the shoreline (Refer 3.4

UKCS/624).

2.4.5 The modal class for chemical spills was also less than 455 litres but 44

releases, or 18 % of the total (including 7 ongoing releases), were of 1 tonne

or more. The largest chemical incident involving a release from the Hunter

Pipeline of 189,840 kg of methanol and 121.6 kg of KI-5351 was caused by a

pipeline failure during an integrity pressure test on 10 November. The

pipeline was removed from service and a replacement installed (Refer 3.1

UKCS/1630).

2.4.6 In Section 3 details are provided for all reports involving releases or

discharges of 2 tonnes or more during 2011 including 8 incidents that

commenced during previous years.

2.5 CATEGORIES AND TYPES OF POLLUTION

2.5.1 Statistics showing the different categories and types of polluting substances

are presented in Table 4. Mineral oils were again the most frequently

recorded category of polluting substances and were identified in 63% of

incident reports. Crude oils accounted for 19% of the identified types of

mineral oil discharges, this figure being 9% less than the previous year’s.

The largest crude oil spill, with an estimated volume of 5.1 tonnes, was

reported by the Byford Dolphin vessel after a pressure relief valve between a

heater and separator lifted prematurely during well test operations. To

prevent any further release of oil the well was closed on the surface at the

choke manifold (Refer 3.15).

2.5.2 Bunker, diesel, fuel and gas oils were spilt in varying quantities from vessels

or oil and gas installations on 165 occasions, 15 more than the previous

year’s total, across all enumeration areas. The spills occurred most frequently

during bunkering operations in the UKCS, western Scotland and eastern

Scotland enumeration areas.

2.5.3 The largest diesel spill of 7.16 tonnes was reported by the Armada

installation after a drain valve was inadvertently left open during a

maintenance operation on 7 February. To prevent any further release the

diesel drain was isolated (Refer 3.13). During adverse weather a

decommissioned fishing vessel ran aground at Corpach in western Scotland

on 9 December causing a 1-tonne diesel oil spillage (Refer 4.8.3).

2.5.4 Lubrication and hydraulic oil spills were recorded in 9 of the 11 survey

enumeration areas with 90 spills attributed to offshore oil and gas

installations. The largest spill of 2.972 tonnes of hydraulic oil was an

ongoing release from the Thelma Manifold that commenced on 1 January

and continued throughout 2011 following a hydraulic system failure. At the

time of reporting the operator had planned a ROV intervention to stem the

leakage (Refer 3.39).

2.5.5 Respondents described the remaining types of mineral oil discharges as bilge

oils, deck oils, emulsified oil, oily water, open drains oil, reformate,

weathered or semi-weathered globules and various waste oils. Three reports

referred to beach pollution incidents involving weathered or semi-weathered

oil globules. The largest of these incidents occurred on 20 December after

lumps of oil were washed ashore between Formby Point and Birkdale. A

clean-up operation was completed over 2 days and at least 50 oiled birds

including gulls and waders were observed (Refer 4.7.5).

2.5.6 A total of 248 reports referred to incidents involving discharges of

substances other than mineral oils during 2011 compared to 173 discharges

during 2010. Eight incidents followed discharges from vessels operating in

ports and in the open sea. A split hose was identified as the source of a 3-

tonne palm oil spill from the tanker NCC Amal (29,168 gt, built 2011)

berthed at Liverpool’s Canada Dock and a clean-up operation was

completed. The Eastern Inshore Fisheries and Conservation Authority

(EIFCA) provided details of a significant beach pollution incident in which

blobs or larger lumps of paraffin wax were found on beaches over a 100 km

stretch of coastline between Suffolk and East Norfolk from 1 January to

September 2011. The source of the wax was believed to have been a

discharge from an unidentified vessel (Refer 4.2.2).

2.5.7 Offshore operators reported 240 new incidents involving accidental

discharges of chemical substances during 2011 compared to 167 discharges

during the previous year. The identified substances were all associated with

exploration and production operations and included barite, glycol solutions,

hydraulic fluids, methanol and sodium. The estimated volumes of accidental

chemical discharges ranged from 0.001 kg to 189,840 kg and 44 discharges

(including ongoing incidents that commenced in previous years) were 1

tonne or more. Further details are provided in Section 3 and Appendix 1.

2.5.8 Chemicals are required to be used as part of offshore exploration and

production operations. The vast majority of chemicals spilled are regulated

under the provisions of the Offshore Chemicals Regulations 2002 (OCR) for

normal operational use and discharge to sea. Under the provisions of OCR

each operational chemical is assessed to determine its potential

environmental impact.

2.6 REPORTED SOURCES OF POLLUTION

2.6.1 The figures in Table 4 show for each enumeration area the numbers of

incidents in which reporting organisations either identified a specific source

of pollution or suspected a vessel or offshore pollution source. The statistics

for the identified sources of pollution are further analysed in Table 5 to show

the different types of vessels. Additional details including the names of

vessels or installations are provided in Appendix 1.

2.6.2 This method of analysis showed that oil and gas installations operating on the

UKCS accounted for 82% of all identified reports of pollution compared to

81% during the previous year. Corresponding statistics for fishing vessels,

pleasure craft, general cargo vessels and the remaining types of vessels were

6.3%, 2.4%, 1.3% and 7.8% respectively.

2.6.3 Reporting organisations identified discharges from oil and chemical tankers

on 10 occasions during 2011 compared to 11 occasions during the previous

year. Port and harbour authorities reported tanker incidents at Fawley, ABP

Humber and Liverpool. The oil/chemical tanker Nena K (4,218 gt, built

2009) berthed at the Fawley Refinery was identified as the source of a 30-

litre paraffin wax discharge on 7 September and a clean-up operation was

completed. A 100-litre diesel oil spill was reported by the chemical and

product tanker Hans Scholl (25,400 gt, built 2004) in the same location on

19 November. The spill occurred during a loading arm disconnection

operation and absorbent pads and booms were deployed to recover the oil

from the water surface.

2.6.4 Six tanker incidents were identified in the English Channel and Irish Sea 3 of

which were believed to have been caused by permissible discharges of

chemicals or vegetable oils in tank washings. The chemical/oil tanker

Sichem Palace (5,451 gt, built 2004) reported an accidental spillage of 20

litres of heavy fuel oil during an internal transfer operation in the English

Channel on 14 June.

2.6.5 At least 31 confirmed discharges were attributed to vessel casualties

including 21 fishing vessels, 8 pleasure craft and workboats, a tug and a

barge. The vessel-casualty spills occurred in all enumeration areas with the

exception of the UKCS. Moreover, vessel casualties accounted for 52% of all

confirmed discharges attributed to fishing vessels and 53% of all discharges

attributed to pleasure craft and workboats. The corresponding statistics from

the 2010 survey were 33% and 46% respectively. Further details identifying

the causes of vessel casualties are provided in Section 4 and Appendix 1.

2.6.6 The Yarmouth Navigator (116 gt, built 1943), a decommissioned Royal

Navy Motor Fishing Vessel, sank in Plymouth Sound on 30 January and a

diesel surface sheen was later observed above the wreck site. In nearshore

waters the 15-metre gill netter Ben-My-Chree sank whilst under tow in a

semi-submerged condition off Gwennap Head on 11 March and a diesel

sheen was later observed floating on the surface above the wreck. On 6

August the MFV Sea Horse sank at Oare Creek near Faversham causing a

10-litre lubrication oil spillage. A clean-up operation was required after the

MTUG Chieftain capsized and sank off Greenwich Pier on 12 August. The

11-metre fishing boat Flowing Tide II sank whilst alongside at Fishguard

Harbour on 18 November following an engine-cooling water pipe failure.

The vessel was later raised and a resulting small diesel sheen dispersed

naturally.

2.6.7 In Scotland the fish farm vessel Lady Laxford partially sank alongside

Kinlochbervie Harbour causing a 100-litre marine gas oil spill. Booms were

deployed around the vessel which was later re-floated. The prawn trawler

Vellee (103 tonnes gt, built 1981) sank in the middle of the Minch off

Benbecula on 6 August and a light fuel oil sheen and floating debris were

later observed near the wreck site. On 18 March the MFV Kestrel ran

aground in Buckie Harbour causing a 27-litre spillage of diesel oil. The

vessel’s vents were plugged to prevent any further release of oil and a clean-

up operation was undertaken by contractors. A SEPA report provided details

of a counter-pollution operation in Dundee Harbour following a diesel oil

leak from a sunken former fishing vessel. A boom was placed across the

harbour but this failed to prevent a small quantity of oil escaping into the

River Tay.

2.7 PROSECUTIONS

2.7.1 Details of 2 successful prosecutions brought by the competent authorities

following pollution offences during 2011 are summarised in Table 6.

Another 2 cases were under consideration or had not been called at the time

of reporting.

2.7.2 Ten reporting organisations provided details of an incident in which palm oil

residues were washed ashore along stretches of the Dorset, Hampshire and

West Sussex coastlines between 11 and 18 January. A back-track analysis

indicated that the pollution originated from the oil/chemical tanker Pretty

Time (30,006 gt, built 2007) whilst the vessel was engaged in tank cleaning

operations in the Outer Nab Anchorage. Following a prosecution brought by

the Maritime and Coastguard Agency at Southampton Magistrates’ Court the

owners of the tanker were fined £20,000 with costs totalling £75,000 (Refer

4.4.2).

2.7.3 The general cargo vessel MV Nina 1 (998 gt, built 1984) was identified as

the source of a 6-litre diesel oil discharge in a report submitted by the Port of

London Authority. A subsequent investigation revealed that the spillage took

place because a scupper had not been blocked as oil was being transferred

forward to the vessel’s bow thruster tank. A successful prosecution was

brought under Section 131 of the Merchant Shipping Act 1995 and

magistrates imposed a fine of £12,391 on the vessel’s owner’ the Baltnautic

Shipping Company Limited (Refer 4.3.3).

2.7.4 Respondents referred to another 2 incidents resulting in oil spills in which

cases were brought by the Maritime and Coastguard Agency for offences

contravening Sections 58 and 100 of the Merchant Shipping Act 1995 and

the Merchant Shipping (Distress and Prevention of Collisions) Regulations

1996.

2.7.5 On 31 January the French-registered stern trawler Jack Abry II (840 gt, built

2005) grounded on the north coast of the Isle of Rum in heavy weather and

oil was later observed around the vessel and in Kilmory Bay. On 2 February

the skipper of the vessel appeared in Stornoway Sheriff Court and pleaded

guilty to failing to maintain a proper lookout and was fined £3,000 (Refer

4.8.2). The MFV Reliant grounded on Sule Skerry to the south-west of the

Orkney Islands on 17 March. A light sheen of marine diesel was later

observed drifting downwind of the vessel. At the Sheriff’s Court in Kirkwall

on 18 January 2012 the skipper of the fishing vessel pleaded guilty of failing

to keep a proper lookout and was fined a total of £1,600 (Refer 4.9.2).

2.8 REPORTING EFFORT

2.8.1 The number of incident reports received from each of 11 reporting

organisations is shown in Table 7. The total number of reports received and

processed was 1,600 from which 669 separate incidents were subsequently

identified and entered into the 2011 database.

2.8.2 Observations from air surveillance patrols and a satellite surveillance

programme were employed to help identify discharges from vessels and

offshore oil and gas installations operating in the UKPCZ (Refer Section 4).

SECTION 3: SUMMARY OF LARGER DISCHARGES (TWO TONNES

OR MORE) DURING 2011

The figures given below are indicative of the reported volumes of releases confirmed in

written reports. However, incidents in which significant amounts of beach material were

mixed with pollutants, or where the volumes released were not indicated or measured, are

not included. The incidents are ordered according to spill size. Those that occurred over a

period of time have been included at the end of this section.

3.1 UKCS 10 November: E.ON Ruhrgas UK E&P Limited Hunter Pipeline (11-

UKCS/1630)

E.ON Ruhrgas UK E&P Limited reported an incident involving a release from the

Hunter Pipeline of 189,840 kg of methanol and 121.6 kg of KI-5351 due to a line

failure during an integrity pressure test. The pipeline was removed from service and

a replacement installed.

3.2 UKCS 13 January: Maersk Oil UK Limited NTvL (11-UKCS/1193)

Maersk Oil UK Limited reported an incident involving a release from the NTvL

installation of 88,800 kg of sodium chloride due to a failure of the transfer hose. The

hose was changed and all sections inspected.

3.3 UKCS 10 April: Dana Petroleum PLC Hudson Manifold (11-UKCS/1296)

Dana Petroleum PLC reported an incident involving a release from the Hudson

Manifold of 26,048 kg of Pelagic 100 due to a failed subsea hydraulic hose. Repairs

were completed and system leak test undertaken.

3.4 UKCS 21 November: unidentified vessel (12-UKCS/624)

The Byford Dolphin Mobile Offshore Drilling Unit (MODU) reported a surface

sheen 5 km to the east of the platform occupying an area of 5 km2 and with a

maximum estimated volume of 25 tonnes. The sheen later dispersed naturally.

3.5 UKCS 22 March: Maersk Oil UK Limited Donan FPSO Global Producer III

(11-UKCS/1268)

Maersk Oil UK Limited reported an incident involving a release from the Donan

FPSO Global Producer III of 21,905 kg of Castrol Transaqua HT2 due to a damaged

O ring within a subsea control module. The connector was replaced and no further

releases were reported.

3.6 UKCS 11 September: Dana Petroleum Limited Guillemot West (Triton) FPSO

(11-UKCS/1550)

Dana Petroleum Ltd reported an incident involving a release from the Guillemot

West (Triton) FPSO of 13,800 kg of Castrol Brayco Micronic SV/3 and 4,200 kg of

Cortron CK929-G due to a subsea chemical umbilical line becoming damaged

during trenching activities. Steps were taken to prevent a recurrence of the incident

(Refer to 3.11 and 3.16 for other discharges relating to this incident).

3.7 UKCS 11 July: BG plc Rowan Gorilla VI (11-UKCS/1445)

BG plc reported an incident involving a release from the Rowan Gorilla VI

installation of 15,400 kg of Carbosea due to an overflow of pollution pans beneath

the rig floor. New procedures, including visual checks of rotary housings and

pollution pans, were introduced to prevent a recurrence of the incident.

3.8 UKCS 4 August: Nexen Petroleum U.K Limited Ettrick FPSO Aoka Mizu (11-

UKCS/1487)

Nexen Petroleum UK Limited reported an incident involving a release from the

Ettrick FPSO Aoka Mizu of 13,110 kg of Transaqua due to a downhole leak which

was confirmed after a DSV survey.

3.9 UKCS 25 August: unidentified vessel (12-UKCS/593)

The Balmoral installation reported a weathered oil sheen with brown streaks and

white emulsion approximately 8 km to the south-east of the platform. The sheen

occupied an area of approximately 0.32 km2 with an estimated maximum volume of

10 tonnes. The sheen was broken up and dispersed by wave action.

3.10 UKCS 21 August: TAQA Bratani Limited Kestrel Subsea Field (11-UKCS/1519)

TAQA Bratani Limited reported an incident involving a release from the Kestrel

Subsea Field of 7.748 tonnes of hydraulic oil due to a subsea hydraulic hose

connection failure. Repairs were completed and visually checked by a ROV to

confirm no further release was occurring to sea.

3.11 UKCS 11 September: Dana Petroleum Limited Guillemot West (Triton) FPSO

(11-UKCS/1551) Dana Petroleum Limited reported an incident involving a release from the Guillemot

West (Triton) FPSO of 5,998 kg of methanol and 1,320 kg of FX2504 due to a

severed subsea chemical umbilical line caused by trenching activities. Amended

procedures and new controls were introduced to prevent a recurrence of the incident.

3.12 UKCS 1 March: Shell U.K. Exploration & Production Limited (NNS) Anasuria

FPSO (11-UKCS/1245) Shell UK Exploration & Production Limited (NNS) reported an incident involving a

release from Anasuria FPSO of 7,273 kg of Oceanic HW525 due to a solenoid

malfunction within the FPSO turret. The leak ceased after the solenoid was replaced.

3.13 UKCS 7 February: BG plc Armada (11-UKCS/1225)

BG plc reported an incident involving a release from the Armada installation of 7.16

tonnes of diesel after a drain valve was left open during a maintenance operation.

The diesel drain was isolated.

3.14 UKCS 20 July: Shell U.K. Exploration & Production Limited (NNS) Shearwater

A Wellhead (11-UKCS/1463)

Shell UK Exploration & Production Limited (NNS) reported an incident involving a

release from the Shearwater A Wellhead installation of 6,360 kg of Versatherm oil-

based drilling fluid due to a leak occurring during operations to test the retraction

status of the SW07 wellhead wear bush lock down pin. During this procedure the lock

down pin was accidentally ejected from the housing and the contents of the riser were

released into the immediate area and ultimately to the sea. Shell has committed to

eliminating the problem by improved design and in the interim has adapted its

procedures.

3.15 UKCS 11 March: BP Byford Dolphin (11-UKCS/1257)

BP reported an incident involving a release from the Byford Dolphin vessel of 5.1

tonnes of crude due to a pressure relief valve between the heater and separator

lifting prematurely during well test operations. The well was closed on the surface at

the choke manifold.

3.16 UKC 11 September: Dana Petroleum Limited Guillemot West (Triton) FPSO

(11-UKCS/1552) Dana Petroleum Ltd reported an incident involving a release from the Guillemot

West (Triton) FPSO of 2,064 kg of Flexoil WM1840 and 2,948 kg of mono-

ethylene glycol (MEG)/water mix due to a severed subsea chemical umbilical line

caused by trenching activities. Amended procedures and new controls were

introduced to prevent a recurrence of the incident.

3.17 UKCS 4 April: TAQA Bratani Limited Kestrel subsea completion P2 (11-

UKCS/1289)

TAQA Bratani Limited reported an incident involving a release from the Kestrel

subsea completion P2 of 4,941.1 kg of Castrol Brayco Micronic 86HT/200 due to a

leak on a subsea hydraulic line fitting. The leak was stopped after the faulty line

fitting was isolated.

3.18 UKCS 16 January: Talisman Energy UK Ltd Clyde (11-UKCS/1195)

Talisman Energy UK Limited reported an incident involving a release from the

Clyde installation of 4,600 kg of fire-fighting foam (1% AFFF-LF Aberdeen Foam)

due to the incorrect installation of the actuator on the AFFF/firewater isolation valve

which resulted in an incorrect indication of the valve position. The valve actuator

was later reinstalled in the correct orientation.

3.19 UKCS 9 November: Britannia Operator Ltd Britannia (11-UKCS/1628)

Britannia Operator Limited reported an incident involving a release from the

Britannia installation of 4,554 kg of EEHC1 due to an overflow from an expansion

drum in the cooling medium system. System improvements were completed to

prevent any recurrence of this release.

3.20 UKCS 25 July: TAQA Bratani Limited Tern (11-UKCS/1469)

TAQA Bratani Limited reported an incident involving a release from the Tern

installation of 2,000 kg of H2S Scav HSW85690 and 2,000 kg of SCW85594 after a

chemical was bunkered into an incorrect storage tank. During subsequent operations

bunkering hoses were checked to confirm that they were clearly marked with signs

indicating the chemical to be bunkered through them.

3.21 UKCS 17 August: unidentified vessel (12-UKCS/623)

The Brent Delta installation reported an oil sheen drifting towards the platform

occupying an area of 15 km2 and with an estimated maximum volume of 4 tonnes.

The sheen later dispersed naturally.

release from the Brent D installation of 3,601 kg of Class G Oilwell (Portland)

Cement and 3.68 kg of FP-16LG due to a cement batch tank overboard discharge

valve being left open. During subsequent operations the overboard line was locked

closed.

3.23 Irish Sea 10 September: MT NCC Amal (7-IS/619)

A split hose was identified as the cause of a 3-tonne palm oil spill from the tanker

NCC Amal (29,168 gt, built 2011) berthed at Liverpool’s Canada Dock. The oil was

contained within the dock system and later recovered.

3.24 UKCS 19 January: BP Exploration Operating Company Ltd Machar Manifold

(11-UKCS/1201)

BP Exploration Operating Company Limited reported an incident involving a release

from the Machar Manifold of 2,780 kg of Oceanic HW443-2 due to a solenoid in a

hydraulically actuated valve not fully opening allowing hydraulic fluid to be

released to sea via a return line. The valve was isolated to prevent any further

releases to sea.

3.25 UKCS 27 August: E.ON Ruhrgas Ensco 100 (11-UKCS/1526)

E.ON Ruhrgas reported an incident involving a release from the Ensco 100

installation of 2,500 kg of Versaclean oil-based mud due to a riser having been

inadvertently disconnected. The riser was then re-latched and sealed again, and the

connection was pressure tested before the operation continued.

3.26 UKCS 10 September: Nexen Petroleum UK Limited Ettrick FPSO Aoka Mizu

(11-UKCS/1547)

Nexen Petroleum UK Limited reported an incident involving a release from the

Ettrick FPSO Aoka Mizu of 2,300 kg of MEG and 71.1 kg of sodium nitrate due to

a failure of the bursting disc within the cooling medium system. A more robust type

of bursting disc was sourced and the closure speed of the valve was reduced. No

more failures have since been reported.

3.27 UKCS 30 June: Talisman Energy (UK) Limited Claymore A Platform (11-

UKCS/1425)

Talisman Energy (UK) Limited reported an incident involving a release from the

Claymore A installation of 2,273 kg of fire-fighting foam (3% Petroseal FFFP) due

to a leak in a deluge/foam system which flowed into open drains and ultimately to

sea. A short-term solution was implemented and in the longer term Talisman intends

to decommission, upgrade, and replace sections of its foam system.

3.28 UKCS 11 April: BP Exploration & Production (UK Offshore) Bruce PUQ (11-

UKCS/1298)

BP Exploration & Production (UK Offshore) reported an incident involving a

release from the Bruce PUQ installation of 2,045.1 kg of Scale Treat 8019 due to the

inappropriate isolation of a chemical injection system allowing scale inhibitor to be

drawn into topsides pipework then to sea via an open hazardous drains caisson. An

investigation was underway to determine the root cause of the release.

3.29 UKCS 15 December: Talisman Energy (UK) Limited Claymore A (11-

UKCS/1681)

Talisman Energy (UK) Limited reported an incident involving a release from the

Claymore A installation of 2,000 kg of fire-fighting foam (3% FFFP) due to an open

firemain/utility system cross-over. A short-term solution was implemented and in

the longer term Talisman intends to decommission, upgrade, replace sections of its

foam system.

UKCS INCIDENTS OCCURRING OVER AN EXTENDED PERIOD DURING 2011

The following reported UKCS incidents occurred over a period of time within the year.

In all cases where the source of the release was from subsea control equipment the fluids

lost were associated with oil and gas exploration and production operational processes. The

operator continued the release to maintain the integrity and functionality of the

wells/equipment until equipment and dive support vessels could be resourced to complete

the necessary repairs.

Each release required the operator to assess the potential environmental impacts associated

with the release. The operator in each case was required to provide regular updates on their

management plan as part of investigations being conducted by DECC. Where necessary,

meetings with the operator took place to discuss aspects associated with the incident.

Discussions included the type and volume of fluid being released, the potential

environmental impact and the operator’s short- and long-term plans to stop the release and,

where relevant, effect a final repair.

3.30 UKCS 1 January: Talisman Energy (UK) Limited Highlander

Template/Manifold (ongoing incident 11-UKCS/1609 from 26 December 2010)

Talisman Energy (UK) Limited reported an incident involving an ongoing release

from the Highlander HS 13 well, via the Tartan Alpha installations produced water

discharge, of 82,968 kg of Pelagic 100 and 42,850 kg of Pelagic 100 HV 45 due to a

failure of a SSSV seal. Talisman reduced the release rate by using a higher viscosity

hydraulic fluid and ultimately prevented further hydraulic fluid release by installing

a hold-open sleeve across the SSSV.

3.31 UKCS 1208 28 January: Shell U.K. Exploration & Production Limited (NNS)

Pierce FPSO (Haewene Brim) (ongoing from 28 January 2011 to 31 December

2011)

Shell UK Exploration & Production Limited (NNS) reported an incident involving a

release from the Pierce FPSO (Haewene Brim) of 20,757 kg of Castrol Transaqua

HT2 due to a leak on the subsea annulus master valve. Until repairs are effected in

quarter 2 of 2013, the valve remains closed during normal operations and is only

opened when monitoring of annulus pressure is required thereby minimising the loss

of fluids.

3.32 UKCS 1 January: Enquest Heather Ltd Heather A (ongoing incident from 1

January 2007)

Enquest Heather Limited reported an incident involving the ongoing release from

the Heather A installation of 20,710 kg of Transaqua HT2. Further investigations

indicated a leak rate of 30 litres per day revised from 60 litres per day; this involved

a full investigation and review of valve performance criteria and hydraulic fluid

requirement for normal operations against what was originally on the PON15 permit.

The operator decided to block in the BR1 well and investigate possible solutions to

resolve this ongoing leak; however, this caused some reservoir stability problems

and the well was flowed again. An intervention and engineering solution has been

devised for the remaining 30 litres-per-day leak. The problem was fixed on 30

November 2012 and monitoring of levels are still continuing.

3.33 UKCS 1 January: Total E&P UK PLC Dunbar Nuggets (ongoing incident

UKCS/1416 from 15 July 2009)

Total E&P UK plc reported an incident involving an ongoing release from the

Nuggets subsea field tied back to the Dunbar installation of 16,335 kg of Oceanic

HW540 subsea hydraulic control fluid. Extensive subsea survey work and testing

eventually located the leak on the wing valve of a subsea ‘xmas tree’. Subsequent

diver intervention effected repairs and stopped further releases to sea.

3.34 UKCS 1 January: BP Exploration Operating Company Ltd Bruce PUQ

(ongoing incident UKCS/1628 from 21 December 2009)

BP Exploration Operating Company Limited reported an incident involving an

ongoing release from the Bruce PUQ of 14,539 kg of Oceanic 540W due to a

suspected hydraulic hose release subsea on the Bravo gas export isolation valve. At

the time of reporting investigations to identify the source of the release were

ongoing.

3.35 UKCS 12 October: CNR International FPSO Petrojarl Banff

(UKCS/1597)(ongoing from 12 October to 10 December 2011)

CNR International reported an incident involving an ongoing release from the FPSO

Petrojarl Banff of 11,594.45 kg of Pelagic 100 Production Control Fluid due to a

hydraulic fluid leak from a subsea isolation valve accumulator. CNR investigated

mitigation measures to reduce the leak rate, and planned for a ROV/DSV

intervention to resolve this issue. The release was ultimately stopped when the FPSO

Petrojarl Banff lost station during a storm in December 2011, and the vessel was

removed from the field.

3.36 UKCS 1 January: BG plc Seymour WHPS (ongoing incident UKCS/1564 from 7

December 2010)

BG plc reported an incident involving an ongoing release from the Seymour WHPS

installation of 8,400 kg of Transaqua HT2 due to failure of a hydraulic port plug

fitting. The well was suspended and a completely new well drilled at a different

location. The new well was fitted with a new xmas tree and SCM, the original

controls umbilical was re-routed and connected up to supply the new tree. Therefore

on completion of this well reconfiguration the previously reported leak was taken to

have been rectified. The original well remains suspended and has no connection to

the hydraulic system.

3.37 UKCS 24 October: Exxonmobil Nevis South Cluster (ongoing incident 11-

UKCS/1606 from 24 October 2011)

Exxonmobil reported an incident involving a release from the Nevis South Cluster of

7,648.7 kg of Oceanic HW 443 due to a hydraulic fluid leak from a subsea control

module (SCM). The SCM was replaced on 11 March 2012 and no further releases

were observed.

3.38 UKCS 1 January: Nexen Petroleum U.K Limited Ettrick FPSO Aoka Mizu

(ongoing incident 11-UKCS/1543 from 18 November 2010)

Nexen Petroleum UK Limited reported an incident involving an ongoing release

from the Ettrick FPSO Aoka Mizu of 5,410 kg of Transaqua due to a release from

subsea supply line connections. On 8 April DSV Acergy Osprey completed subsea

works including visual inspection of lines. No leaks were observed and the incident

was closed.

3.39 UKCS 1 January: CNR International Thelma Manifold (ongoing incident

UKCS/1178 from 23 June 2006)

CNR International reported an incident involving the ongoing release from the

Thelma Manifold of 2.972 tonnes of Brayco Micronic hydraulic fluid from 1

January 2011 to 31 December 2011 due to failure of the hydraulic system. CNR

undertook an investigation to analyse data to identify the source of the losses, and a

ROV/DSV intervention was planned to resolve this issue.

3.40 UKCS 1 January Talisman Energy (UK) Limited Clyde (ongoing incident

UKCS/1284 from 5 May 2010) Talisman Energy (UK) Limited reported an incident involving an ongoing release

from the Clyde installation of 2,120 kg of Castrol Transaqua HT2 due to a hydraulic

leak from the production wing valve actuator seal on a subsea well. An over-ride

actuator was installed to "piggy-back" the defective component during a DSV

intervention on 12 February 2011 which proved successful.

4.1 North-east England (Northumberland, Tyne & Wear, Durham,

Yorkshire and The Humber)

4.1.1 Reporting organisations identified 9 pollution incidents attributed to vessels

during 2011 compared to 15 incidents during the previous year. All recorded

incidents were in ports and harbours and the polluting substances reported

included crude oil, fuel oils and hydraulic oils in quantities ranging from 5

litres to 25 litres.

4.1.2 At the Port of Tyne spill recovery equipment was deployed after a bucket of

bilge water overflowed on the deck of the MV Eems during a storm and

entered the River Tyne. A burst hose on an excavator boom caused a 25-litre

spillage of hydraulic oil from the backhoe dredger Maricavor (571 gt, built

1996) in the same location on 23 June. The oil was quickly dispersed by

mechanical agitation.

4.1.3 PD Teesport submitted details of 2 pollution incidents attributed to vessels

during 2011 compared to 4 incidents during 2010. The sand suction dredger

Sand Weaver (3,497 gt, built 1975) was identified as the source of a 5-litre

hydraulic oil spill on 23 January. Another hydraulic oil spill was attributed to

the workboat Tie Venture IV (71 gt, built 1993) in the same location on 6

April.

4.1.4 A POLREP and a Port of Sunderland report described a diesel oil discharge

from an unidentified fishing vessel at Greenwell Quays. The fishing cobble

Star of Hope sank in Seaton Sluice Harbour on 20 November. A surface

sheen was later observed on the water surface which later dispersed

naturally.

4.1.5 Three incidents were reported in the River Humber area during 2011.

Associated British Ports (ABP) Humber Estuary Services provided details of

a 15-litre crude oil spill from the MTANKER NS Consul (105,926 gt, built

2006) at the Tetney Mono Buoy on 22 February. The spill occurred during a

line-washing operation and the resulting surface sheen dispersed naturally. A

tug was identified as the source of a 25-litre oil discharge into the River

Humber on 16 November. The hopper dredger Heortnesse (604 gt, built

1959) reported a 10-litre lubrication oil leakage through a defective stern

gland at Hull. The incident occurred within an enclosed dock area and a

clean-up was completed by the vessel’s crew.

4.2 Eastern England (Lincolnshire, Norfolk & Suffolk)

4.2.1 Seven incidents were recorded during 2011, two fewer than the total

recorded during the previous year. Three discharges occurred in ports and

harbours, 2 in the open sea, 1 in nearshore waters and a significant beach

pollution incident was also reported. Accidental discharges from offshore oil

and gas installations operating off eastern England were included in the

statistics for the UKCS (Refer to 4.11).

4.2.2 The Eastern Inshore Fisheries and Conservation Authority (EIFCA) provided

details of a significant beach pollution incident in which blobs or larger

lumps of paraffin wax were found on beaches over a 100-km stretch of

coastline between Suffolk and East Norfolk from 1 January to September

2011. The source of the wax was believed to have been a discharge from an

unidentified vessel.

4.2.3 A satellite image alerted the authorities to a suspected oil discharge in the

open sea off Great Yarmouth on 1 May. A surveillance flight was tasked to

the area and confirmed the presence of small strips of sheen which could

have been part of a larger patch of oil. The Sean installation reported a

surface sheen attributed to an unidentified passing vessel on 19 September.

4.2.4 A workboat lost three 200-litre drums of lubrication oil overboard in

nearshore waters off Spurn Point on 16 September. Two of the drums were

punctured, releasing their contents into the sea where the oil was quickly

broken up and dispersed by wave action.

4.2.5 Elsewhere, pollution incidents were recorded in ports and harbours at

Boston, Grimsby and Lowestoft. A small diesel sheen was observed after a

pleasure craft sank at the entrance to Boston’s Black Sluice Lock on 23

April. A POLREP provided details of a diesel spillage from the survey vessel

Chartwell (106 gt, built 1989) at the Grimsby Fish Dock Marina. Absorbent

booms were deployed in a clean-up operation. Absorbent booms were also

deployed in a clean-up operation at Lowestoft following a 50-litre diesel spill

from a fishing vessel.

4.3 Essex and Kent

4.3.1 Twelve incidents were recorded by reporting organisations during 2011, the

same number as the previous year. Ten discharges occurred in ports and

harbours and 2 discharges in the open sea and nearshore waters. The source

of the pollution was identified on 11 occasions and there were 11 references

to fuel oils as polluting substances.

4.3.2 Port and harbour authorities reported vessel discharges at Dover, London,

Medway and Ramsgate. Four incident reports submitted by the Dover

Harbour Board described diesel oil spills of 1 litre or less from 3 P&O ferry

lifeboats and a pleasure craft. On each occasion the surface sheens dispersed

naturally and no further actions were required.

4.3.3 The general cargo vessel MV Nina 1 (998 gt, built 1984) was identified as

the source of a 6-litre diesel oil discharge in a report submitted by the Port of

London Authority. A subsequent investigation revealed that the spillage took

place because a scupper had not been blocked as oil was being transferred

forward to the vessel’s bow thruster tank. A successful prosecution was

brought under Section 131 of the Merchant Shipping Act 1995 and

magistrates imposed a fine of £12,391 on the vessel’s owner, the Baltnautic

Shipping Company Limited. A clean-up operation was required after the

MTUG Chieftain capsized and sank off Greenwich Pier on 12 August.

4.3.4 Peel Ports Medway recorded 3 pollution incidents during 2011. On 6 August

the MFV Sea Horse sank at Oare Creek near Faversham causing a 10-litre

lubrication oil spillage. The fishing vessel was later refloated and the oil

dispersed naturally. Back pressure in a fuel line was identified as the cause of

a 5-litre fuel oil spill from a pleasure craft on 9 September. A light fuel oil

sheen was observed after a fishing vessel sank at Queenborough Creek on 26

October.

4.3.5 A Ramsgate Port Authority report described a 25-litre diesel oil spill from

the MV Nab Cat 2 on 6 March. Absorbent booms and pads were deployed in

a clean-up operation. The incident was caused after a hose became

disconnected from a pump during a contaminated fuel removal operation.

4.3.6 On 11 July a marine pollution surveillance aircraft reported sighting a

surface sheen in the Dover Strait extending for 250 metres and with an

estimated maximum volume of 16.6 litres. A POLREP and a Kent County

Council report referred to an incident on 14 October after the motor cruiser

Layia Nija suffered a sudden ingress of water and was beached off Sandgate.

A ribbon of diesel oil was later observed in the vicinity of the wreck.

4.3.7 In addition a beach clean-up and recovery operation was carried out by

Dover District Council personnel after 5 five-litre plastic containers were

washed ashore after being lost overboard from a passing vessel. Each was

found to be in a leaking condition and partly filled with an unidentified green

viscous liquid.

4.4 Southern England (East and West Sussex, Hampshire, Dorset and the

Isle of Wight)

4.4.1 Reporting organisations identified 16 pollution incidents attributed to vessels

during 2011 compared to 28 incidents during 2010. Eleven recorded

incidents were in ports and harbours, compared to 20 during the previous

year, and there were 3 references to vegetable oil discharges. Sources of

pollution were identified on 15 occasions including 6 tankers and 4 pleasure

craft.

4.4.2 Four discharges in the open sea were attributed to chemical/oil tankers. Ten

reporting organisations provided details of an incident in which palm oil

residues were washed ashore along stretches of the Dorset, Hampshire and

West Sussex coastlines between 11 and 18 January. The residues were in the

form of small white pellets and larger yellow waxy balls and most were

removed by beach cleaning operations completed by local authority

personnel over several days. A back-track analysis indicated that the

pollution originated from the oil/chemical tanker Pretty Time (30,006 gt,

built 2007) whilst the vessel was engaged in tank-cleaning operations in the

Outer Nab Anchorage. The source was later confirmed by chemical analyses

which showed that palm oil samples taken from the vessel and the beaches

were the same. Following a prosecution brought by the Maritime and

Coastguard Agency at Southampton Magistrates’ Court, the owners of the

tanker were fined £20,000 with costs totalling £75,000.

4.4.3 A light oily film extending for 10 km was observed trailing in the wake of

the chemical/oil tanker Chemroad Quest (20,333 gt, built 2010) in the mid-

channel area on 11 January. Subsequent communications with the tanker

indicated a permissible discharge of soya bean oil residues in tank washings.

Another permissible discharge of vegetable oil in tank washings was

attributed to the chemical tanker Venezia D (6,875 gt, built 1998) on 24 May.

An accidental spillage of 20 litres of heavy fuel oil during an internal transfer

operation was reported by the chemical/oil tanker Sichem Palace (5,451 gt,

built 2004) in the English Channel on 14 June.

4.4.4 A POLREP provided details of a beach pollution incident after small lumps

of well-weathered oil were washed ashore in the Bexhill area during the

second week of February. Oil samples were analysed but the source of the

pollution could not be identified.

4.4.5 Oil discharges resulting mainly in surface sheens were reported in local ports

and harbours at Hayling Island, Lymington, Newhaven, Poole, Southampton,

and Weymouth. Chichester Harbour Conservancy provided details of a 2-

litre hydraulic oil spill from a pleasure craft at the Northney Marina. A clean-

up was quickly completed by marina staff. Lymington Harbour

Commissioners recorded 2 ten-litre diesel spills from pleasure craft during

2011. A crane barge was identified as the source of a 5-litre hydraulic oil

spill at Newhaven on 18 March and a clean-up operation was completed. A

70-litre hydraulic oil spill from the high-speed catamaran Condor Rapide

(5,007 gt, built 1997) berthed in Poole Harbour was broken up and dispersed

by propeller wash. A pleasure craft was identified as the source of a bilge oil

discharge in the same location on 15 August. An Environment Agency (EA)

report referred to a diesel oil spill from the high-speed catamaran Condor

Express (5,005 gt, built 1996) in Weymouth Harbour on 8 March.

4.4.6 Four incidents were recorded in the Port of Southampton during 2011.

Booms were deployed during a clean-up operation following a bunkering

spill from the multicat vessel Willendeavour (91.87 net tonnage, built 2007).

On 7 August the MFV Lilly Gina sank on her mooring in the River Itchen

and absorbent booms were deployed in a clean-up operation. Fareham

Borough Council and Hampshire County Council reports described a 30-litre

paraffin wax discharge from the oil/chemical tanker Nena K (4,218 gt, built

2009) berthed at the Fawley Refinery and a clean-up operation was

completed. A POLREP and a New Forest District Council report identified a

100-litre diesel oil spill from the chemical and product tanker Hans Scholl

(25,400 gt, built 2004) in the same location on 19 November. The spill

occurred during a loading arm disconnection operation and absorbent pads

and booms were deployed to recover the oil from the water surface.

4.4.7 In addition, a Havant Borough Council report referred to an incident in

which a 5-litre drum filled with sodium hypochlorite was washed ashore at

Sandy Point on 13 May.

4.5 South-west England (Devon, Cornwall, Isles of Scilly, Channel Islands

and the Western Approaches)

4.5.1 Seventeen vessel-source pollution incidents were identified by reporting

organisations during 2011 compared to 11 incidents during 2010. Seven

discharges occurred in local ports and harbours, 2 in the open sea and 5

discharges in nearshore waters and estuaries. A source of pollution was

identified on 12 occasions, the sources including 5 pleasure craft, 3 fishing

vessels and a general cargo vessel.

4.5.2 In the open sea, details of 2 oil discharges from vessels were provided by

POLREPS. A satellite image identified a small patch of oil in the English

Channel which was later confirmed as a fuel oil sheen by 2 passing vessels.

On 19 April a marine pollution surveillance aircraft detected visually and by

radar a surface sheen with an estimated maximum volume of 864 litres.

4.5.3 In nearshore waters the 15-metre gill netter Ben-My-Chree sank whilst under

tow in a semi-submerged condition off Gwennap Head on 11 March and a

diesel sheen was later observed floating on the surface above the wreck.

Another diesel sheen from a sunken pleasure craft was reported on 11 April

near Looe Island. A small boat ran aground on Porthbeor Beach in Cornwall

causing a spillage of 11 litres of petrol which quickly evaporated.

4.5.4 The EA carried out a successful clean-up operation on the Torridge estuary

following a diesel oil leakage from the derelict dredger/hopper barge

Panurgic moored on the foreshore. It was thought that the dredger was holed

as a result of corrosion and fuel escaped as it filled with water on an

incoming tide. Approximately 450 litres of diesel were recovered from the

vessel using a skimming device and absorbents. Another fuel recovery

operation was undertaken by the EA in the same locality following a diesel

leakage from the barge Southern Beaver moored near Appledore. The costs

of the operation were recovered from the vessel’s owner who was also issued

with a formal warning letter.

4.5.5 Two POLREPS described beach pollution incidents at Sennen Cove.

Globules of black, semi-solid emulsified oil ranging in size from 2cm to

25cm were removed from the high water mark of the beach on 9 January.

Similar black globules were washed ashore along the entire length of the

beach on 11 May.

4.5.6 Elsewhere, oil spills were recorded in ports and harbours at Falmouth,

Paignton, Penryn, Plymouth and St.Mary’s in the Isles of Scilly. A light

diesel sheen was dispersed by propeller wash following a bunkering spill

from the bulk carrier Mulberry Wilton (39,964 gt, built 2004) in Falmouth

Bay on 27 May. A written warning from the Harbour Master was later issued

to the Master of the vessel. Falmouth Harbour Commissioners described

another incident following a hydraulic oil spillage on the deck of the MFV

Bon Accord, where a small quantity escaped to sea through the scuppers of

the fishing vessel, and the crew sprayed detergent into the water to disperse

the oil. A written warning was later issued to the owner of the vessel by the

Harbour Master. A pleasure craft was believed to have been the source of a

bilge oil discharge in Paignton Harbour on 2 April.

4.5.7 Cornwall Council provided details of 2 diesel oil spills from pleasure craft

berthed in Penryn Harbour during 2011. The Yarmouth Navigator (116 gt,

built 1943), a decommissioned Royal Navy Motor Fishing Vessel, sank in

Plymouth Sound on 30 January and a diesel surface sheen was later observed

above the wreck site. A generator dismantling operation, in which fuel pipes

had been left open, was identified as the cause of a 4-litre diesel spill from

the passenger boat Blue Hunter in St Mary’s Harbour on 2 May. A clean-up

operation was completed by harbour staff using absorbent pads.

4.6 Bristol Channel and South Wales (Somerset, Bristol, Gloucestershire,

Monmouthshire, Newport, Cardiff, Vale of Glamorgan, Bridgend, Neath

Port Talbot, Swansea, Carmarthenshire and Pembrokeshire)

4.6.1 Eight accidental oil discharges from vessels were reported during 2011

compared to 6 discharges during the previous year. Seven incidents occurred

in ports or harbours. A source of the pollution was identified on each

occasion, including discharges from 4 fishing vessels, 2 tugs, a dredger and a

barge.

4.6.2 The Milford Haven Port Authority provided details of 6 incidents attributed

to vessels and a source of pollution was identified on each occasion.

Estimated volumes were provided for each of the discharges and the median

volume was 10 litres, compared to 4 litres during 2010. Individual spill

volumes ranged from 3 to 65 litres and clean-up operations were completed

on 5 occasions.

4.6.3 A burst pipe was identified as the cause of a 65-litre hydraulic oil spill from

the backhoe dredger Piet Ponton (262 gt, built 1987) on 16 March. The

resulting surface sheen was broken up and dispersed by propeller wash. A

tank overflow caused a 15-litre diesel spill from the beam trawler Den

Hoope 389 gt, built 2001) at Milford Dock on 8 December. Absorbent pads

were deployed during a clean-up operation following a 10-litre diesel spill

from the MTUG Flying Spindrift (259 gt, built 1986) on 22 March. The

cause of the incident was later identified as a tank overflow during an

internal oil transfer operation.

4.6.4 Elsewhere a POLREP provided details of a hydraulic oil spill from the work

barge Sarah Grey operating off Tenby. The 11-metre fishing boat Flowing

Tide II sank whilst alongside at Fishguard Harbour on 18 November

following an engine cooling water pipe failure. The vessel was later raised

and a small diesel sheen dispersed naturally.

4.7 Irish Sea (Ceredigion, Gwynedd, Isle of Anglesey, Conwy, Denbighshire,

Flintshire, Merseyside, Lancashire, Cumbria, Isle of Man and all

Districts of Northern Ireland)

4.7.1 Respondents identified 8 oil discharges, 2 chemical discharges and a palm oil

discharge from vessels during 2011 compared to 14 oil discharges during the

previous year. Six discharges in ports and harbours were reported, 4

discharges were in the open sea and a beach pollution incident was also

reported. Two spills were attributed to vessel casualties compared to 9 spills

during 2010. Details of accidental spills from oil and gas installations

operating in Morecambe Bay are included in the statistics for the UKCS

(Refer to 4.11).

4.7.2 Five incidents were recorded in the ports and harbours of north-west England

during 2011. An EA report described a 10-litre diesel spill from the MSV

Husvik Supporter (290 gt, built 1978) at Barrow in Furness on 19 May. A

clean-up operation was quickly completed and no further actions were

required. Whitehaven Harbour Commissioners provided details of a 500-litre

diesel spill from a fishing vessel on 20 May. The spill occurred in the non-

tidal North Harbour and a clean-up operation was completed on the same

day. A pleasure craft sank in Glasson Dock on 29 September causing a 1-litre

diesel oil spill. Absorbent booms and pads were deployed in a clean-up

operation and the vessel was later refloated. A split hose was identified as the

source of a 3-tonne palm oil spill from the tanker NCC Amal (29,168 gt,

built 2011) berthed at Liverpool’s Canada Dock and a clean-up operation

was completed. A hydraulic coupling leak under a stern ramp was identified

as the cause of a 50-litre hydraulic oil spill from the Ro-Ro container carrier

Atlantic Cartier (58,358 gt, built 1987) alongside Liverpool’s Seaforth

Terminal on 21 December. Contractors completed a clean-up operation

within the enclosed dock area and no oil entered the River Mersey.

4.7.3 A single incident was reported in the mid-Wales and North Wales area

during 2011. An EA report referred to a 10-litre diesel oil spill after a

pleasure craft sank in Fforyd Harbour on 3 March. The vessel was refloated

on the next incoming tide and the surface sheen dispersed naturally.

4.7.4 POLREPS provided details of 2 chemical discharges and 2 oil discharges in

the open sea. On 11 February a surface sheen was identified trailing in the

wake of the chemical tanker Brovig Bora (2,627 gt, built 2006). Subsequent

communications with the vessel determined a permissible discharge of tank

washings containing residues of the bio-diesel product, fatty acid

methylester. The chemical tanker Stolt Egret (3,853 gt, built 1992) was

identified as the source of another permissible fatty acid discharge on 19

July. On this occasion the surface sheen was detected by a marine pollution

surveillance aircraft and extended for a distance of 16.8 km with a maximum

estimated volume of 605 litres. The Douglas installation reported 2 surface

oil sheens attributed to unidentified passing vessels on 29 March and 18 July

and both were dispersed by wave action.

4.7.5 A Sefton Council report and a POLREP referred to a beach pollution incident

after lumps of oil were washed ashore between Formby Point and Birkdale

on 20 December. A clean-up operation was completed over 2 days and at

least 50 oiled birds including gulls and waders were observed.

4.8 Western Scotland (Dumfries and Galloway, Ayrshire, Inverclyde, Argyll

& Bute and the Highlands & Islands Region to Cape Wrath)

4.8.1 Reporting organisations submitted details of 18 vessel-source pollution

incidents compared to 9 during the previous year. Fourteen incidents in local

ports or harbours were reported and a fishing vessel was identified as the

source of pollution on 13 occasions. Further analysis of the information

contained in annual returns showed 6 oil spills following vessel casualties, 2

more than the total for 2010.

4.8.2 Vessel-casualty spills were recorded during January, June, August and

December 2011. On 31 January the French-registered stern trawler Jack

Abry II (840 gt, built 2005) grounded on the north coast of the Isle of Rum in

heavy weather carrying up to 140 tonnes of marine gas oil and 7,000 litres of

lubricating oil. Some oil was later observed around the vessel and in Kilmory

Bay, the oil being later broken up and dispersed by wave action. On 2

February the skipper of the vessel appeared in Stornoway Sheriff Court and

pleaded guilty to failing to maintain a proper lookout and was fined £3,000.

4.8.3 The fish-farm vessel Lady Laxford partially sank alongside Kinlochbervie

Harbour causing a 100-litre marine gas oil spill. Booms were deployed

around the vessel which was later re-floated. A small fishing vessel sank in

Bowling Harbour on 11 June. The prawn trawler Vellee (103 gt, built 1981)

sank in the middle of the Minch off Benbecula on 6 August and a light fuel

oil sheen and floating debris were later observed near the wreck site. During

adverse weather a decommissioned fishing vessel ran aground at Corpach on

9 December causing a 1-tonne diesel oil spillage. A clean-up operation was

required following a 100-litre diesel spill from a barge that sank in Rosyth

Docks on 21 December.

4.8.4 Reporting organisations identified at least 3 bunkering spills from fishing

vessels in ports during 2011 compared to 2 during the previous year. A

POLREP and a Highland Council Harbours report provided details of a 50-

litre diesel spill from the MFV Belinda in Uig Harbour. Absorbent booms

were deployed during a clean-up operation following a 30-litre bunkering

spill from the MFV Pierre Matigny in Lochinver Harbour on 11 July. The

third bunkering spill from a fishing vessel was recorded in the same location

on 3 August.

4.8.5 The Mallaig Harbour Authority submitted details of 3 oil pollution incidents

recorded during 2011 and the costs of clean-up operations undertaken by

harbour authority staff were charged to the owners of vessels. A Scottish

Environment Protection Agency (SEPA) report identified a burst hydraulic

pipe as the cause of a 20-litre hydraulic oil spill from Island Class ferry Eigg

(69 gt, built 1975) in Oban harbour on 17 February. A clean-up operation

was later completed by harbour staff. A local resident reported diesel oil

leaking from a decommissioned fishing vessel in poor condition berthed in

Rodel Harbour. Another decommissioned fishing vessel was identified as the

source of an oil sheen reported at Glencaple Quay on the River Nith. Booms

were deployed following an accidental discharge of gearbox oil from a

submarine at the Faslane Naval Base on 12 October.

4.9 Orkney and Shetland Islands

4.9.1 Five oil pollution incidents attributed to vessels were reported during 2011

compared to a single incident during 2010. Four incidents occurred in ports

and harbours and another in the open sea. Accidental spills from oil

installations operating in the West Shetland Basin were included in the

statistics for the UKCS (Refer to 4.11).

4.9.2 In the open sea the MFV Reliant grounded on Sule Skerry to the south-west

of the Orkney Islands on 17 March. A light sheen of marine diesel was later

observed drifting downwind of the vessel. At the Sheriff Court in Kirkwall

on 18 January 2012 the skipper of the fishing vessel pleaded guilty of failing

to keep a proper lookout and was fined a total of £1,600.

4.9.3 Three incidents were reported in Shetland during 2011 and on each occasion

the oil dispersed naturally. On 10 April a stern gland leak was believed to

have been the source of a slight sheen trailing behind the MFV Venture in

Sullom Voe Harbour. Another fishing vessel was believed to have been the

source of an oil sheen reported at Tofts Voe Pier on 23 April. A third oil

sheen was attributed to a discharge from the MFV Cornucopia at Brownies

Taing Pier on 22 June.

4.9.4 Orkney Islands Council Marine Services submitted a single report describing

a 200-litre diesel spill from the MFV Obsession in Kirkwall Harbour on 14

April. The oil was later recovered using absorbents in a clean-up operation.

4.10 Eastern Scotland (the northern and eastern shores of the Highlands and

Islands Region from Cape Wrath, Moray, Aberdeenshire, Angus, Fife,

Lothian and Scottish Borders)

4.10.1 Twenty-three discharges from vessels were recorded by reporting

organisations during 2011, six fewer than the previous year’s total. Analysis

of incidents by marine environmental zones showed 18 spills occurred in

ports and harbours, 3 in the open sea and 2 in nearshore waters. The source

of the pollution was identified on 20 occasions, sources including 9 fishing

vessels and 4 support/supply vessels, and there were 13 references to fuel

oils as polluting substances. At least 8 incidents were caused by vessel

casualties involving 7 fishing vessels and a rigid-hulled inflatable boat

(RHIB). No details were provided by respondents of any successful

prosecutions brought by the competent authorities during 2011.

4.10.2 Aberdeen Harbour Board provided details of 7 oil discharges during 2011

compared to 6 discharges during 2010. The source of each discharge was

identified and sources comprised 5 support/supply vessels, one dredger and

one freight ferry. Estimated volumes were provided for 6 discharges and the

median volume was 7.5 litres compared to 5 litres during the previous year.

On 23 September the multi-purpose offshore vessel Skandi Foula (3,252 gt,

built 2002) reported a 1-tonne diesel spill at the North Shell Quay. The area

around the vessel was immediately boomed off and skimmers were deployed

to recover the oil and none was reported outside the dock area. The cause of

the spillage was later identified as severe corrosion and consequently the

vessel was taken into dry dock to undergo repairs. A 30-litre bunkering spill

was reported by the standby safety vessel Grampian Protector (573 gt, built

1983) at the Pocra Quay on 26 April. A clean-up operation successfully

recovered the diesel oil and no further actions were required. The MFV New

Dawn reported a 10-litre diesel spill at the Commercial Quay on 4 October

and the sheen later dispersed naturally.

4.10.3 Four oil pollution incidents were reported by the Montrose Harbour

Authority during 2011 compared with 5 incidents during 2010. All incidents

were caused by accidental discharges of hydraulic oil from barges in

volumes ranging from 5 to 20 litres. A clean-up operation was completed by

contractors after each spill and no further actions were required.

4.10.4 Elsewhere, reporting organisations recorded 7 oil spills in ports and harbours

at Anstruther, Buckie, Cromarty, Dundee, Peterhead and Port Seaton. A Fife

Council report referred to a 100-litre spill of contaminated bilge water in

Anstruther Harbour from the MFV Reaper, which is part of the historic boat

collection managed by the Scottish Fisheries Museum. A clean-up operation

was carried out over 7 days using booms and absorbent materials. On 18

March the MFV Kestrel ran aground in Buckie Harbour causing a 27-litre

spillage of diesel oil. The vessel’s vents were plugged to prevent any further

release of oil and a clean-up operation was undertaken by contractors. Two

incidents were recorded in Cromarty Harbour during 2011 and both were

caused by diesel oil spills following fishing-vessel casualties. Absorbent

mats were deployed in a counter-pollution operation after the steel prawn

trawler Invader sank on 6 December. An oil sheen covering an area of

approximately 80m2 was observed after the prawn trawler Christine sank in

the same location on 29 December.

4.10.5 A SEPA report provided details of a counter-pollution operation in Dundee

Harbour following a diesel oil leak from a sunken former fishing vessel. A

boom was placed across the harbour but this failed to prevent a small

quantity of oil escaping into the River Tay. The MFV Miranda Faye sank at

the entrance to Peterhead Harbour on 9 September and the resulting diesel oil

sheen was broken up and dispersed by wave action. A POLREP described a

similar incident after the MFV Sea Spray sank in Port Seaton Harbour on 26

August.

4.10.6 Reporting organisations identified 5 oil discharges from vessels in the open

sea and in nearshore waters and all were broken up and dispersed by wave

action and posed no threat to the shoreline. A diesel sheen mixed with

flotsam was observed after the MFV Silver Rock sank in rough seas off

Lossiemouth on 24 February. The rigid-hulled inflatable boat (RHIB) North

Coast Explorer sank in Gloup Cave on West Stroma Island in the Pentland

Firth following an engine breakdown. A fishing vessel reported sighting a

rainbow sheen extending for 1 km off North Cove Harbour on 2 August and

on the same day a survey vessel reported sighting a dark slick in the open sea

which was later dispersed by wave action. On 31 August a marine pollution

surveillance aircraft operating in the open sea to the north of Fraserburgh

detected visually and by radar a surface sheen extending for 6 km with an

estimated maximum volume of 28 litres.

4.11 United Kingdom Continental Shelf

4.11.1 The discharge statistics for this enumeration area were again compiled from

POLREP reports submitted by the MCA and PON1 reports submitted by

DECC. Operators are required to notify DECC where an unauthorised

discharge of oil or chemical to sea, regardless of volume, has occurred. Each

discharge is reported on a PON1 to DECC’s Offshore Environmental

Inspectorate, MCA, the Joint Nature Conservation Committee (JNCC) and

Marine Scotland.

4.11.2 DECC reviews every PON1 that is received in order to assess the potential

for environmental impact and the need for any counter-pollution response

actions. In addition, each incident reported is reviewed to ensure actions are

being undertaken to prevent recurrence and to assess whether there has been

any contravention of legislation. This is undertaken in accordance with

DECC's investigation and enforcement policies.

4.11.3 Separate data sets have been presented for accidental discharges attributed to

offshore oil and gas installations and for discharges attributed to vessels,

whether identified or not, operating within the UKCS area (Refer to Figure

2).

Oil and gas installations

4.11.4 All accidental/unplanned discharges or spills of oil or chemicals, regardless

of volume, must be notified to the relevant authorities. Accidental discharges

of operational chemicals being used subsea as part of the exploration and

production process may take place over a significant period of time. In some

cases it may be many months before remedial work can be undertaken due to

the timescales involved in sourcing vessels and equipment to carry out the

work. Environmental risk assessments have shown that these events, which

are normally of relatively small volume, usually cause negligible impact on

the marine environment. Details of all discharges, including a cause when

known, are listed in Appendix 1 (UKCS oil and gas installations) and further

information is provided in Section 3 for those spills of 2 tonnes or more.

4.11.5 A total of 279 accidental oil discharges were attributed to oil and gas

installations during 2011 including those operating in the west Shetland

Basin, Liverpool Bay and off the coast of eastern England. The 2011 annual

total was the same number as the mean annual total of 279 accidental oil

discharges reported between 2000 and 2010.

4.11.6 Analysis of oil types showed that 33% of reported discharges were identified

as fuel oils, followed by 32% lubrication and hydraulic oils and 23% crude

oils. Estimated volumes for 279 oil spills were reported (including 3 ongoing

releases) and of these approximately 94% were less than 455 litres.

4.11.7 In addition, the authorities were notified of 240 accidental discharges of

chemical substances during 2011 compared with 167 discharges during the

previous year. These chemicals are routinely used in oil and gas exploration

and production operations and included barite, glycol solutions, methanol

and sodium.

4.11.8 The estimated volumes of accidental chemical discharges ranged from 0.001

kg to 189,840 kg and 44 discharges were 1 tonne or more. It should be noted,

however, that many of the larger discharges associated with operational

chemicals being used sub-sea occurred over an extended interval due to the

requirement for operators to source vessels and equipment to locate the

source of the release and undertake repairs. Further details are provided in

Section 3.

4.11.9 Of the total chemicals spilled in 2011 over 80% were considered under the

OSPAR list of substances used and discharged offshore as Posing Little or

No Risk to the Environment (PLONOR). None of the chemicals spilled are

included in the OSPAR list of chemicals for priority action, which are

considered to pose the greatest hazard. None of the spill incidents resulted in

a significant environmental impact.

4.11.10 The PON1 reports again indicated that remedial actions were taken by

operators following most accidental discharges. The responses included

identification of root causes of spills and improvements in operational

control procedures, recommendations concerning preventative actions and

carrying out any necessary repairs and modifications to faulty or damaged

equipment.

Vessels

4.11.11 A further 24 incidents were identified in POLREPS and PON1s. Most

involved surface sheens or thicker oil slicks reported by the nearest offshore

installation but attributed to a discharge from a ‘third party’ elsewhere that

was generally assumed to have been a passing vessel. The maximum

estimated volumes, where known, ranged from less than 1 litre to 25 tonnes.

In each incident the pollution was believed to have dispersed naturally

without posing a threat to wildlife or the coastline.

4.11.12 On 21 November a surface slick occupying an area of approximately 5 km2

with an estimated maximum volume of 25 tonnes was reported 5 km to the

east of the Byford Dolphin Mobile Offshore Drilling Unit (MODU). The

Balmoral installation and a passing fishing vessel reported a surface slick

with an estimated maximum volume of 10 tonnes on 25 August. Another

surface sheen occupying an area of approximately 15 km2 was observed

drifting towards the Brent Delta installation.

4.11.13 The multipurpose offshore installation vessel Saipem 7000 (117,812 gt, built

1987) reported a 29-litre hydraulic oil spill following a hose connector

failure on 3 June. On 12 June the MSV Vos Challenger (1,217 gt, built

1975) reported a 200-litre fuel oil spillage to sea through a purifier vent. The

oil was later broken up and dispersed by wave action. A burst line was

identified as the cause of a 4-litre biodegradable hydraulic oil spill from the

research/survey vessel Fairfield New Venture (2,192 gt. built 1986).

SECTION 5: QUALITY ASSURANCE PLAN AND

ACKNOWLEDGEMENTS

5.1 Quality Assurance Plan

As part of a comprehensive quality assurance plan data verification procedures were

again applied at all stages during the 2011 survey. For example, other sources of

information were consulted including the casualty section of Lloyd’s List, and any

relevant reports were cross-referenced with the appropriate questionnaire returns.

The approaches for data entry and analysis were clearly shown in the relevant tables

and all entries to the enumeration area sub-sets were subject to independent checking

to detect any errors. On some occasions unexpected values were confirmed with the

various data-sources. Multiple reports were again received from DECC and MCA

describing accidental discharges from oil and gas installations operating on the

UKCS. For the remaining discharges from vessels 28% of individual incident reports

were compiled from questionnaire returns submitted by two or more reporting

organisations which was 6% more than the previous year.

As part of the quality assurance plan the complete database is provided for scrutiny in

Appendix 1 thereby enabling stakeholder groups to determine the reliability and

accuracy of the survey results. Furthermore, the annual survey reports are made

available to all interested parties through the MCA's website (www.dft.gov.uk/mca)

and the ACOPS website (www.acops.org.uk).

MCA/ACOPS ANNUAL SURVEY OF VESSEL/OFFSHORE

SOURCED MARINE POLLUTION

GUIDELINES TO AID COMPLETION OF THE SURVEY FORM

The survey questionnaire requests information on incidents involving a range of

pollutants including mineral oil (MARPOL I), noxious liquid substances (MARPOL

II), packaged dangerous/harmful goods (MARPOL III) and garbage (MARPOL V).

Reports are requested for pollution incidents attributed to discharges, or suspected

discharges, from vessels and offshore installations operating within the UK Pollution

Control Zone, harbour waters and national waters. Information is not required for

pollution incidents attributed to land-based sources such as industrial premises,

sewers and outfalls.

Annual returns can be made either by email or by post. If using email please click on

the boxes provided and insert other information in the spaces provided. The email and

postal versions of the survey form can be downloaded from the ACOPS website at

www.acops.org.uk (under ACOPS Reports on the main menu) together with previous

survey reports.

Please complete a separate form for each incident. Thank you for your continued

support in providing this important information.

Question Guideline

1 Please insert name of reporting organisation. If you have nothing to report

please tick the no pollution to report box, add respondent details (Question

17) and return the questionnaire to the address shown.

2 Identify the category of vessel/offshore sourced pollution from the options

given (tick appropriate box). Note that in the case of garbage, an incident

report is requested only in circumstances involving an illegal or accidental

discharge from a specific vessel/offshore installation. An incident report is

not required following observations of litter at sea or on beaches from an

unknown source.

3 Insert dates of the beginning and end of the incident.

4 Please identify one or more zones affected by the pollution (tick

appropriate box). The shoreward boundary of the open sea zone is defined

as 3 nautical miles (5.6 km) from nearest land.

5 State how long the pollution remained in days or weeks (insert values).

6 Identify the location of the incident using geo-coordinates, national grid

reference, place name or other suitable method (insert details).

7 If known, insert the estimated volume of pollution in litres, tonnes or cubic

metres. Please state any other units of volume used.

8 Please indicate the overall distance in km, or state the names of settlements

at the extremities, of the affected stretch of coastline.

9 Indicate whether a vessel or offshore installation was identified as a source

of pollution or simply suspected on the basis of available evidence (tick

most appropriate box from the options given). Note that incidents

attributed to land-based sources of pollution are not included in this

survey.

10 Please identify the most appropriate category which indicates the scale of

pollution at sea or deposited on the shoreline (tick appropriate boxes).

11 If known, identify the type of vessel or offshore installation causing the

pollution (tick one of the boxes to select the appropriate response to the

options given).

12 Please state the name of the vessel/offshore installation, whether UK or

foreign registered and operating company if known (insert details in the

spaces provided).

13 Identify the category and type of pollution from the options given (tick the

appropriate box or insert relevant details in the spaces provided).

14 Indicate whether a partial or complete clean-up operation was undertaken

(Circle appropriate option).

15 Please add details of any legal proceedings initiated against alleged

offenders including fines and penalties (identify options as indicated and
