Welcome to the Shared Savings Program Annual Training, provided by the Department of Management Services’ Division of State Group Insurance 1.2

In this interactive training, you will learn about the State of Florida’s Shared Savings Program and how it can benefit you and your family. There will be questions throughout the training as well as at the end, to ensure you understand the differences between the different benefits. You must make 80% to pass. At the end of the training, if you earned 80% or higher on the quiz questions, you will be prompted to fill out a quick compliance survey to show you completed the mandatory training. If you make less than 80%, you will need to retake the training until you make 80% or higher. This training is narrated, so check your speakers to ensure they work. On each slide there is a pause and a play button for you to utilize should something interrupt your training. For any resources mentioned in this training, such as the ADA compliant script for this training, you can find them in the Resources tab in the upper right-hand corner of the training module.

In January 2019, the State of Florida began offering a Shared Savings Program to reward you for making informed decisions about your healthcare. The Shared Savings Program is a voluntary program that is available to you and your dependents who are enrolled in a State Group Insurance health plan. The purpose of the Shared Savings Program is to reduce healthcare costs and reward you for making informed and cost-effective decisions about your healthcare. 1.4

So why shop for healthcare? Did you know that in-network prices for the same procedure can vary by over 500%, depending on the facility you choose? We are taught to shop and compare prices for everything, but so many of us do not realize we can shop for our healthcare. Most people do not realize there is a 3 to 10 times variance in cost and quality for in-network healthcare procedures.

Healthcare Bluebook provides an online transparency tool that empowers you as the employee to make informed decisions regarding your healthcare, and allows you to ShopSmart for non-emergency healthcare procedures based on cost and quality. First, you use Healthcare Bluebook to “shop” for a rewardable healthcare procedure. Please Note: Not all procedures are rewardable. Look for the ‘Go Green to Get Green’ tile. 1.12

Second, you receive the healthcare procedure at a Green Facility.

And third, Healthcare Bluebook uses your search history to validate that you received a rewardable procedure by using its online transparency tool prior to your procedure.

After Healthcare Bluebook validates that you shopped for, and used a green facility, your reward will be credited to your Chard Snyder account within three to four months from the procedure completion date. 1.15

Even your family members who are participating in a State Group Insurance health plan can shop and earn rewards. Please note: rewards are always paid to the primary account holder’s savings and spending account.

SugeryPlus is a voluntary benefit that provides pre-planned, non-emergency surgical procedures. SurgeryPlus is separate from your health plan and has its own network of high-quality providers. Surgery Plus offers surgical procedures that are covered under the State Group Health Insurance Program but is different from your health plan because the healthcare services are “bundled” together. For example, rather than paying separately for the surgeon, facility, anesthesiologist, and radiologist, SurgeryPlus negotiates one “bundled” rate. By packaging surgical expenses into one simple, bundled rate, SurgeryPlus is able save money for you and the State of Florida.

It’s important to note: you will never pay more going through Surgery Plus than you would using your traditional healthcare coverage. As mentioned, Surgery Plus network includes only top-quality surgeons of excellence in their field and the credentialing process is taken very seriously. This means that you will receive the best quality care for your procedure. Surgeons of excellence credentialing includes:

- Board Certification, which requires an active state medical license - Only experienced Surgeons- this means Surgery Plus seeks out surgeons with high procedure volume and low

complication rates - Surgical Fellowship which requires additional specialized training other than residency that is focused on a

unique specialty - Background checks that check for disciplinary action from state medical board and criminal records - Last a Malpractice review is conducted on any claims against the surgeon based on merit.

When you contact Surgery Plus you will be assigned a dedicated Care Advocate that will help you throughout your entire surgical experience. Each time you call, you will be directed to your care advocate, so they know your situation, and can anticipate your needs. Your care advocate will take the administrative burden away from you so all you have to do is focus on feeling better. Your care advocate will recommend at least three best fitting surgeons of excellence, book all of your appointments, transfer medical records, ensure you have all of your needs met and are completely satisfied. Surgery Plus has a large network across the United States, and is continuing to add more surgeons of excellence within their network.

In the event there is not a provider near you, SurgeryPlus offers a travel benefit. This benefit includes:

- Mileage reimbursement - Hotel if needed - And per diem per day for you and a companion.

Prior to any of your appointments, your care advocate will mail you a pre-loaded debit card, so you do not have to be responsible for any out-of-pocket expenses. 1.26

SurgeryPlus covers over 350 surgeries. The most common covered procedures, ranging from ankle replacement to pacemaker device replacement. SurgeryPlus does not cover:

- Brain Surgery - Vision - Dental - Maternity, or - Cancer

By utilizing Healthcare Bluebook and SurgeryPlus, you can earn financial incentives. For Healthcare Bluebook, your financial incentive can range from $25-$5,000! For SurgeryPlus, your financial incentive can range from $500-$6,000! The savings and spending account selections available to you are based on your health plan. Employees enrolled in the standard plan can choose from a Health Reimbursement Account (HRA) or a healthcare Flexible Spending Account (FSA). Employees enrolled in the High Deductible Health plan can choose from a Health savings account (HSA), a Post-deductible health reimbursement account, or a limited purpose FSA. 1.33

You select a savings and spending account to which your rewards will be credited. Rewards will automatically be credited to the savings and spending account of your choice after you receive a rewardable healthcare service. You may make your selection through People First at any time throughout the year.

You do not have to make a saving and spending account election to earn a reward. Chard Snyder will open an account for you based on your health plan and send you a welcome packet including your Chard Snyder Benefit Card. If you have a Standard Health Plan and do not make a selection before you earn a reward the funds will go to an HRA automatically set up for you by Chard Snyder. If you have a High Deductible Health plan and have not reached your maximum for HSA your rewards will be credited to your HSA then to a post deductible HRA. You can find specific rules and guidelines pertaining to each of these plans on the Saving and Spending Accounts tab of the myBenefits website. Mybenefits.myflorida.com 1.35

The rewards that are earned and deposited into your savings and spending account can be used on eligible expenses.

• Paying copays each month for doctor visits or prescription drugs? • Maybe you have a child that needs braces or glasses, and you’re just not sure how you can afford them. • Maybe you need to purchase hearing aids or diabetes medication. • Maybe you’ve been wanting to try acupuncture. Use your rewards!
