Important Announcement: All proposals and attachments shall be submitted through a designated secure Sharepoint Site and not on HlePRO, See change 3 and 8. Subcontractors wishing to submit financial information separately from the offeror, may do so through a secure Sharepoint site issued by the SPO, see change 3 and 9.

The following changes are made to the RFP:

1. Replace Addendum 9, in its entirety, to include missing pages 2 and 4.

4. Change Section 1.9.1 Use of HlePRO:

The Hawaii eProcurement System ("HlePRO") [pronounced "Hi-Pro"] is an electronic system used by the State in the RFP Process. The State will use HlePRO to issue the RFP, recei·.'e Offers, and issue Addenda to the RFP. The State will also use HlePRO to receive the written questions regarding the RFP and to issue responses to such questions. Such responses shall be issued through HlePRO in the form of Addenda to the RFP.

6. Delete Section 1.9.4:

If a ContraGt is terminated or i:esoineled through no fault of Contraotor at the beginning of the ContraGt or within si>Ety (60) days of the Notioe of Award, and no amount paid on the ContraGt, Contraotor will become eligible to reoei¥e a oredit of suoh administrati¥e fees paiel for and to ContraGtor's aooount 'Nith HIC. To reoei¥e the oredit, Contraotor must make a request to HIC for a oredit.

7. Change Section 1.9.5 Electronic Submission Of Proposals:

a) Offeror's Proposal shall be submitted and received electronically through HlePRO a designated secure SharePoint site. This electronically submitted Proposal shall be considered the original. Any Proposals received outside of the designated secure SharePoint site, including faxed or e-mailed Proposals, shall not be accepted or considered for award. Any Proposal received after the due date and time shall be rejected.

b) Offeror shall re¥i&'N all speoial instruGtions looated on the HlePRO solioitation. Offerors are responsible for ensuring that all necessary files are included in their response upon submission in HlePRO by the due date and time.

c) Offerors are advised not to wait until the last minute to submit their Proposal in HlePRO the designated secure SharePoint site. Offerors should allow ample time to review their submittals on HlePRO, including all attachments, prior to the due date/time. Submission must be completed and submitted by due date and time. If submission is not completed and submitted by the due date and time, HlePRO SPO will not accept the offer.

• Submit an intent to offer via email that includes: o Name of Company o Physical Address o Phone Number o RFP Number and Title o Contact information for person who will be submitting proposal and

attachments, (one person per offeror) to include: • Name; • Email Address; and • Phone Number

• Submit the intent to offer by the date and time designated in Section 1.6. • Submit intent to offer by email to: state.procurement.office@hawaii.gov

9. Add to Section 1.9.5 Electronic Submission of Proposals as follows:

f) Subcontractor Submission of Financials

Subcontractors wishing to submit financial information separately from the offeror, may do so through a secure Sharepoint site issued by the SPO. Each subcontractor shall be issued a single account (login/password) to submit financials upon following instructions below:

• Submit a request to submit financials separately via email that includes: o Name of Company submitting financials o Physical Address o Phone Number o RFP Number and Title o Name of Primary Contractor o Contact information for person who will be submitting financials, (one

person per subcontractor) to include: • Name; • Email Address; and • Phone Number

• Submit request by the date and time designated in Section 1.6. • Submit request by email to:

10. Change Section 3.12.1 Proposal Format:

a) Proposals shall be submitted as attaohments on HlePRO.

b) Files should be in MS Word format and compatible with MS Word 2007; and for cost submission forms and other calculated data in Excel format for Cost Submission form and other calculated data. Files uploaded to HlePRO secure SharePoint site will remain iR HlePRO in their original format.

11. Change Section 3.13.1:

Proposals will be •1iewed in HlePRO on or after the date and time speoified in &eGtion 1.1, RFP Sohedule and Signifioant Dates, or as amended. Proposals shall not be made available to the public and shall be received and receipt verified by two or more procurement officials.

12. Change Section 3.14 Best and Final Offer (BAFO):

If the Do TAX determines a BAFO is necessary, it shall request one from each priority listed Offeror. Offeror shall submit its BAFO electronically through tRe HlePRO the designated SharePoint site. Any BAFO received after the deadline or not received in the designated SharePoint site shall not be considered. Priority listed Offerors shall also be informed that if they do not submit a notice of withdrawal or a BAFO, their immediately previous offer will be construed as their BAFO.

13. Change Section 3.15.2:

Any changes, to the Proposal may be made prior to the Proposal Due Date and time.

14. Change Section 5.7.1 :

15. Change RFP 13-013-SW Appendix A Section A 1.1.2:

A 1.1.2 Offeror's authorized signature on the Offer Form, OF-1 shall be an original signature in ink, which shall be required before an award, if any, can

be made. Ink signatures are not required for electronic submission of the Proposal on HlePRO but shall be required upon Notice of Award. The submission of the Proposal on HlePRO shall indicate Offerer's intent to be bound.

16. Change RFP 13-013-SW Appendix A Section A2.1.3:

A2.1.3 Offerer's authorized signature on Attachment 1: Offer Form OF-1 shall be an original signature in ink, which shall be required before an award, if any, can be made. Ink signatures ·are not required for electronic submission of the Proposal on HlePRO but shall be required upon Notice of Award. The submission of the Proposal on HlePRO shall indicate Offeror's intent to be bound.

17. Change RFP 13-013-SW Appendix A Section A2.1.4:

A2.1.4 Pricing shall be submitted on Attachment 2: Offer Form OF-2. The price shall be the all-inclusive cost, including the GET. No other costs to the State that are related to this RFP will be honored. Any unit prices shall be inclusive including GET. Grand Total should be submitted in HlePRO and match the Grand Total in Attachment# 4, Cost Summary Worksheet Tab.
