B. Executive Summary Diversified services Network, Inc. (DSN) would like to thank the State of New Mexico, General Services Department/State Purchasing Division for this opportunity to respond to the Request for Proposal (RFP) for establishing Master Agreements with qualified offerors to provide Temporary Employment Services. Highlight of the major features of the proposal and meeting requirements:

DSN’s proposal is for Information Technology Professional Services category as defined within the scope of this RFP for the State of New Mexico in conjunction the all Participating States.

DSN meets all requirements as defined within the scope of this RFP for the IT Temporary Services category.

DSN has responded to each item in “Section 6” in this RFP and meets all the requirements

DSN has responded to each item in the Business Requirements section 6.7 in this RFP and meets all the requirements

DSN has responded to each item in the Technical Requirements, section 6.8 “Attachment B: Scope of Work” in this RFP and meets all the requirements

DSN acknowledges that we have read, understands and accepts all the terms and conditions of the Master Agreement that includes, but not be limited to, the NASPO ValuePoint Standard Terms and Conditions in Attachment A, the New Mexico, the Lead State specific terms and conditions required to execute a Master Agreement in Attachment E, the Scope of Work in Attachment B and selected portions of DSN’s Proposal.

C. Insurance Requirements (Section 6.3)

6.3 Insurance To be eligible for award, the Offeror agrees to acquire insurance from an insurance carrier or carriers licensed to conduct business in each Participating Entity’s state at the prescribed levels set forth in Section 21 of the NASPO ValuePoint Master Agreement Terms and Conditions. Describe your insurance or plans to obtain insurance satisfying the requirements in Section 21. Diversified Services Network, Inc. (DSN) utilizes “The Hartford” insurance which is licensed to conduct business nationwide. DSN’s certificate of insurance is at the prescribed levels set forth in Section 21 of the NASPO ValuePoint Master Agreement Terms and Conditions. Campaign Contribution Disclosure Form Pursuant to NMSA 1978, § 13-1-191.1 (2006), any person seeking to enter into a contract with any state agency or local public body for professional services, a design and build project delivery system, or the design and installation of measures the primary purpose of which is to conserve natural resources must file this form with that state agency or local public body. This form must be filed even if the contract qualifies as a small purchase or a sole source contract. The prospective contractor must disclose whether they, a family member or a representative of the prospective contractor has made a campaign contribution to an applicable public official of the state or a local public body during the two years prior to the date on which the contractor submits a proposal or, in the case of a sole source or small purchase contract, the two years prior to the date the contractor signs the contract, if the aggregate total of contributions given by the prospective contractor, a family member or a representative of the prospective contractor to the public official exceeds two hundred and fifty dollars ($250) over the two year period. Furthermore, the state agency or local public body shall void an executed contract or cancel a solicitation or proposed award for a proposed contract if: 1) a prospective contractor, a family member of the prospective contractor, or a representative of the prospective contractor gives a campaign contribution or other thing of value to an applicable public official or the applicable public official’s employees during the pendency of the procurement process or 2) a prospective contractor fails to submit a fully completed disclosure statement pursuant to the law. THIS FORM MUST BE FILED BY ANY PROSPECTIVE CONTRACTOR WHETHER OR NOT THEY, THEIR FAMILY MEMBER, OR THEIR REPRESENTATIVE HAS MADE ANY CONTRIBUTIONS SUBJECT TO DISCLOSURE. The following definitions apply: “Applicable public official” means a person elected to an office or a person appointed to complete a term of an elected office, who has the authority to award or influence the award of the contract for which the prospective contractor is submitting a competitive sealed proposal or who has the authority to negotiate a sole source or small purchase contract that may be awarded without submission of a sealed competitive proposal. “Campaign Contribution” means a gift, subscription, loan, advance or deposit of money or other thing of value, including the estimated value of an in-kind contribution, that is made to or received by an applicable public official or any person authorized to raise, collect or expend contributions on that official’s behalf for the purpose of electing the official to either statewide or local office. “Campaign Contribution” includes the payment of a debt incurred in an election campaign, but does not include the value of services provided without compensation or unreimbursed travel or other personal expenses of individuals who volunteer a portion or all of their time on behalf of a candidate or political committee, nor does it include the administrative or solicitation expenses of a political committee that are paid by an organization that sponsors the committee. “Family member” means spouse, father, mother, child, father-in-law, mother-in-law, daughter-in-law or son-in-law.

“Pendency of the procurement process” means the time period commencing with the public notice of the request for proposals and ending with the award of the contract or the cancellation of the request for proposals. “Person” means any corporation, partnership, individual, joint venture, association or any other private legal entity. “Prospective contractor” means a person who is subject to the competitive sealed proposal process set forth in the Procurement Code or is not required to submit a competitive sealed proposal because that person qualifies for a sole source or a small purchase contract. “Representative of a prospective contractor” means an officer or director of a corporation, a member or manager of a limited liability corporation, a partner of a partnership or a trustee of a trust of the prospective contractor. Response to Business Requirements (Section 6.7) SECTION 6: Administrative and Technical Response Requirements 6.1 Mandatory Minimum Administrative Proposal Requirements This section contains the minimum requirements that must be met in order to be considered for the evaluation phase. All of the items described in this section are non-negotiable. All Offerors must state willingness and demonstrate ability to satisfy these requirements in the proposal submitted for consideration. Diversified Services Network, Inc. (DSN) is committed to meet and satisfy all the requirements required in the RFP number 50-000-15-00058 for IT Temporary Services and has demonstrated its ability in this proposal. DSN accept all items in this section without exception. 6.2 NASPO ValuePoint Master Agreement Acceptance NASPO ValuePoint Master Agreement(s) resulting from this RFP will constitute the final agreement except for negotiated terms and conditions specific to a Participating States Participating Addendums. The Master Agreement will include, but not be limited to the NASPO ValuePoint Standard Terms and Conditions in Attachment A, the Lead State specific terms and conditions required to execute a Master Agreement in Attachment E, the Scope of Work in Attachment B and selected portions of the Offerors Proposal. This section highlights particular terms and conditions of NASPO ValuePoint Master Agreement Terms and Conditions, Offerors will be bound to all the terms and conditions when executing a Master Agreement as shown in Attachment A, Offerors must acknowledge in the Letter of Transmittal, Attachment E, Item 10, Question #10, they have read and understand all of the terms and conditions as shown in the Master Agreement Attachment A. DSN understands that the Master Agreement will include, but not be limited to the NASPO ValuePoint Standard Terms and Conditions in Attachment A, the Lead State specific terms and conditions required to execute a Master Agreement in Attachment E, the Scope of Work in Attachment B and selected portions of the DSN’s Proposal. DSN has read and understood NASPO ValuePoint’s terms and Conditions (Attachment A), the New Mexico’s Additional Terms and Conditions (Attachment E) and acknowledged in the Letter of Transmittal, Attachment E, Item 10, Question #10, that DSN has read, understands and agree to all of the terms and conditions as shown in the Master Agreement Attachment A. We understand that include, but not be limited to the NASPO ValuePoint Standard Terms and Conditions. DSN takes no exceptions for any of the terms and conditions in Attachment A and Attachment E. 6.3 Insurance To be eligible for award, the Offeror agrees to acquire insurance from an insurance carrier or carriers licensed to conduct business in each Participating Entity’s state at the prescribed levels set forth in Section 21 of the NASPO ValuePoint Master Agreement Terms and Conditions. For over 27 years, DSN has been insured to cover to all the prescribed level set by our customers. DSN’s insurance is with Harford Insurance Company. DSN is committed to acquire insurance from an insurance carrier or carriers licensed to conduct business in each Participating Entity’s state at the prescribed levels set forth in Section 21 of the NASPO ValuePoint Master Agreement Terms and Conditions. 6.4 NASPO ValuePoint Administrative Fee and Reporting Requirements To be eligible for award, the Offeror agrees to pay a NASPO ValuePoint administrative fee as specified in Section 6 of the NASPO ValuePoint Master Agreement Terms and Conditions. Moreover, specific summary and detailed usage reporting requirements are prescribed by Section 7 of NASPO ValuePoint Master Agreement Terms and Conditions. Offerors shall identify the person responsible for providing the mandatory usage reports in the Letter of Transmittal, item 5. (This information must be kept current during the contract period). Contractor will be required to provide reporting contact within 15 days of Master Agreement execution. DSN agrees to pay a NASPO ValuePoint administrative fee as specified in Section 6 of the NASPO ValuePoint Master Agreement Terms and Conditions. DSN’s report shall comply with all the detailed usage reporting requirements are prescribed by Section 7 of NASPO ValuePoint Master Agreement Terms and Conditions. DSN has identified the person responsible for providing the mandatory usage reports in the Letter of Transmittal, item 5. DSN is committed to provide reporting contact within 15 days of the Master Agreement execution. 6.5 Lead State Terms and Conditions Refer to Attachment E for the Lead State Special Terms and Conditions that apply to this solicitation. Offeror shall indicate in its Letter of Transmittal, Attachment B, Item 10, Question #5, that they have read and understand all of the requirements shown in the Lead State Terms and Conditions. DSN has read and understands the entire requirements in Attachment E for the Lead State Special Terms and Conditions that apply to this solicitation and indicated it in the letter of transmittal 6.6 Participating State Terms and Conditions In addition to the Lead State conducting this solicitation, the following Participating States have requested to be named in this RFP as potential users of the resulting Master Agreement: Hawaii, Illinois, Louisiana, Nevada, New Mexico, Oregon, Utah, Vermont, Virginia, and Washington. Other entities may become Participating Entities after award of the Master Agreement. Some States may have included special or unique terms and conditions for their state that will govern their state Participating Addendum. These terms and conditions are being provided as a courtesy to proposers to indicate which additional terms and conditions may be incorporated into the state Participating Addendum after award of the Master Agreement. The Lead State will not address questions or concerns or negotiate other States’ terms and conditions. The Participating States shall negotiate these terms and conditions directly with the supplier. State-specific terms and conditions are included in Attachment J. DSN understands that some States may have included special or unique terms and conditions for their state that will govern their respected state’s Participating Addendum. unexpected problems and issues that crop up. When these issues arise, DSN has the experience and the processes to deal with them promptly with right solution ensuring complete customer’s satisfaction. DSN has processes in place to deal with any unexpected issues quickly and effectively, thus mitigating any impact on the customer. DSN management is keenly aware that unresolved issue could create unnecessary risk conflicts and delays of engagement. DSN’s Policy and Procedure Our policy focuses on customer satisfaction and our process starts immediately as a soon as DSN is notified of an assignment where an issue has occurred.

DSN’s Account Manager, and/or other senior managers, will be available to meet with the State of New Mexico and/or a Participating States’ agency in the same day, and typically within a few hours of being notified of an issue. DSN works to establish local offices that are located within at most an hour away from most State agencies and staffed with an Account Manager who is dedicated for that State’s IT Temporary Services contract. At a bare minimum, DSN will establish voice contact within one hour of being notified of an issue occurring with any assignment.

DSN’s Account Manager will promptly meet customer’s counterpart early on to evaluate the issue and discuss ways to remedy the problem and agree on expectations that become the next step for DSN.

Should any customer issue not be resolved to the customer’s satisfaction, the issue is immediately escalated to the DSN’s Vice President and/or DSN’s Director of Operation to provide an immediate resolution for the issue in a short response time.

In the event that a resource is needed to be terminated from the engagement,

DSN’s Account Manager, and/or other senior management will be available to meet with the State of New Mexico and NASPO Value Point representative on the same day to address any concern and take any immediate corrective action.

If customer requests to terminate a resource immediately, a DSN representative will be on hand to talk to the resource and escort him/her outside the work place in a professional manner such that it does not disrupt the work environment. DSN will counsel the resource outside the work place and ensure that all customer policies in addition to DSN’s HR policies are followed. DSN will return any customer’s belonging to the designated state representative.

In case customer gives an advance notice for terminating a resource, DSN will follow customer’s policies in handling the termination of the resource and ensures seamless transition of work. DSN will provide appropriate follow-up action items, documented by management reports, to mitigate any impact on the assignment.

DSN has the resources ready to backfill any vacancy in timely and professional manner. We have a proven capability to rapidly recruit, train and deploy candidates to meet the customer’s requirements. DSN shall prioritize the need for a replacement candidate, and the Account Manager will escalate the matter to DSN senior management. DSN management will act immediately on the State Agency’s request to ensure that a replacement is found within the time specified by the state agency

In addition, DSN’s policy requires the Account Manager to prepare and present a final briefing to customer in the Post Engagement Assessment Meeting to address the results of individual Task Orders. In addition DSN uses HireRight for Background Screening for each person before the start of their employment with DSN. DSN recruits local employees, thus eliminating travel costs, duplicate residences and reducing employee turnover. If local employees are not found, the search expands to other avenues available in our recruitment system to find suitable personnel to fill the requirements. candidates’ performance varies from task to task but the general organization structure continues to follow our traditional strategy, allowing for DSN accountability for its agreed-upon performance. Our approach to monitor candidates’ performance is to work closely with our customer through the formation of a strong project team, dedicated to technical excellence and customer satisfaction. For effective performance monitoring of candidates and per customer’s policies and procedures, DSN’s management recommends periodic face-to-face performance review meetings with customer’s procurement agency or project leads to determine the status of all assignments’ activities.

DSN conducts candidates’ performance evaluation as follows:

DSN’s Account Manager and Director of Talent Acquisition initiate Performance Monitoring Evaluation annually or when an assignment ends. The main considerations in the performance monitoring and evaluation is customer feedback in assessing candidates’ quantity of work, work habits, interpersonal relations and adaptability to job conditions.

The goal of DSN’s Internal Performance Monitoring and Evaluation is to ensure retaining the best qualified resources to meet and exceed customer expectations in addition to the mutual benefit of DSN and its resources. In addition, DSN’s management utilizes Performance Monitoring Evaluations in decisions about employment conditions, training, merit pay increases, job assignments, Resource development, promotion and retention.

The process starts by DSN’s employee or subcontractor conducting a self assessment. The purpose is to provide the opportunity for employees/subcontractor to evaluate their own performance and express how they think they’ve performed without being influenced by their supervisor’s judgments.

Continuous Improvement Review: This process is linked to DSN’s quality improvements and productivity training policy. DSN has a continuous improvement program which offers employees IT classes to enhance employee growth and keep them abreast of changes in technology. These classes are offered in the evening and weekend free of charge to employees. These classes are taught by local colleges exclusively designed for DSN employees. DSN provides extra training to employees if it is beneficial to our customers and required to learn or enforce technical skills. In addition, DSN offers tuition reimbursement for continuing education courses.

It is very important for DSN to ensure that all of the resources that we provide our customers meet and exceed the requirements set by them for their project needs. Over our 27 year history in providing professional IT resources to our customers, DSN has developed a strong network of thousands of readily accessible resources with pre-verified skills. All of these resources have been entered into our internal database of resources to allow DSN to quickly locate and place the best available IT resource with our customer. The majority of the resources that DSN provides to our customers come from this pre-verified pool of resources, or directly from within our own employee base. As a part of DSN’s candidate qualification screening process with any potential candidate, DSN recruiters perform a detailed review of each potential candidate’s resume, paying particular attention to the technical skills and specific technology platforms that the candidate references in his/her resume. The DSN recruiter then begins a potential candidate’s resume assessment process, which includes reviewing each applicable technical skill and skill level required by the customer need, and determines the years of experience with each skill, the level of skill that the candidate has with that skill, and reviews how/when/where that candidate achieved that level of skill, and whether or not that candidate has any certifications proving that skill level. This initial assessment of the candidate’s skills is done during the initial candidate pre-screen interview, either by phone or via Skype. During this initial skills evaluation process with the potential candidate, the DSN recruiter reviews the candidate’s resume in detail, ensuring that each customer required or desired skill is fully addressed in the candidate’s resume. Validating technical training and skill development of Resource. Subsequent to the candidate initial skills evaluation, the DSN recruiter begins the verification process of determining the validity of the candidate’s claimed skills and skill levels. This verification process begins with following up on candidate provided references and ensuring that the skills and skill levels claimed on the resume are verified by the reference as being accurate. DSN uses a third party (HireRight) to verify applicant's current and previous work history, including dates of employment; job titles, responsibilities and reasons for leaving, attendance employee rating, eligibility for rehire

DSN recruiter will review any training courses that the candidate may list on his/her resume and request certificates of completion if available. If the candidate has stated on their resume that they have been certified in any skill by a nationally or internationally known certification organization, the DSN recruiter will then verify that the certification actually was issued and whether or not the certification is still valid. All verifications and validations steps are documented and included with candidate credentials. Once all of these items have been verified and documented, and if the candidate is deemed to be a potential match for a particular customer need, the DSN recruiter will then submit the candidate for initial management review. The initial DSN management review process consists of a review of the candidate resume and skills matrix by DSN’s Director of Talent Acquisition to ensure that nothing in the initial screening process has been overlooked. Once a candidate has passed the initial management review process, the candidate is presented for specific technical review. Depending upon the specific technology skills mix, DSN may conduct this technical review utilizing DSN internal resources, or it may use the services of an independent third-party technical assessment organization, to conduct the candidate technical review. During this technical review process, the candidate’s skills are evaluated independently of the recruiting process to provide an un-biased technical opinion of the true skill level of the candidate. DSN uses a variety of methods to conduct these technical reviews, including online tests and verbally provided tests. DSN, at times, uses the services of Derrico Computers, and other independent third-party testing organizations, to provide even a more detailed skills evaluation of many of the newer or emerging technologies to ensure that candidate skill levels meet the customer requirements.

Once a candidate has completed DSN’s technical review process, the candidate is re-assessed to ensure that they are fully capable of meeting the requirements of our customer’s project. Once that re-assessment is completed, then the candidate is presented to the Account Manager in charge of the customer account for final approval of the candidate and to submit to the customer. The Account Manager of the customer account is DSN’s final arbiter of the quality of candidate’s submittal to the customer. Skills Development: Whereas we have a stringent initial technical review process for potential candidates to be submitted for any DSN customer opportunity, we also have ongoing annual reviews of our consultants to ensure that their skills are maintained and improved upon. At DSN, our consultants are our biggest asset. The ongoing attention to the development and welfare of our consultants over the years has allowed DSN to grow and remain in business for over 27 years. As part of our annual review process, we set goals for each consultant to continue to develop their skills further, and to acquire new skills or certifications, to make each consultant more valuable to our customers, and to DSN. In the life cycle of an assignment, there will be unexpected problems and issues that crop up. When these issues arise, DSN has the experience and the processes to deal with them promptly with right solution ensuring complete customer’s satisfaction. DSN has processes in place to deal with any unexpected issues quickly and effectively, thus mitigating any impact on the customer. DSN management is keenly aware that unresolved

issue could create unnecessary risk conflicts and delays of engagement. DSN’s Policy and Procedure Our policy focuses on customer satisfaction and our process starts immediately as a soon as DSN is notified of an assignment where an issue has occurred.

DSN’s Account Manager, and/or other senior managers, will be available to meet with the State of New Mexico and/or a Participating States’ agency in the same day, and typically within a few hours of being notified of an issue. DSN works to establish local offices that are located within at most an hour away from most State agencies and staffed with an Account Manager who is dedicated for that State’s IT Temporary Services contract. At a bare minimum, DSN will establish voice contact within one hour of being notified of an issue occurring with any assignment.

DSN’s Account Manager will promptly meet customer’s counterpart early on to evaluate the issue and discuss ways to remedy the problem and agree on expectations that become the next step for DSN.

Should any customer issue not be resolved to the customer’s satisfaction, the issue is immediately escalated to the DSN’s Vice President and/or DSN’s Director of Operation to provide an immediate resolution for the issue in a short response time.

In the event that a resource is needed to be terminated from the engagement, DSN’s Account Manager and/or other senior management will be available to meet with the State of New Mexico and NASPO Value Point representative on the same day to address any concern and take any immediate corrective action.

resource immediately, a DSN representative will be on hand to talk to the resource and escort him/her outside the work place in a professional manner such that it does not disrupt the work environment. DSN will counsel the resource outside the work place and ensure that all customer policies in addition to DSN’s HR policies are followed. DSN will return any customer’s belonging to the designated state representative.

In case customer gives an advance notice for terminating a resource, DSN will follow customer’s policies in handling the termination of the resource and ensures seamless transition of work. DSN will provide appropriate follow-up action items, documented by management reports, to mitigate any impact on the assignment.

DSN has the resources ready to backfill any vacancy in timely and professional manner. We have a proven capability to rapidly recruit, train and deploy candidates to meet the customer’s requirements. DSN shall prioritize the need for a replacement candidate, and the Account Manager will escalate the matter to DSN senior management. DSN management will act immediately on the State Agency’s request to ensure that a replacement is found within the time specified by the state agency

DSN will provide appropriate follow-up action items, documented by a management report, to correct deviations from the budget and schedule. In addition, DSN will add to the report to “lesson Learned”’ and save it in DSN repository.

DSN performs a thorough pre-employment screening with each potential candidate. DSN recruiters perform a detailed review of each potential candidate’s resume, paying particular attention to the technical skills and specific technology platforms that the candidate references in his/her resume. The DSN recruiter then begins a potential candidate’s resume assessment process, which includes reviewing each applicable technical skill and skill level required by the customer need, and determines the years of experience with each skill, the level of skill that the candidate has with that skill, and reviews how/when/where that candidate achieved that level of skill, and whether or not that candidate has any certifications proving that skill level. This initial assessment of the candidate’s skills is done during the initial candidate pre-screen interview, either by phone or via Skype. Subsequent to the candidate initial skills evaluation, the DSN recruiter begins the verification process of determining the validity of the candidate’s claimed skills and skill levels. This verification process begins with following up on candidate provided references and ensuring that the skills and skill levels claimed on the resume are verified by the reference as being accurate. DSN uses a third party (HireRight) to verify applicant's current and previous work history, including dates of employment; job titles, responsibilities and reasons for leaving, attendance employee rating, eligibility for rehire and work habits along with other information as it become necessary. DSN recruiter will review any training courses that the candidate may list on his/her resume and request certificates of completion if available. If the candidate has stated on their resume that they have been certified in any skill by a nationally or internationally known certification organization, the DSN recruiter will then verify that the certification actually was issued and whether or not the certification is still valid. All verifications and validations steps are documented and included with candidate credentials. Once all of these items have been verified and documented, and if the candidate is deemed to be a potential match for a particular customer need, the DSN recruiter will then submit the candidate for initial management review. DSN uses a variety of methods to conduct these technical reviews, including online tests and verbally provided tests. This RFP is soliciting proposals from Temporary Employment Companies to provide services relating to Administrative Support (Including Office and Clerical), Commercial/Industrial Workers, and Healthcare Staffing Services, Information Technology Professional Services, and Professional Services as defined within the scope of this RFP. Awarded Offerors shall be independent contractors and not employees of the State of New Mexico or Participating Entities. The Awarded Offeror’s staff, including temporary assigned individuals shall also not be considered employees of the Participating Entities.

These services are as needed and upon request from State agencies and political subdivisions. Contractor(s) will be responsible for hiring, firing, taxes, workers’ compensation, benefits, etc., for the candidates who are not employees of the individual participating entities. Candidates will not be provided employee benefits from the State agencies or political subdivisions. The State will not guarantee any minimum level of usage for any resulting contract(s) under this RFP. It is our policy that no candidate will start an assignment at customer’s site without having completed and signed an on-boarding document. Our number one focus is on customer satisfaction and we are proactive in that pursuit.

1.5. All temporarily assigned individuals will be appropriately dressed for the assignment and shall

maintain a professional demeanor. Dress code policy is established by the individual agencies. Temporary Candidate must dress according to the requirements of the Procuring Agency requesting the assignment.

It is DSN’s recruiting policy that the Account Manager and the recruiter conduct an on boarding session with the candidate to thoroughly explain customer’s requirements regarding the appropriate dress for the assignment and maintaining a professional demeanor. It is our policy that no candidate will start an assignment at customer’s site without having completed and signed an on-boarding document. Our number one focus is on customer satisfaction and we are proactive in that pursuit.

1.6. Temporarily assigned individuals should be available for the entire length of the assignment; however, if a replacement is required, a qualified replacement must be provided within twenty-four (24) hours of notification, including weekends and holidays.

Turnover can be one of the most detrimental impacts to an information technology project and thus on the customer. DSN prides itself on having less than a 2% turnover per year. DSN management philosophy and approach to managing and retaining scarce human resources is to focus on developing long term relationships with our employees, to understand and meet their needs and expectations. DSN has a well established resource planning, recruiting and screening processes to ensure that assigned individuals should be available for the entire length of the assignment. If a replacement is needed, DSN has an extensive network of resources that allows us to accommodate staffing needs and will ensure that a qualified replacement will be provided within twenty-four (24) hours of notification, including weekends and holidays.

DSN understands that these services are as needed and the length of the assignment could be reduced upon request from the procurement agency. 2. Work Hours

2.1. The exact work hours for temporarily assigned personnel will be determined by the Procuring Agency. Generally, work hours begin at 8:00 a.m. and end at 5:00 p.m. Monday through Friday excluding officially observed holidays by the Procuring Agency. Temporarily assigned personnel will work no more than eight (8) hours per day, excluding one (1) hour for lunch; or a total of forty (40) hours per week.

For over 27 years, DSN has been providing a full spectrum of IT Temporary Employment and Consulting Services at all skill levels to both the Public and Private Sectors meeting customer’s schedules. DSN’s has versatile IT staff that is ready to accommodate customer’s schedule, a 40 hours a week, 8:00 a.m. to 5:00 p.m. Monday through Friday excluding officially observed holidays by the Procuring Agency. DSN understands that the temporarily assigned personnel will work no more than eight (8) hours per day, excluding one (1) hour for lunch; or a total of forty (40) hours per week.

2.2. Temporarily assigned individuals will not be paid for their lunch hour.

We strongly believe that our 27+ years of extensive experience in state governments systems, IT staffing for multiple states coupled with our vast resources for qualified IT experts provide us an in depth understanding of our customers schedule needs, policies and procedures. DSN understands that our temporary IT staff will not be paid for their lunch hour.

2.3. Agencies have the right to request temporarily assigned individuals for holiday, evening/night, weekend or shift work.

For over 27 years, DSN has been providing a full spectrum of IT Temporary Employment and Consulting Services at all skill levels to both the Public and Private Sectors meeting customer’s schedules. DSN’s has versatile IT staff that is ready to accommodate customer’s schedule and needs for services. DSN’s IT Temporary workforce is ready to work holiday schedule as well as evening/night, weekend or shift work.

2.4. Hours may vary per Procuring Agency.

DSN understands that hours may change according to the need of the Procuring Agency. Our 27 years experience in providing a full spectrum of IT Temporary Employment and Consulting Services at all skill levels to both the Public and Private Sectors made us keenly aware of the variation customer’s schedules. DSN’s has versatile IT staff that is ready to accommodate customer’s schedule and needs for services any hour during the 24 hours day.

2.5. The Procuring Agency reserves the right to request a replacement of any individual. If for any

If the Procuring Agency requests a replacement of a resource, DSN representative will be on hand to talk to the resource and escort him/her outside the work place in a professional manner that does not disrupt the work environment. DSN will follow the customer’s policies and procedures. DSN will not charge the Procurement Agency for an individual who is determined to be unsatisfactory as follows:

If for any reason a replacement is required within the first eight (8) hours of service, there will be no charge to the Procuring Agency.

Any time beyond the initial eight (8) hours of service, the temporarily assigned individual is determined to be unsatisfactory; the DSN agrees to issue a credit invoice to the Procuring Agency for the total charges from the point the Procuring Agency notifies the DSN to request a replacement.

2.6. The Awarded Offeror agrees to replace an unsatisfactory individual within one (1) business day; however, the Procuring Agency has the option to contact a different Awarded Offeror for the service.

DSN has the resources and the proven capability to replace a candidate to meet customer’s requirement within one (1) business day. DSN understands and agrees that the Procurement Agency may choose to contact a different Awarded Offeror for the service.

2.7. The Procuring Agency shall be the sole judge as to whether a temporarily assigned individual is satisfactory and is fulfilling the Procuring Agency’s requirements.

Our customer is the sole judge as to whether DSN’s assigned individual is satisfactory and is fulfilling the Procuring Agency’s requirements. Absolutely, the Procuring Agency shall be the sole judge as to whether a temporarily assigned individual is satisfactory and is fulfilling the Procuring Agency’s requirements.

3. Listing of Position Classifications & Pay Rate

Each Procuring Agency will provide a listing of position classifications to the Awarded Offeror; however, the listing is not meant to be all-inclusive. Individual states or agencies may request other temporary positions by way of providing the Awarded Offeror(s) with the employee pay rate, position classification/title and description of duties. Describe how your company captures and provides additional positions requested by the Procuring Agencies.

4. Placement

Offeror shall describe how their company will provide assistance for Procuring Agencies with the placement of any candidate(s). At a minimum, include problem (conflict) and resolutions and the following items below:

4.1. Agencies may refer a candidate to be hired to the Awarded Offeror to sign up to perform specific services needed or may request the Awarded Offeror to recruit and provide the temporary Candidate. The Procuring Agency will not pay a placement or conversion fee for individuals who are a direct

DSN welcomes the referral of candidates and does not except to be paid a placement or conversion fee for individuals who are a direct referral from the Procuring Agency. 4.2. Upon a request for service from the Procuring Agency, the Awarded Offeror will provide expedient temporary employment services. An e-mail, facsimile, or telephone call from the Procuring Agency will constitute a request for service.

DSN goes to a great length to develop a deep understanding of our customers so that we can work in collaboration to meet our customers ‘needs. An e-mail, facsimile, or telephone call from the Procuring Agency will constitute a request for service and DSN will act immediately to ensure that the needs are met in a timely manner. DSN strives to establish partnership with various specialties companies.

4.3. The Procuring Agency reserves the right to interview the candidate to determine their qualifications for the required position (but this does not negate the Awarded Offeror’s responsibility of qualifying candidate(s)).

DSN prides itself on making the utmost rigorous and concerted effort for delivering qualified candidates with relevant experience to the Procuring Agency the first time, on time, within budget and with complete customer’s satisfaction. DSN’s believes that the ultimate responsibility of qualifying candidate(s) is the Awarded Offeror’s responsibility. 4.4. The Procuring Agency may reject and/or remove any candidate who does not meet the requested experience or is deficient in the performance of the assignment.

In the life cycle of an assignment, there will be unexpected problems and issues that crop up. When these issues arise, DSN has the experience and the processes to deal with them promptly with right solution ensuring complete customer’s satisfaction. DSN has processes in place to deal with any unexpected issues quickly and effectively, thus mitigating any impact on the customer. DSN management is keenly aware that unresolved issue could create unnecessary risk conflicts and delays of engagement. Therefore, DSN agrees and understand fully that “The Procuring Agency may reject and/or remove any candidate who does not meet the requested experience or is deficient in the performance of the assignment.”

4.5. Procuring Agencies may select Awarded Offeror(s) within their geographic region based on the preference of the Procuring Agency.

To the extent possible, DSN recruits local employees, thus eliminating travel costs and ensures stability and low turnover. Therefore, DSN understands

We understand that the goal of the Procurement Agency is to fulfill its need for a qualified IT candidate(s) on time and within budget. DSN understands and agree that “Multiple Awarded Offerors may be contacted to fill the same position.”

. 5. Contractor’s Responsibilities

5.1. The Awarded Offeror is responsible to obtain the information as described in the Scope of Work and any other information necessary to determine what job category satisfies the service request.

DSN’s process starts by identifying and understanding the current and anticipated needs of our customer. DSN believes that customer satisfaction requires thorough understanding of every requirement. DSN is committed to matching resources to “the information as described in the Scope of Work and any other information necessary to determine what job category satisfies the service request.” Mismatching resources to customer's requirements can be one of the most detrimental impacts to an information technology project. Therefore, DSN performs d iligent analysis of the information as described in the Scope of Work and any other information necessary to determine what job category satisfies the service request.”

5.2. The Awarded Offeror will inform the Procuring Agency point of contact of the proposed job classification and applicable rate to obtain authorization to proceed with the service request.

DSN is committed to timely and accurate communication with the Procuring Agency point of contact of the proposed job classification and applicable rate to obtain authorization to proceed with the service request. DSN believes that obtaining authorization to proceed with service request is of utmost importance for the success of an assignment. DSN has a proven history of working closely with the Procurement Agency point of contact to ensure proper and orderly process to proceed with service requested.

5.3. Placing candidates out of applicable job classification is considered an abuse of the contract. Periodic checks of requests and assignments will be performed by the Procuring Agency to ensure this does not occur.

DSN’s business practices and philosophy is based on creating transparency and clear communications with customer’s point of contact regarding any existing issues. DSN is committed to following customer’s policies, procedures and standards. DSN’s number one focus is on customer satisfaction and we are proactive in that pursuit. DSN understands that periodic check of requests and assignments will be performed by the Procuring Agency to ensure that placement of candidates out of the applicable job classifications don’t occur.

5.4. The Awarded Offeror is responsible for conducting appropriate background and reference checks on potential candidates prior to any assignments and should be prepared to conduct more extensive background investigations when required by the Procuring Agency. Awarded Offeror must send notification to the Procuring Agencies of the compliance of the background and reference checks.

Failure to provide notification of compliance will be considered a violation of the contract and may result in rejection of the candidate and possibly jeopardize future placements by offending Awarded Offeror.

Thorough screening of each candidate qualifications against customer requirements and candidate’s ability against customers works environment and culture. In depth evaluation of applicant technical skills takes place and a telephone interview, personal interview, technical interview and testing. Applicant's references and education are checked thoroughly.

DSN utilizes a set of test papers for evaluation of candidates. We conduct an on-line technical test for the candidates to evaluate their technical skills. DSN uses a third party testing facilities to help in assisting in the evaluation of the technical skills of the potential hire; DSN uses Derrico Computers and Software Unlimited for the testing purposes.

DSN uses a comprehensive and thorough Drug Screening and Background check process to qualify personnel for employment at the firm. DSN uses several qualified medical facilities nationwide such as Quest diagnostics and its affiliates nationwide to conduct drug screening on all employees before the start of their assignment. In addition, DSN uses HireRight for Background Screening for each person before the start of their employment with DSN.

E-Verify – DSN performs an e-Verify screen through the Department of Homeland Security on all resources prior to customer start.

Quality assurance – DSN is continually evaluating the performance of each resource during a customer engagement to ensure that both the customer and the resource are satisfied with the progress of project.

Engagement end assessment – At the completion of any DSN assignment, resources are evaluated for future placements and lessons learned are documented.

DSN is committed to timely communication with Procuring Agencies to ensure compliance of the background and reference checks.

5.5. These services are as needed and upon request from State agencies and political subdivisions. Awarded Offeror will be responsible for liability insurance, federal and state payroll requirements including but not limited to insurance coverage for any candidate sent to the Procuring Agency, payroll taxes, payroll reports, workers’ compensation, benefits, hiring and firing etc., for the candidates .

DSN understands that these services are on as needed basis and upon request from Sate agencies and political subdivisions. DSN has a strong track record and proven history of working as a U.S. corporation since 1989 with good standing. DSN has liability insurance that meets or exceeds the requirements of this RFP. Ac copy of the insurance certificate is included in this response. Since our inception, DSN has been in full compliance with law and fully responsible for federal and state payroll requirements including but not limited to insurance coverage for any candidate sent to the Procuring Agency, payroll taxes, payroll reports, workers’ compensation, benefits, hiring and firing etc., for the candidates.

5.6. The Awarded Offeror is responsible for conducting periodic quality assurance checks with the Procuring Agency’s point of contact to verify that the Procuring Agency’s requirements are being

fulfilled by the candidate. At a minimum, these checks should be completed at the end of the first week of any assignment. Procuring Agencies may request quality assurance checks at any interval during the term of the candidate’s placement.

A key component in our approach to assess customers’ satisfaction is to assess and ensure that the quality of the work being performed meet or exceed their expectations. Therefore, DSN is continually evaluating the performance of each resource during a customer engagement to ensure that both the customer and the resource are satisfied with the progress of project. Our quality assurance process is based on working closely with the Procurement Agency’s point of contact to ensure and verify that the Procuring Agency’s requirements are being fulfilled by the candidate. DSN’s Account Manager, at a minimum will meet with Procuring Agency at the end of the first week of any assignment to check and verify that Procuring Agency’s requirements are being fulfilled by the candidate. DSN prides itself on its strong track record and proven history of complying with customers policies and procedures and follow their Quality Assurance standards.

5.7. Candidates may be hired as a permanent employee of the State or Procuring Agency if, the Procuring Agencies and hiring processes have been complied with and if the candidate elects to accept employment with the State or Procuring Agency. Such occurrence will create no further obligation (financial or otherwise) on the part of the Procuring Agency.

DSN’s employment policy does not prevent candidates from being hired as a permanent employee of the State or Procuring Agency if the Procuring Agency’s and hiring processes have been complied with and if the candidate elects to accept employment with the State or Procuring Agency.” DSN’s policy regarding such occurrence “creates no further obligation (financial or otherwise) on the part of the Procuring Agency.”

5.8. The Procuring Agency will not be responsible for the Awarded Offeror’s candidate who voluntarily leaves the Awarded Offeror’s employment or engages in employment with another company.

DSN has 27 years experience in providing IT Temporary service and similar contracts with many state governments and in addition to the many Fortune 500 companies. As a result, DSN is keenly aware of such situation and fully understand and agree that “The Procuring Agency will not be responsible for the Awarded Offeror’s candidate who voluntarily leaves the Awarded Offeror’s employment or engages in employment with another company.”

5.9. The Awarded Offeror agrees to ensure candidates agree to be bound by the Participating State’s security regulations, policies, and standards as required by the Procuring Agency (e.g., Department of Corrections). This will vary based on the individual Procuring Agency’s requirements.

DSN has 27 years experience in providing IT Temporary service and similar contracts with many state governments and in addition to many Fortune 500 companies. As a result, DSN is keenly aware of such situations and fully understand and agrees that “The Procuring Agency will not be responsible for the Awarded Offeror’s candidate who voluntarily leaves the Awarded Offeror’s employment or engages in employment with another company.”

5.10. Awarded Offeror shall ensure adequate backup documentation (such as Candidate timesheets) are attached to invoice or billing requests. The timesheet should include the following:

a. Name of the Procuring Agency; b. Name of the temporarily assigned individual; c. Dates worked; d. Beginning and ending time;

DSN is committed to ensure that timesheet for New Mexico and NASPO Value Point shall include the following:

a. Name of the Procuring Agency; b. Name of the temporarily assigned individual; c. Dates worked; d. Beginning and ending time; e. Number of regular hours worked each day; and f. If applicable, number of overtime hours worked each day

5.11. The Awarded Offeror is responsible and may be held financially liable for the negligent acts of its Candidates.

Throughout our 27 history of providing excellent IT Temporary Services to state governments and fortune 500 companies, DSN has been responsible for any financially liabilities as a result of negligent acts of its candidates. DSN’s business practices and philosophy is based on being a responsible vendor creating transparency with our customer to address any such issues that may arise.

6. Bonding

6.1. The Awarded Offeror shall have the ability to bond candidates as directed by the Procuring Agency.

Throughout our 27 history of providing excellent IT Temporary Services to state governments and fortune 500 companies, DSN has been ready to bond candidates as directed by our customers. DSN’s business practices and philosophy is based on being committed to bonding candidates as directed by the Procuring Agency.

6.2. The fee for this service will be borne by the Procuring Agency.

DSN’s has over 27 years experience in recruiting and providing IT Temporary services to state governments and fortune 500 companies. DSN understands that “The fee for this service will be borne by the Procuring Agency.” DSN is ready to accept any changes in your policies regarding bonding.

6.3. Selection of the bonding insurer is at the Awarded Offeror’s discretion; however, each insurance policy shall be:

7. Travel

DSN’s travel policy for candidates on assignment at New Mexico or NASPO participating states for travel expenses submitted for reimbursement shall follow Procuring Agency’s internal policies.

7.3. Valid travel reimbursements will be reimbursed at the Procuring Agencies current travel rate.

DSN’s travel policy for candidates on assignment at New Mexico or NASPO participating states for valid travel reimbursements will be reimbursed at the Procuring Agencies current travel rates.

7.4. The candidate and the Procuring Agency’s contact person must sign the travel expense form.

Throughout our 27 history of providing IT Temporary Services to state governments and fortune 500 companies, DSN’s travel policy for candidates on assignment is based on following customer’s travel policies. DSN’s travel policy for candidates on assignment at New Mexico or NASPO participating states for travel expenses shall require the candidate and the Procuring Agency’s contact person to sign the travel expense form.

7.5. The form must be submitted with the Awarded Offeror’s invoice for services with the travel expense as a separate line item on the invoice in order to be reimbursed by the Procuring Agency.

Throughout our 27 history of providing IT Temporary Services to state governments and fortune 500 companies, DSN’s travel policy for candidates on assignment is based on following customer’s travel policies. DSN is committed to ensure that travel expense form for candidates on assignment at New Mexico will be submitted as a separate line item on DSN’s invoice for services in order to be reimbursed by the Procuring Agency.

7.6. The type of position requiring travel will vary based on the individual Procuring Agency’s requirements.

As a result of our long history in providing IT Temporary services, DSN is keenly aware that the type of position requiring travel will vary based on the individual Procuring Agency’s requirements.

8. Equipment, Property and Damages

8.1. The Awarded Offeror shall be responsible for the proper maintenance and custody of any personal

DSN shall be shall be responsible for the proper maintenance and custody of any personal tangible property owned and real property furnished by the Procuring Agency for the use in connection with the performance of the contract.

8.2. The Awarded Offeror will reimburse the Procuring Agency for such property’s loss or damage caused by the Awarded Offeror’ assigned individual, with the exception of normal wear and tear.

Throughout our 27 history of providing excellent IT Temporary Services to state governments and Fortune 500 companies, DSN has been responsible for any property’s loss or damage caused by any of DSN’s candidates on assignment at our customers. Therefore, DSN will reimburse the Procuring Agency for such property’s loss or damage caused by DSN’s assigned individual, with the exception of normal wear and tear.

8.3. The equipment used may include computers, copy machines, phones, printers, etc. Equipment may vary depending on the Candidate assignments.

DSN’s candidates on assignment at our customers use different types of equipment. DSN is keenly aware that the equipment used may include computers, copy machines, phones, printers, etc. Equipment may vary depending on the DSN’s candidate assignments.

9. Procuring Agency’s Responsibilities

9.1. Prior to contacting the Awarded Offeror (s), the Procuring Agency is responsible to define details of the request to include, but not be limited to:

9.1.1. Number of individuals needed; 9.1.2. Job duties; 9.1.3. Equipment to be used; 9.1.4. Knowledge, skills and education and/or experience; 9.1.5. Computer software to be used; 9.1.6. Hours of work; 9.1.7. Expected length of assignment; 9.1.8. Job related attire; 9.1.9. Position location; 9.1.10. Procuring Agency contact person; and 9.1.11. Other pertinent job-related information.

10.3. It is reasonable to expect employment eligibility and references will be required for all candidates; background checks for referrals by the Procuring Agency will be at the discretion of the Procuring Agency.

Our process for conducting appropriate employment eligibility and references checks on potential candidates prior to any assignments is as follows:

E-Verify – DSN performs an e-Verify screen through the Department of Homeland Security on all resources prior to customer start.

DSN verifies the legal status for all H1B and TN holders and confirms their employment eligibility before assigning them to any of customers. In addition, DSN ensures that the resources that work under the visa program are authorized to work if offered the position.

Thorough screening of each candidate qualifications against customer requirements and candidate’s ability against customers works environment and culture. In depth evaluation of applicant technical skills takes place and a telephone interview, personal interview, technical interview and testing. Applicant's references and education are checked thoroughly.

DSN utilizes a set of test papers for evaluation of candidates. We conduct an on-line technical test for the candidates to evaluate their technical skills. DSN uses a third party testing facilities to help in assisting in the evaluation of the technical skills of the potential hire; DSN uses Derrico Computers and Software Unlimited for the testing purposes.

DSN uses a comprehensive and thorough Drug Screening and Background check process to qualify personnel for employment at the firm. DSN uses several qualified medical facilities nationwide such as Quest diagnostics and its affiliates nationwide to conduct drug screening on all employees before the start of their assignment. In addition, DSN uses HireRight for Background Screening for each person before the start of their employment with DSN.

10.5. Other background checks will be at the discretion of each requesting Procuring Agency.

DSN understands that “other background checks will be at the discretion of each requesting Procuring Agency.” Should an additional background check be required due to the nature of the assignment, DSN is ready to perform extra checks as requested by the Procurement Agency.

10.6. Procuring Agencies reserve the right to request and conduct pre-employment background checks and drug testing prior to the potential candidates starting date.

Cost Proposal (Binder 2)

Attachment C: Cost Form Temporary Employment Services will be based on the Offeror’s Administrative Service Fee (based on a percentage) for each Temporary Employment Services. Offeror’s Administrative Service Fee is a not to exceed maximum percentage fee; however, the Offeror’s Administrative Service Fee may be negotiated at a lower percentage. The Offeror’s Administrative Service Fee may be tied to the Producers Price Index (PPI) but must not be more than the PPI. The Awarded Offeror’s Administrative Service Fee shall remain firm for the duration of the NASPO Value Point Master Agreements including all optional renewals. Each of the Temporary Employment Services must have an Offeror’s Administrative Service Fee for all Participating Entities. The Lead State is exempt from federal excise taxes and no payment will be made for any taxes levied on the Awarded Offeror’s or any Subcontractor’s employee’s wages. The Lead State will pay for any applicable Lead State or local sales or use taxes on the products provided or the services rendered. Taxes shall be included as a separate line item on an Awarded Offeror’s invoice. The tax rules with respect to other Participating Entities may vary and are expected to be addressed in the Participating Addenda. DSN has read, understands and agrees on the requirements of the Administrative Service Fee for the Temporary Employment Services.
